119구급대를 통해 3차 병원으로 재이송된 환자분석 (Analysis of patients retransferred to a tertiary hospital through paramedics)

  • Purpose: The study aimed to collect the first aid activity log data of patients who were retransferred through paramedics and to analyze the data in the hospital to improve the plan. Methods: We analyzed 434 retransferred patients, out of the 18,197 patients who visited the emergency medical center in the C area in G metropolitan city, from January 2017 through December 2018. The collected data were analyzed using the SPSS software. Results: The patients were retransferred for various reasons: 17.7% (77 cases) due to the absence of specialists, 15.0% (65 cases) required first aid, and 5.3% (23 cases) due to absence of medical department. In addition, the major medical department with the largest number of retransfer was the department of emergency medicine with 38.2% (166 cases). In the prehospital stage, 38.5% (167 cases) were classified as severe, but in the hospital stage, they were classified as mild. In addition, as a result of hospitalization and discharge, 60.4% (262 cases) were discharged. Conclusion: Most of the retransferred patients were non-emergency patients, and were discharged for mild conditions. The overcrowding in the emergency room of a tertiary hospital can be prevented by reducing the retransmission.

Fresh Agricultural Products Online Retailer's Emergency Logistics Capability Framework During the Pandemic

  • Purpose: During the pandemic, a large number of orders forced online retailers of fresh produce to quickly change their emergency logistics capabilities to meet the needs of ordinary consumers during the pandemic. Our research aims to help online retailers establish an emergency physical distribution framework for fresh produce during disasters to meet consumer needs. Research design and methodology: 160 effective responses were collected from the online response team in Wuhan, China, and exploratory factor analysis was used to determine the emergency logistics capability framework. Twelve experts were invited online to collect their scoring opinions and use the entropy method to determine the importance of emergency logistics capabilities. Results: Our results have identified a total of 17 emergency logistics factors for online retailers, and we found that Transportation route planning and reconstruction capabilities, Emergency plan planning ability, and Supply chain real-time information sharing capability are the most important in the overall framework. Conclusions: This research completely established the physical distribution framework of fresh agricultural products online retailer in emergency situations. It enriches academic resources in the field of emergency distribution and provides a scientific basis for corporate managers to improve their physical distribution capabilities in emergency situations.

  • This paper considers urban emergency service systems that dispatch vehicles (facilities) from fixed bases with the objective of finding an optimum allocation of emergency facilities. A case study of fire station problem of Seoul city is conducted to analyze the deployment of fire-fighting resources and develop a class of improved deployment strategies with Parameteric Allocation Model (P.A.M.). The study shows a long term plan leading to balanced fire protection for most fire hazardous districts as well as low alarm districts.

  • 재난상황하에서는 신속한 지역단위 재난복구를 전문적으로 담당하는 인력이 필요하다. 그러나, 현재의 재난대응체계에서는 대민지원, 자원봉사 등 수준에서 재난복구를 시행하고 있으므로 전문성 부족으로 인한 초기대응 및 재난복구 지연, 재난 지속시 대응 미흡 등 다양한 문제점이 발생하고 있다. 그러므로, 재난상황시 지자체단위 초기 대응능력 구비와 신속히 재난복구를 할 수 있는 전문성을 가진 부대의 창설이 필요하다. 따라서, 본 연구에서는 재난상황하 긴급 재난복구를 통한 지역안정과 재해 주민구호, 재난이전으로의 신속한 전환을 위한 예비군 긴급대응부대를 창설과 운용방안 수준의 방향성을 제시하였다.

이상강우에 대비한 성주댐의 홍수조절 능력 분석 (Evaluation of Flood Control Capacity for Seongju Dam against Extreme Floods)

  • As a fundamental research to establish a safety operation plan for irrigation dams, this study presents hydrologic analysis conducted in Sungju Dam watershed based on various rainfall data. Especially those reservoirs without flood control feature are widely exposed to the risk of flooding, a safe and optimized operation program need to be improved against arbitrary flooding. In this study, reservoir routing program was developed and simulated for reservoir runoff estimation using WMS hydrology model. The model simulated the variations of reservoir elevation under the condition of open or closed emergency gate. In case of closed emergency gate, water surface elevation was given as 193.15 m, and this value exceeds the dam crest height by 1.65 m. When the emergency gate is open, the increment of water surface elevation is given as 192.01 m, and this value exceeds dam crest height by 0.57 m. As an alternative plan, dam height increase can be considered for flood control under the PMP (Probable Maximum Precipitation) condition. Since the dam size is relatively small compare to the watershed area, sound protection can be expected from the latter option rather than emergency gate installation.

응급의료센터를 위한 위험기반 운영계획 모델 (Risk-based Operational Planning and Scheduling Model for an Emergency Medical Center)

  • In order to deal with high uncertainty and variability in emergency medical centers, many researchers have developed various models for their operational planning and scheduling. However, most of the models just provide static plans without any risk measures as their results, and thus the users often lose the opportunity to analyze how much risk the patients have, whether the plan is still implementable or how the plan should be changed when an unexpected event happens. In this study, we construct a simulation model combined with a risk-based planning and scheduling module designed by Simio LLC. In addition to static schedules, it provides possibility of treatment delay for each patient as a risk measure, and updates the schedule to avoid the risk when it is needed. By using the simulation model, the users can experiment various scenarios in operations quickly, and also can make a decision not based on their past experience or intuition but based on scientific estimation of risks even in urgent situations. An example of such an operational decision making process is demonstrated for a real mid-size emergency medical center located in Seoul, Republic of Korea. The model is designed for temporal short-term planning especially, but it can be expanded for long-term planning also with some appropriate adjustments.

  • Recently, the power supply situation has deteriorated and the plan has issued to utilize of emergency generators for securing standby power. If a part of Emergency Generators was utilized by peak shaving, it can be avoiding over-investment in supply power facility. In this paper we investigate management of Emergency Generator and understand the current situation. So, we verify demand response resource possibility of Emergency Generator.

국내 주요 수변시설물 EAP 프레임워크 개발 (Development of a Framework of Emergency Action Plan for Domestic Water Front Critical Infrastructure)

  • 강우는 자연과 인간에게 중요한 자원이다. 적절한 강우는 농작물의 성장과 토지의 온도를 일정하게 유지시켜줌으로써 우리 사회가 안정적으로 유지되도록 하는 매우 중요한 역할을 한다. 그러나 태풍 및 집중호우 등과 같이 특정 지역에 계획강수량보다 더 많이 발생한 집중강우는 시설물 및 인간에게 커다란 피해를 입히기도 한다. 이에 따라 미국, 일본 등 국외 선진국의 수재해 대응기관들은 시설운영자와 재난전문가, 응급대응인력들에게 가이드라인을 제공하기 위해 수재해 EAP(Emergency Action Plan)를 작성 운영하여 시설물을 보호하고 강화하는데 활용하고 있다. 대표적인 예로, 미국의 FEMA와 DHS는 시설물과 인명을 보호하기 위해 댐, 제방 등을 위한 다양한 EAP를 운영하고 있는데, 특히 FEMA는 EAP를 시설물이나 인명에 가해지는 피해나 기능적 충격과 같은 응급상황을 감소시키기 위해 작성하는 공식문서로 규정하고 있으며, 이 EAP를 통해 응급 시에 발생할 수 있는 모든 상황뿐만 아니라 평상시 발생할 수 있는 시설물관리상의 문제까지 대응이 가능하도록 규정하고 있다. 이에 본 연구에서는 미국, 일본 등 국외주요국과 국내에서 사용중인 EAP 관련 규정을 분석하여 국내 상황에 적합한 EAP 체계를 제안하였다. EAP는 예방, 준비, 대응, 복구의 재난단계별로 시설물과 인명을 보호하기 위해 재난전문가들과 시설운영자들이 취해야 할 업무, 응급장비 운용 등의 내용을 담고 있다. 본 연구를 통해 개발된 EAP 프레임워크는 테스트베드 적용을 통해 적정성을 평가받았으며, 국내 홍수재난 위험을 감소시키기 위한 절차와 방법 개선에 기여할 것으로 기대된다.

인천국제공항의 응급환자 발생 및 이송 현황에 관한 연구 (A study on the occurrence and transfer status of emergency patients at Incheon International Airport)

  • Purpose: The study aimed to analyze the occurrence and transfer status of emergency patients at Incheon International Airport in Korea. Methods: This study design included 810 patients for eight years, from July 1, 2014 to June 31, 2022. The collected data were analyzed with SPSS statistics version 25.0. Results: For the demographics, 531 (65.6%) were males, 151 (19.5%) were in the highest age group (in their 50s), and 289 (35.9%) were foreigners. The most common place of occurrence was in protected areas in the passenger terminal, with 341 (42.1%) of cases occurring here. The time of arrival at the site (z=-3.444, p=.001), stay duration at the site (z=-8.145, p=. 001), and transfer time (z=-3.623, p=.000) were all significantly longer. Conclusion: It is necessary to consider a rapid emergency transportation plan, such as developing a system that simplifies immigration procedures when emergency patients move from protected areas to general areas. Moreover, in order to quickly respond to foreign emergency patients, detailed characteristics will need to be identified. Lastly, considering the long transfer time, an instructing doctor should accompany an ambulance and actively perform advanced resuscitation.

철도 비상사고 비상대응 훈련 프로르갬 개발에 관한 연구 (A Study on Developing the Training Program for the Emergency Response against Railroad Emergency Accidents)

  • The Railroad Safety Act was implemented in 2005 due to the increased concerns regarding railroad safety since the Daegu subway fire in 2003. In line with this, the Act became a standard to build the total railroad safety systems which has been overlooked compared to the quantitative growth of the railroad business. The minister of the ministry of Land, Transportation and Maritime Affairs lays down the Act for railroad Companies to conduct emergency programs in response to the emergency occurring on the railroad such as fire, explosion, derailment, etc. By enacting the Safety Act, the nation's construction of the contingency management system for railroad emergency increased", and it made the railroad company prepare the Emergency SOP by establishing 'The guideline on the Establishment of the railroad Emergency Plan' in order to support the efficiency of the Act. In line with this, I would like to analyze a matter of system development and the main function of the "railroad Emergency Training Program" for activation improvement of standardized operation procedure developed through the 'Total capitalize Safety Technology development'.
