Analysis of Maternal and Neonatal Transport by the 1339 Emergency Medical Information Center in Busan Area (부산 지역 응급의료정보센터를 통한 산모와 신생아 전원에 대한 연구)

  • Purpose: In relation to perinatal healthcare, medical institutions and resources are limitative and also are in a state of flux due to the therapeutic specialty. We analyzed requests for interhospital transfers received by Busan 1339 Emergency Medical Information Center (EMIC) to grasp the state of perinatal healthcare delivery system. Methods: This study was conducted on the basis of data inputted into the computing system of Busan 1339 EMIC, between January 1 and December 31, 2009. In connection with 378 pregnant women and 136 newborns who were required to transfer, retrospective analyses were made of the success rate of transfer (SR), the number of contacted hospitals, the time required for transfer and the reason of transfer and refusal. Results: In the case of pregnant women, the SR were 65.5%. They came in contact with 2.7 hospitals, and it took 24.4 minutes. As for the reason of transfer, preterm labor accounted for the highest proportion. In the case of newborns, the SR were 71.3%. They came in contact with 2.4 hospitals, and it took 15.6 minutes. The most common reason of transfer were respiratory symptoms. In the reason of refusal with pregnant women and newborn, the lack of medical staff, medical equipments and wards accounted for great. Conclusion: Many pregnant women and newborns have been transferred to hospitals by EMIC, but the SR has not been higher yet. Accordingly, there is a need to evaluate the propriety of perinatal treatment system, as well as to set up effective perinatal healthcare delivery system.

  • The rise of the Internet of Things (IoT) devices have greatly influenced many industries and one of them is healthcare where wearable devices started to track all your daily activities for better health monitoring accuracy and even down to tracking daily food intake in some cases. With the amounts of data that are being tracked and shared between from these devices, questions were raised on how to uphold user's data privacy when data is shared between these IoT devices and third party. With the blockchain platforms started to mature since its inception, the technology can be implemented according to a variety of use case scenarios. In this paper, we present a system architecture based on the healthcare system and IoT network by leveraging on multiple blockchain networks as the medium in between that should enable users to have direct authority on data accessibility of their shared data. We provide proof of concept implementation and highlight the results from our testing to show how the efficiency and scalability of the healthcare system improved without having a significant impact on the performance of the Electronic Medical Record (EMR) that mostly affected by the previous solution since these solutions directly connected to a public blockchain network and which resulted in significant delays and high cost of operation when a large amount of data or complicated functions are involved.

Influence of Multiple Rib Fracture upon Traumatic Hemo-pneumothorax (다발성 늑골골절의 외상성 혈기흉 발생에 대한 영향)

  • Purpose: Multiple rib fracture (MRF) and a hemopneumothorax accompany with most blunt chest traumas. We aimed to analyze the factors increasing the probability of a hemopneumothorax. In addition, other injuries accompanying MRF were analyzed. Methods: We retrospectively reviewed the medical records of 154 mutiple rib fracture patients who visited our hospital between January 2005 and December 2007. The medical records were reviewed for sex, age, mechanism of injury, location, number of fractures, distance of dislocated rib fragments, and presence of complications. We measured the distance of bony dislocations by using the PACS (Picture Archiving and Communication System). Results: The average number of rib fractures was $3.7{\pm}2.1$, and the number of rib fractures significantly influenced the incidence of a hemothorax (p<0.001). The risk of a hemothorax was increased in a bilateral MRF compared to a unilateral MRF (p=0.027). The distance of dislocated rib fragments influenced the probability of a hemothorax significantly (p=0.018), and subcutaneous emphysema and lung contusion were significantly associated with a pneumothorax (p=0.021, p=0.036). Conclusion: The number of MRFs did not influence the risk for a pneumothorax, but did influence the risk for a hemothorax. The laterality, distance of dislocation, also had an influence on the risk for a hemothorax. Also, subcutaneous emphysema and lung contusion were increased in cases with a pneumothorax. We must consider the possibility of a hemothorax even when the initial chest X-ray shows no evidence of a hemothorax. If a lung contusion is present, then an occult pneumothorax must be considered.

A study on improving the interior design of ambulance (구급차 내부 디자인 개선에 관한 연구)

  • Purpose: This research project addressed the need to designing more safe and efficient interior of the future ambulance in Korea. Methods: The study sample contained 760 paramedics in 4 districts. Data was collected by using a revised and complemented questionnaire based on literature review. Results: In relation to the efficacy and safety of work, answers related to storage closet showed to be the highest, and the most difficult part of paramedic work in an ambulance was lurching. CPR is the most frequently used emergency care inside an ambulance, but 66% of the paramedics responded that accurate CPR is not possible during vehicle transfer. Safety belts are not worn for 82.8% of the time, because of discomfort (51.3%). 13.8% of the paramedics responded that stretchers are unstable, 29.5% had an experience of having patients fall off the stretcher inside an ambulance. There were comments on installing equipments to prevent noise, and assist communication. Conclusion: The suggested practical layout contains five main modifications 1. Developing specially designed belt is needed for paramedic safety & efficient work. 2. The seats are molded to be ergonomically friendly. 3. Equipments to secure the body and safety devices for CPR are needed. 4. System improvement for communication between the driver seat and paramedics is needed. 5. The stretchers are molded to be maximize efficiency and minimize injury.

  • In this paper, aim to suggest the medical information wireless transmission system to provide the mobility of medical service by means of wireless area network which makes it possible, home or at a long range, to check and oversee the state of patients, and to can out a simulation. The proposed method converts medical information to digital data in an emergency and sends them to mobile terminals such as PDAs to make possible swift first aid. The simulation took advantage of the OFDM transmission method based on IEEE 802.1la in order to send reliable medical information in mobile wireless channel environment, and analyzed the system performance by applying convolution encoding to transmit reliable information in AWGN and 3-ray mobile multipath fading channel environment.

  • The purpose of this study was to survey a development of the wireless transmission system of medical images for ubiquitous medicine. There have been many changes in medical equipments and medical record medical treatment and medical record within hospital and PACS(Picture Archiving Communication System) which is picture management system for patients can be typical cases. It is difficult to use these automated medical systems unless they are within hospital and in case of rapid image reading in the emergency cases or in absence of doctor, it is difficult to perform it immediately. The present study implemented an image transmission system using CDMA connection so that images in the server can be viewed at any time and in any place. Remote wireless diagnosis based on medical images using PDA is applicable to medical areas that require mobility, and the use of PDA can be an ideal alternative for point of care. The use of PDA enables prompt and accurate access to digital medical images, which in turn reduces medical accidents and improves the quality of medical services through high productivity and efficiency of medical practitioners' works. It also enables quick response to patients' demands and high-quality medical services and, consequently, patients' high satisfaction.

Interference Priority: A New Scheme for Prioritized Resource Allocation in Wireless

  • A standard paradigm for the allocation of wireless resources in communication demands symmetry, that is, all users are assumed to be on equal footing and hence get equal shares of the system's communication capabilities. However, there are situations in which "prime users" should be given priority, as for example in the transmission of emergency messages. We examine prioritization policies that could be implemented at the physical layer and propose a new one, termed interference priority (IP), which is shown to have excellent performance. We evaluate the performance of these prioritization techniques both in controlled settings and within the context of a full cellular system and discuss the impact of prioritized use of resources on the unprioritized users.

  • The author Nakajima was involved in the field of disaster communications and emergency medical care as guest research scientist at the Fukushima Nuclear Accident Independent Investigation Commission established by the National Diet of Japan and reviewer of the Commission's report, and Kurokawa was the chairman of this Commission. Looking back over a decade, we are on the liability issue of bureaucrats and telecom operators, so it's becoming clear what was hidden at the time. The battery of NTT DoCoMo's mobile phone repeaters had a capacity of only about 24 hours, and communication failures increased after one day. The Government also failed to issue an announcement of "Vent from reactor" under the Telecommunications Act Article No. 129. This mistake lost the opportunity to use the third-party telecommunications (e.g. taxi radios). Furthermore, as a result of LASCOM (telecommunications satellite network for local governments via GEO) and a variety of unexpected communication failures, the evacuation order "Escape!" could not be notified to the general public well. As a result, the general public was exposed to unnecessary radiation exposure. Such bureaucratic slow action in emergencies is common in the response to the 2020 coronavirus.

  • In this study we show a telemedicine system using a DSP, which gives fast and exact medical data such as the ECG signal of the external emergency patient and enables the patient to get temporary treatments under the direction of a doctor in the hospital. This transceiver, which is able to treat the real time transmission of dynamic medical data captured by the measuring instrument and bidirectional communication of voice signal, is implemented using DSB-SC as modulation and demodulation technique and digital filter of each terminal are implemented as FIR filter. The system designed with DSP in this study is very small and compact and it can k, furthermore, to support additional biomedical signals just by renewing the software.

  • This paper describes the implementation of wireless realtime monitoring system using modified CSMA/CA protocol. This system consists of wireless modem, central monitor, mobile station, DB server, and offer advantage of mobility, reduced installation time, long-term cost savings, and so on. And this system offers patient position pursuit service. Patient position pursuit service must be offered to deal with emergency which can be occured during patient movement. This paper proposes modified CSMA/CA protocol and patient position pursuit algorithm, implements wireless realtime monitoring system using it.
