Isolated penetrating gluteal stab injury with uncontrolled bleeding in Türkiye: a case report

  • Arterial injuries in the gluteal region caused by a knife are rare but serious, with mortality rates of up to 25%. This case report presents the management of a young male patient admitted to the emergency department in hypovolemic shock, with uncontrollable bleeding from an isolated penetrating gluteal injury. Additionally, the details of the surgical approach employed are discussed.

IT Manager Behavior in Crisis Response: Understanding Crisis Cases Using Recommendations from the Crisis Management Literature

  • In their role as stewards of organizational information technology (IT), IT managers participate in crisis management activities. While much has been said about the power of technology in improving preparation for emergencies, the behavior of IT managers in crisis situations is not well understood. This paper addresses IT manager actions during the crisis response effort, when appropriate actions need to be taken at short notice. Recognizing that few guidelines exist for IT managers in these situations, we use recommendations from the crisis management literature in analyzing five earthquake cases from Japan and Taiwan. We identify several recommendations from this set for IT managers, which are related mainly to communications and leadership behaviors, suggesting that the IT manager role is a vital one in crisis response. The research additionally shows that recommendations from the crisis management literature have value also when applied to IT managers. Finally, we conclude on several ways that our understanding of IT manager crisis response could be developed by future research.

  • Rapid screening for internal contamination by alpha- and beta-emitting radionuclides is essential in situations involving radiation workers or radiation accidents. This study focused on the use of urine samples and liquid scintillation counting to quickly and accurately assess contamination. Calibration of the alpha and beta detection areas ensured precise measurement results. The major radionuclides recommended for surveillance during accidents were also considered. This study evaluated the effectiveness of the method by examining various parameters, including the limit of detection, linearity, sensitivity, selectivity, accuracy, ruggedness, and blind test sample analysis. The liquid scintillation counting method is an effective tool for screening urinary samples to detect alpha- and beta-emitting radionuclides, particularly during radiation emergencies, despite some limitations in precision.

  • This study proposes an algorithm that combines a Kalman Filter method with effective decay constant correction to improve the accuracy of predicting radiation dose rate distribution during emergencies. The algorithm addresses the limitations of relying solely on measurement data by incorporating calculation data and refining the estimations. The effectiveness of algorithm was assessed using hypothetical test scenarios, which demonstrated a significant improvement in the accuracy of dose rate prediction compared to the model predictions. The estimates generated by the algorithm showed a good agreement with the measured data, and the discrepancies tend to decrease over time. Furthermore, the application of the effective decay constant correction accelerated the reduction of prediction errors. In conclusion, it was confirmed that the combined use of the Kalman filter method and effective decay constant correction is an effective approach to improve the accuracy of dose rate prediction.

Fire Safety Labeling System for Household Goods to Prevent House Fires

  • Data released by the National Fire Agency showed that the total incidence of fires from 2014 to 2023 was 410,497, among which 75,215 were house fires, constituting 18% of the total occurrences. The fatalities resulting from house fires numbered 1,435. Fatalities stemming from prior house fires occurred between midnight and 6 am, coinciding with periods of sleep and diminished responsiveness to fire emergencies. A fire safety labeling system is proposed to prevent fire hazards associated with household products. This system primarily entails indicators of thermal energy and inherent fire attributed to household products. Furthermore, the Korea Fire Safety Institute has suggested adopting a risk label as a concrete measure plan for implementing the fire safety labeling system for household products. The results promise to safeguard the public against fires and increase the exports of household commodities by enhancing the corporate reputation through positive association with fire safety measures.

  • One of the most important purpose of multi-purpose reservoir is storing a large amount of water for utilization in a dry season. However, multi-purpose reservoirs that were constructed according to the limited hydrologic information available at the time of construction may encounter problems such as decreased water inflow due to climate change and an inability to cope with a water shortage. To solve these problems, in 2015, the Ministry of Land, Infrastructure and Transport suggested a revised water supply standard in case of water shortage for reservoirs. However, the revised standard has not been sufficiently discussed to determine its effectiveness. In addition, multi-purpose reservoirs in South Korea have secured and stored water for emergencies, but there is currently no way to utilize the stored water. Determining how to utilize the stored water effectively may be a useful method for preparing drought. Therefore, this article discusses the revised water supply standard as it relates to a water shortage in reservoirs and a method of utilizing the water stored for emergencies in multi-purpose reservoirs. The options for utilizing the water storage were evaluated using a water shortage safety degree index, and the results showed that the options may slightly and limitedly increase the water supply capacity. However, the evaluation also showed that a complex application of two options may overcome the exisiting problem and to supply water more effectively.

Survey on the Educational Needs for Smartphone Based Emergency Situations - Targeting Parents with Elementary School Children (스마트폰 기반 응급상황 대처 교육에 대한 요구도 조사 - 초등학생 자녀를 둔 부모를 대상으로)

  • This study investigates the educational needs of parents of elementary school children, who use smartphones for emergencies. This is a descriptive cross-sectional study, and data of 200 parents of students under the 6th grade was collected from October 17 to 27, 2017. The study results indicate that 67.5% parents visited the emergency department due to a child's emergency accident. Of these, 74% respondents replied that they were unaware of how to deal with the emergency at that instance. Most parents had never received education, and were using smart-phones to acquire information on first aid. Ed. Parents' demand for education on child emergency response scored high, with an average of 3.40 (±0.42) out of 4 points. Among the educational categories, "the management of foreign body airway obstruction and cardiopulmonary resuscitation (CPR)" scored the highest average for educational needs (3.64±0.52). This research provides baseline information for understanding the educational needs of parents for first aid for the child. Our findings indicate that the smartphone is potentially an effective educational tool for parents.

SEDATION PRACTICES IN DENTAL OFFICE : A SURVEY OF MEMBERS OF THE KOREAN ACADEMY OF PEDIATRIC DENTISTRY (진정요법사용에 관한 설문조사)

  • Monitoring of patients is essential in assuring the safety of sedation in the dental office. The purpose of this study was to explore the utilization and preferences of sedation, utilization of monitoring methods, and the preparation for emergencies during sedation. Members of the Korean Academy of Pediatric Dentistry were surveyed(n=245) from 1998. 10. 1 to 1998. 12. 20 to establish current trends in the use of sedation practices and monitoring methods in Korea. Findings of the survey include: 1. Twenty nine percent of pediatric dentists use sedative agents in their practices. Seventy one percent of pediatric dentists do not use any sedative agents in their practices 2. Thirty nine percent of pediatric dentists who use sedative agents do so without monitoring devices 3. Nitrous oxide inhalation sedation was the most frequently utilized sedation method by pediatric dentists and chloral hydrate was the most frequently utilized sedative by pediatric dentists. Pulse oximeter was the most frequently utilized monitoring method during sedation in dental office. 4. Only twenty eight percent of pediatric dentists obtain the informed consent of the child's legal guardian. 5. Only sixty percent of pediatric dentists prepare for emergencies during sedation.

Simulation of Counting Efficiencies of Portable NaI Detector for Rapid Screening of Internal Exposure in Radiation Emergencies (방사선비상시 내부피폭 신속 분류를 위한 휴대용 NaI 검출기의 계측효율 전산모사)

  • In case of radiation emergencies, radioactive materials released into environments can cause internal exposure of members of the public. Even though whole body counters are widely used for direct measurement of internally deposited radionuclides, those are not likely to be used at the field to rapidly screen internal exposure. In this study, we estimated the counting efficiencies of portable NaI detector for different size BOMAB phantoms using Monte Carlo transport code to apply handheld gamma spectrometers for rapid screening of internal exposure following radiological accidents. As a result of comparison for two counting geometries, counting efficiencies for sitting model were about 1.1 times higher than those for standing model. We found, however, that differences of counting efficiencies according to different size are higher than those according to counting geometry. Therefore, we concluded that when we assess internal exposure of small size people compared to the reference male, the body size should be considered to estimate more accurate radioactivity in the human body because counting efficiencies of 4-year old BOMAB phantom were about 2.4~3.1 times higher than those of reference male BOMAB phantom.

  • Emergencies and disasters can happen any time without any warning, and things can change and escalate very quickly, and often it is swift and decisive actions that make all the difference. It is a responsibility of the building facility management to ensure that a proven evacuation plan in place to cover various worst scenario to handled automatically inside the facility. To mapping out optimal safe escape routes is a straightforward undertaking, but does not necessarily guarantee residents the highest level of protection. The emergency evacuation navigation approach is a state-of-the-art that designed to evacuate human livings during an emergencies based on real-time decisions using live sensory data with pre-defined optimum path finding algorithm. The poor decision on causalities and guidance may apparently end the evacuation process and cannot then be remedied. This paper propose a cloud connected emergency evacuation system model to react dynamically to changes in the environment in emergency for safest emergency evacuation using IoT based emergency exit sign system. In the previous researches shows that the performance of optimal routing algorithms for evacuation purposes are more sensitive to the initial distribution of evacuees, the occupancy levels, and the type and level of emergency situations. The heuristic-based evacuees routing algorithms have a problem with the choice of certain parameters which causes evacuation process in real-time. Therefore, this paper proposes an evacuee routing algorithm that optimizes evacuation by making using high computational power of cloud servers. The proposed algorithm is evaluated via a cloud-based simulator with different "simulated casualties" are then re-routed using a Dijkstra's algorithm to obtain new safe emergency evacuation paths against guiding evacuees with a predetermined routing algorithm for them to emergency exits. The performance of proposed approach can be iterated as long as corrective action is still possible and give safe evacuation paths and dynamically configure the emergency exit signs to react for real-time instantaneous safe evacuation guidance.
