Analysis of Quantitative Electroencephalography (QEEG) Following Acupuncture Treatment in Patients with Insomnia: Z Scored Absolute Power and sLORETA (불면증 환자에 대한 침치료 전후 정량화 뇌파 분석: Z Scored Absolute Power and sLORETA)

  • Objectives: To investigate the neurophysiological effect of acupuncture treatment on insomnia patients using quantitative electroencephalography (QEEG) and standardized Low Resolution Brain Electromagnetic Tomography method (sLORETA).Background: Insomnia is one of the commonly encountered symptoms in primary medical care. Recent studies of acupuncture for insomnia reported that the acupuncture groups showed significant improvements compared with the control groups. However, the neurophysiological mechanism of acupuncture in the treatment of insomnia has not been revealed and a few studies have measured the effect of acupuncture treatment using QEEG.Methods: Participants who had some problems in initiating or maintaining sleep, or had non-restorative sleep for more than 3 days a week and ISI scores above 8 and below 21 were treated by acupuncture for 2 weeks (3 times a week, total 6 times). We assessed the effectiveness of acupuncture for insomnia by the PSQI (Pittsburgh Sleep Quality Index) at baseline and at 2 weeks after the end of treatment (4th week). Also, we performed EEG and analysed the EEG data at baseline and at the end of treatment (2nd week) on the linked ears montage using the Neuroguide software program and sLORETA.Results: Thirty-two participants were enrolled and 2 participants dropped out because of personal reasons. Among the 30 participants, EEGs of 12 participants were included in the analysis of QEEG and sLORETA. Total score on the ISI and PSQI was significantly decreased after acupuncture treatment. The number of electrodes exceeding the range of 90% (±1.65) or 95% (±1.96) in the z scored absolute power of beta was significantly decreased after acupuncture treatment. There was no significant change in brain activation between pre- and post-acupuncture using sLORETA.Conclusions: The deviation of absolute power compared to the normative database was significantly decreased after acupuncture treatment in the alpha and beta ranges. Therefore, we suggest that acupuncture treatment for insomnia might be effective through the central nervous system especially in the brain. There are many limitations to drawing any conclusion. Further studies are needed in the future to overcome these limitations.

  • Syncope in children and adolescents have a common occurrence according for up to 15% before adulthood. Micturition syncope, a kind of situational syncope, can be considered a form of reflex syncope. It can typically occur in healthy young men after rising from bed in the early morning who experience sudden loss of consciousness during or immediately after urination. The mechanism of micturition syncope is not completely understood, but it has been suggested that vasovagal reflex mediated bradycardia and peripheral vasodilation and decreased venous return due to Valsalva effect and standing position lead to the decrease in cerebral blood flow resulting in syncope. The causes of syncope are variable. So complete history taking, physical examination, electrocardiography, exercise stress test, echocardiography, head-up tilt table test, electroencephalography(EEG), brain magnetic resonance image and urodynamic study should be required for the diagnosis of micturition syncope. There were several reports about micturition syncope. However, literature of micturition syncope at the pediatric age has rarely been reported in Korea so far. Therefore, we report a case of a 9-year-old boy with micturition syncope with typical EEG findings of high amplitude delta wave and flattening during syncope.

Study on the Variation of Driver's Biosignals According to the Color Temperature of Vehicle Interior Mood Lighting (자동차 실내 무드조명의 색온도에 따른 운전자의 생체신호 변화)

  • The purpose of this work is to suggest the optimal color temperature, which induces a sense of comfort for autonomous vehicle users through the analysis of biosignal using electroencephalography (EEG) and photoplethysmography (PPG). To achieve this purpose, we applied lighting with a color temperature of 3000 K, 4000 K, 5000 K, and 6000 K to the autonomous driving environment. We experimented in a laboratory equipped with a graphic driving simulator. The experimental procedure is as follows: 1) stabilization (5 min). 2) Uchida-Kraepelin test (3 min). 3) Automatic driving + lighting (3 min). This procedure was repeated four times under different color temperatures. We performed frequency analysis on a collected time-series data and calculated the power value for each frequency band through power spectrum analysis. In the case of EEG, we analyzed α- and β-waves, which are indicators of stability and arousal, respectively. In the case of PPG, we analyzed the sympathetic nervous system activity. To reduce deviations between the subjects, we normalized the data before analysis. The result of the first analysis revealed that α-wave increased only at 5000 K, while the β-wave increased at almost all color temperatures. In addition, in the case of PPG, sympathetic nervous system activity (SNSA) increased under driving conditions. The result of the second analysis revealed that the difference between β-wave and SNSA is insignificant. In conclusion, the increase in α-waves showed that EEG was most stable at 5000 K. The results of this study can be applied to the upcoming autonomous driving era to induce high driver satisfaction. Furthermore, this approach could eventually lead to the acceptance of autonomous vehicles by suggesting a positive effect of autonomous driving.

A Study of Intrinsic Alpha Rhythm, Electroencephalography, and Heart Rate Variability Index as Indicators of Cognitive Function and Health in Elderly Adults (노년기 인지기능 및 건강상태를 반영하는 지표로써 Alpha 고유리듬과 뇌파 및 HRV 지표와의 관계 연구)

  • This study was an examination of the relevance and clinical significance of electroencephalographic (EEG) indexes (e.g., mental/physical stress and attention) and indexes of heart rate variability (HRV) with regard to cognitive function and physiological health conditions in elderly people. A device was used to record two-channel EEGs of the frontal lobe and a one-channel ECG simultaneously. Subjects were 76 people average aged 73. The significant findings are as follows: First, subjects whose intrinsic alpha rhythm had high amplitude, regardless of peak, showed higher resistance to mental stress and lower physical stress than did subjects with low-amplitutde intrinsic alpha rhythm. Second, HRV, SDNN, and RMSSD indexes showed strong positive correlations between the two groups of subjects regardless of the division of groups. Third, the alpha asymmetry of the left and right sides of the brain in subjects with low-amplitude intrinsic alpha rhythm was larger, and the delta/alpha ratio (reflecting physical stress) and theta/sensorimotor rhythm (SMR) ratio (showing the decline in attention) were bigger. Fourth, the subjects in whom intrinsic alpha rhythm peak occurred during slow rhythm had a higher theta/SMR ratio than did subjects whose peak occurred during fast rhythm, which was related to a steeper decline in attention. Therefore, the presence or absence of intrinsic alpha rhythm peak and amplitude on quantitative EEG may be an index reflecting the cognitive function and physiological health of elderly people.

The Performance of Ictal Brain SPECT Localizing for Epileptogenic Zone in Neocortical Epilepsy (신피질성 간질에서 발작기 $^{99m}Tc$-HMPAO 뇌혈류 SPECT의 간질병소 국소화 성능)

  • The epileptogenic zones should be localized precisely before surgical resection of these zones in intractable epilepsy. The localization is more difficult in patients with neocortical epilepsy than in patients with temporal lobe epilepsy. This study aimed at evaluation of the usefulness of ictal brain perfusion SPECT for the localization of epileptogenic zones in neocortical epilepsy. We compared the performance of ictal SPECT with MRI referring to ictal scalp electroencephalography(sEEG). Ictal $^{99m}Tc$-HMPAO SPECT were done in twenty-one patients. Ictal EEG were also obtained during video monitoring. MRI were reviewd. According to the ictal sEEG and semiology, 8 patients were frontal lobe epilepsy, 7 patients were lateral temporal lobe epilepsy, 2 patients were parietal lobe epilepsy, and 4 patients were occipital lobe epilepsy. Ictal SPECT showed hyperperfusion in 14 patients(67%) in the zones which were suspected to be epileptogenic according to ictal EEG and semiology. MRI found morphologic abnormalities in 9 patients(43%). Among the 12 patients, in whom no epileptogenic zones were revealed by MRI, ictal SPECT found zones of hyperperfusion concordant with ictal SEEG in 9 patients(75%). However, no zones of hyperperfusion were found in 4 among 9 patients who were found to have cerebromalacia, abnormal calcification and migration anomaly in MRI. We thought that ictal SPECT was useful for localization of epileptogenic zones in neocortical epilepsy and especially in patients with negative findings in MRI.

$^{99m}Tc$-HMPAO Brain SPECT in Seizure Disorder: Comparison Brain SPECT, MRI/CT and EEG (경련성 질환에서 $^{99m}Tc$-HMPAO 뇌혈류 SPECT : SPECT, CT/MRI와 EEG의 비교)

  • We studied 115 patients with seizure who had been performed brain SPECT, brain MRI or CT and EEG. To evaluate the pattern of brain SPECT in seizure patients 28 of them had secondary epilepsies, 87 had primary epilepsies. In primary epilepsies, 42 were generalized seizure and 45 were partial seizure. The causes of secondary epilepsies were congenital malformation, cerebromalacia, cerebral infarction, multiple sclerosis, AV-malformation, granuloma and etc, in order. In 28 secondary epilepsies, 25 of them, brain SPECT lesions was concordant with MRI or CT lesions, 3 were disconcordant. The brain SPECT findings of generalized seizure were normal in 22 patients, diffuse irregular decreased perfusion in 8, decreased in frontal cortex in 4, temporal in 5 and frontotemporal in 3. In 45 partial seizure, 19 brain SPECT were concordant with EEG (42.4%).

  • Recent development of 3D technologies made it possible that observers perceive 3D depth from two dimensional images. Despite this kind of technological development, when observers watch 3D display they experience 3D visual fatigue that they do not usually experience in real life. It is critical to measure visual fatigue in order to overcome the problem of 3D visual fatigue. The purpose of the present study was to develop a protocol to measure 3D visual fatigue based on an EEG signal and to examine its validity. The first experiment explored the possible ERP components that reflected visual fatigue in 2D and 3D conditions. The second experiment examined whether the feature of the component found in the first experiment was affected by the amount of binocular disparity. Both in Cz and Pz channels, the peak amplitude of P3 component was much lower in 3D rather than in 2D conditions, and it decreased as the amount of binocular disparity increased. The subjective 3D visual fatigue also increased with the amount of binocular disparity. These results imply that the peak amplitude of P3 component at Cz and Pz channels can be used as an index of 3D visual fatigue.

Desyncronization And Correlation Coefficient of Brain Wave on Scientific Reasoning (과학적인 사고과정에서 나타난 뇌파의 비동기화와 상관계수)

  • The purpose of this study was to test whether the degrees of desyncronization in cranial nerves were related to correlation coefficient between brain waves measured on scalp. Based on the hypothesis. this research intended to describe the patterns of brain wave during scientific reasoning. EEG (electroencephalography) signals in 19 electrodes were recorded from elementary (6th grade) and middle (7th, 8th grade) school students as they performed roller-ball tasks: planning experiment, cognitive conflict, bridging, metacognition, and at rest (eyes closed and eyes opened). The results showed that the smaller the correlation coefficient was, the bigger the degrees of desyncronization. As compared to resting periods, the correlation coefficients in prefrontal and frontal lobe decreased during scientific reasoning. And the nerves in prefrontal and frontal lobes were most active during scientific reasoning. These results suggest that the correlation coefficient in addition to frequency of brain waves be analysed to get meaningful results of researches on scientific reasoning by brain wave.

A Preliminary Evaluation of NeuroGuide and IVA + Plus as Diagnostic Tools for Attention-Deficit Hyperactivity Disorder (주의력결핍 과잉행동장애의 진단 보조도구로 뉴로가이드와 IVA + Plus의 유용성에 관한 예비연구)

  • Objectives : This study was designed to investigate the usability of IVA + Plus (Continuous Performance Test) and Neuro-Guide [Quantitative electroencephalography (EEG) normative database] as an auxiliary diagnostic tools for attention-deficit hyperactivity disorder (ADHD). Methods : The scores of IVA + Plus and resting EEG were obtained from 34 elementary school-aged children. Also, the Korean ADHD Rating Scale (K-ARS) and the Diagnostic Interview Schedule for Children version IV (DISC-IV) was done for the parent of them. From the result of the DISC-IV, we divided them into three groups, ADHD Not Otherwise Specified (NOS), and Normal Control (NC). Using NeuroGuide, the z-scores of relative power for delta (1-4 Hz), theta (4-8 Hz), alpha (8-12 Hz), and beta (12-25 Hz) were calculated. Then the correlation and variance analysis were done to investigate the differences between three groups. Results : The scores of IVA + Plus were negatively correlated with the K-ARS. IVA + Plus have successfully discriminated the ADHD from NC and NOS. The z-scores of relative power of delta and theta were positively correlated with the K-ARS. The z-scores of relative power of alpha and beta were negatively correlated with the K-ARS. Conclusions : The IVA + Plus and NeuroGuide QEEG test are expected to be used as the valuable tools for diagnosing ADHD accurately.

Epilepsy in children with a history of febrile seizures

  • Purpose: Febrile seizure, the most common type of pediatric convulsive disorder, is a benign seizure syndrome distinct from epilepsy. However, as epilepsy is also common during childhood, we aimed to identify the prognostic factors that can predict epilepsy in children with febrile seizures. Methods: The study comprised 249 children at the Korea University Ansan Hospital who presented with febrile seizures. The relationship between the subsequent occurrence of epilepsy and clinical factors including seizure and fever-related variables were analyzed by multivariate analysis. Results: Twenty-five patients (10.0%) had additional afebrile seizures later and were diagnosed with epilepsy. The subsequent occurrence of epilepsy in patients with a history of febrile seizures was associated with a seizure frequency of more than 10 times during the first 2 years after seizure onset (P<0.001). Factors that were associated with subsequent occurrence of epilepsy were developmental delay (P<0.001), preterm birth (P =0.001), multiple seizures during a febrile seizure attack (P =0.005), and epileptiform discharges on electroencephalography (EEG) (P =0.008). Other factors such as the age at onset of first seizure, seizure duration, and family history of epilepsy were not associated with subsequent occurrence of epilepsy in this study. Conclusion: Febrile seizures are common and mostly benign. However, careful observation is needed, particularly for prediction of subsequent epileptic episodes in patients with frequent febrile seizures with known risk factors, such as developmental delay, history of preterm birth, several attacks during a febrile episode, and epileptiform discharges on EEG.
