Colorectal Cancer Screening with Computed Tomography Colonography: Single Region Experience in Kazakhstan

  • Background/Aims: The aim of our study was to determine the efficacy of computed tomography colonography (CTC) in screening for colorectal cancer (CRC). Methods: A total of 612 females and 588 males aged 45 to 75 years were enrolled in CTC screening. CTC was performed following standard bowel preparation and colonic insufflation with carbon dioxide. The main outcomes were the detection rate of CRC and advanced adenoma (AA), prevalence of colorectal lesions in relation to socio-demographic and health factors, and overall diagnostic performance of CTC. Results: Overall, 56.5% of the 1,200 invited subjects underwent CTC screening. The sensitivity for CRC and AA was 0.89 and 0.97, respectively, while the specificity was 0.71 and 0.99, respectively. The prevalence of CRC and AA was 3.0% (18/593) and 7.1% (42/593), respectively, with the highest CRC prevalence in the 66-75 age group (≥12 times; odds ratio [OR], 12.11; 95% confidence interval [CI], 4.45-32.92). CRC and AA prevalence were inversely correlated with Asian descent, physical activity, and negative fecal immunochemical test results (OR=0.43; 95% CI, 0.22-0.83; OR=0.16; 95% CI, 0.04-0.68; OR=0.5; 95% CI, 0.07-3.85, respectively). Conclusions: Our study revealed high accuracy of CTC in diagnosing colonic neoplasms, good compliance with CTC screening, and high detection rate of CRC.

A comparative analysis of the efficacy of tiny child cardiopulmonary resuscitation (CPR) by chest compression methods (가슴압박 방법에 따른 소아 심폐소생술의 효율성 비교)

  • Purpose: This study aimed to compare the effectiveness of one-hand compression method (one-hand) and two-thumb compression methods (two-thumb) using a 3-year-old pediatric mannequin. Methods: Participants in the experiment were randomly assigned to 21 people with one hand and 21 people with two thumbs, who had completed a basic life support training course. The mannequin used a few junior QCPR manikis (Laerdal, Norway), based on an age of 3. The study followed the guidelines for two-rescuer pediatric CPR, performing chest compressions and ventilations in a 15:2 ratio for 5 cycles, 10 minutes. The chest compression number, depth, speed rate, and recoil rate were measured after 1, 3, and 5 cycles, respectively. Data were analyzed using SPSS 23.0. Results: Comparing the efficiency of chest compressions performed during 1, 3, and 5 cycles, the depth of chest compressions was 40.84±3.10 mm for the two thumbs and 51.48±4.79 mm for the one-hand, which was significantly different (p<.001). According to pediatric CPR guidelines, the frequency corresponding to the compression depth range of 40.00-50.00 mm was statistically higher at 57.1% (12 people) of two thumb than at 23.8% (5 people) of one hand (p<.001). Conclusion: While the current guidelines recommend one- or two-thumb encircling hand compressions for larger children, the use of one- or two-handed wrapped thumb compressions should be considered for smaller children, depending on the rescuer's judgment, when distinguishing between a child and an infant is difficult.

Three-dimensional analysis of decompression efficacy and influencing factors in the maxillofacial cystic lesions: a retrospective study

  • Objectives: To evaluate the effectiveness of decompression and various parameters that may affect volume change in cystic lesions. Patients and Methods: This retrospective study included patients who visited the Department of Oral and Maxillofacial Surgery at Ewha Womans University Medical Center between 2012 and 2022 for decompression of cystic lesions of the jaw. To measure volume changes, pre- and post-decompression cone-beam computed tomography was performed and reconstructed in three dimensions using Mimics 25.0 software (Materialise NV). A comparative analysis was performed based on sex, age, initial cyst volume, location, degree of cortical layer expansion, and pathologic diagnosis using the Mann-Whitney U and Kruskal-Wallis tests. Results: In all 20 cases, the duration of decompression was 7.84±3.35 months, and all patients successfully completed the decompression period without any complications. Significant differences were observed in the reduction rate and shrinkage speed based on the degree of cortical layer expansion. However, only the shrinkage speed (not the reduction rate) showed a significant difference with respect to the initial cyst volume. Significant differences were not observed based on sex, age, location, or pathologic diagnosis. Conclusion: Although the present study involved a small number of cases, the effectiveness of decompression was confirmed. In particular, 3D analysis overcame the shortcomings of previous studies of decompression and allowed earlier resection. Further studies with more patients are required to provide a rationale for these results and identify factors that influence decompression.

Seedborne Fungi and Fungicide Seed Treatment of Ginseng

  • The incidence of fungi and their possible contribution to low vigour were examined in a collection of ginseng (Panax quiquefolius) seed from Ontario. When examined after one winter of stratification in the field and storage at 4f for five months in the laboratory, the collection exhibited low vigour (plant stand 16.7% of seeding rate six weeks after seeding) and high incidence (94%) of discolored or soft seed. Fungi isolated (and incidence) from 1,304 endosperm halves recovered from surface-sterilized seed were, in order of abundance, Fusarium rostrum (22.2%), Chaetomium crispuum (14.3%), Funriud oxysporum (9.0%), Fusarium sdani (9.0%), iwmor sp. (8.4%), Alternaria sp. (8.1%), Zowieua lucotricha (7.8%), Cylindruarpn sp. (0.9%), Fusarium avenacmm (0.9%), and Vdudla iliata (0.4%). Most of these fungi, including known and potential pathogens of ginseng (species of Alerraria, Cylindrocarpon, Fusarium, and Trichodirma), were associated with both healthy and diseased seed. Application of Benlate (benomyl), Thiram (thiram), or UBI-2584 (tebuconazole) to seed caused slight to pronounced reduction in emergence and did not significantly affect plant stand six weeks after seeding. The study demonstrated the high level of infection by fungi, including known and potential pathogens of the cry, in an arbitrary collection of ginseng seed from commercial sources, and the phytotoxicity of the fungicides tested when applied to moist stratified seed. The lack of efficacy of the fungicides precluded determination of the contribution of seedborne fungi to low vigour of the seed.

The Dffects of Retinoids on CRABPII cRNA Induction amd Collagen Synthesis on Human Dermal Fibroblast

  • Retinoids are essential regulators of spithelial cell growth and celluar differentiation. They are also known to be effective in photoaging. It was reported that topical application of retinoic acid improves facial wrinkle carsed by collagen synthesis reduction in photodamaged skin. Collagen synthesis by retinoic acid may contribute to the wrinkle effacement. Since celluar retinoic acid binding protein II is slsctively induced in human skin and dermal fibroblasts in vitro by retinoic acid, this response can be used to mesure retinoids potency and activity. In order to know the activity of retinoids and their relations with collagen synthesis, we treated dermal fibroblasts with retinoids for 48 hours at 10-6-10-7M and measured CRABPII mRNA level by quantitative Nortern blotting. We also measured the rate of collagen systhesis by retinoids using 3-dimensional dermal equivalent. CRABPII mRNA level was increased 3-fold by retinoic acid, 2.1-fold by retinol and 1.4-fold by retinaldehyde. Collagen systhesis was increased 34% by all-trans retinioc acid, 26% by retinol, 17% by retinaldehyde and 7% by retinyl palmitate. From the above results, retinoids were found to be a potent indecers of CRABPII mRNA and collagen synthesis. Though retinoic acid was the most effective, its use has been restricted because of the side effects. Instead, retinol can be a best candidate in cosmetics for the treatment of photodamaged skin in terms of efficacy and safety.

An Analysis on the Damage Compensation of Hanwoo Farmers as a Result of the Korea-U. S. Free Trade Agreement (한.미 FTA 체결에 따른 한우농가 피해보전효과 분석)

  • To help improve the current government practice of direct damage-compensation policies, resulting from the loss of profit, sustained by Hanwoo farmers, as a result of the recent Korea-U. S. Free Trade Agreement (FTA), this research aims to examine any problems or issues caused by said policies. To accomplish this task, we have established Hanwoo-SIMO model and estimated the damage of Hanwoo farmers, one without the implementation of the FTA and another with the FTA, to compare and contrast the two. We then analyzed the efficacy of the current government policies. According to our analysis, the current direct compensation policies for the loss of profit on the part of Hanwoo farmers are insufficient. To address this problem, we recommend the government enact a new direct damagecompensation law to address the following issues. First, as the base formula of damage-compensation, the government should use current price of the beef rather than the annually changing flexible price. Second, the flexible control index should remain fixed at 1.0 rate while the government prepares the adequate amount of the damage compensating direct payment resulting from the FTA. Third, the direct government compensation policy should extend beyond the current 15 years (2013-2026) as the profit loss is expected to increase after the midpoint of the FTA.

The Effects of Korean Medical Treatment Combined with Embedding Acupuncture on Patients with Chronic Lower Back Pain: a Retrospective Study (만성 요통에 대한 매선요법을 병행한 한방치료의 효과에 대한 후향적 연구)

  • Objectives : The purpose of this study was to investigate the clinical effects of Korean medical treatments combined with Embedding acupuncture on patients with chronic lower back pain. Methods : We reviewed the medical records of 40 patients with chronic lowerback pain hospitalized at Dongshin Korean Medicine Hospital from March, 2015 to February, 2016. They were divided into two groups: the embedding acupuncture group(20 patients) and the non-embedding acupuncture group(20 patients). To evaluate the efficacy of the treatments, the 40 patients were asked to complete a Numerical Rating scale (NRS) and the Oswestry Disability Index (ODI) four times during admission. Results : The mean NRS of the embedding acupuncture group decreased more significantly than the non-embedding acupuncture group at days three and ten of admission. The ODI change and ODI rate of change of the embedding acupuncture group were significantly greater than the non-embedding acupuncture at days three and ten of admission. Conclusion : Korean medical treatment combined with embedding acupuncture might be effective in reducing pain and improving the life quality of patients with chronic lower back pain. We hope that further studies will be done to produce more clinical data and ensure effective application of these results.

Phase II Study on Breast Conservative Surgery Plus Chemo- and Radiotherapy in Treating Chinese Patients with Early Staged Breast Cancer

  • Purpose: To evaluate the efficacy of conservative surgery plus chemo-, radio-therapy in treating patients with early stage breast cancer. Patients and Methods: Eligible patients were treated by postoperative chemotherapy as well as whole-breast irradiation with tumor bed boost. Postoperative radiotherapy consisted of 6 MV whole breast linear accelerator irradiation with two tangential half fields to a total dose of 45~50 Gy, followed by $10{\sim}15MeV{\beta}$ boost irradiation to tumor bed for 10~20Gy, total dose 56~66Gy. Results: Fifty-two patients were enrolled. Overall 1-, 2- and 3 year survival rates were 98.1%, 92.3%, and 90.4%, respectively, with a local recurrence rate of 5.77%. Cosmetic results were evaluated as good by doctors in 90.4% of patients. Conclusions: Breast conservative surgery combined with chemo- radio-therapy could be a treatment option for Chinese patients with early stage breast cancer.

The Effect of Exercise on Cardiovascular and Musculoskeletal Variables and Quality of Life in Elderly Women (여성노인에서 운동프로그램이 심혈관계와 근골격계 그리고 삶의 질에 미치는 효과)

  • Objectives: The aim of this study was to evaluate the effects of exercise on the cardiovascular and musculoskeletal variables and quality of life in elderly women. Methods: The study subjects were 22 in experimental group and 21 in the control group, all of who were aged 65 years and over. The subjects in the experimental group were under an exercise program 3 times a week for one year. To assess the effects of exercise in the study group, blood pressure, heart rate, lipid profile, bony metabolism, grip strength, flexibility, reaction time, and quality of life were measured. Results: Exercise showed the significant differences between experimental and control groups on the variables of the diastolic blood pressure, HDL-cholesterol, urinary deoxypyridinoline, flexibility, and quality of life. Compared to before the exercise in the study group, the urinary DPD level and quality of life were significantly improved at 3 months, and diastolic blood pressure, serum HDL-cholesterol, and flexibility were significantly improved by 1 year. Conclusions: The study results underscore the efficacy of exercise and the need for regular exercise programs that are designed for older persons.

Bordetella bronchiseptica is a potent and safe adjuvant that enhances the antigen-presenting capability of dendritic cells

  • We previously demonstrated that Bordetella bronchiseptica (B. bronchiseptica) antigen (Ag) enhances the Mycoplasma hyopneumoniae Ag-specific immune response. The focus of this study was whether acellular bacterin of B. bronchiseptica could be used as an adjuvant to increase antigen-presenting capability of dendritic cells (DCs) by increasing the level of activation. The metabolic activity of DCs was increased by B. bronchiseptica, similar to lipopolysaccharide (LPS). Flow cytometry analysis revealed that B. bronchiseptica increases the expression of major histocompatibility complex class-2, cluster of differentiation (CD)40, CD54, and CD86 which are closely related to DC-mediated immune responses. B. bronchiseptica enhanced the production of cytokines related to adaptive immune responses. Furthermore, the survival rate of B. bronchiseptica-injected groups was 100% at 15 and 20 mg/kg doses, whereas that of LPS-injected groups was only 20%, 0% at 15 and 20 mg/kg doses respectively, and so B. bronchiseptica is likely to be safer than LPS. Taken together, these results indicate that B. bronchiseptica can be used as an adjuvant to enhance the antigen-presenting capability of DCs. B. bronchiseptica is a candidate for producing vaccines, especially in case of DC-mediating efficacy and safety demands. This study provides researchers and clinicians with valuable information regarding the usage of B. bronchiseptica as a safe bacteria-derived immunostimulating agent for developing efficient vaccines.
