Retrospective Statistical Analysis on Patients Admitted to a Korean Medicine Hospital by Traffic Accident (교통사고로 한방병원에 입원한 환자에 대한 후향적 통계 분석)

  • Objectives: The purpose of this study was to investigate characteristics of patients who were admitted to an oriental medicine hospital by traffic accident. Methods: The medical charts of 346 patients admitted to an oriental medicine hospital from June 1, 2017 to May 31, 2018 were analyzed. The Numbering Rating Scale (NRS) and duration of hospitalization were used to evaluate characteristics of the patients. Results: Acupuncture, Moxibustion, Infralux were used to treat all the patients. The most frequently used herbal medication was Danggwisu-san(22.25%). 87 patients(25.14%) visited the outpatient department after being discharged from the hospital. The most frequent complaint in terms of pain was cervical pain(82.7%) and of systemic symptom was headache(23.7%). Men and younger aged patients showed higher therapeutic effect than women and older ages. The most common duration of hospitalization was 2~4 days(42.73%) and positively correlated with therapeutic effect. The most frequent interval between time of injury and visit to the hospital was from 0-1 days(68.90%) and showed no relationship with therapeutic effect. The most frequent admission pathway was "Directly to the hospital"(57.51%). Admission pathway was proportionally associated with duration of hospitalization and treatment results were not. The most common vehicle type involved in the traffic accidents was a sedan(72.25%), accident type was a rear-end collision(43.64%) and showed no relationship with therapeutic effect. Conclusions: In this study, therapeutic effects were highly correlated among men, younger ages, and duration of hospital stay, and was not for interval days, admission pathway, vehicle type, and accident type.

  • Objectives: Heavy menstrual bleeding has a significant effect on women's daily life and health consequences for theirs. Attention to women's and girls' menstrual health is critical for their health. This study presents the results of a novel research on the effect of chamomile capsule on the amount of bleeding, and the duration and interval of menstrual cycles. Methods: The current study was a randomized, double-blind, clinical trial, which was carried out on 118 female students living in dorms of Guilan University of Medical Sciences. The participants were divided into two groups of 59, each receiving either chamomile capsule 250 mg or placebo three times a day, for an interval beginning from 7 days before the starting menstruation till the next onset. Higham chart is the pictorial blood loss assessment to determine the amount and duration of menstrual bleeding. Data were analyzed using SPSS version 20. Alpha was set at 0.05 for all analyses. Results: The average amount of bleeding in the chamomile group decreased after taking the capsule (p = 0.001). However, statistical tests did not show any significant difference in terms of duration and interval between two bleeding in both the experimental and control groups (p > 0.05). Conclusion: The present study showed that the chamomile capsule decreases the amount of menstrual bleeding and can be used as a therapeutic method. A Survey on Clinical Outcomes in Climacteric womens had improved by Received only Daejo-hwan at 3 months after the End of Treatment (대조환(大造丸) 투약만으로 호전된 갱년기 환자에서 치료 3개월 후 임상경과에 대란 조사연구)

  • Purpose : To assess the duration of the therapeutic effect and recurrence symptoms after stopping Herb Medication therapy for climacteric womens. Methods : 28 climacteric womens received only Daejo-hwan(DJH): 2pills(4g) a time, three times a day for a total 8 weeks as maintenance therapy and were followed up for 3 months. Grading of climacteric symptoms were measured by Kupperman's index(K.I.), Greene scale(G.S.) and MRS. Results : The results were as follows. 1. After DJH-treatment, the climacteric symptoms were improved and each mean score of K.I., G.S., and MRS reduced significantly. 2. At 3 months after the end of treatment(EOT), each mean score of K.1., G.S., and MRS reduced significantly as compared with Baseline, and increased significantly as compared with EOT. 3. At 3 months after EOT, each climacteric symptoms kept up the favorable turn and the mean score of MRS reduced significantly as compared with Baseline, but made no significant difference as compared with EOT. Conclusion : In this study, we can consider the DJH-treatment therapeutic effect was kept up for 3 months after EOT and recurrence symptoms did not appeared after EOT to climacteric womens.

  • Women were used as their own controls in the comparison of presterilization and poststerilization menstrual patterns. Five parameters were studied: regularity of cycle length, duration and amount of flow and incidence of dysmenorrhea and intermenstrual bleeding. Three parameters in the electrocoagulation group (regularity of cycle length and duration and amount of flow) and one parameter in the tubal ring group (duration of flow) showed significant changes after sterilization. However, by controlling for the effect of previous contraceptive methods used, no significant menstrual pattern changes following sterilization were discerned in either technique group.

Therapeutic Effect of Weizhong(BL40) Venepuncture on Low Back Pain (위중혈(委中穴) 자락(刺絡)의 요통(腰痛)에 대(對)한 치료효과(治療效果))

  • Objective : Since ancient times venepuncture has been widely used as a useful treatment for low back pain. This study is made in order to objectively evaluate the therapeutic effect of Weizhong(BL40) on low back pain. Methods : Total 46 cases of low back pain patients were divided into 2 groups; one group took only acupuncture therapy, and the other group took acupuncture and venepuncture therapy together. Each patient's pain was evaluated before the treatment and the 3 or 4 days later after the treatment. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. Results : Both acupuncture therapy group and acupuncture & venepuncture group showed good effect on low back pain. Though the latter group showed better pain release effect than the former, there was no statistical significance between pain rating score of two groups. Sciatica group showed no significant difference from non-sciatica group in pain rating score. Palpation on Weizhong(BL40) group showed significant difference only in pain duration from non-palpation group. Exposed vessel around Weizhong(BL40) group showed significant difference in pain rating score from non-exposed vessel group. Conclusion : Weizhong(BL40) venepuncture can be highly recommended as a useful therapy for low back pain patients especially who show exposed vessels on the popliteal area.

Effects of therapeutic horse-riding program on the walking ability of students with intellectual disabilities

  • The purpose of this study was to determine if an 8-week therapeutic riding (TR) program was effective in improving the walking ability of students with intellectual disabilities. Thirteen students diagnosed with intellectual disabilities participated in the TR program. TR sessions were conducted twice a week (30 min per session), with a total of 16 rides taking place over an 8-week period. A gait measurement analyzer was used to measure progress based on a turn test (6-m walking and turning test), walk test (10-m walking), and timed up and go (TUG) test. Measurements were made three times: before horse-riding (P0), after 4 weeks (8 rides) of horse-riding (P1), and after 8 weeks (16 rides) of horse-riding (P2). Data analysis was conducted using SPSS software (ver. 22.0). Descriptive statistics were generated on the general characteristics of the subjects, and the Kolmogorov-Smirnov test was used to verify the normality of the data. Because of the lack of normality, the data were analyzed using a nonparametric method and the significance level was set to 0.05. Measurements of the duration of the forward gait cycle (s) in the turn test and the forward gait speed (m/s) in the walk test indicated improved walking ability after the TR program (p < 0.001); the stride length (% height) also increased significantly (p < 0.05). The walk test revealed a significant effect of the program on the duration of the forward gait cycle (p < 0.05), while there were significant improvements on the left and right of the elaborated strides (p < 0.001). No significant improvement in TUG test performance was observed after the TR program. In this study, an 8-week TR program had positive results on gait. Therefore, further research is merited, where TR programs are likely to improve the walking ability of individuals with intellectual disabilities.

  • Transcutaneous electrical nerve stimulation (TENS) is one of the representative physiotherapical modalities used for the treatment of various musculoskeletal disorders by the application of electrical stimuli. In dental practice, it has long been used in the treatment of acute and chronic orofacial pain conditions including temporomandibular disorders. TENS is the delivery of therapeutic electrical stimuli with a variety of electrical intensity, frequency and duration to stimulate peripheral nerve through surface electrodes with various form and placement. While controversy still remains over the clinical effectiveness and application of TENS, basic understanding of its electrical properties and the expected biological reactions is important to increase the therapeutic effect and decrease the risk of possible side effects. This review, therefore, focuses on basic understanding of TENS including its underlying mechanisms and stimulation parameters.

  • Objective : This study is designed in order to evaluate the therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) on acute low back pain Methods : We investigated 10 patients suffering from acute low back pain were treated from Mach. 1st 2005 to Jun. 30th 2005. 10 patients, were diagnosed only straightened curvature on lateral view of lumbar spine X-ray and get Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. Results : Both Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture therapy showed good effect on acute low back pain. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. In the evaluation of treatment effect, Exellent was 2(20%), Good was 5(50%), Moderate was 3(30%), Poor was 0(0%). Conclusion : The therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture on acute low back pain can be recommended as a useful therapy to treat acute low back pain.

Histopathologic Change of External Abdominal Irradiation on Urinary Bladder of Mice at Total Dose and Intervals (백색 마우스의 전복부 조사량 및 회복기간에 따른 방광의 병리조직학적 고찰)

  • The object of this study was to determine the radiation effect on the urinary bladder and to establish the basic data for optimal fraction schedule on the whole abdominal irradiation of the mice. Although radiation damage of the urinary bladder is one of the dose-limiting factor for treatment of lower abdominal cancer, such as uterine cervical or rectal cancer, systematic histopathological study of total dose and recovery duration is very rare, especially in conventional fractionation regimen of clinical use. Authors used 198 mice and analyzed histopathological findings according to total dose(40 & 50GY) and recovery duration(1-15 weeks after completion of irradiation). The results were summarized as follows : 1. No definite difference of radiosensitivity was noted between male and female group. 2. Most of mucosal injuries were recovered within 14 weeks in 40 GY irradiated group. 3. Vascular injury and change of connective tissue were prominent and persisted even mild degree until 15 weeks after completion of irradiation in 50 GY irradiated group. 4. Although follow up duration of this study(105 days) was not enough to compare life span of mice, this study emphasized that precious schedule for treatment planning was necessary for preventing or reducing of later complication.

Relationship between paravertebral muscle twitching and long-term effects of radiofrequency medial branch neurotomy

  • Background: To achieve a prolonged therapeutic effect in patients with lumbar facet joint syndrome, radiofrequency medial branch neurotomy (RF-MB) is commonly performed. The purpose of this study was to evaluate the prognostic value of paravertebral muscle twitching when performing RF-MB in patients with lumbar facet joint syndrome. Methods: We collected and analyzed data from 68 patients with confirmed facet joint syndrome. Sensory stimulation was performed at 50 Hz with a 0.5 V cut-off value. Patients were divided into 3 groups according to the twitching of the paravertebral muscle during 2 Hz motor stimulation: 'Complete', when twitching was observed at all needles; 'Partial', when twitching was present at 1 or 2 needles; and 'None', when no twitching was observed. The relationship between the long-term effects of RF-MB and paravertebral muscle twitching was analyzed. Results: The mean effect duration of RF-MB was 4.6, 5.8, and 7.0 months in the None, Partial, and Complete groups, respectively (P = 0.47). Although the mean effect duration of RF-MB did not increase significantly in proportion to the paravertebral muscle twitching, the Complete group had prolonged effect duration (> 6 months) than the None group in subgroup analysis. (P = 0.03). Conclusions: Paravertebral muscle twitching while performing lumbar RF-MB may be a reliable predictor of long-term efficacy when sensory provocation under 0.5 V is achieved. However, further investigation may be necessary for clarifying its clinical significance.
