급성기(急性期) 요통(腰痛)의 치료(治療)에 자락요법(刺絡療法)의 병행(竝行)이 미치는 영향(影響) (The Effect of the Treatment with Venesection on Treating Low Back Pain of Acute Stage)

  • Objective : This study is designed in order to evaluate the therapeutic effect of venesection on acute low back pain. Methods : From Nov. 1st 2003 to Apr. 30th 2004, 40 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took only acupuncture therapy, and the other group(test group) took acupuncture and venesection therapy. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. PRS and VAS was checked twice, before the treatment and after the treatment. Treatment was done twice a day. Results : Both acupuncture therapy and acupuncture with venesection therapy showed good effect on low back pain of acute stage. And the latter group showed better effect on decreasing pain than former. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. But it was not proved statistically. Conclusion : Only acupuncture and acupuncture with venesection can be recommended as a useful therapy to treat low back pain of acute stage.

기분장애에서 risperidone의 양면성 (Risperidone as a Janus in Mood Disorder)

  • To examine the double-faced thymoleptic(antidepressant and antimanic) effects of risperidone in mood disorders, this article reviews the psychotropic-induced mania, thymoleptic effects of antipsychotics, therapeutic effects of risperidone and risperidone(RIS)-induced mania(RIM) in mood disorders, risk factors of RIM, possible neurochemical mechanism of these thymoleptic effects, pathophysiological and clinical significance of thymoleptic effects, and suggestive clinical guideline of RIS in mood disorders. RIS appeared effective for bipolar disorder at a lower dose than that recommended for schizophrenia, especially in the cases of maintenance of mood stabilizers, and gradual titration from low doses. Manic induction/exacerbation can occur by chance during RIS treatment in mood disorders, schizoaffective disorders, and schizophrenias. The possible risk factors for RIM are refractory mood disorder, especially in bipolar I disorder with poor initial response ; refractory schizoaffective disorders, especially in bipolar type with poor initial response ; refractory chronic schizophrenias, especially with initial responses ; psychotic features ; higher initial doses ; rapid titration ; combined therapy with antidepressants in refractory depression ; and RIS monotherapy in mania/hypomania. RIS is a drug that preferentially block 5-HT2 receptors. The effects of low dose are due mainly to the blockade of 5-HT2 receptors. There are more gradual increase in D2 blockade with increasing dose and this D2 blocking properties become apparent at higher doses. This may be related to a modulation of dopaminergic transmission by 5-HT2 antagonism at lower doses with the direct action of RIS on DA receptors coming into play at higher dose. The serotonergic antagonistic effect may be important for its effects on depressive symptoms. This, together with adequate blo-ckade of D2 receptors, may not necessarily lead to destabilization of mood disorder, but rather to more therapeutic effects. Therefore, this dose-receptor affinity relationship with both antidepressant and antimanic effects according to treatment duration can explain a continuum of antidepressant effect, antimanic effect, behavioral stimulation, and manic/hypomanic induction/exacerbation. It was the recognition of a useful psychiatric side effects by a thoughtful observer with fertile minds that led to their ultimate utilization as psychotropic drugs, i.e., phenothiazine, MAOI, TCA, and lithium. And, in vivo pharmacological challenge by novel psychotropics, as a neurochemical probe, with more specific actions is a useful tool to select pharmacologically homogeneous subgroup of the same phenotypical(clinical) condition, to further study the unknown underlying pathogenesis of various mental illnesses. Finally, RIS may be a useful alternative or adjunctive drug for patients with mood disorders without psychotic features or refractory to treatment with standard antipsychotic drugs. The more conservative doses(tirated slowly from 1-3 mg/d) of RIS, and maintenance of mood stabilizer in the cases with risk factors of RIM are recommended in mood disorder.

방사선 보호제(Diethone)의 랫드 피부반응에 대한 수식작용 (Skin Radioprotector (Diethone) Modifying Dermal Response of Radiation on Rats)

  • Diethone의 피부에 대한 방사선 보호작용을 평가하기 위하여 Wistar계 랫드에 단일 X-선(20, 30, 40 Gy) 조사 후 시간경과와 선량변화에 따른 급성 피부반응을 연구하였다. 40Gy 선량에서 Diethone군의 피부반응기간은 24.7일로 대조군(29.8일)과 바셀린(29.2일) 투여군에 비하여 약$17\%$의 피부반응 기간이 뚜렷이 감소되었다. 피부반응이 최고도에 달한 10일 동안의 평균치는 Diethone군(2.43)이 대조군(2.91)과 바셀린(2.81)군에 비하여 현저히 감소되었다. 피부반응 Score 2.5에서 구한 등가효과 선량의 반응감소 계수(DRF)는 1.41로 피부보호효과가 있었다. 본 실험 결과로 보아 방사선 피부보호제를 국소적으로 도포함으로써 일반적인 치료선량보다 많은 방사선량을 거대하고 방사선내성이 있는 종양에 조사함으로로써 치료효과를 향상시킬 수 있을 것으로 기대한다.

Therapeutic Effect of Low-Energy Nitrogen Plasma Pulses on Tinea Pedis

  • Superficial fungal infections with dermatophytes, nondermatophyte molds, or yeasts are treated primarily with topical and/or systemic antifungal agents. Additional or alternative treatment modalities, particularly energy-delivering modalities, however, are used widely to induce fungicidal effects via selective photothermal reactions. In addition to light- or laser-based devices, plasma therapy also has antifungal properties. This report describes a Korean male patient with mycologically confirmed tinea pedis that was treated effectively with two sessions of nitrogen plasma treatment at one-week intervals using a plasma delivering system. Nitrogen plasma was prepared by loading a 0.28-ml inert nitrogen gas/pulse that was activated by a microwave generator. The other treatment settings were a nozzle diameter of 5 mm, pulse energy of 0.75 J, pulse duration of 7 msec, and two passes. One week after the first session of nitrogen plasma treatment, the patient exhibited marked reductions in scale and inflammation. One month after the final treatment, no clinical features of recurrence were found, and successive potassium hydroxide testing revealed negative results.

Cordycepin protects against β-amyloid and ibotenic acid-induced hippocampal CA1 pyramidal neuronal hyperactivity

  • Cordycepin exerts neuroprotective effects against excitotoxic neuronal death. However, its direct electrophysiological evidence in Alzheimer's disease (AD) remains unclear. This study aimed to explore the electrophysiological mechanisms underlying the protective effect of cordycepin against the excitotoxic neuronal insult in AD using whole-cell patch clamp techniques. ${\beta}$-Amyloid ($A{\beta}$) and ibotenic acid (IBO)-induced injury model in cultured hippocampal neurons was used for the purpose. The results revealed that cordycepin significantly delayed $A{\beta}$ + IBO-induced excessive neuronal membrane depolarization. It increased the onset time/latency, extended the duration, and reduced the slope in both slow and rapid depolarization. Additionally, cordycepin reversed the neuronal hyperactivity in $A{\beta}$ + IBO-induced evoked action potential (AP) firing, including increase in repetitive firing frequency, shortening of evoked AP latency, decrease in the amplitude of fast afterhyperpolarization, and increase in membrane depolarization. Further, the suppressive effect of cordycepin against $A{\beta}$ + IBO-induced excessive neuronal membrane depolarization and neuronal hyperactivity was blocked by DPCPX (8-cyclopentyl-1,3-dipropylxanthine, an adenosine $A_1$ receptor-specific blocker). Collectively, these results revealed the suppressive effect of cordycepin against the $A{\beta}$ + IBO-induced excitotoxic neuronal insult by attenuating excessive neuronal activity and membrane depolarization, and the mechanism through the activation of $A_1R$ is strongly recommended, thus highlighting the therapeutic potential of cordycepin in AD.

단기간의 요부안정화 운동이 만성요통환자의 근력과 자세균형에 미치는 영향 (The Effect of Short-term Lumbar Stabilization Exercise for Lumbar Muscle Strength and Postural Balance on Chronic LBP)

  • PURPOSE: The purpose of this study was to investigate the effect of short-term lumbar stabilization exercise to patients suffering from chronic low back pain(CLBP) on the strength of their lumbar flexor and extensor and the postural balance of them and to suggest effective treatment duration for them. METHODS: Study subjects were 20 patients with CLBP who's been reported the pain for more than 12 weeks. For these subjects, muscle strength of both extensor and flexor of lumbar and the balancing ability were measured before and after the intervention. Lumbar stabilization exercise was composed of matt, ball and sling exercise. The intervention was applied for total 12 sessions for 4 weeks (3 times a week) and each session was for 40 minutes. RESULT: For muscle strength of both extensor and flexor of lumbar, there was significant differences after the application of lumbar stabilization exercise(p<.05). For balancing ability, stabilization shows significant differences in all of the positions except PC(eyes closed on pillows)(p<.05). Distribution of weight shows no significant differences but it was generally distributed within normal range after the exercise and Fall index shows significant differences between before and after the exercise(p<.05). CONCLUSION: These results could have positive effect on deciding different periods of therapeutic intervention.

레크리에이션 치료가 재미 한인노인의 신체적, 정신적 건강에 미치는 영향 (The Effect of Recreation Therapy in Physical and Mental Health for Korean American Senior)

  • 본 연구는 미국에 거주하고 있는 한인 노인을 대상으로 현재 뉴욕 시에서 시행하고 있는 데이케어 프로그램(Adult Day Care Program)의 일환인 치료 레크리에이션의 신체적, 정신적 효과를 평가하고자 시행되었다. 연구 방법은 비동등성 대조군 사후 실험 연구로 2017년 2월부터 4월까지 진행되었다. 본 연구에 참여한 대상자는 실험군 35명, 대조군 35명이었으며 이 중 66명의 자료를 분석하였다. 본 연구에 참여한 대상자의 평균연령은 79.4세 이었으며 미국에 거주한 기간은 평균 24.6년 이었다. 치료 레크리에이션 프로그램은 데이케어센터에서 제공하는 프로그램으로서 본 연구결과, 데이케어센터 치료 레크리에이션에 참여한 노인의 일상생활 수행능력, 우울 및 외로움에 효과가 있는 것으로 나타났다. 따라서 향후 문화적 감수성을 고려한 노인 대상 다양한 치료 레크리에이션 프로그램이 개발, 운영되어져야 할 것이며 이에 대한 효과를 평가하기 위하여 근거기반 연구가 수행되어져야 할 것이다.

FCST의 음양균형장치를 활용한 경련성 사경증 증례보고 (Two Cases of Spasmodic Torticollis Managed by Yinyang balance appliance of FCST for the Meridian and Neurologic Balance)

  • 본 증례에서는 후천성으로 나타난 경련성 사경증 2례에 대해 외래 진료로 각각 3개월(91일간 54회), 5개월 반(167일간 90회)동안 FCST의 음양균형장치를 주 치료수단으로 하고 침구치료, 도수치료, 약물치료를 보조 치료수단으로 하여 치료하였다. 2례 모두 시각적 상사 척도(VAS)를 적용하여 평가해보았을 때 완전한 호전이 이루어졌다고 판단되었고, 임상적 관찰과 환자의 주관적 구술에서도 모든 증상의 소실과 정상 회복을 확인하고 치료를 종료하였다. 이후 각각 5개월에서 10개월 간 추적 관찰한 결과 모두 정상적인 생활을 하고 있음을 확인하고 치료 효과의 정상 유지를 관찰했다. 이로써 아탈구된 상부경추와 뇌신경계 및 경락체계의 불균형과 경련성 사경의 유의한 관련성을 유추해볼 수 있었으며, 이의 회복에 음양균형장치를 활용하여 악관절의 다차원적인 이상적 음양중심균형위치를 찾아줌으로써 척추를 비롯해 뇌와 경락체계를 조절해줄 수 있다는 FCST의 치료원리가 유의한 결과로 입증된 경우라 사료된다.

라미부딘 내성 소아 청소년 만성 B형 간염에서 Entecavir 치료 경험 (Experience with Entecavir Therapy for Lamivudine-Resistant Chronic Hepatitis B in Korean Children and Adolescents)

  • 목 적: 라미부딘에 내성을 보인 소아 청소년 만성 B형 간염에서 엔테카비어 단독요법의 바이러스 억제효과를 확인하고자 하였다. 방 법: 라미부딘에 내성을 보이는 소아 청소년 만성 B형 간염 환자 23명 중 6명(남 4, 나이 15.1~24.6세, 평균 17.5세)에게 엔테카비어 1 mg으로 치료하였으며 (평균 13.4개월, 1~21.1개월) 아데포비어로 치료한 11명과 비교하였다. 치료 시작 후 HBV DNA 역가가 1 $log_{10}$ 이하, 2 $log_{10}$ 이하, 357 IU/mL 이하로 감소하는데 걸리는 기간을 각각 구하여 서로 비교하였다. 결 과: HBV DNA 역가가 2 $log_{10}$ IU/mL 이상 감소하는데 걸린 기간은 각각 2.4${\pm}$2.3개월, 9.2${\pm}$7.3개월로 (p=0.025) 두 군 간에 유의한 차이가 있었다. HBV DNA 역가가 1 $log_{10}$ IU/mL 이상 감소하는데 걸린 기간 및 357 IU/mL 이하로 감소하는데 걸린 기간은 두 군 간에 유의한 차이가 없었다. 결 론: 라미부딘 내성 소아 청소년 만성 B형 간염에서 엔테카비어 단독요법은 아데포비어 단독요법과 비교해 볼 때 HBV DNA 역가가 2 $log_{10}$ IU/mL 이상 감소하는데 걸린 기간이 유의하게 짧았다. 특별한 부작용은 관찰되지 않았다. 엔테카비어의 치료 효과를 알기 위하여 더 많은 환자를 대상으로 한 장기 치료가 필요하다.

Effects of Culture Duration, Follicle Stimulating Hormone (FSH) Type, and Activin A Concentration on In Vitro Growth of Preantral Follicles and Maturation of Intrafollicular Oocytes

  • The objective of this study was to establish an in vitro culture system for ovarian preantral follicles of B6D2F1. First, we optimized the in vitro preantral-follicle culture by culture duration, follicle stimulating hormone (FSH) type, and activin A concentration. Duration of in vitro culture for 9, 11, and 13 days was sufficient for the normal development of preantral follicles to antral follicles. Formation of cumulus cell-oocyte complex (COC) was induced by treatment with human chorionic gonadotropin (hCG; 2.5 IU/mL) and epidermal growth factor (EGF; 5 ng/mL). In addition, metaphase II (MII) oocytes formed during this in vitro culture of preantral follicles. In vitro preantralfollicle culture for 9 days showed higher rates of growth and maturation, thus yielding a greater number of antral follicles, and there were significant differences (p < 0.05) in the number of MII oocytes (that formed from these preantral follicles via differentiation) between the 9-day culture and 11-day or 13-day culture. The follicles cultured for 9 days contained a tightly packed well-defined COC, whereas in follicles cultured for 11 days, the COC was not well defined (spreading was observed in the culture dish); the follicles cultured for 13 days disintegrated and released the oocyte. Second, we compared the growth of the preantral follicles in vitro in the presence of various FSH types. There were no significant differences in the growth and maturation rates and in differentiation into MII oocytes during in vitro culture between preantral follicles supplemented with FSH from Merck and those supplemented with FSH from Sigma. To increase the efficiency of MII oocyte formation, the preantral follicles were cultured at different activin A concentrations (0 to 200 ng/mL). The control follicles, which were not treated with activin A, showed the highest rate of differentiation into antral follicles and into MII oocytes among all the groups (0 to 200 ng/mL). Therefore, activin A (50 to 200 ng/mL) had a negative effect on oocyte maturation. Thus, in this study, we propose an in vitro system of preantral-follicle culture that can serve as a therapeutic strategy for fertility preservation of human oocytes for assisted reproductive medicine, for conservation of endangered species, and for creation of superior breeds.
