조등산(Diaoteng-san, Choto-san, 釣藤散)과 GB34(陽陵泉) 전침치료를 활용한 약인성파킨슨증후군 환자의 증례 1례 (A Case Report of Drug-Induced Parkinsonism Treated with Jodeung-san and Electro-acupuncture at GB34)

  • Drug-induced Parkinsonism is the most frequently observed type among the cases of secondary Parkinsonism. Besides typical parkinsonian symptoms, such as tremor, rigidity and bradykinesia, drug-induced Parkinsonism manifests with additional simultaneous symptoms like orobuccolingual dyskinesia, mixed type of tremor (resting, action), and symmetry of expressions. We present a case of drug-induced Parkinsonism, affected by taking the antiulcer drug cimetidine. Jodeung-san extract (Choto-san, Tsumura Co. 10) was administered for 7 days and acupuncture (electronic, auricular, pharmacopuncture) was conducted 3 times. The clinical outcomes were then evaluated through the patient's global impression of change, visual analogue scale, and Hoehn and Yahr stage. After the treatment, the clinical features, such as tremor and orobuccolingual dyskinesia, disappeared. The combination of Jodeung-san and electro-acupuncture at GB34 could therefore be a remedy for the patients with drug-induced Parkinsonism.

  • Objectives: Drug-induced Parkinsonism has similar symptoms to Parkinson's disease, but each has different causes. Drug-induced Parkinsonism accounts for the largest proportion of secondary Parkinsonism We report a outpatient case of drug-induced Parkinsonism after taking Risperidone, an atypical antipsychotic. Method: With discontinuing of antipsychotic drug, modified Yigan-san extract was administered for 12 weeks, and acupuncture and electroacupuncture procedures were performed 20 times. Results: Abnormal Involuntary Movement Scale (AIMS) score decreased from 23 to 3 during 59 days of treatment period without adverse events and worsening of depression. The Patient was highly satisfied. Conclusion: Modified Yigan-san and electroacupuncture (GB34) can be used as an treatment option in patients with drug-induced Parkinsonism.

사상처방을 활용한 약물 유발성 속발성 파킨슨증 치험 2례 (Two Cases of Drug-induced Secondary Parkinsonism Treated with Sasang Constitutional Therapy)

  • Objectives : This study is about two patients of drug-induced secondary parkinsonism caused by antipsychotic drugs. The purpose of this study is to report the clinical effects of Sasang constitutional medicine. Methods : These two patients were treated by Sasang constitutional herbal medications based on "Donguisusebowon". We evaluated the symptoms through the score of the Unified Parkinson's Disease Rating Scale(UPDRS). Results and Conclusions : The patients' chief complaints were improved. This study shows that Sasang constitutional herbal medications are an effective treatment for drug-induced secondary parkinsonism patients, but further studies are still necessary.

약인성 파킨슨증후군 환자의 한방 치험 1례 (Case of Drug-Induced Parkinsonism Treated with Traditional Korean Medicine)

  • Objective: This clinical study reports on the effect of Korean medicine on a patient with drug-induced Parkinsonism.Method: We used herbal medicine (Yigan-san), acupuncture, moxibustion, pharmacopuncture, and bee venom acupuncture to treat a patient with drug-induced Parkinsonism during hospitalization for 11 days. We observed the changes of symptoms using the unified Parkinson’s disease rating scale (UPDRS), the abnormal involuntary movement scale (AIMS), and the Beck depression inventory (BDI).Results: After treatment, the patient’s symptoms showed improvement in tremor, gait disturbance, and general condition. The UPDRS, AIMS, and BDI scores were also improved after treatment.Conclusion: This clinical case study suggests that traditional Korean medicine treatment could be effective in the treatment of drug-induced Parkinsonism.

보행장애를 호소하는 약인성 파킨슨 증후군 환자의 한방복합치료 1례 (Case Report of Drug-Induced Parkinsonism with Gait Disturbance Treated with Adjuvant Korean Therapy)

  • Objective: This case study reported the effectiveness of adjuvant Korean therapy on gait disturbances induced by drug-induced Parkinsonism. Method: A patient suffering from frontotemporal lobe dementia was diagnosed with drug-induced Parkinsonism and treated with adjuvant Korean therapy, including herbal medicine and pharmaco-acupuncture. The evaluation was performed by monitoring the length of time and number of steps during an 8 m gait, using the Unified Parkinson's Disease Rating Scale (UPDRS). Results: After 17 days of adjuvant Korean therapy, the UPDRS score improved from 32 to 16. The length of time for the 8 m gait improved from 20 seconds to 14 seconds. The patient also showed a decrease in the number of steps during the 8 m gait from 43 to 22. Conclusion: This case suggests that adjuvant Korean therapy can be effective for drug-induced Parkinsonism.

  • This study examined the effectiveness of Korean medical treatment in a patient with persistent drug-induced parkinsonism after drug discontinuation. The changes in symptoms were assessed using the unified parkinson's disease rating scale (UPDRS), postural instability-gait disturbance (PIGD) score, and the 20 m gait time and steps. After 22 days of hospitalization, the UPDRS, PIGD score, and 20 m gait time and steps showed clinically significant improvement. The improvement persisted after discharge. This study indicated that Korean medical treatment could be an effective alternative therapy for treating persistent drug-induced parkinsonism after drug discontinuation.

  • Manganese (Mn) intoxication causes a parkinsonian syndrome. It may be difficult to distinguish Mn-induced parkinsonism from idiopathic Parkinson disease (IPD). Neuropathological descriptions on the brains with Mn intoxication showed the preferential damage in the globus pallidus and substantia nigra pars reticularis.(omitted)

도파민운반체 영상의 임상적 유용성 (Clinical Usefulness of Dopamine Transporter Imaging)

  • Imaging of the dopamine transporter (DAT) provides a marker for the integrity of presynaptic nigrostriatal dopaminergic system. DAT density is reduced in Parkinson disease, multiple system atrophy, and progressive supranuclear palsy. In patients with suspicious parkinsonism, normal DAT imaging suggests an alternative diagnosis such as essential tremor, vascular parkinsonism, or drug-induced parkinsonism. DAT imaging is a useful tool to aid clinician's differential diagnosis in parkinsonism.

Perphenazine and trifluoperazine induce mitochondria-mediated cell death in SH-SY5Y cells

  • Drug-induced parkinsonism has been associated with an increased risk for Parkinson's disease. Antipsychotic drugs have long been known to cause parkinsonian symptoms. However, it remains unclear whether antipsychotics can directly damage the nigrostriatal pathway. In the present study, we investigated the toxicity mechanism of two typical antipsychotics, perphenazine and trifluoperazine, in a human dopaminergic cell line, SH-SY5Y. Perphenazine and trifluoperazine induced mitochondrial damage as evidenced by fragmentation of mitochondria, activation of Bax, cytochrome c release and a decrease in cellular ATP level. In addition, activation of caspase-3 and apoptotic nuclei were observed following the drug treatment. However, pan-caspase inhibitor did not suppress the cell death induced by the antipsychotics, suggesting that the initiated apoptosis was possibly shifted to necrosis upon caspase inhibition. Damaged mitochondria may have induced oxidative stress since the drug-induced cell death was partially suppressed by an antioxidant. Taken together, our results suggest that perphenazine and trifluoperazine can induce apoptotic cell death in a dopaminergic cell line via mitochondrial damage accompanied by oxidative stress.
