기능성 소화불량의 의한 협진과 통합의료에 대한 한의사, 의사, 환자의 인식도 차이 (Perception Difference between Korean Medicine Doctors, Western Medicine Doctors, and Patients on the Collaborative and Integrated Medicine for the Functional Dyspepsia)

  • Objectives: The aim of this study was to investigate the perception difference between Korean Medicine doctors, Western Medicine doctors, and functional dyspepsia (FD) patients on the 'Collaborative and Integrated Medicine (CIM)' for FD management. Methods: Between February and April 2021, 545 Korean Medicine doctors, 121 Western Medicine doctors, and 186 FD patients were invited to participate in this survey. A questionnaire was developed by both the Korean and Western Medicine doctors and consisted of questions about general perceptions of FD and perceptions of CIM for FD. Results: Responses from 424 Korean Medicine doctors, 113 Western Medicine doctors, and 153 FD patients were analyzed for the CIM of FD. Significant perceptional differences were noted between Korean Medicine doctors, Western Medicine doctors, and FD patients on the CIM for FD, including necessity, effectiveness, safety, cost, and reliability of CIM for the management of FD (p<0.001). The FD patients also more strongly supported the necessity of developing CIM for FD than did the Korean and Western Medicine doctors (p<0.001), and 87.6% of the FD patients strongly agreed or agreed with the use of CIM for FD. Conclusions: A significant perception difference was detected between Korean Medicine doctors, Western Medicine doctors, and FD patients regarding the use of CIM for FD. Patients with FD favored the necessity and were more willing to accept CIM for FD than were the doctors. Therefore, more efforts may be warranted for doctors to promote CIM for FD.

신생아집중치료실 캥거루케어에 대한 간호사와 의사의 인식 (Perception of Nurses and Physicians in Neonatal Intensive Care Units on Kangaroo Care)

  • Purpose: To identify the perception and practices of kangaroo care in nurses and doctors working in neonatal intensive care units (NICU) in Korea. Methods: One hundred forty-nine nurses and nineteen doctors working in the NICU from six university hospitals completed a survey questionnaire. Results: Most agreed that Kangaroo care promoted attachment and parental confidence as well as physical health of the infant. However, nurses and doctors showed a negative perception in providing kangaroo care for premature infants under 1,000 grams or within several hours after birth. Major barriers for kangaroo care were worrying about extubation and safety problems of premature infants. Married or senior nurses showed a more positive perception than others. Also nurses who worked in hospitals where kangaroo care was provided had a lower barrier perception than other nurses. Conclusion: Nurses and doctors working in NICU worried about adverse effects of kangaroo care even though they perceived positive effects. Standard education programs and manuals should be developed before dissemination of kangaroo care in Korea.

  • The objectives of this study were (1) to describe doctors' perception and attitudes toward patient safety culture and medical error reporting in their working unit and hospitals, (2) to examine whether these perception and attitudes differ by doctors' characteristics, such as sex, position, and specialties, and (3) to understand the relationship between overall perception of patient safety in their working unit and each sub domain of patient safety culture. A survey was conducted with 135 doctors working in a university hospital in Korea. After descriptive analyses and chi-square tests of subgroup differences, a multivariate-regression of overall perception of patient safety in their unit with sub-domains of patient safety culture was conducted. Overall, a significant proportion of doctors expressed negative perception of their working units' patient safety culture, many reporting potentials for patient safety problems to occur in their unit. They also negatively viewed their hospital leadership's commitment on patient safety. Regarding the patient safety in their working unit, doctors were most worried about staffing level and observance of safety procedures. Most doctors did not know how and which medical error to report. They also perceived that medical errors would work against them personally and penalize them. About 22 percent of respondents believed that even seriously harmful medical errors were not reported.

임상영양서비스에 대한 부산.경남지역 의료진과 영양사의 인식 차이 (Different Perceptions of Clinical Nutrition Services between Doctors and Dietitians in the Busan-Gyeongnam Area)

  • The purpose of this study was to evaluate the difference in perception of clinical nutrition service (CNS) between doctors and dietitians working in hospitals in Busan and the Gyeongnam area. Research was performed through questionnaires (from November to December 2011) at over 100 beds. 73.3% of dietitians were aware of the Nutrition Support Team (NST), while only 15.6% of doctors were aware of it. Due to heavy work and lack of medical staff, doctors didn't participate in NST, although most of them recognized the necessity of NST. 61.7% of dietitians screened and managed malnourished patients, whereas only 29.8% of doctors did. The main reason dietitians didn't treat malnourished patients was the absence of a treatment system in the hospital. Less than 50% of dietitians participated in the doctor's round to malnourished patients. As for why dietitians didn't participate in doctor's rounds, 71% of doctors chose understaffed dietitians and 38.1% of dietitians chose the doctors' unawareness of the importance of the dietitian in doctor's rounds. For the lower rate of nutrition counseling in provincial regions, compared to the capital region, 46.8% of doctors cited a lack of connection between doctors and clinical dietitians, while 43.3% of dietitians cited the lack of doctors' awareness on the importance of nutrition counseling. Although 87.3% of the doctors and 91.6% of the dietitians answered that CNS is important for treatment, the perception of onsite performance status on CNS was found to be low in both groups. 48.9% of doctors and 50.0% of dietitians regarded dietitians in the hospital as personnel in charge of food services, rather a member of the medical team. To improve the awareness of the importance of the CNS, and the image of clinical dietitians, 31.2% of doctors answered "to introduce a professional dietitian license for each disease" and 26.7% of dietitians answered "to change the system in the hospital". Most subjects found that a separation of clinical nutrition services from the food service part is needed. These results suggest that it is important to narrow the difference in perceptions of clinical nutrition services between doctors and dietitians for an organized clinical nutrition management of patients in hospitals in Busan and the Gyeongnam area.

임상영양서비스에 대한 의료진의 인식 및 요구도 (Doctors' Perception and Needs on Clinical Nutrition Services in Hospitals)

  • The purpose of this study was to evaluate the perception and needs of doctors on clinical nutrition services. A cross-sectional survey design was used. The doctors' perception and needs were assessed by questionnaires that had been specifically designed for the study. The research was conducted from February 14 to March 15, 2011 for 544 doctors at 42 large hospitals (with over 400 beds). Ninety-eight percent of doctors responded that clinical nutrition service was important. The mean scores of importance on clinical nutrition service were 4.45 for 'nutrition screening at admission', 4.50 for 'treatment of malnutrition', and 4.43 for 'nutrition education and counseling'. The mean scores of needs for clinical nutrition service were 4.42 for 'individual nutrition counseling & education' 4.39 for 'nutrition management for malnourished patients' and 'nutrition management for tube fed patients'. The medical specialists showed significantly (P<0.01) higher scores than the residents on the importance and needs for clinical nutrition services. Eighty-five percent of the doctors recognized the necessity of the specialized dietitians classified by diseases. The medical specialists (93.8%) showed significantly (P<0.01) higher recognition of this necessity than the residents (77.7%). The low residents' perceptions of the importance and needs for clinical nutrition services will necessitate clinical nutrition education plans. The departments of clinical nutrition in universities should improve the ability and skills of the clinical dietitians.

원격의료 도입에 대한 한의사의 인식 (Korean Medicine Doctors' Perception of Telemedicine)

  • Objectives : This study aimed to investigate the perception level of Telemedicine among Korean medical doctors. Methods : The on-line survey was administered for Korean Medicine doctors. The survey consisted of 46 questions, addressing issues on concepts and adoption model of Telemedicine. Data were collected from 528 Korean medical doctors, and were analyzed using frequency analysis, t-test, ANOVA, and Kruskalwallis test. Results : The results showed that more than half of respondents had positive attitude towards implementation of Telemedicine in Korean Medicine and conventional medicine areas. Respondents were most positive about the aspect that Telemedicine could reduce 'hospital use by people with disabilities', while they were most concerned about the possibility that patients could be concentrated in large-scale hospitals. As prerequisites for the introduction of Telemedicine, accurate information delivery during remote communication between doctors and patients, clarification of responsibility for Telemedicine at the policy level, and development of equipment for accurate information delivery at the technical level received high responses. Conclusions : Korean Medicine doctors were positive about the implementation of Telemedicine, and they preferred to remote monitoring between doctors and patients. The development of medical equipment for accurate patient information delivery and the establishment of an institutional basis for clarifying responsibilities in case of medical accidents are required.

전공의들의 전문직인식이 직업만족도에 미치는 영향 (Interns' and Residents' Professional Job Perception and Its Effect to Their Job Satisfaction)

  • In ordo to investigate the professional job perception and job satisfaction of interns and residents and its related factors, the author conducted survey using self administered questionnaire for 2 months(August and September, 1996). The study subjects were 562 interns and residents who worked at one university hospital(tertiary hospital) and two general hospitals(secondary hospital) in Taegu, Korea. Among them, 297 (52.8%) responded completely. The results were as follows ; Respondents thought that people component was most important, science and status components were next among professional value scales which meant the desirable attitudes required to conduct professional works. But, the score of professional value scales was generally high and not affected by other variables in multiple linear regression analysis. They seemed to have very normative and ideal perceptions about professional values. The score of professionalism scales, which were attitudinal and behavioral traits about professional job, of high grade residents was significantly higher than that of interns and lower grade residents. The score of reference to professional organization and autonomy factor were increasing significantly as the grade increased. Working conditions and perception for socioeconomic status of doctors influenced the score of professionalism scale significantly. It seemed that professional socialization was made during the training periods of interns and residents. Most of respondents answered that current socioeconomic status of doctors were middle and high strata but they responded that the socioeconomic status of doctors would fall in the future. They seemed to have a pessimistic thought about doctor's status. Generally the respondents thought that they were satisfied with doctor job and fit to the job, but 51.9% answered that if possible they would get other jobs. It seemed to reflect their critical thinking on doctors' status. Perceptions about socioeconomic status of doctors, professional value and professionalism influenced job satisfaction significantly. The interns and residents had high sense of calling to doctors and thought that doctors were socially important job. Generally they were satisfied with their job. Interns and residents had normative thoughts about the trait which compentent doctors must have. During the training period, they seemed to have attitude and perception as a professional and to make professional socializations.

한의학에 대한 의사들의 인식과 태도 변화에 관한 체계적 문헌 고찰 (Systematic Review of Changes in the Perception and Attitude of Medical Doctors toward Traditional Korean Medicine)

  • Objective : To find a collaboration strategy between western medicine and traditional Korean medicine (KM), this study aims to figure out the changes in the perception and attitude of medical doctors toward KM through systematic review. Method : Systematic literature searches were performed on six Korean databases. Studies were categorized according to the respondents and question items and analyzed by the context of questions, similarity of respondents and measurement scale. And we analyzed the changes of response regarding to medical doctors' and medical students' perspective and attitude to KM. Results : Eighteen survey studies including attitude of medical doctors and medical students toward KM were selected, which were conducted from 1993 to 2011. Although the attitude toward KM did not show any positive change, medical doctors have had more interest in acupuncture than herbal medicine and appreciated KM's treatment effect for musculoskeletal disease. In spite of little KM education experience, they had intentions for acupuncture education at least. Many medical doctors have listed the unscientific aspect as a major reason why they cannot trust KM. Medical doctors working for cooperative practice showed more positive attitude than other medical doctors and medical students had more positive attitude in general than medical doctors Conclusion : Though the growth of KM service and cooperative practice since 1990s, medical doctors' attitude toward KM seems to become more negative. To improve their attitude, making scientific evidences for KM is required as well as giving more education and treatment experience.

개원의사들의 전문직업성과 직무스트레스가 직업만족도에 미치는 영향 (Professional Job Perception, Job Stress and Job Satisfaction of Doctors Practicing in Local Clinic in Daegu City)

  • Objective : This study was conducted in order to investigate professional job perception, job stress and job satisfaction in doctors. Method : The authors conducted a survey using a self-administered questionnaire, conducted between August and September,2001. The study subjects were 457 doctors practicing at local clinics in Daegu City. Results : For the professionalism scale, the score for 'sense of calling to field' and 'feeling of autonomy' were relatively high, Age, working hours per day, and perception of socioeconomic status significantly influenced the professionalism scale scores (p<0.01). For the job stress scale, the scores for 'clinical responsibility/decision' factor were the highest of all the surveyed factors. Working hours per day significantly influenced the job stress scores (p<0.01). To investigate the overall relationship between the variables, the authors conducted a linear structural equation model analysis. The equation was statistically appropriate and a good fit. The job stress, and the professionalism factors, respectively influenced job satisfaction negatively and positively (T>2.0). The working hours per day and status perception also influenced their job satisfaction (T>2.0). Conclusion : To promote the iob satisfaction of doctors, the development of coping tools and other intervention methods are needed to increase doctor's professional job perception and reduce their job stress. Further studies are required to understand the characteristics of job satisfaction and for its promotion with doctors.

성폭력 피해자 한의의료지원시스템 구축을 위한 한의계 인식 조사연구 (Korean Medical Doctors' Perception Survey Regarding the Establishment of an Oriental Medicine Support System for Sexual Violence Victims)

  • Objectives: The purpose of this study was to prepare basic data to build an Oriental medicine support system for sexual violence victims through a survey on perceptions of Korean medical doctors about sexual violence. Methods: From 18 September 2019 to 24 September 2019, the online survey was conducted by both Korean medical doctors and students of the Korean Medical College. Thereafter, a quantitative statistical analysis was conducted on data from a total of 1,011 respondents, including 749 doctors and 262 students. Results and Conclusions: Many of the doctors and the students who answered were not fully aware of public services and legal systems related to sexual violence. Most Korean doctors had very little experience in treating victims of sexual violence. Most respondents thought that the number of designated Oriental medicine institutions for sexual violence victims should increase, and related education and Oriental medicine manuals would be essential. Most respondents said that information about trauma treatment would be essential. In addition, the Korean medicine group showed overall good scores in the perception of sexual violence. However, there were statistically significant differences in perception between males and female, and among age groups. We hope that this study could be used as basic data for establishing an Oriental medicine support system for sexual violence victims.
