The National Status of Tuberculosis Using Nationwide Medical Records Survey of Patients with Tuberculosis in Korea

  • Background: The epidemiology of tuberculosis (TB) has been assessed based on the data of the analysis of TB patients notified to the surveillance system in Korea. However, the national status of TB is not validated through this surveillance system. The objective is to determine the epidemiology of TB and to understand the accurate status of TB patients treated in private institutions. Methods: Medical records of 53,579 patients who had been diagnosed with TB in 2008 were analyzed. Results: Among 53,579 patients, the number of sputum smear positive cases was 15,639(29.2%) and the number of new cases was 39,191 (73.1%). The drug resistance rate of new cases was 5.3%, while the rate stood at 13.3% for TB patients with treatment history. The number of multi-drug resistant TB (MDR-TB) patients was 2,472 (4.6%), which consists of 2.9% of new cases and 9.3% of TB patients with prior treatment history. The number of extensively drug-resistant TB patients was 749 (1.4%), consisting of 1.1% of new cases and 2.2% of TB patients with prior treatment history. In terms of treatment outcomes, 66.4% of all TB patients, 70.5% of new cases, 64.4% of relapse cases, and 46.8% of MDR-TB cases were cured or completed. It was inferred that in 2008, the total number of TB patients reached 70,767, 145.6 per 100,000 people (95% confidence interval, 145.5~145.7). Conclusion: We conclude that the medical records review of the Health Insurance Review and Assessment Service (HIRA) data can be very effective in promoting the understanding of the current status of TB in private institutions.

Construction of Luminescence- and Fluorescence-Tagged Burkholderia pseudomallei for Pathogen Tracking in a Mouse Model

  • Molecular imaging is a powerful method for tracking various infectious disease-causing pathogens in host organisms. Currently, a dual molecular imaging method that can provide temporal and spatial information on infected hosts at the organism, organ, tissue, and cellular levels simultaneously has not been reported for Burkholderia pseudomallei, a high-risk pathogen that causes melioidosis. In this study, we have established an experimental method that provides spatiotemporal information on infected hosts using luminescent and fluorescent dual-labeled B. pseudomallei. Using this method, we visualized B. pseudomallei infection at the organism, organ, and tissue levels in a BALB/c mouse model by detecting its luminescence and fluorescence. The infection of B. pseudomallei at the cellular level was also visualized by its emitted fluorescence in infected macrophage cells. This method could be an extremely useful and applicable tool to study the pathogenesis of B. pseudomallei-related infectious diseases.

Respiratory syncytial virus prevention in children with congenital heart disease: who and how?

  • Respiratory syncytial virus (RSV) is a major cause of respiratory infection in children. Most of the pediatric population have RSV infection before the age of 2, and recurrent infections are common even within one season. Chronic lung disease, prematurity, along with congenital heart disease (CHD) are major risk factors in severe lower respiratory infection. In hemo-dynamically significant CHD patients with RSV infection, hospitalization is usually needed and the possibility of treatment in intensive care unit and the use of mechanical ventilator support are known to increase. Therefore the prevention of RSV infection in CHD patients is mandatory. The current standard for RSV prevention is immunoprophylaxis by palivizumab. Immunoprophylaxis is recommended monthly in hemodynamically significant CHD patients, up to 5 months. Motabizumab, a second generation drug and newly developing RSV vaccines are also expected to play a key role in RSV prevention in the future. The prophylaxis of RSV infection in CHD patients is cost-effective in both the medical aspect of the patients as well as the socio-economic aspect. Therefore an effort to promote prevention should be made by not only the family of the patients but also by the government.

A Profile of Glucose-6-Phosphate Dehydrogenase Variants and Deficiency of Multicultural Families in Korea

  • Vivax malaria incidence in Korea is now decreased and showing a low plateau. Nowadays, vivax malaria in Korea is expected to be successfully eliminated with anti-malaria chemotherapy, primaquine, and vector control. The glucose-6-phosphate dehydrogenase (G6PD) deficiency is associated with potential hemolytic anemia after primaquine administration. This inborn disorder has a pivotal polymorphism with genetic variants and is the most prevalent X-chromosome-linked disorder. The prevalence of G6PD deficiency was previously reported negligible in Korea. As the population of multicultural families pertaining marriage immigrants and their adolescents increases, it is necessary to check G6PD deficiency for them prior to primaquine treatment for vivax malaria. The prevalence of G6PD variants and G6PD deficiency in multicultural families was performed in 7 counties and 2 cities of Jeollanam-do (Province), Gyeonggi-do, and Gangwon-do. A total of 733 blood samples of multicultural family participants were subjected to test the phenotypic and genetic G6PD deficiency status using G6PD enzyme activity quantitation kit and PCR-based G6PD genotyping kit. The G6PD phenotypic deficiency was observed in 7.8% of male adolescent participants and 3.2% of materfamilias population. Based on the PCR-based genotyping, we observed total 35 participants carrying the mutated alleles. It is proposed that primaquine prescription should seriously be considered prior to malaria treatment.

경남도민의 COVID-19 감염예방행위에 미치는 영향요인: 불안, 건강신념을 중심으로 (Gyeongnam Inhabitants' Effects of COVID-19 Infection Prevention Behavior: Focus on Anxiety and Health Belief)

  • Purpose: This study was aimed to identify the influencing factors on the COVID-19 infection prevention behavior of adults residing in Gyeongsangnam-do by examining relationships between anxiety, health belief and COVID-19 infection prevention behavior. Methods: The participants of this study were recruited from a website, and the total number of participants was 186. The self-report questionnaires were distributed during July 2020. Results: Multiple regression analysis revealed the significant factors affecting the COVID-19 infection prevention behavior of adult residing in Gyeongsangnam-do were perceived barriers(��=-3.68, p<.001), perceived benefits(��=3.39, p=.001), perceived susceptibility(��=2.83, p=.005), sex(��=2.59, p=.010). The total explanatory power of the study variables for COVID-19 infection prevention behavior was 35.0%(F=17.61, p<.001). Conclusions: Based on these findings, in order to promote COVID-19 infection prevention behavior, there is needed to reduce obstacles for carrying out infection prevention behaviors, and develop multi-faceted promotional strategies for infection prevention behaviors.

간호사의 함께있음 프로그램이 만성질환 노인의 자살예방에 미치는 효과 (Effects of a Nurse Presence Program on Suicide Prevention for Elders with a Chronic Disease)

  • Purpose: The purpose of this study was to identify the effects of a nurse presence program on suicide prevention for elders with chronic disease. Method: The subjects were recruited from two different elderly institutions located in D city and K province, Korea. Twenty subjects in the control group received no intervention and nineteen subjects in the experimental group received a nurse presence program. Result: There were more significant decreases in suicide ideation, and the cortisol level and increases in life satisfaction in the experimental group compared to the control group. Conclusion: According to the above results, a nurse presence program for elders with a chronic disease decreased stressful events like suicide ideation and increased self esteem through therapeutic interaction. These findings suggest that this program can be used as an efficient independent nursing intervention for elders in a critical situation.

한국 성인 남녀에서 지방섭취제한 행동 변화단계에 따른 영양상태와 건강관련 행위에 대한 신념 비교 (Comparison of Nutritional Status and Beliefs on Health Behavior Regarding Stages of change in Dietary Fat Reduction among Korean Men and Women)

  • This study describes the application of the stages of change construct to fat intake by examining the associations of the stages of change with nutritional status and beliefs on health behavior. Data were obtained from apparently healthy 596 adults(326 females and 270 males) residing in large cities. Stages of change assessed by an algorithms based on 6 items were designed each subject into one of the 5 stages: precontemplation(PC), contemplation(CO), preparation(PR), action(AC) and maintenance(MA). Beliefs on health behavior were assessed by self efficacy as well as 4 belief scales from the Locus of Illness Control(LIC) developed using factor analysis such as internal disease cure and prevention and external disease cure and prevention. Energy and fat intakes were measured by a 39 item short form food frequency questionnaire. Regarding the 5 stages of change, MA stage comprised the largest group(37.9%), followed by Ac(30.7%), PC(11.4%), CO(10.4%) and PR(9.6%). Subjects who were females, older or healthier were more likely to belong to either AC or MA. Stage assignment of individuals was corroborated by their nutritional variables. Those in PC had the most energy and fat and those in MA ate the least for females. BMI was higher in PR than any other stages for both males and females. Those in PC were distinctive in that they were more externally oriented in terms of health control showing higher scores on external disease prevention(for males) and external disease cure(for females), and lower score on internal disease cure. On the other hand, those in MA received the highest scores on internal disease prevention and self efficacy, which suggested that they were more internally oriented. Canonical discriminant function analysis indicated that the 5 stages were importantly discriminated by BMI, self efficacy, internal disease prevention and external disease prevention for males and by fat intake, self efficacy and external disease cure variables for females. The results of our study confirm differences in stages of change in fat intake in terms of nutritional status and beliefs on heath behavior and indicate the need for taking these phases of change into account in nutrition advice. 노인 성병예방 프로그램 개발 및 효과 (Development and effects of sexually transmitted disease prevention programs for the elderly)

  • Purpose: An STD (Sexually transmitted disease) prevention program was developed for the elderly, and its effect on their knowledge about STDs, STD prevention attitudes, social support, STD prevention self-efficacy, STD prevention behaviors, and STD presence were verified. Method: The subjects of the study were 50 people (25 in the experimental group and 25 in the control group). The experimental group was provided with a six-session STD prevention program. Result: In the experimental group, the knowledge about STD (t=105.00, p<.001), STD prevention attitudes (t=7.60, p<.001), and social support (F=87.11, p<.001), STD prevention self-efficacy (U=28.00, p<.001), and STD prevention behavior (t=7.38, p<.001) were improved significantly. Conclusion: The results highlight the need for nursing intervention for STD prevention to the elderly using the program developed for the prevention of STDs for the elderly.

  • Because of the imminent market opening of livestock and poultry products, Korean poultry industry is faced an influx of contagious diseases from abroad. Due to the increasing trend of intensive and automatic poultry farming, the risks of catching various poultry epidemics are ever more increasing. For the prevention of poultry epidemics, the principles of biosecurity should be observed rather strictly. In addition, regular checking to confirm the presence or absence of latent diseases through serological test, post-mortem examination, and laboratory test, should be carried out routinely. Finally, all kinds of stress factors should be minimized to keep the flock healthy and productive, so that they can achieve their maximum genetic potential.
