Analysis of the Nursing Interventions Performed by Home Health Care Nurses in a Hospital : An Application of NIC (병원중심 가정간호중재 분석: NIC 체계 적용)

  • The purpose of the study was to investigate the characteristics of the clients registered in the department of home health care nursing in a hospital and to analyze nursing intervention activities recorded in charts by application of Nursing Intervention Classification (NIC) system. For the descriptive survey study, data were collected by reviewing charts of 572 home health care clients between May, 1997 and July, 2000 at K hospital in Seoul. The average age of the clients was 66 years and the number of clients in their 70s ranked first with 28.2 percent(158 people). The mean length of home care service was 47 days with the highest frequency of less than four weeks (56 %). With regard to medical diagnosis, cancer showed the highest frequency (48%, 271 people), followed by cerebrovascular disease (19%), and pulmonary disease (6.9%). According to analysis of nursing interventions by the NIC system, the most frequently used nursing interventions in level 1 were interventions in the Physiological: Complex domain which were used 3,663 times (33%) among 11,107 total interventions. The Safety domain was the second most frequently used intervention, followed by the Physiological: Basic, and the Behavioral domains. In level 2, the Risk Management class was the most frequently used interventions with 3,108 interventions (27.9%), followed by Drug Management, and Tissue Perfusion Management classes. In level 3 interventions, Vital Sign Monitoring was the most frequently used intervention, 569 times (5.1%), followed by Health Screening, and Neurological Monitoring interventions. In sum, half of the clients in the study had cancer and were in their 70s. The most frequent reason for ending home care was death (40%), followed by readmission (28%). These findings represent clients with severe conditions referred to the home care nursing department as it was a University teaching hospital. Further research on analyzing nursing interventions performed in each institution needs to be conducted to develop a standardized list of nursing interventions to use in home health care settings.

A Study on The Life Tablefor Specific Causes of Death in Korea (사망원인과 특정사인생명표에 관한 연구)

  • This study was conducted to make the life tables from specific causes of death in Korea. Both "Life tables of Korea in l978-79" and "the statistics on causes of death statistics in 1980" issued by Economic Planning Board were used as source of data for this study. Among the 58, 187 death certificates reported to the concerned authorities, 39, 801 causes were drawn for the purpose of this study. As a result, it is revealed that two thirds of men in Korea died from these 10 major causes of death. The summarized results are as follows: 1. According to recent statistics, 10 major causes of death in 1980 were shown in the order of 1) malignant neoplasms, 2) cerebrovascular disease, 3) accidents and adverse effects, 4)hypertensive disease, 5) ischaemic heart disease and heart attack, 6) chronic liver disease and cirrhosis, 7) tuberculosis, 8) pneumonia, bronchitis, emphysema and asthma, 9) suicide, 10) diabetes mellitis. 2. The major causes of death in Korea were very similar to those of developed countries such as West Germany, Denmark and Japan. This means that our pattern of death causes is almost approaching to that of developed countries. 3. Our crude death rate in 1980 was on the line of 6.6 per 1, 000 people. This is very low level, compared with 12.1 in West Germany and 10.0 in Denmark, however, our age sepcific death rate was on the verge of doubled level in each age category as to that of West Germany, Denmark and Japan. The fact tells us that our death rate is very high yet, especially in young and prime adult age, and the proportion of the aged is quite low. 4. Average ages of people died from malignant neoplasms, cerebro vascular diseases and hypertensive diseases were 63.1, 66.6, 67.3 respectively, however, that of accidents and adverse effect was only 42.5. This shows that accidents occur indifferently from age. 5. In the curve of eventual death probability, the curve of malignant neoplasms was the highest of all curves before 60 in age. However, the probability curve of eventually dying from accidents and adverse effects tends to decline with age. 6. In this study five life tables from major causes of death (four leading causes of death and of tuberculosis) were constructed for 1979. These life tables are reflecting accurately the effects of age distribution on the specific cause of death. In the surviving curje of these tables we can see that the curve of accidents is adversely related to age. While curves of neoplasms, hypertension and tuberculosis are not diminishing before 40 in age, they are going sharply downward after 50 in age.ard after 50 in age.

Prevalence of giardiasis of stray cats in the Daejeon city

  • Giardiasis is widespread all over the world, and it is a disease that causes both acute and chronic digestive symptoms. It is zoonotic disease that affects animals and humans. There are few studies on giardiasis in stray cats due to difficulties in catching and sampling. Therefore, this study evaluated the prevalence of giardiasis in stray cats in the Daejeon city because of increasing interest as zoonotic disease. The specimens were the feces of stray cats captured for the neutering project (TNR) in Daejeon; 30 fecal samples were collected from 2021 to 2022 in each of 5 districts in Daejeon. A total of 150 samples were collected. All samples were tested for giardiasis using the Giardia SNAP kit (SNAP test, IDEXX Laboratories. Inc., Westbrook, ME). The overall prevalence rate was 46 out of 150 cats (30.7%). By age, 25 out of 71 juvenile cats (35.2%) were positive, and 21 out of 79 adult cats (26.6%) were positive. A total of 19 out of 69 cats (27.5%) with diarrhea were positive, and 27 out of 81 asymptomatic cats (33.3%) were positive. For gender, 38 out of 99 females (38.4%) were positive, and 8 out of 51 males (15.7%) were positive. The positive rate of giardiasis in stray cats was over 30%, which is high compared to other research results. It is necessary to increase the public's awareness of the value of deworming stray cats and the sanitation of people who have come into contact with them.

Effects of Gossypetin from Hibiscus sabdariffa on Interleukin-6 Production in Porphyromonas gingivalis Lipopolysaccharide-Stimulated Human Gingival Fibroblasts

  • Background: Periodontal disease is a major cause of tooth loss in adults and is a representative oral disease commonly suffered by most people around the world. Mainly the proliferation of Gram-negative bacteria and secreted virulence factors cause an inflammatory response and destroy periodontal tissue. Gossypetin, isolated from Hibiscus sabdariffa L, is known to have various pharmacological effects, including antibacterial and anticancer activities. We aimed to confirm the anti-inflammatory effect of gossypetin through interleukin-6 (IL-6) regulation in human gingival fibroblasts (HGFs) stimulated with lipopolysaccharide (LPS) of Porphyromonas gingivalis, a major cause of adult periodontitis. Methods: CCK-8 assay was performed to confirm the concentration-dependent cytotoxicity of gossypetin against HGFs. The secretion level and mRNA expression of IL-6, an inflammation-related cytokine, and the effect of gossypetin on these in HGFs stimulated with P. gingivalis LPS were confirmed by ELISA and qRT-PCR analysis, respectively. Results: Up to a concentration of 100 µM gossypetin with or without P. gingivalis LPS, the survival rate for HGFs was maintained at over 95% and showed no toxicity. ELISA and qRT-PCR analysis results showed that P. gingivalis LPS increased IL-6 secretion and mRNA levels in HGFs compared to the control group. However, this increase in IL-6 was significantly down-regulated by gossypetin treatment in a dose-dependent manner. In particular, 80 µM gossypetin inhibited IL-6 production to the level of the control group. Conclusion: These results indicated that gossypetin attenuated IL-6 production in HGFs stimulated by P. gingivalis LPS, which may ultimately suppress the inflammatory response in periodontal tissue. Therefore, gossypetin may have potential as a natural ingredient for the prevention and treatment of periodontal disease.

Factors associated with early recognition of myocardial infarction in adult smokers: Using data from 2021 Community Health Survey (성인 흡연자에서 심근경색증 조기 증상 인지와 관련된 요인:2021 지역사회 건강조사 자료 활용)

  • The purpose of this study was to understand the degree of early recognition of myocardial infarction in adult smokers and to analyze factors related to cognition. In a descriptive survey using raw data from the 2021 Community Health Survey, 210,899 out of 229,942 people who completed the survey, excluding 18,343 subjects with insufficient responses, were selected as study subjects. Age, marital status, education level, smoking cessation plan, health examination_cancer examination, diabetes diagnosis, subjective health level, and social physical environment were significantly related to early symptoms of myocardial infarction. Based on the results of the study, active promotion and education for high-risk groups with a high risk of developing myocardial infarction are needed.

A Study on the Blood-Letting Therapy in Elementary Questions (("황제내경소문(黃帝內經素問)" 중(中) 사혈(瀉血)에 관한 연구(硏究))

  • Blood-Letting Therapy is a rational and ecological medical treatment by which we can heal most of the diseases by removing the static blood which precipitates in the blood vessel and blocks the flowing of blood. And the static blood is the generic term for the injurious, bad, dead and precipitated blood which is blocked the capillary vessel. The Yellow Emperor's Canon of Internal Medicine says that "the patient is treated with drugs internally and stone acupuncture externally. "In the old texts, the blood-letting therapy is mentioned as blood-letting, network vessel pricking, bloodletting, pricking, and arousing pulses etc and it is noted down as the method of network vessel pricking in 'On the Application of Needles' of Spiritual Pivot. Nine-pricking therapy, twelve-pricking therapy and five-pricking therapy are recorded in the methods of network vessel pricking and among them, the method of squeezing blood after pricking the affected part is explained as the network vessel pricking. There are four methods of network vessel pricking, pricking, picking, cluster needling and scatter-pricking and they are fluidly applied to the various symptoms of diseases. In 'On Discriminative Treating for Patients of Different Regions' in Elementary Questions, Ki-baek emphasizes "most of the local people, there are black in skin and loose in striate, and their diseases are mostly of carbuncle kind. It is suitable to treat the disease with stone therapy to prick with stone, so the stone therapy is transmitted from the east. "And in 'On the Corresponding Relation Between the Eum and Yang of Man and All Things' in Elementary Questions, when the Emperor asked Ki-Back, he answered "sthenia means the sthenia of evil, and deficiency means the deficiency of healthy energy. When the blood is sthenic, the evil should be discharged by pricking when out letting the blood; deficiency of vital energy is the asthenia of channels and network vessels, so the energy should drain from the channel which is not deficient, to replenish. "And in this case we can use the methods of 'Breaking out the static bloods', 'driving out the static bloods', blood-letting'. With this we can infer that the blood-letting therapy is made use of the important medical treatments from the ancient times. Especially in referring to the principles of treatment in The Yellow Emperor's Canon of Internal Medicine, it mostly alluded to acupuncture therapies and only eleven times to medicinal treatments. This is to verify that the blood-letting therapy formed the foundation of the medical art. In Dong's Therapy of Acupuncture-Moxibustion and Bloodletting, Dong Kyeong-Chang gave emphasis on the points that there must be extravasated bloods without exception in the serious illnesses which is old, unnatural, accompanying acute pains and so we can revive our body‘s sprit by circulating 'gi' and static blood piled up in the network vessel, regulating the weakness and strength, and controling the disharmony of the internal organs. The blood-letting therapy has effect on the orifice in emergency, such as fore draining, freeing network vessels, harmonizing gi and blood, relieving pain, dispersing swelling and concretion, sedation, resolving toxin as well as strengthening the heart, relieving itching. So it has distinguished effect on all kinds of medical treatment to the modern people. But by the change of social customs and the confucianism of confucius - it is widely spread on the period of North and South Dynasties, 'Wi' and 'Jin' in china and the period of the Three States in korea - The blood-letting therapy which was regarded as the most important medicinal treatment withered rapidly. And Confucius accentuated the importance of our body and all its members, loyalty and filial piety and banned any damage of our body under no circumstances. As a result of it, the therapy of blood-letting had a rapid decrease and barely kept itself in existence in both countries. What is worse, at the period of Japanese colonial rule of korea and our nation's founding of early stage, it has been withered by the high-handed policy to change Oriental Medicine into modern medical science. So the therapy of blood-letting barely kept itself in existence in some Buddhist temples. Another case, it has handed down as a old-fashioned quick fix in folk remedies. But all kinds of the contamination of heavy metals and the misuses of antibiotics are widely spread nowadays, which increased diseases of adult people and incurable diseases as modern society unavoidably made its way into a highly industrial society. To make us more miserable, the western medical science - the antibiotics and surgical operation medical science - already reveals itself into a limit. The necessity of a new medical science which can give a security to the patients who are suffering from the diseases of adult people and the incurable diseases is especially come into the force nowadays. In view of the results after bibliographically studying on the blood-letting Therapy in Elementary Questions of the Yellow Emperor's Canon of Internal Medicine, the blood-letting therapy has acted for the important Oriental medicinal science and has been clarified the prominent effects on the diseases of adult people and the incurable diseases. So it is regarded as an appropriate thing that we lay out a determined theory of the blood-letting therapy and of course prevent the unwanted side effects from inappropriate medicinal treatments, and make full use of clinic by elevating the curative value and that we win back our self-respect of medical treatment which is dominated from the western medical science and ultimately contribute to national medical welfare.

The Present and Future of Children′s Hospice Care in Korea (한국 아동 호스피스의 현황과 전망)

  • Purpose: This study is to identify the present situation of children's hospice and to find the developing strategies for child hospice system in Korea. Method: The data was collected from both literatures and the recent data provided by the government. The direction of future of children's hospice cared in Korea was predicted based on the literature analysis and the report and policy of government. Result: In Korea, the system of the child hospice is not processing. There are the importance differences between children and adult in that the characteristics and approach of the hospice care. All medical personnel and the people related to hospice care including children and their family should be recognized the necessity of the children's hospice care. The following strategies is needed for setting up the child hospice :the principles and standards, recognizing of the necessities, developing of educational program for the specialist and the systemic children's hospice program, and the organization of child hospice. Conclusion: Directions suggested from this study have the importance of child hospice to establish and develop well in Korea for both children with life-threatening disease and their families.

The Use and expenditure of the Complementary and Alternative medicine in Korea (우리 나라 국민의 대체요법 이용 및 비용지출 현황)

  • Objectives : To document the use and out-of-pocket expenditure of complementary and alternative medicine(CAM) in Korean adult population. Methods : Nationwide, random-sampled, and population-weighted telephone survey was conducted. The sample size of respondents was 1,000(age over 18). The respondents were asked about their use, out-of pocket expenditure of CAM in the past 12 months. SAS 8.0 statistic package was used for checking the relevance between each variables by performing $x^2-test$ and variance evaluation. Results : In 2001, 64.0% of people experienced more than one alternative therapy and the beneficiary took therapy average figure of 2.07. Alternative therapies were generally used for health promotion(73.7%) rather than curing the disease(26.3%). The most common therapies included Medication(30.2%), Physical-therapy(21.9%) health implements(20.8%), herb medication(19.2%), diet therapy(14.3%) etc. Average annual out-of-pocket expenditure was £<192,186. Use varied according to age, living province, income, and education, while cost expenditure did according to sex, health condition, income, education. Conclusions : The use of CAM in Korea is very large and the expenditure for them is 22.6% of national medical expenditure. It shows great need of political and academic approach.

Factors Affecting Unmet Healthcare Needs among Adults with Chronic Diseases (만성질환을 가진 성인의 미충족 의료 영향요인)

  • Purpose: In adults with chronic diseases, unmet healthcare needs are likely to increase the risk of worsening the illness and complications. This study was conducted to explore factors affecting unmet healthcare needs among adults with chronic diseases. Methods: A cross sectional study was performed using the data from the seventh Korea National Health and Nutrition Examination Survey (KNHANES VII), 2016-2017. The sample of this study consisted of 6,104 adults with chronic diseases. The data were analyzed by Rao-Scott 𝑥2 test and logistic regression using complex samples analysis. Results: The proportion of subjects who experienced unmet healthcare needs was 10.9% of adults with chronic diseases. The factors affecting unmet healthcare needs of adults with chronic diseases were age, gender, employment status, household income, subjective health status, activity limitation, and perceived stress. Conclusion: The findings of this study suggest that various factors may be associated with their unmet healthcare needs. Finally, the development of strategies to reduce unmet healthcare needs for people with chronic diseases should consider the associated factors presented in the study.

Weight Loss Effect of Wax Gourd (동아의 체중조절 효과)

  • This study attempted to evaluate the effectiveness of function of Wax Ground for obesity and the related disease of adult people can be developed by the combination of the 17 food materials screened in this study. The experiments using Sprague-Dawley male rats show that seventeen kinds of food material inhibited the increase of body weight gain, energy intake, fat pads weight, liver triglycerides content, atherogenic index, and blood glucose concentration induced by cafeteria diet. Oral administration of wax gourd decreased most body weight gain, energy intake, fat pads weight, and blood glucose concentration. Liver triglycerides content was the lowest in the group administered mulberry stem extract and the group administered pine leaves showed the lowest atherogenic index. The combined administration of wax gourd, mulberry stem extract and pine leaves improved more the obesity and related symptoms than the individual administrations.
