Objectives : This study was designed to make beneficial proposal for clinical application on some of the most common disorders treated by Oriental medicine by analyzing treatment process and prognosis. Methods : Number of peculiar attributes pertaining to a specific disorder were analyzed and based on those attributes, patterns associated with process and prognosis were interpreted in reference with classical literatures. Results : 1. Factors which can influence the progression and prognosis include time of onset, intensity of symptoms, course of passage, effects of risk factors, condition of the patient's righteous qi(正氣), accuracy of differential diagnosis made by the practitioner, accuracy of treatment methods, and other unexpected external influences. 2. Correlation between the condition of disorders and treatment progression is closely associated with proper treatment procedures and performances. The time of onset and intensity play critical roles in the treatment process and prognosis and showed pattern tendency with mutual interactions. 3. When there is complication of various disorders, it is ideal to give priority to more urgent illness and take care of moderate illness later. If there isn't any correlation between disorders, treat them in the order of acute to chronic disorders. The approach is reversed when disorders are related, treating in the order of most chronic to most acute. 4. In a case of complication of various disorders, depending on the disorder being acute or chronic, intensity, and accuracy of treatments, either a domino effect or gradual fade out of symptoms were witnessed. 5. The concept of "Five Evils Theory" according to Nan Jing(Difficult Classic) is essential in grasping disease progression due to interrelationships between zangfu organs. Conclusions : Predicting of disease process and prognosis for vast array of disorders treated by Oriental medicine is a very difficult task, yet evaluating the disorder's peculiar properties and influential factors resulted in few principles which can be effectively applied into clinical applications.
Purpose: Women have been often underdiagnosed and undertreated when they have as high mortality as men of ischemic heart disease, such as angina. One of the reasons of women's under treatment is associated with either vague, non-traditional symptoms or longer delay in seeking professional help when they experience ischemic heart disease. This study was conducted to investigate the characteristics of women's anginal pain induced by the treadmill test and to explore the potential relationship(s) between anginal pain and other psycho-physiologic factors. Methods: Of 22 female patients referred to treadmill test, 7 with positive finding participated in this study. Anginal pain in the past was analyzed by Rose questionnaire, whereas anginal pain induced by the treadmill test was identified by McGill pain scale, visual analogue scale and present pain index. Results: Women expressed more heaviness than sharp pain, and complained splitting more often than subjects in previous study that included both men and women. Pain intensity by VAS $3.64{\pm}3.94$, Pain rating index was $5.14{\pm}7.29$, present pain index was $1.57{\pm}1.81$, duration of pain was $5.14{\pm}4.8$ min. Exercise intensity was $6.0{\pm}4.63$ METs, exercise duration was $364.29{\pm}141.39$ sec, ST change was -2.0mm and rate of perceived exertion was $16.29{\pm}2.06$. The relationships between pain intensity and pain duration, ST segment changes were significant. Correlation among pain measures was significant. Conclusion: Generalization of these pilot findings may be inappropriate, and therefore, further larger study is needed.

Vocal vibrato is a characteristic feature in the singing of performers trained in the western classical tradition and vibrato is generally considered to result from modulation in frequency amplitude and timbre. Vocal tremor is also characterized by periodic fluctuations in the voice frequency or intensity and vocal tremor is symptom of a neurological disease as Spasmodic dysphonia , Parkinson's disease. Vocal vibrato and Vocal tremor may have many of the same origins and mechanisms in the voice production systems. The purpose of this study is to find acostic character of Korean traditional song Pansori singer's vibrato and Spasmodic dysphonia patient's vocal tremor. twelve Pansori singers and seven Spasmodic dysponia patients participated to this study. Power spectrum and Real time Spectrogram are used to analyze the acoustic characteristics of Pansori singing and Spasmodic dysphonia patient's voice The results are as follows; First, vowel formant differences between Pansori singing and Spasmodic dysphonia patient's voice are higher F1, F3. Second, The vibrato rate show differences between Pansori singing and Spasmodic dysphonia patients;$4^{\sim}6/sec$ and $5{\sim}6/sec$ Vibrato rate of pitch is 5.7 Hz ${\sim}$ 42.4 Hz for Pansori singing , 3.8 Hz ${\sim}$ 27.9 Hz for Spasmodic dysphonia patients ;Vibrato rate of intensity range is 0.07 dB ${\sim}$ 8.26 dB for Pansori singing and 0.07 dB ${\sim}$ 4.81 dB for Spasmodic dysphonia patients

Background: Some matrix metalloproteinases (MMPs) are involved in invasion and metastasis of head and neck squamous cell carcinoma (HNSCC). However, there are few studies on association between stromelysin-2 (ST-2) and invasive behavior of HNSCC. The purpose of this study was to investigate Stromelysin-2 expression by immunohistochemistry. Materials and Methods: This study was conducted on 81 specimens, including 61 HNSCC and 20 non neoplastic epithelium. Sections with 5 micron thickness were prepared and stained with immunohistochemistry technique. Then expression of ST-2 was evaluated according to percentage of stained cells and intensity of staining. Data were analyzed by SPSS (V.21) using Kruskal-Wallis and Tukey tests (P<0.05). Results: The 61 HNSCC specimens were grades I 36.1%, II 34.4% and III 29.5%. The level of ST-2 expressions were moderate (++) and intensive (+++) in 21.3% and 78.7% of tumors, respectively. The ST-2 expression level was only significant between the tumors with grade I and grade III (P=0.016). Tumors presented ST-2 expression with staining intensity of mild 6.6%, moderate 26.2% and strong 67.2%. Staining intensity of ST-2 in grade I tumors was significantly lower than grade II and grade III (P<0.05), and there was no significant difference between grades II and III (P=0.99). Conclusions: According to this study, the expression of ST-2 is associated with histopathological grade and tumor differentiation in HNSCCs.

Purpose: To study the overall treatment time (OTT) and acute toxicity of intensity-modulated radiotherapy (IMRT) treatment for nasopharyngeal carcinoma (NPC). Methods: This retrospective study covered all NPC patients who underwent radical IMRT treatment at the Penang General Hospital from June 2011 to February 2012. Patients of any age and stage of disease with histologically proven diagnosis were included. Information was collected on patient demographics, clinical stage, treatment received, including any neoadjuvant and/or concurrent chemotherapy, acute toxity and completion of IMRT within the OTT. Results: A total of 26 NPC patients were treated with IMRT during the study period; 88.5% had stage III/IV disease. 45.2% received neo-adjuvant chemotherapy while 50.0% were given concurrent chemo-irradiation. All patients completed the treatment and 92.3% within the 7 weeks OTT. Xerostomia was present in all patients with 92.3% having grade 2. Severe grade III/IV acute toxicity occurred in 73.1% of patients, the commonest of which was oral mucositis (57.6%). This was followed by dysphagia which occurred in 53.8%, skin reactions in 42.3% and weight loss in 19.2%. However, haematological toxicity was mild with only one patient having leucopaenia. Conclusion: IMRT treatment for NPC is feasible in our center. More importantly, it can be delivered within the 7 weeks OTT in the majority of patients. Severe grade 3/4 toxicity is very common (73.1%) and thus maximal nutritional and analgesic support is required throughout the treatment.
This study investigated the effects of high light intensity (100 KLw) and high temperature (45 ℃, dark) on enzyme (glucose-6-phosphate dehydrogenase, acid phosphatase, catalase, peroxidase, and proteinase) activities and characteristics of Panax ginseng C.A. Meyer leaves. Enzyme activity and protein content decreased rapidly under treatment with high light intensity In P ginseng the thermal stabilities of catalase and peroxidase were high (above 70%), and the coagulation rates of soluble proteins were low (below 17%). Therefore, the decrease in enzyme activity and protein content was not caused by increase in leaf temperature due to the high light intensity, but by increase in proteolytic activities. The photochemical formation rate of superoxide radical (O-2) was higher in the P ginseng leaf extracts than in Solanum nigmm, and was accelerated by addition of crude saponin to the buffer extracts.

Purpose: Traditionally, three-dimensional conformal radiation therapy (3D-CRT) is used for neoadjuvant chemoradiation in locally advanced rectal cancer. Intensity-modulated radiation therapy (IMRT) was later developed for more conformal dose distribution, with the potential for reduced toxicity across many disease sites. We sought to use the National Cancer Database (NCDB) to examine trends and predictors for IMRT use in rectal cancer. Materials and Methods: We queried the NCDB from 2004 to 2015 for patients with rectal adenocarcinoma treated with neoadjuvant concurrent chemoradiation to standard doses followed by surgical resection. Odds ratios were used to determine predictors of IMRT use. Univariable and multivariable Cox regressions were used to determine potential predictors of overall survival (OS). Propensity matching was used to account for any indication bias. Results: Among 21,490 eligible patients, 3,131 were treated with IMRT. IMRT use increased from 1% in 2004 to 22% in 2014. Predictors for IMRT use included increased N stage, higher comorbidity score, more recent year, treatment at an academic facility, increased income, and higher educational level. On propensity-adjusted, multivariable analysis, male gender, increased distance to facility, higher comorbidity score, IMRT technique, government insurance, African-American race, and non-metro location were predictive of worse OS. Of note, the complete response rate at time of surgery was 28% with non-IMRT and 21% with IMRT. Conclusion: IMRT use has steadily increased in the treatment of rectal cancer, but still remains only a fraction of overall treatment technique, more often reserved for higher disease burden.

Automatic detection of disease helps medical institutions that are introducing digital images to read images rapidly and accurately, and is thus applicable to lesion diagnosis and treatment. The aim of this study was to apply a symmetry contribution algorithm to unsharp mask filter-applied MR images and propose an analysis technique to automatically recognize brain tumor and edema. We extracted the skull region and drawed outline of the skull in database of images obtained at P University Hospital and detected an axis of symmetry with cerebral characteristics. A symmetry contribution algorithm was then applied to the images around the axis of symmetry to observe intensity changes in pixels and detect disease areas. When we did not use the unsharp mask filter, a brain tumor was detected in 60 of a total of 95 MR images. The disease detection rate for the brain was 63.16%. However, when we used the unsharp mask filter, the tumor was detected in 87 of a total of 95 MR images, with a disease detection rate of 91.58%. When the unsharp mask filter was used in the pre-process stage, the disease detection rate for the brain was higher than when it was not used. We confirmed that unsharp mask filter can be used to rapidly and accurately to read many MR images stored in a database.
The purpose of this study was to examine the effects of manipulating loudness and pitch in terms of speech intelligibility and voice of a patient with Parkinson's Disease. The subject, who was diagnosed as a patient with Parkinson's disease 11 years ago, demonstrated a severely breath voice with low intensity. The accuracy of articulation in consonants was intelligible only at the single word level, and the overall intelligibility in continuous speech was low. The results showed that the subject's articulation accuracy and speech intelligibility was significantly improved after having loudness and pitch training. Habitual Fo, Jitter, Shimmer, Fo tremor, Amp tremor were decreased after training. In addition, the value of HNR also increased after training. It was shown that the changes of these acoustic parameters were closely related to the decrease of breathiness in Parkinson's voice, and this decrease of breathiness affected speech intelligibility considerably. Based on the experimental results, it was claimed that the vocal training by manipulating the loudness and pitch could be highly effective in improving the voice quality and speech intelligibility in Parkinson's Disease.

We evaluated the carbon (C) and nitrogen (N) status of litterfall in a natural red pine (Pinus densiflora) stand damaged by pine wilt disease in Jinju City, which was one of the areas severely affected by the disease in Korea. A significant correlation (P < 0.05) was found between tree density and basal area and the C and N status of litterfall components, but C and N status was not correlated with mean diameter at breast height in the pine wilt disease stands. Needle-litter C and N concentrations were linearly related (P < 0.05) to basal area in pine wilt disease stands. Needle-litter C concentration decreased with a decrease in damage intensity due to pine wilt disease, whereas litter N concentration increased with an increase of basal area in pine wilt disease stands. The linear regression equations developed for litterfall C and N inputs were significant (P < 0.05), with basal area accounting for 50-86% of the variation, except for cone and flower litter. The results indicated that the incidence of pine wilt disease could impact the quality and quantity of C and N in litterfall of pine stands suffering from pine wilt disease.
