Background: Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by synovial hyperplasia and joint destruction. The synovial fibroblasts express cell adhesion molecules and have a role in adhesive interation with inflammatory cells in synovial tissue. It has been suggested that hypoxic conditioins are thought to exist in arthritic joints, and several studies indicate that reactive oxygen species (ROS) produced in hypoxic condition can initiate events that lead to pro-adhesive changes via increased expression of adhesion molecules. So, this study wsa designed to examine whether antioxidant can inhibit hypoxia-induced expression of ICAM-1 in cultured human synovial fibroblasts. Methods: Synovial fibroblasts were isolated from synovial tissue in patients with RA and cultured at hypoxic condition. Antioxidant, PDTC (pyrrolidine dithiocarbamate) were pre-treated for an hour before the hypoxic culture and synovial fibroblasts were harvested at 0, 6, 12, 24, 48 hours time points. Cell surface ICAM-1 expression in synovial fibroblasts was examined by the flow cytometric analysis. To analyse the expression of ICAM-1 mRNA, reverse-transcriptase polymerase chain reaction (RT-PCR) was performed. The levels of cytokines in culture supernatants were measured by ELISA, and activation of NF-${\kappa}B$ was analysed by electrophoretic mobility shift assay. The adhesive reaction between synovial fibroblasts and lymphocytes was assayed by measurement of fluorescent intensity of BCECF-AM in lymphocytes. Results: Hypoxic stimuli up-regulated the ICAM-1 expression as well as the adhesive interaction of human synvial fibroblasts to lymphocytes in a time-dependent manner, and PDTC inhibited hpyoxia-induced ICAM-1 expression and cell-cell interaction. PDTC also inhibited the hypoxia-induced activation of intracellular transcription factor, NF-${\kappa}B$. PDTC decreased the amount of hypoxia-induced production of IL-$1{\beta}$ and TNF-${\alpha}$. Conclusion: These studies demonstrate that PDTC inhibit the hypoxia-induced expression of the adhesion molecule, ICAM-1 and activation of NF-${\kappa}B$ in cultured human synovial fibroblasts.
Objectives: The purpose of this study is to identify prevalence and related factors of the elderly, who took health examination, with metabolic syndrome. Methods: The health examination and lifestyle survey were performed for 21,512 adults at 60 years of age or older who took health examination in H health promotion center during January-March 2009. Results: The prevalence of metabolic syndrome for the subject was 24.0%. Of the subject with metabolic syndrome, the prevalence of the diseases was obesity 60%, abdominal obesity 78.5%, hypertension 82.6%, dyslipidemia 89.7% and diabetes 51.9%. In comparison of the relationship between metabolic syndrome and other diseases, the male subject with metabolic syndrome were significantly higher in BMI, waist circumference, systolic/diastolic blood pressure, hemoglobin, AST, ALT, $\gamma$-GTP, TG, AC glucose, creatinine than normal male(p<0.001). In comparison of the relationship between metabolic syndrome and lifestyle, more drinking frequency and amount in male and more drinking frequency in female were associated with increased risk of metabolic syndrome(p<0.01). Regardless of exercise intensity, practice of exercise contributed to reduce the risk of metabolic syndrome(p<0.01). Conclusion: In conclusion, TLC program, focused on lifestyle behaviors which is strongly associated with the prevalence of metabolic syndrome, should be developed for the improvement of life quality in the elderly with metabolic syndrome.
Photoplethysmogram (PPG) and pressure pulse waveform (PPW) were compared and evaluated for the efficacy of stimulating knuckles by using the pulsed magnetic field. Both signals were observed simultaneously while the knuckles were exposed for 10 min to the pulsed magnetic field, with maximum field intensity of 0.8 T and transition time of 0.126 msec. After 5 min stimulation of the knuckles, the results showed that the aging indexes calculated from the second derivative of the PPG were increased from -1.913 to 0.072, and that of the PPW from -0.063 to 0.387. However, for the relatively long-term stimulation for 10 min, we found that the values of both the aging indexes of the second derivatives and augmentation index of the PPW returned to the starting level. The changes observed in characteristic factors such as the aging indexes of the second derivatives and augmentation index of the PPW indicate the potential of pulsed magnetic field stimulation as a therapeutic method for the treatment of patients with peripheral vascular disease.
Immunostaing intensity of Dynamin II protein in ameloblast during mouse tooth development showed a significant increase of 48% at the postnatal day 3 and a significant increase of 50% at the postnatal day 5 as compared with the postnatal day 1, but showed a significant decrease of 16% at the postnatal day 7 and a significant decrease of 12% at the postnatal day 10 as compared with the postnatal day 1. From the above results, Dynamin II had relevance to secretion of amelogenin, ameloblastin, enamelin and matrix metalloproteinase-20 proteins for enamel formation in ameloblast. Dynamin II may be involved in the transport of vesicles containing proteins for enamel formation through the acceleration of vesicular formation and may be had a good possibility of secretory regulation of proteins for enamel formation in ameloblast. Therefore, Dynamin II have potential for being used in the field of gene theraphy for periodontal disease and in the regeneration for enamel and dentin tissues lost to dental caries.

Squamous cell carcinoma is a neglected disease entity in orthopedic oncology. The purpose of this study was to analyze overall survival and the role of surgery on survival and to evaluate the significance of possible prognostic factors. From Oct, 1986 to Aug, 1996, 57 patients were enlisted and 42 patients ere eligible. Inclusion criteria included more than one year follow-up and no distant metastasis at the first visit. Staging and survival followed AJC classification and Kaplan-Meier plot. Stage II included 17 cases and stage III, 25 cases. Thirty-eight patients underwent operations, chemotherapy, and/or radiotherapy, and the remaining four had operations only. The chemotherapeutic regimen was adriamycin-cisplatin. The average follow-up period was 45 months. The ten-year actuarial survival rate of whole patients was 65.4%. Location of primary lesion, stage, pathologic grading, and intensity of chemotherapy in the same stage showed a significant difference in survival. Nine out of 42 patients had local recurrence. Seven patients had inadequate wide margins and two had intralesional margins. Average period of recurrence from operation was 13(4-35)months. The operation itself had no impact on survival but a surgical margin of no less than 3cm from the lesion was important for local control. Pathological grade and staging were significant variables for long term survival. Acral lesion had a significantly higher chance of regional and distant metastasis but actual survival showed no difference. In stage II, aggressive chemotherapy could delay or reduce the chance of regional or distant metastasis.
For investigating a disease specific psychological mechanism in various dermatological disorders, we tried to explore the difference in correlation among stress perception, locus of control as a coping strategy, depressive symptoms and pruritic symptoms in the patients with psychological pruritis and chronic urticaria. The subjects were composed of 32 patients with psychol-ogical pruritis and 67 patients with chronic urticaria(subject group), and 25 patients with organic pruritis and 59 patient with major depression(control group). Global assesment of recent stress scale(GARS), I-E locus of control scale, Beck Depression Inventory(BDI) and medical college of wisconsin center pain follow up questionnaire were used for assesement. The results were as follows: 1) There were a significant positive correlation between stress perception and depressive symptom in patients with psychological pruritis, chronic urticaria, and major depression but not hi organic pruritis. 2) In relationship between locus of control and depressive symptom, patients with psychological pruritis, organic pruritis, and major depression except chronic ruticaria showed a significant negative correlation. 3) For intensity and pattern of pruritis, there were positive relations with depressive symptom and stress perception only in patients with chronic urticaria. Above results indicate that stress perception and locus of contorl may play a significant role in the formation of psychological and dermatological symptoms in psychological pruitis and chronic urticaria. But the mechanisms of these processes are different in either disorder.

For this, in this paper, we would like to develop child ocular inspection, auscultation diagnosis instruments, using Oriental medicine principle that living body signal of five organs and six hallow organs which reflects patients face and voice We would like to get more accurate diagnosis result for child's symptoms from doctor's intuition on the basis of diagnostic sight visualization, objectification, quantization itself. This paper develops color revision, YCbCr application, and face color selection and five sensory organs and nose or apex extraction method etc, in child ocular inspection by first work achievement sequence among the whole development systems. Also, in occasion of child auscultation, crying characteristics of colics through pitch, intensity and formant analysis is numerized and objectifies doctor's intuition through this. Finally, experiments are performed to verify the effectiveness of the proposed methods.

There is much confusion in the field of Rhinitis regarding how to best measure disease severity objectively, Therefore, we aimed to establish a new adequate scoring system for Rhinitis, that should be based on comparison analysis of various scoring systems. We report as follows. We researched for data relating to severity scoring systems for rhinitis in Entrez PubMed from 1995 to 2005 and in Kiss Kstudy. Results and Conclusions: Properties of severity scoring systems were validity, sensitivity of change and ease of use. The essential items of severity scoring systems were subjective symptoms. The criterion of severity were divided into subjective symptoms and complication and Quality of Life. Intensity items are nasal obstruction, rhinorrhea, sneezing, itching, Postnasal drip, nasal mucosa swelling, nasal mucosa color, complication. Subjective symptoms is difficulty of Life. The significant items of severity scoring system are nasal symptoms. The whole score does with the maximum 30 scores. As it were, we assumed nasal symptoms accounted for around 80% of each total score, with complication and difficulty of Life representing 20%.
Background: The effect of lumbar spinal stenosis (LSS) and peripheral vascular disease (PVD), which occurs with similar degenerative conditions, when seen together, has not been studied. The aim of this study is to examine and compare the relationship between pain, balance, disability, fear of falling, and kinesiophobia in LSS patients with intermittent vascular claudication (IVC). Methods: Seventy-two patients diagnosed with LSS using magnetic resonance imaging participated in this study. Thirty-five patients with IVC symptoms and showing vascular lesions by lower extremity venous and arterial Doppler ultrasonography imaging were included in the IVC-LSS group. The pain, static balance, dynamic balance, disability, fear of falling, and kinesiophobia were evaluated using the numeric rating scale, single leg stance test, Time Up and Go (TUG), the Oswestry Disability Index (ODI), Fall Efficacy Scale-International (FES-I), and Tampa Scale for Kinesiophobia (TSK), respectively. Results: Age and female sex were found to be higher in the IVC-LSS group (P = 0.024; P = 0.012). The IVC-LSS group had a shorter single leg stance time and TUG test duration, pain intensity, ODI, FES-I, and TSK scores were higher than patients with LSS (P = 0.001). Pain, fear of falling, and kinesiophobia were moderately correlated with disability in the IVC-LSS group. No relationship was found between pain and dynamic balance. Also, the pain was not related to kinesiophobia. Conclusions: The findings indicated that IVC causes loss of balance and an increase in pain, disability, fear of falling, and kinesophobia in patients with LSS.

Malaria remains one of the leading health burdens in the developing world, especially in several sub-Saharan Africa countries; and Uganda has some of the highest recorded measures of malaria transmission intensity in the world. It is evident that the prevalence of malaria infection, the incidence of disease, and mortality from severe malaria remain very high in Uganda. Although the recent stable political and economic situation in the last few decades in Uganda supported for a fairly good appreciation of malaria control, the declines in infection, morbidity, and mortality are not sufficient to interrupt transmission and this country is among the top 4 countries with cases of malaria, especially among children under 5 years of age. In fact, Uganda, which is endemic in over 95% of the country, is a representative of challenges facing malaria control in Africa. In this study, we evaluated an active case detection program in 6 randomly selected villages, Uganda. This program covered a potential target population of 5,017 individuals. Our team screened 12,257 samples of malaria by active case detection, every 4 months, from February 2015 to January 2017 in the 6 villages (a total of 6 times). This study assessed the perceptions and practices on malaria control in Kiyuni Parish of Kyankwanzi district, Uganda. Our study presents that the incidence of malaria is sustained high despite efforts to scale-up and improve the use of LLINs and access to ACDs, based on the average incidence confirmed by RDTs.
