Diagnosis of Unruptured Intracranial Aneurysms Using Proton-Density Magnetic Resonance Angiography: A Comparison With High-Resolution Time-of-Flight Magnetic Resonance Angiography

  • Objective: Differentiating intracranial aneurysms from normal variants using CT angiography (CTA) or MR angiography (MRA) poses significant challenges. This study aimed to evaluate the efficacy of proton-density MRA (PD-MRA) compared to high-resolution time-of-flight MRA (HR-MRA) in diagnosing aneurysms among patients with indeterminate findings on conventional CTA or MRA. Materials and Methods: In this retrospective analysis, we included patients who underwent both PD-MRA and HR-MRA from August 2020 to July 2022 to assess lesions deemed indeterminate on prior conventional CTA or MRA examinations. Three experienced neuroradiologists independently reviewed the lesions using HR-MRA and PD-MRA with reconstructed voxel sizes of 0.253 mm3 or 0.23 mm3, respectively. A neurointerventionist established the gold standard with digital subtraction angiography. We compared the performance of HR-MRA, PD-MRA (0.253-mm3 voxel), and PD-MRA (0.23-mm3 voxel) in diagnosing aneurysms, both per lesion and per patient. The Fleiss kappa statistic was used to calculate inter-reader agreement. Results: The study involved 109 patients (average age 57.4 ± 11.0 years; male:female ratio, 11:98) with 141 indeterminate lesions. Of these, 78 lesions (55.3%) in 69 patients were confirmed as aneurysms by the reference standard. PD-MRA (0.253-mm3 voxel) exhibited significantly higher per-lesion diagnostic performance compared to HR-MRA across all three readers: sensitivity ranged from 87.2%-91.0% versus 66.7%-70.5%; specificity from 93.7%-96.8% versus 58.7%-68.3%; and accuracy from 90.8%-92.9% versus 63.8%-69.5% (P ≤ 0.003). Furthermore, PD-MRA (0.253-mm3 voxel) demonstrated significantly superior per-patient specificity and accuracy compared to HR-MRA across all evaluators (P ≤ 0.013). The diagnostic accuracy of PD-MRA (0.23-mm3 voxel) surpassed that of HR-MRA and was comparable to PD-MRA (0.253-mm3 voxel). The kappa values for inter-reader agreements were significantly higher in PD-MRA (0.820-0.938) than in HR-MRA (0.447-0.510). Conclusion: PD-MRA outperformed HR-MRA in diagnostic accuracy and demonstrated almost perfect inter-reader consistency in identifying intracranial aneurysms among patients with lesions initially indeterminate on CTA or MRA.

Optimal Localization through DSA Distortion Correction for SRS

  • 신경 외과적 수술의 한분야인 정위적 방사선 수술은 두 개강 내의 병변의 위치 계산 후, 고선량의 방사선을 조사하여 병변을 치료하는 방법이기 때문에, 효과적인 수술을 위해서는 병변의 정확한 위치 정보가 무엇보다도 중요하다. 본 연구에서는 DSA(Digital Subtraction Angiography) 영상이 내재적으로 이미지 왜곡이라는 문제점을 가지고 있기 때문에, 이것의 보정을 통하여 더욱 정확한 target 위치를 계산하였다 이미지 왜곡을 보정하기 위하여 grid 팬텀을 제작하였고, localization 알고리즘의 정확도를 평가하기 위하여, target 팬텀을 제작하였다. Image Intensifier의 앞쪽에 grid 팬텀을 부착하고, target 팬텀을 Leksell Frame에 고정시킨 후, DSA 영상을 얻었다. 본 실험을 위하여 개발된 프로그램을 이용하여, Anterior and Posterior, Left and Right 영상에서 bilinear transform을 적용하여 왜곡을 보정한 후, target 위치를 계산하였다. 그리고, 이와 같은 방법을 통하여 계산된 target 위치 좌표와 target 팬텀의 절대 좌표의 비교를 통하여 localization 오차가 계산되었다. 이번 실험의 결과는 왜곡을 보정하지 않은 경우, localization 오차는 $\pm$0.41mm, 왜곡 보정을 한 경우는 $\pm$0.34mm이었다. 따라서 본 연구에서 개발된 알고리즘 정밀도가 인정되며, 환자의 치료에 적합한 것으로 사료된다.

Slice Interpolation기법의 고해상도 자기공명혈관조영술을 이용한 뇌동맥류의 진단 : 디지탈 감산 혈관조영술과 자기공명 혈관조영술의 비교 (Evaluation of Cerebral Aneurysm with High Resolution MR Angiography using Slice Interpolation Technique: Correlation wity Digital Subtraction Angiography(DSA) and MR Angiography(MRA))

  • 목적: 자기공명혈관조영술(magnetic resonance angiography: MRA)을 이용하여 뇌동맥류를 비침습적 방법으로 진단하고자 하는 시도가 있었지만 크기가 3mm 이하인 경우 진단에 애로가 많았다. 뇌동맥류의 진단에 있어 최근 개발된 고해상도, 고속의 slice interpolation 자기 공명혈관조영술과 디지탈 감산 혈관조영술(digital subtraction angiography: DSA)의 결과를 전향적으로 비교하여 보편적인 선별 검사법으로 가능성을 확인하고자 하였다. 대상 및 방법: 총 19명의 환자에서 26개의 뇌동맥류를 대상으로 하였다. 모두 자기공명혈관조영술을 먼저 촬영하여 기원혈관, 동맥류경부의 확인, 인근 작은 혈관과의 연관관계등을 확인한 후 디지탈 감산 혈관조영술을 시행하여 비교하였다. 영상은 1.5T 초전도형기계(Vision, Siemens, erlangen, Germany)를 이용하여 slice interpolation 을 이용한 자기공명혈관조영술로 촬영하였다. 촬영은 TR / TE / FA = 30 / 6.4 / 25, matrix $512{\times}160$, 관찰면 $200{\times}150$, 촬영시간 7분 42초, 유효 두께 0.7mm, 총 두께가 102.2mm로 하여 대공(foramen magnum)에서 전뇌동맥(anterior cerebral artery)의 A3 부위까지 충분히 포함되도록 하였다. 영상분석은 최대강도투사(maximum intensity projection: MIP)를 사용하였으며 두개강내 동맥류가 있는 경우 다면재구성(multiplanar reconstruction: MPR) 기법을 사용하였다. 결과: 19명의 환자중 2명이 3개, 3명이 2개, 나머지 14명이 각각 1개씩의 두개강내 동맥류를 가져 모두 26개 였으며 파열된 동맥류가 14개였고 파열되지 않은 동맥류가 12 개이었다. 크기가 2mm 이하가 8개, 3-5mm가 9개, 6-9mm가 7개이며 10mm이상이 2개가 있었다. 처음 검사에서 자기공명혈관조영술과 디지탈 감산 혈관조영술에서 23개의 동맥류중 내경동맥에 1mm 크기의 동맥류 1개를 제외한 25개를 각각 발견할 수 있어 96%의 예민도를 보였으나 진음성과 위음성은 없어 특이도를 측정할 수 없었다. 크기와 모양을 확인하는데 자기공명혈과조영술과 다면재구성을 동시에 사용한 경우 디지탈 감산 혈관조영술과 같은 성적을 보인 반면 동맥류 경부와 기원혈관을 확인하는데도 자기공명혈관조영술의 다면재구성을 동시에 사용한 경우가 자기공명혈관조영술 또는 디지탈 감산 혈관조영술만 사용한 경우 보다 월등히 좋았다. 결론: Slice interpolation 기법을 이용한 고해상 자기공명혈관조영술은 두개강내 동맥류를 검사하는데 디지탈 감산 혈관조영술과 동일한 성적을 보여 앞으로 비침습적 일차 선별 검사법으로 가능할 것으로 생각된다.

Internal Carotid Artery Reconstruction Using Multiple Fenestrated Clips for Complete Occlusion of Large Paraclinoid Aneurysms

  • Objective : Although surgical techniques for clipping paraclinoid aneurysms have evolved significantly in recent times, direct microsurgical clipping of large and giant paraclinoid aneurysms remains a formidable surgical challenge. We review here our surgical experiences in direct surgical clipping of large and giant paraclinoid aneurysms, especially in dealing with anterior clinoidectomy, distal dural ring resection, optic canal unroofing, clipping techniques, and surgical complications. Methods : Between September 2001 and February 2012, we directly obliterated ten large and giant paraclinoid aneurysms. In all cases, tailored orbito-zygomatic craniotomies with extradural and/or intradural clinoidectomy were performed. The efficacy of surgical clipping was evaluated with postoperative digital subtraction angiography and computed tomographic angiography. Results : Of the ten cases reported, five each were of ruptured and unruptured aneurysms. Five aneurysms occurred in the carotid cave, two in the superior hypophyseal artery, two in the intracavernous, and one in the posterior wall. The mean diameter of the aneurysms sac was 18.8 mm in the greatest dimension. All large and giant paraclinoid aneurysms were obliterated with direct neck clipping without bypass. With the exception of the one intracavenous aneurysm, all large and giant paraclinoid aneurysms were occluded completely. Conclusion : The key features of successful surgical clipping of large and giant paraclinoid aneurysms include enhancing exposure of proximal neck of aneurysms, establishing proximal control, and completely obliterating aneurysms with minimal manipulation of the optic nerve. Our results suggest that internal carotid artery reconstruction using multiple fenestrated clips without bypass may potentially achieve complete occlusion of large paraclinoid aneurysms.

Four-Year Experience Using an Advanced Interdisciplinary Hybrid Operating Room : Potentials in Treatment of Cerebrovascular Disease

  • Objective : To describe our experiences with a fully equipped high-end digital subtraction angiography (DSA) system within a hybrid operating room (OR). Methods : A single-plane DSA system with 3-dimensional rotational angiography, cone-beam computed tomography (CBCT), and real-time navigation software was used in our hybrid OR. Between April 2014 and January 2018, 191 sessions of cerebrovascular procedures were performed in our hybrid OR. After the retrospective review of all cases, the procedures were categorized into three subcategorical procedures : combined endovascular and surgical procedure, complementary rescue procedure during intervention and surgery, and frameless stereotaxic operation. Results : Forty-nine of 191 procedures were performed using hybrid techniques. Four cases of blood blister aneurysms and a ruptured posterior inferior cerebellar artery aneurysm were treated using bypass surgery and endovascular trapping. Eight cases of ruptured aneurysm with intracranial hemorrhage (ICH) were treated by partial embolization and surgical clipping. Six cases of ruptured arteriovenous malformation with ICH were treated by Onyx embolization of nidus and subsequent surgical removal of nidus and ICH. Two (5.4%) of the 37 cases of pre-mature rupture during clipping were secured by endovascular coil embolization. In one (0.8%) complicated case of 103 intra-arterial thrombectomy procedures, emergency surgical embolectomy with bypass surgery was performed. In 27 cases of ICH, frameless stereotaxic hematoma aspiration was performed using $XperGuide^{(R)}$ system (Philips Medical Systems, Best, the Netherlands). All procedures were performed in single sessions without any procedural complications. Conclusion : Hybrid OR with a fully equipped DSA system could provide precise and safe treatment strategies for cerebrovascular diseases. Especially, we could suggest a strategy to cope flexibly in complex lesions or unexpected situations in hybrid OR. CBCT with real-time navigation software could augment the usefulness of hybrid OR.

2D 혈관조영술에서 직접 측정한 혈관 직경과 MR 영상에서 단면적 기반 환산 직경의 비교 분석 (Comparative Analysis between Directly Measured Diameter in 2D Angiography and Cross-Sectional Area-Converted Diameter in MR Image)

  • This study aimed to quantitatively compare the diameters measured directly from the coronal plane or sagittal plane of 2D digital subtraction angiography (DSA) and the cross-sectional area-converted diameters calculated from contrast-enhanced MR (CE-MR) imaging. A retrospective analysis was conducted on 20 patients who underwent both 2D DSA and CE-MR imaging. Firstly, the venous diameters of the superior sagittal sinus (SSS) and transverse sinus (TS) were directly measured from 2D DSA. Subsequently, the axial planes for SSS diameter and the sagittal plane for TS in CE-MR imaging were utilized to calculate cross-sectional area-based converted diameters. The numerical values obtained from 2D DSA and CE-MR imaging were compared pairwise at each location. For SSS, the diameter measured by 2D DSA was 27% larger than the conversion-based diameter from CE-MR imaging (9.8±1.4 mm vs. 7.1±1.3 mm, P<0.05). Similarly, for the right TS, the difference was 16% (8.8±3.2 mm vs. 7.4±2.0 mm, P<0.05), and for the left TS, the difference was 22% (8.4±2.8 mm vs. 6.6±1.3 mm, P<0.05). In conclusion, the diameter measured directly in conventional 2D DSA may be larger than the diameter converted based on the cross-sectional area. Therefore, when selecting the size of the stent, it is crucial to make precise determinations while keeping this fact in mind.

외상 후 발생한 얕은관자동맥 가성동맥류의 치험례 (A Case Report of Posttraumatic Pseudoaneurysm of the Superficial Temporal Artery)

  • Posttraumatic pseudoaneurysm of the superficial temporal artery is very rare and occurs secondary to trauma. Clinical diagnosis is based on past history of trauma and physical examination and can be confirmed by duplex ultrasonogram, digital subtraction angiography, CT and MRI. Ligation of proximal and distal ends of the superficial temporal artery and excision of the pseudoaneurysm has been the standard treatment. Compressive therapy, endovascular coil embolization, percutaneous thrombin injection under ultrasound guidance have been reported as alternative treatment methods. When surgical excision of the superficial temporal artery pseudoaneurysm is performed, surgeon must be concerned about the anatomical relation between superficial temporal artery and temporal branches of the facial nerve. In this article, we report a rare case of superficial temporal artery pseudoaneurysm with some review of the literatures about anatomical relation between superficial temporal artery and temporal branches of facial nerve.

External Carotid Artery Angioplasty and Stenting Followed by Superficial Temporal Artery to Middle Cerebral Artery Anastomosis

  • A 31-year-old man presented with right hemiparesis, and magnetic resonance imaging revealed a small infarct at left basal ganglia. Digital subtraction angiography showed left cervical internal carotid artery (ICA) occlusion and severe stenosis of the ipsilateral external carotid artery (ECA) with collateral cerebral circulation fed by ECAs. Based on the results of a functional evaluation of cerebral blood flow, we performed preventive ECA angioplasty and stenting for advanced ECA stenosis to ensure sufficient blood flow to the superficial temporal artery. Eight weeks later, superficial temporal artery to middle cerebral artery (STA-MCA) anastomosis was performed. His postoperative course was uneventful and no additional transient ischemic attacks have occurred. To our knowledge, this is the first report of preventive angioplasty and stenting for advanced narrowing of an ECA before STA-MCA anastomosis for ipsilateral ICA occlusion.

Cavernous Hemangioma in the Middle Cranial Fossa & Cavernous Sinus

  • Extracerebral cavernous hemangiomas are rare vascular tumors that are very difficult to remove because of severe intraoperative bleeding. We report a case of 57-year-old male with extracerebral cavernous hemangioma with review of 126 cases in the literature. Patient presented with blurred vision, diplopia, numbness on the left side of his face. Magnetic resonance imaging revealed a well defined mass of $3{\times}4{\times}3cm$ size with heterogenous iso-or hypointensity on T1-weighted image showing strong homogenous contrast enhancement and marked hyperintensity on T2-weighted image. Digital subtraction angiography[DSA] revealed a faint tumor blush by feeders from the left internal carotid artery[ICA] and left external carotid artery[ECA] in the delayed phase. Even with profuse intratumoral bleeding, near total removal was achieved. In addition to preoperative neurologic deficits such as ophthalmoplegia, facial numbness in the V1-2 dermatomes, ptosis appeared postoperatively.

Ruptured Total Intrameatal Anterior Inferior Cerebellar Artery Aneurysm

  • Among the distal anterior inferior cerebellar artery (AICA) aneurysms, a unique aneurysm at the meatal loop inside the internal auditory meatus is extremely rare. The authors report a case of surgically treated total intrameatal AICA aneurysm. A 62-year-old female patient presenting with sudden bursting headache and neck pain was transferred to our department. Computed tomography and digital subtraction angiography showed subarachnoid hemorrhage at the basal, prepontine cistern and an aneurysm of the distal anterior inferior cerebellar artery inside the internal auditory meatus. Surgery was performed by retrosigmoid craniotomy with unroofing of the internal auditory meatus. The aneurysm was identified between the seventh and eighth cranial nerve in the meatus and was removed from the canal and clipped with a small straight Sugita clip. After operation the patient experienced transient facial paresis and tinnitus but improved during follow up.
