A Subjectivity Study on the Meaning of Aging for Elders (노인의 의미에 대한 주관성 연구)

  • This study is designed to investigate what elders think about the meaning of aging. We have used Q-methods to identify meaning of aging from elders, and developed self-referenced surveys to analyze characteristics In this study, we used a non-structured method to select Q sampling. From 183 Q populations, we selected 36 Q sampling. A total of 32 persons sixty-years or older were randomly selected for P samples, When the Q-sorting was complete, we interviewed the participants at both end of the extremes(agree or disagree), and documented their responses. We used PC QUANL to process the data and used principal component analysis for Q factor analysis. There were five subjective types for the meaning of aging by elders. Of the 32 P-samples of elders, 11 were identified as Type 1, 7 as Type 2, 2 as Type 3, 8 as Type 4, and 4 as Type 5. Type 1 : 'Matured elders' Elders wished the well being of their children, thought older persons should maintain good health, worried about becoming senile, and dependent God believing in life after death. Type 2 : 'Assertive-Rights' Elders categorized as Assertive-Rights insisted on their rights to life as a person. Type 2 elders characterized themselves as people who should keep themselves healthy, become weak and lack sexual desires, act selfish like a child, need to be protected, and be financially independent. Type 3 : 'Passive-Dependents' Elders characterize themselves as those who pray for their children's well being, worry about the children even after their death. and becoming senile. Type 4 : 'Hopeless' The 'Hopeless' type of elders characterized aging as a time to pray for their children, insignificant beings, thoughts were selfish and child-like, poor, worried about going senile, regret their life overall, and preferred to die than to live as an old person. Type 5 : 'Attached-Present' The 'Attached-Present' type of elders thought elderly characterized themselves as acting selfish and child-like, wiser, anxious, regret their life, stand aloof of greed and worldly things, being a model for the society, and deserving to be treated with filial respect. Thus far, Korean elders seemed to have a positive and negative meaning of aging due to the current changes in the society, value system, and family structures. The above five subjective meanings of aging confirm that we need to approach and nurse the elderly differently. Years of aging are a part of and a natural process of life with various physical, psychological, and sociological changes. Nurses need to assist elderly to find the positive meaning of their life by providing appropriate physical, psychological, and social support at an earlier stage in nursing. Based on this study, we could derive the following two implication from the perspectives of science of nursing to care for elders. 1) Based on the studies investigating the type of meaning of aging, we could develop tools to assist in nursing intervention programs for elderly. 2) Based on research on the meaning of aging for different developmental stages of life, we could develop a model for roles for different family members in nursing and caring for the elders.

Aberrant Expression of Markers of Cancer Stem Cells in Gastric Adenocarcinoma and their Relationship to Vasculogenic Mimicry

  • Background: Gastric cancer is the second leading cause of cancer-related death in Asia, and the majority type is gastric adenocarcinoma (GAC). Most GAC patients die of recurrence and metastasis. Cancer stem cells (CSCs) have been thought to be responsible for the initiation, development, metastasis, and ultimately recurrence of cancer. In this study, we aimed to investigate expression and clinical significance of CSCs markers, CD133 and Lgr5, and vasculogenic mimicry (VM) in primary GAC. Materials and Methods: Specimens from 261 Chinese patients with follow-up were analyzed for CD133, Lgr5 protein expression and VM by immunohistochemical and histochemical staining. The Pearson Chi's square test was used to assess the associations among the positive staining of these markers and clinicopathological characteristics. Postoperative overall survival time was were studied by univariate and multivariate analyses. Results: In GAC tissues, positive rates of 49.0%, 38.7%, and 26.8% were obtained for CD133, Lgr5, and VM, respectively. The mean score of microvessel density (MVD) was $21.7{\pm}11.1$ in GAC tissues. There was a significantly difference between the positive and negative groups. There was a positive relationship between the VM, the expression of CD133 and Lgr5, and the score of MVD and the grades of tumor, lymph node metastasis, TNM stages (all p<0.05). The overall mean survival time of the patients with CD133, Lgr5, VM, and MVD (${\geq}22$) positive expression was lower than that of patients with negative expression. The score of MVD, positive expression of CD133 and VM were independent prognostic factors of GAC (p<0.05). Conclusions: VM, and expression of CD133, Lgr5, and the score of MVD are related to grades of tumor, lymph node metastasis, TNM stages, and overall mean survival time. It is suggested that CSCs and VM could play an important role in the evolution of GAC.

Prognostic Value of Leptin in Terminally Ill Cancer Patients (말기암환자의 여명 예측 요인, 혈중 렙틴 농도의 효과)

  • Purpose: Most terminally ill cancer patients die from cancer anorexia-cachexia syndrome. This study evaluated a prognostic role of plasma leptin levels in terminally ill cancer patients. Methods: This study enrolled 69 terminally ill cancer patients who were aged above 20 years old from July 2009 to July 2010. For univariate analysis, an association between leptin levels and patient's characteristics or other variables was examined using Spearman's correlation analysis, Wilcoxon's rank-sum test or Kruskal-Wallis test, as appropriately. For multivariable analysis, Cox's proportional hazard regression model was used to evaluate a clinical significance of plasma leptin levels as a prognostic factor and to determine factors which affect the risk of death in terminally ill cancer patients. Results: A statistically significant positive correlation between plasma leptin levels and survival time was found. Univariate Cox's proportional hazard regression analyses also showed a moderately significant association between plasma leptin levels and survival time. However, after adjusting variables for sex, white blood cell counts, total bilirubin, AST, ALT, albumin and CRP levels, plasma leptin levels were not significantly associated with survival time. Conclusion: No significant association was found between plasma leptin levels and survival time in terminally ill cancer patients. However, this study suggested a prognostic value of plasma leptin levels in gastrointestinal cancer patients.

Biogeochemistry of Metal and Nonmetal Elements in the Surface Sediment of the Gamak Bay (가막간 표층퇴적물 중의 금속 및 비금속 원소의 생지화확적 분포특성)

  • In order to evaluate die geochemical characteristics of sediment in a semi-enclosed bay used as shellfish and fish farming area, the concentrations of metallic(V, Cr, Mn, Fe, Co. Ni, Cu, Zn, Ag, Cd, Hg, Ph, As) and non-metallic(P, Se) elements and uranium were measured in the surface sediment samples collected from 19 stations of Gamak Bay in April 2010. Metal contamination status in the sediments were also evaluated using the sediment quality guidelines(SQGs) proposed by the National Oceanic and Atmospheric Administration(NOAA) and the enrichment factor(EF). The concentrations of elements in sediment were mainly controlled by quartz-dilution effect(V, Cr, Fe, Co and Ni), the dilution effect of organic matter(Cd and U), and metal redistribution by the decomposition of organic matter(Mn, Ag, As, and Se). The concentrations of metals, except As and Ni, in sediments from all sampling stations were lower than ERL values of NOAA. Conclusively, the surface sediment of Gamak Bay was slightly polluted with Ni, Ag, Cd, and Cd but was not polluted with other elements on the basis of EF results. Our results suggest that the surface sediment in Gamak Bay is not polluted by metallic elements.

A Study on the Thermo-Mechanical Stress of MEMS Device Packages (마이크로 머신(MEMS) 소자 패키지의 열응력에 대한 연구)

  • Unlike common device, MEMS(micro-electro-mechanical system) device consists of very small mechanical structures which determine the performance of the device. Because of its small mechanical structure inside. MEMS device is very sensitive to thermal stress caused by CTE(coefficient of thermal expansion) mismatch between its components. Therefore, its characteristics are affected by material properties. process temperature. and dimensions of each layer such as chip, adhesive and substrate. In this study. we investigated the change of the thermal stress in the chip attached to a substrate. With computer-aided finite element method (FEM), the computer simulation of the thermal stress was conducted on variables such as bonding material, process temperature, bonding layer thickness and die size. The commercial simulation program, ABAQUS ver5.6, was used. Subsequently 3-layer test samples were fabricated, and their degree of bending were measured by 3-D coordinate measuring machine. The experimental results were in good agreement with the simulation results. This study shows that the bonding layer could be the source of stress or act as the buffer layer for stress according to its elastic modulus and CTE. Solder adhesive layer was the source of stress due to its high elastic modulus, therefore high compressive stress was developed in the chip. And the maximum tensile stress was developed in the adhesive layer. On the other hand, polymer adhesive layer with low elastic modulus acted as buffer layer, and resulted in lower compressive stress. The maximum tensile stress was developed in the substrate.

Effects of Emulsifier Additions on the Physical Properties of Extruded Psyllium (유화제 첨가에 따른 차전자피 압출성형물의 물리적 특성)

  • This study aims to investigate the physical properties of extruded psyllium husk upon the addition of emulsifiers. Three different emulsifiers-glycerol monostearate (GMS), polyglycerol ester (PGE), and sugar ester (SE)-were added to the mixture of psyllium husk and rice powder before extrusion. Extrusion was performed using a twin-screw extruder at 140℃ die temperature, 200 rpm screw speed, and 16% feed moisture content. The physical properties of psyllium husk extrudates including expansion ratio, specific length, piece density, texture profile, color properties, water soluble index, and water absorption index were evaluated. It was observed that the expansion ratio was the highest while the specific length and piece density were the lowest in the control which had no emulsifiers. Texture profile analysis showed that the apparent elastic modulus and breaking strength were highest in the extrudate with a PGE of 0.1%. The adhesiveness was found to be lowest in the extrudates with an SE of 0.1% and GMS of 0.5%. Lightness value was highest in the extrudate with a PGE of 0.1%. Color difference, water soluble index, and water absorption index were highest in the control. The results reveal that some physical properties of extruded psyllium husk were improved with the addition of emulsifiers. This finding provides useful information for the development of psyllium snacks with good physical characteristics.

Causes of Weakening Tree Vigor of Pinus thunbergii in Hanbando Coastal Forest in Shinangun, Jeollanamdo Province (전남 신안군 한반도해송숲의 곰솔 수세약화 원인 분석)

  • This study intended to identify causes of poor tree vigor in the Hanbando coastal forest by investigating its geographical environment, climate condition, soil physicochemical characteristics, and growth condition of Pinus thunbergii. It divided the forest into an area with poor tree vigor or dead standing trees and a control area with good tree vigor and examined them separately. The survey showed that stand density was significantly higher in the area with poor tree vigor. In contrast, the crown width in the area with good tree vigor was wider. The number of dead standing trees and the stand density showed a negative correlation. The stand density and diameter at breast height (DBH), tree height, crown height, and crown width also showed a negative correlation. The result indicated that, as the tree's stand density increases, the crowns of individual trees overlapped and the lower branches died. Then crown height and crown width decreased, and the number of leaves and photosynthesis was reduced, leading to lower tree height and weaker growth of breast diameter. As a result, tree vigor weakened, and combined with environmental pressures from the lack of moisture and nutrients in coastal soil and salty wind, P. thunbergii in coastal areas is expected to die massively. Although the causes of dead standing trees and poor tree vigor of P. thunbergii in the Hanbando coastal forest are complicated, poor management of adequate tree density in response to the growth of P. thunbergii is the primary cause. The secondary cause is external environmental pressures, including unfavorable soil conditions and salty and strong wind that obstruct the growth of P. thunbergii.

A Study on the Ontological Meaning of Life in The Canonical Scripture (『전경』에 나타난 생명의 존재론적 위상)

  • This study aims at determining the meaning of Life in The Canonical Scripture of the Daesoon Thought upon the ontological bases of occidental philosophies; especially upon those of Plato and Bergson. In western philosophy, the word ontology designates investigating the meanings of being, which is derived from the Greek 'onto (being)' and 'logia (logical discourse).' The various meanings of life have been sought from ancient times all over the world, for these are the critical and vital questions that pertain to the nature of human existence. Plato had asserted that life, in his word, soul, had three different kinds of aspects of meaning. Immortal, reciprocal, and divine. Plato scheme was such that the soul could die, but after death it could became reborn into another various forms of living creatures. The real inner life of humans, the soul, would live eternally. Henri-Louis Bergson, a famous French philosopher from the 20th century, claimed that life had three different kinds of aspects. Self-identity, Élan vital (vital impetus) and liberty. Bergson insisted especially the real meaning of life had been characterized by "unité multiple et multiplicitéune," "unity as something multiple and multiplicity something singular." The meaning of life in Daesoon Thought could be said to have three different characteristics, solidarity, earthly immortality and grievance-resolution. Some similarities can be found between certain western ontological meanings of life and those of Daesoon Thought. Namely, the qualities of eternity, reciprocity, and divinity.

The Relationship between Optimal End-of-Life Care and Well Dying of Nurses (간호사의 최상의 임종간호 및 웰다잉 관계 연구)

  • The purpose of this study was to investigate the relationship between the optimal end-of-life care and well-dying perceived by nurses to provide the optimal end-of-life care for patients and their families. This subjects of the study were 121 nurses working in the ward or intensive care unit of a general hospital in Seoul, and the data collected from April 28 to May 12, 2021 were analyzed. Data were analyzed using t-test, ANOVA, and Pearson correlation coefficients with the SPSS/WIN 27.0 program. The total average of the subjects' perceptions of the optimal end-of-life care was 4.04±0.82 on a five-point scale, indicating the overall level of 'agree' with the optimal end-of-life care. The total average of well dying perceived by the subjects was 3.63±1.14 on a five-point scale, and the most important recognized questions was to die peacefully. Of general characteristics, there was a significant differences on optimal end-of-life care according to age (t=-.97, p=.013). The nurse group in their 30s showed higher consent for end-of-life care than those in their 20s. An analysis of the correlation between the optimal end-of-life care and well-dying showed that there was no significant correlation. This study suggest that education should be provided to raise awareness of end-of-life care for young nurses with low experience.

A Study of Perceptions among Middle-aged and Older Koreans about Dying Well: Focusing on Typology (중노년층의 좋은 죽음에 대한 인식: 유형화를 중심으로)

  • In recognition of the increasing social attention paid to the notion of how to die well, this study explored what it is that middle-aged and older Koreans think of as "dying well." Specifically, it was aimed at classifying the perceptions people middle-aged and older have regarding dying well. To this end, we used data from the National Survey on Well-Dying, which was conducted in 2018 by the Korea Institute for Health and Social Affairs, and employed Latent Class Analysis. The demographic characteristics of each of the classified subgroups were identified through technical statistics. The types identified were multilayered depending on the imminence of death, perspectives of the last stages before dying, and the meaning of death in the context of social relationships. These types differed according to gender and subjective health conditions. Based on our findings in this study, we put forward policy suggestions about awareness improvement of personal and social levels, promoting on comprehensive death preparation, providing a system to reduce the cost of medical and nursing expense at the end of one's life, promoting of the right to self-determination regarding death, raising social attention to groups that are least prepared for dying well.
