Huge Intraabdominal and Thoracic Desmoid Tumor -Surgical experience in one case - (복강 및 흉강 내 거대 데스모이드 종양 수술 치험 1예)

  • Desmoid tumors are benign neoplasms with high rates of recurrence. A case of huge desmoid tumor of the intrathoracic and intraabdominal space is presented. The patient was treated with resection, which involved hepatic left lobectomy and diaphragmatic resection and partial pericardiectorny and wedge resection of left lower lobe of lung. The resulting defect over the pericarium and diaphragm was reconstructed by bovine pericardium and Marlex mesh. Prevention of presumed local recurrence of desmoid tumors requires wide excision margin.

  • 염증성 복부질환에서 늑막침윤이 일어나는 기전에 대하여는 잘 알려져 있으나, 비염증성 복수의 늑막강내로의 이동에 관여하는 기전에 대하여는 여러가지 가설이 제시되어 있다. 저자들은 만성 신부전증 환자에서 Tenckhoff 카테타의 복강내 이식후 발생한 우측 수흉이 투석액의 이동에 의한 것임을 동위원소주사법을 통하여 확인하였는 바, 이에 관여하는 기전에 대한 문헌고찰과 함께 보고하는 바이다.

A Study on EMG Activation Changes of Spinal Stability Muscles during Forced Respiratory Maneuvers

  • Purpose: The purpose of this study was to investigate whether changes in electromyography (EMG) activations of spinal stability muscles with respiratory demand change were due to changes in respiratory demand or in postural demand. Methods: Forty healthy subjects (19male, 21female, $20.8{\pm}1.9$years old) performed quiet breathing and four different forced respiratory maneuvers (FRM) (Pulsed Lip Breathing, Diaphragmatic Breathing, Combination breathing, and respiratory muscle endurance training breathing) while in sitting and standing positions. EMG data for four muscles (TrA/IO, EO, RA, and ES) were collected and filtered using a band pass filter (20~200Hz) and a notch filter (60, 120, 180Hz). Results: There were no significant differences on percentage of change on %MVIC between QB and FRM (PLB, CB, DB, and RMET) between positions (all p>0.05).

  • A 2-year-old, female Pomeranian dog was referred for dyspnea. Thoracic radiographs revealed left-sided mediastinal shift, increased soft tissue opacity in the caudal aspect of left thorax with loss of the left diaphragmatic silhouette, and dorsal elevation of mediastinal structures and heart from the sternum by lung tissue. The left main bronchus was visualized as an air-bronchogram and observed to abruptly discontinue at the level of the 10th rib. Thoracic computed tomography (CT) revealed absence of the left lung parenchyma and left pulmonary vessels with a rudimentary left main bronchus. The case was congenital pulmonary aplasia diagnosed via radiography and CT.

  • Hiccups have more than 100 etiologies. The most common etiology has gastrointestinal origins, related mainly to gastric distention and gastroesophageal reflux disease. Intractable hiccups are rare but may present as a severe symptom of various diseases. Hiccups are mostly treated with non-invasive or pharmacological therapies. If these therapies fail, invasive methods should be used. Here, we present a patient on whom we performed a blockage of the phrenic nerve with the guidance of a nerve stimulator. The patient also had pneumothorax as a complication. Three hours after intervention, a tube thoracostomy was performed. One week later, the patient was cured and discharged from the hospital. In conclusion, a stimulator provides the benefit of localizing the phrenic nerve, which leads to diaphragmatic contractions. Patients with thin necks have more risk of pneumothorax during phrenic nerve location.

  • Pathological studies, on the lung of 27 cases of bovine parasitic bronchitis which were secured from abattoirs in Korea, were performed and discussed. The chief pathological findings were as follows. Grossly, the lungs were shown various degree of lobular consolidation, collapse and alveolar emphysema around the bronchi in which nematodes of Dictyocaulus viviparus were present. Emphysema and edema were observed in the interlobular septa. The lesions were encountered mostly in the caudal and ventral borders of the diaphragmatic lobes. In all the cased of parasitic bronchitis in which thickening of the visceral pleura and fibrinous deposition on the pleural surfaces were not seemed to be entirely due to the lungworm infection. Microscopically, bronchopneumonia, peribronchial and peribronchiolar lymphoid hyperplasia, lympho-reticular broncho-occlusive legion, and bleakthrough lesion were examined in the affected lungs.

  • 1. Hepatic ascariasis represented by milk spots on the liver surface was found in 475(31.7%) out of 1,500 pigs. 2. For histopathological studies, 170 cases were collected at random from the milk spotted liver From these cases ascaris larva was found in 14 cases(8.2%). 3. Grossly, the occurrence of milk spots was not confined to any particular lobes of the liver. The lesions, however, were more frequently found in the diaphragmatic surface than in the visceral surface. The extent of the spots was limited to the subcapsular region and the deeper tissue was not affected. 4. Histologically, hemorrhage and eosinophilic infiltration were observed in the area invaded by the larva. The increase of connective tissue followed by a form of chronic lymphocytic hepatitis was observed at the late stage of infection. There was numerous eosinophilic infiltration around the larva trapped in the liver tissue. Consequently, encapsulation of the larva was followed by organization and calcification.

The Successful Removal of a Foreign Body in the Spleen via Diaphragm Laceration Site by Video-Assisted Thoracoscopic Surgery

  • A 73-year-old man, who, in an inebriated state, had slipped in a flowerbed and was wounded on the left flank, was transferred to Trauma Center, Gil Medical Center, Gachon University College of Medicine. Based on the chest and abdominopelvic computed tomography, he was diagnosed with multiple rib fractures and hemopneumothorax on the left hemithorax and was found to have a bony fragment in the spleen. He had not presented peritonitis and exsanguinous symptoms during the observation period. Seven days later, computed tomography of the abdomen showed suspected diaphragmatic injury and a retained foreign body in the spleen. On exploration by video assisted thoracoc surgery (VATS), a herniated omentum through the lacerated site of the diaphragm was observed. After omentectomy using Endo Gia, the foreign body in the spleen was observed through the lacerated site of the diaphragm. Traumatic diaphragm rupture with a foreign body, in the spleen, was successfully managed by video assisted thoracic surgery via the lacerated site of the diaphragm.

  • Diaphragm injuries are very important because, if both thoracic and abdominal viscera are damaged, a combination of shock and acute respiratory distress may develop. It can be highly lethal. This evaluation was based on the reviews of 17 cases of traumatic diaphragm injuries treated at the Department of Cardiovascular Surgery, Seoul Adventist Hospital during 5 years from March 1993 to February 1997. The mean age of the patients was 37.2 years and sex ratio was 3.2:1 with male dominance. Blunt trauma(N=5, Rt.=4, Lt.= 1) was 29.5%, penetrating trauma(N= 12, Rt.=5, Lt.=7) was 70.5%. Dyspnea(76%) was the most common symptom. Blunt trauma(9.8$\pm$3.7 Cm) was larger than the penetrating trauma(3.2$\pm$ 1.3 Cm)(P<0.05) in the size(mean$\pm$SD) of the injury. All of the patients had associated injuries and repaired immediatley with thoracic approach 11 cases(64%), abdominal approaih 3 cases(18%) and thoracoabdominal approach 3 cases(18%). f cases of penetrating diaphragmatic t auma was diagnosed on the operation of other organ injury Now we suggest that diaphragmatic injury should be suspected in all patients with penetrating as well as blunt injury of the chest and abdomen to protect the patient from its late complications.

Intrathoracic Ectopic Liver - A case report - (흉강내 이소성 간 - 1예 보고 -)

  • Although intrathoracic liver in association with a congenital diaphragmatic hernia has been well documented, the finding of intrathoracic ectopic liver tissue in the presence of an intact diaphragm is an extremely rare congenital anomaly. We have experienced a case of intrathoracic ectopic liver without any diaphragmatic hernia. A 37-year-old woman was admitted for the treatment of an incidentally detected right lung mass, A chest computed tomography scan revealed a right lower lobe lung mass close to the diaphragm, and this was suspicious for bronchial carcinoid tumor. Upon surgery, 2 round solid masses $3.5{\times}3.5 cm$ and a $2.0{\times}2.0 cm$ in size were noted, with their bottoms attached to the diaphragm dome. The masses were completely resected. Histologically, they were confirmed to be intrathoracic ectopic livers. The patient had an uncomplicated postoperative course.
