Background: Secondary hyperparathyroidism (SHPT) is common in patients with chronic kidney disease, affecting most of those who are receiving dialysis. Cinacalcet, a novel calcimimetic, targets the calcium-sensing receptor to lower PTH levels in dialysis patients. Objective: This study aimed to assess efficacy, safety and appropriateness of use of cinacalcet in dialysis patients. Method: This retrospective study was performed on total 24 cases with identified intact parathyroid hormone (iPTH), serum calcium and phosphorus levels before and 4 weeks after cinacalcet initiation at a teaching hospital from July 1st, 2011 to October 31st, 2012. Results: Cinacalcet decreased iPTH by 19% from baseline after 4weeks treatment and it was statistically significant (p<0.001). Cinacalcet also significantly decreased iPTH levels regardless of dialysis modality (hemodialysis group versus peritoneal dialysis group) and severity of SHPT (iPTH 300-800 pg/ml group versus iPTH >800 pg/ml group). Serum calcium, phosphorus and Ca x P levels were decreased without statistical significance. Gastrointestinal events, headache and hypocalcemia were the most common side effects. Monitoring for iPTH and serum calcium was not performed appropriately. 43.7% patients initiated cinacalcet therapy at serum calcium level< 9.0 mg/dl. Conclusion: In conclusion, cinacalcet lowers parathyroid hormone levels with no serious side effects. However, it is required to avoid cinacalcet treatment in patients with low serum calcium levels and monitor iPTH and serum calcium levels during cinacalcet administration.
Many dialysis modalities such as peritoneal dialysis (PD), hemodialysis (HD) and continuous hemofiltration or hemodialysis (CRRT) are available for the management of pediatric patients with acute renal failure (ARF). PD is a relatively simple, inexpensive modality and can be used in hemodynamically unstable patients. But, it may not be the optimal therapy for patients with severe volume overload or life threatening hyperkalemia. HD is the preferred modality for the treatment of severe volume overload, severe hyperkalemia, but it needs vascular access. Improvements in the HD equipment have allowed HD to be performend in small children. Recents technological improvements in CRRT therapies have enabled pediatric patients who are less stable to be treated. CRRT is becoming the preferred method of acute therapy in pediatric intensive care units. A sound knowledge of the underlying principles of dialysis and awareness of recent technological advancements in differnet dialysis modalities will hopefully result in improved management of children with ARF.
In this study, we prepared all-trans retinoic acid (ATRA)-encapsulated, surfactant-free, PLGA nanoparticles. The nanoparticles were formed by nanoprecipitation process, after which the solvent was removed by solvent evaporation or dialysis method. When a nanoparticle was prepared by the nanoprecipitation - solvent evaporation method, the nanoparticles were bigger than the nanoparticles of the nanoprecipitation - dialysis method, despite the higher although loading efficiency. Nanoparticles from the nanoprecipitation - dialysis method were smaller than 200 nm in diameter, while the loading efficiency was not significantly changed. Especially, nanoparticles prepared from DMAc, 1,4-dioxane, and DMF had a diameter of less than 100 nm. In the transmission electron microscopy (TEM) observations, all of the nanoparticles showed spherical shapes. The loading efficiency of ATRA was higher than 90% (w/w) at all formulations with exception of THF. The drug content was increased with increasing drug-feeding amount while the loading efficiency was decreased. In the drug release study, an initial burst was observed for $2{\sim}6$ days according to the variations of the formulation, after which the drug was continuously released over one month. Nanoparticles from the nanoprecipitation - dialysis method showed faster drug release than those from the nanoprecipitation - solvent evaporation method. The decreased drug release kinetics was observed at lower drug contents. In the tumor cell cytotoxicity test, ATRA-encapsulated, surfactant-free, PLGA nanoparticles exhibited similar cytotoxicity with that of ATRA itself.
Purpose: This study was done to identify stress, fatigue, and self efficacy in patients on dialysis and identify their effect on quality of life. Method: The participants (n=149) who agreed to participate in the research were selected from among those who were on dialysis treatment at C University Medical Centers in Seoul and Inchon, and at C University Medical Center in Jeju. The data were collected from July, 2001 to March, 2002 using interviews. Data analysis was done with t-test, ANOVA, and multiple stepwise regression using the SAS program. Result: Quality of life was significantly different according to economic status, and type of dialysis. Quality of life for the group with higher economic status was better than for the group with lower economic status, and the group on peritoneal dialysis had higher quality of lift scores than the group on hemodialysis. As for self-efficacy, there were significant difference according to duration of disease, type of dialysis, and duration of dialysis. Stress was significantly different according to marital status. Quality of life was significantly predicted by stress (40.02%) and fatigue (3.85%). Conclusion: To improve quality of life for people on dialysis, their stress should be treated positively, also multi-dimensional nursing interventions to provide emotional support and nursing interventions to diminish fatigue are required.
본 연구의 목적은 말기신부전증으로 복막투석을 받는 환자의 식이적응 경험의 의미와 구조를 규명하고 기술하는 것이다. 연구 방법으로 Colaizzi의 현상학적 연구방법을 적용한 질적 연구로서, 자료는 G광역시 일개 대학병원을 다니고 있는 복막투석 6개월 이상 된 9명의 환자를 대상으로 심층면담을 통해 수집되었다. 자료 분석을 위해 모든 면담은 녹취되고 필사되었고, Colaizzi의 현상학적 방법을 이용하여 분석하였다. 본 연구결과, 복막투석환자의 식이적응 경험은 네개의 주제모음과 15개의 주제로 구조화되었다. 네 개의 주제모음은 '복막투석과 식이적응의 이중적 고통', '막막하고 답답한 식이조절', '체험하면서 느끼게 된 식이조절의 고됨', '투석과 식이조절 병행의 중요성을 깨닫고 삶의 목표 재설정'이었다. 결론적으로 본 연구를 통해 복막투석환자들이 겪는 현실적인 식이적응과정은 복막투석과 식이요법적응의 이중적 고통으로 삶이 고되었지만, 순응하고 더 나은 삶을 위해 나아가는 과정이었다. 이를 바탕으로 복막투석환자 식이적응의 어려움에 대해 깊이 있게 이해하고, 식이관련 맞춤형 중재연구 개발에 필요한 기초자료로 활용하고자 한다.
Binding of sulfaethidole to bovine serum albumin was studied by equilibrium dialysis, fluorescence probe technique, uv difference spectrophotometry and circular dichroism. Equilibrium dialysis method enabled us to estimate the total number of drug binding sites of albumin molecule. For sulfaethidole, albumin had 6 primary and 40 secondary binding sites. The primary and secondary binding constants were 0.9 * 10/sup 5/ M/sup -1/ and 0.2 * 10/sup 6/ M/sup -1/, respectivitely. 1-Anilino-8-naphthalenesulfonate (ANS) and 2-(4-hydroxylbenzeneazo)- benzoic acid (HBAB) were used as the fluorescence probe and the uv spectrophotometric probe, respectively. In fluorescence probe technique, results indicated that the number of higher affinity drug binding site of albumin was 1 and the number of lower affinity drug binding sites of albumin was 3, and the primary and secondary drug binding constants for bovine serum albumin were 2.15 * 10/sup 5/M/sup -1/ and 1.04 * 10/sup 5/ M/sup -1/, respectively. In uv difference spectrophotometry, binding sites were 3 and binding constant was 1.88 * 10/sup 5/M/sup -1/. The present study was designed to compare the nutritional status of hemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD) patients. Subjects were 58 HD patients (male/female = 29/29) and 33 CAPD patients (male/female = 23/10) undergoing dialysis treatment in Artificial Kidney Unit of Chonbuk National University Hospital. For nutritional assessment, %IBW (ideal body weight), %TSF (tricep skin fold), %MAC (mid arm circumference), %MAMC (mid arm muscle circumference), serum albumin, serum transferrin, TLC (total lymphocyte count), SGA (subjective global assessment) and estimated energy and protein intakes by 1-month food frequency method were used. Between HD and CAPD group, mean age (50 $\pm$ 12 vs. 52 $\pm$ 12 yr), dialysis durations (37 $\pm$ 36 vs. 30 $\pm$ 26 mon), dietary energy intakes (28.3 $\pm$ 9.0 vs. 28.8 $\pm$ 8.6 kcal/kg/day), dietary protein intakes (1.1 $\pm$ 0.4 vs. 1.2 $\pm$ 0.3 g/kg/day) and incidence of co-morbid conditions (69.0% vs. 69.7%) were not significantly different. Data by using SGA showed a higher incidence of malnutrition in CAPD patients (45.6%) than in HD patients (36.2%). %IBW (p < 0.001), %TSF (p < 0.001) and %MAC (p < 0.001) were higher in CAPD patients than were in HD patients. But serum albumin (p < 0.001) and transferrin (p < 0.001) were significantly lower in CAPD patients than were in HD patients. A higher incidence of malnutrition was shown in CAPD Patients than in HD Patients due to different dialysis type. A significant finding was that CAPD showed protein deficient malnutrition and HD did calorie deficient malnutrition. It suggests that an adequate dietary intake considering dialysis type prevents a prevalence of malnutrition.
본 연구는 만성신부전으로 30년 이상 혈액투석치료를 받는 8명의 환자를 대상으로 투석경험의 의미와 본질을 탐색하고자 Colaizzi(1978)의 현상학적 방법을 적용한 질적 연구이다. 2017년 2월 27일부터 5월 30일 까지 심층면담에 의한 자료를 수집하였다. 연구결과 장기간 혈액투석환자의 투석경험은 전혀 다른 삶의 시작의 범주는 '힘든 투석생활', '투석치료라는 굴레에 매임', '끝나버린 이전의 삶', 다시 일어서는 삶의 범주는 '투석을 받아들임', '힘이 되는 울타리', '사회생활로 다가서는 삶', 현존하는 삶의 범주는 '투석경험을 나누는 삶', '피할 수 없는 신체적 한계', '살고자 하는 마음가짐'의 3개의 범주와 9개의 주제모음과 22개의 주제로 도출되었다. 장기간 혈액투석환자의 투석경험은 전혀 다른 삶으로 시작되는 초기 투석경험을 통해 힘듦을 이겨내고 각 개인에 맞는 자기 관리 방법을 알고 투석경험을 나누며 장기간의 투석생활을 현존의 삶으로 이어가고 있었다. 혈액투석간호는 투석경험에 따른 다양한 교육방법의 개발과 만성질환자의 투석에 대한 적응 및 지속적인 자기관리 향상을 가져오는 간호가 필요하다.
Purpose: This study was done to provide fundamental data for nursing interventions to prevent and reduce fatigue and to identify fatigue and kidney disease symptoms in female patients on hemodialysis and evaluate factors associated with this fatigue. Method: A cross-sectional study design was used with self-administered questionnaires which included general characteristics and the fatigue scale developed by Brown, Dittner, Findly, & Wessely(2005)(Cronbach's $\alpha=0.98$ and for present study $\alpha=0.96$) and a review of laboratory data. From eight dialysis units, 84 women were enrolled. Data were analyzed using the SAS program. Results: Mean score for fatigue was 49.4(range $16{\sim}78$) and fatigue by research variables was significantly different by age(0.046), employment status(0.041), menopause(0.009), hypoalbuminemia(0.022), length of time on dialysis(0.48) and kidney disease symptoms(0.000). Correlations between fatigue and lack of strength, dizziness, and cramps after dialysis were significantly higher. Factors affecting fatigue were kidney disease symptoms and length of time on dialysis, explaining 49.2% of fatigue. Conclusion: A comprehensive approach considering kidney disease symptoms, length of time on hemodialysis, age, menopause, and hypoalbuminemia are required for interventions to reduce fatigue in female patients on hemodialysis.
Purpose: The purpose of this study was to explore the experience of patients on the waiting list for kidney retransplantation. Methods: The data were collected by individual in-depth interviews of nine patients who were dialyzed after primary kidney graft dysfunction and were waiting for deceased donor kidney retransplantation. All interviews were audio-taped and transcribed, and were analyzed using a phenomenological method. Results: The findings included 5 theme clusters and 13 sub-themes. The 5 clusters were 'Diagnosed with chronic renal failure and dialysis: Broken daily life and crisis', 'Kidney transplantation: The only way to escape from dialysis', 'Kidney graft failure: Inevitable moving backward to hemodialysis', 'Self-management of re-dialysis patients: Growth through pain', and 'The waiting for kidney retransplantation: To try again in the hope of getting a new life'. Conclusion: This study provides a deep understanding of patients with dialysis who are waiting for deceased donor kidney retransplantation. On the basis of the findings of this study, health professionals can provide customized information and develop effective nursing interventions to improve the self-management of these patients.
