Comparative Assessment of Diagnostic Performances of Two Commercial Rapid Diagnostic Test Kits for Detection of Plasmodium spp. in Ugandan Patients with Malaria

  • Prompt diagnosis of malaria cases with rapid diagnostic tests (RDTs) has been widely adopted as an effective malaria diagnostic tool in many malaria endemic countries, primarily due to their easy operation, fast result output, and straightforward interpretation. However, there has been controversy about the diagnostic accuracy of RDTs. This study was conducted to evaluate the diagnostic performances of the 2 commercially available malaria RDT kits, RapiGEN Malaria Ag Pf/Pv (pLDH/pLDH) and Asan $EasyTest^{TM}$ Malaria Ag Pf/Pv (HRP-2/pLDH) for their abilities to detect Plasmodium species in blood samples collected from Ugandan patients with malaria. To evaluate the diagnostic performances of these 2 RDT kits, 229 blood samples were tested for malaria infection by microscopic examination and a species-specific nested polymerase chain reaction. The detection sensitivities for P. falciparum of Malaria Ag Pf/Pv (pLDH/pLDH) and Asan $EasyTest^{TM}$ Malaria Ag Pf/Pv (HRP-2/pLDH) were 87.83% and 89.57%, respectively. The specificities of the 2 RDTs were 100% for P. falciparum and mixed P. falciparum/P. vivax infections. These results suggest that the 2 RDT kits showed reasonable levels of diagnostic performances for detection of the malaria parasites from Ugandan patients. However, neither kit could effectively detect P. falciparum infections with low parasitaemia (<$500parasites/{\mu}l$).

진단검사의 특성 추정을 위한 표본크기 (Sample Size Requirements in Diagnostic Test Performance Studies)

  • There has been increasing attention on sample size requirements in peer reviewed medical literatures. Accordingly, a statistically-valid sample size determination has been described for a variety of medical situations including diagnostic test accuracy studies. If the sample is too small, the estimate is too inaccurate to be useful. On the other hand, a very large sample size would yield the estimate with more accurate than required but may be costly and inefficient. Choosing the optimal sample size depends on statistical considerations, such as the desired precision, statistical power, confidence level and prevalence of disease, and non-statistical considerations, such as resources, cost and sample availability. In a previous paper (J Vet Clin 2012; 29: 68-77) we briefly described the statistical theory behind sample size calculations and provided practical methods of calculating sample size in different situations for different research purposes. This review describes how to calculate sample sizes when assessing diagnostic test performance such as sensitivity and specificity alone. Also included in this paper are tables and formulae to help researchers for designing diagnostic test studies and calculating sample size in studies evaluating test performance. For complex studies clinicians are encouraged to consult a statistician to help in the design and analysis for an accurate determination of the sample size.

영상 인식 기반 신속 인플루엔자 자동 판독 기법 개발 (Development of Automated Rapid Influenza Diagnostic Test Method Based on Image Recognition)

  • To examine different types of influenza diagnostic test kits automatically, automated rapid influenza diagnostic test method based on image recognition is proposed in this paper. First, the proposed methods classify a variety of the rapid influenza diagnostic test kit based on support vector machine that analyzes the kits' feature point. Then, to improve the accuracy of test, the proposed methods match the histogram of both the target image of influenza kit and the input image of influenza kit for minimizing the effect of environment factors, such as lighting and exposure variations. And, to minimize the effect from composition of the hand-helds devices, the proposed methods extract the feature point and match point-by-point between target image of influenza kit and input image of influenza kit. Experimental results of 124 experimental group show that the proposed methods significantly have effectiveness, which shows 90% accuracy in moderate antigen, for the preliminary examination of influenza, and provides the opportunity for taking action against influenza.

  • This test was performed to assess the correlation between insulation diagnostic tests and breakdown voltage strength of the stator winding of 6.6kV class induction motor in insulation deterioration condition which have been in service for 5 years after being installed in 1998. The insulation diagnostic tests include resistance, polarization index(P.I), dissipation factor(${\triangle}tan{\delta}$), maximum partial discharges(Qmax) and AC breakdown test. we evaluated the correlation between insulation diagnostic test and AC breakdown test for stator winding of high voltage induction motor.

위 조직 생검 시료의 Helicobacter pylori 균 검출에 사용되는 진단검사의 특성을 추정하기 위한 비선형 모형의 응용 (Diagnostic Performance for Detection of Hezicobacter Pyzori Infection in Gastric Biopsy Specimens with No Gold Test: Non-linear Regression Approach)

  • 감염된 환축을 찾는 진단과정에 완벽하지 못한 진단검사를 사용하는 경우 진단검사 결과는 흔히 왜곡되어 나타난다. 본 연구에서 저자는 Helicobacter pylori 감염을 진단하는데 사용되는 urease 검사, PCR 검사 및 조직학적인 검사법을 단독으로 사용하는 경우와 병행하여 사용하는 상황으로 구분하여 각각 진단적 특성을 평가하였다. 비선형 회귀모형 분석결과 민감도, 특이도, 양성우도비 및 음성우도비는 urease 검사법의 경우 99.9%, 99.9%, 99.9%, 99.6%, PCR 검사의 경우 88.6%, 99.9%, 99.9%, 70.5%, 조직검사법의 경우 78.3%, 97%, 78.3%, 97%fh 나타났다. 예측도는 유병율의 변화에 따라 다양한 값을 보였으며 Helicobacter pylori 감염의 유병율이 35% 이상일 때 조직 검사상 양성결과는 90% 이상의 일치도를 보였고, 유병율이 25% 미만일 때 조직 검사상 음성결과는 90% 이상의 일치도를 보였다. 본 연구결과 임상에서 감염된 개체를 스크리닝하는 목적으로 세가지 진단검사를 병행하는 것은 실질적인 이익이 없으며 단독검사로서 urease 검사와 PCR 검사가 가장 적합한 것으로 나타났다.

  • This paper proposes a method for improving efficiency of high-voltage motors off-line insulation diagnostic test. The conventional manual test method has measurement errors and excessive processing time. We developed the KITAS system and solved these problems by using the instrument remote control and communication technology. The result of proficiency testing by interlaboratory comparisons using Power Factor Tip-Up test was '${\mid}E_n{\mid}$ < 1'. It was possible to confirm the validity of acquired data from the KITAS and establish a remote diagnostic system for easy access without physical limitations.

턱관절균형의학의 기본 진단검사법 (Diagnostic Evaluation Methods in TMJ Balancing Medicine)

  • Diagnostic evaluation methods frequently adopted in Temporomandibular Balancing Medicine (TBM) are briefly reviewed as to their basic concepts and procedures. TBM diagnostic evaluation methods may be classified into direct and indirect ones. Indirect methods are to observe any change in the manifestations with regard to the (1) posture, (2) pain, (3) muscle strength, and (4) stretching. Direct methods are to evaluate through (1) cervical palpation test, (2) restricted cervical rotation test, and (3) lateral cervical tension test. These methods may provide clinicians valuable information on the current yinyang balance and potential therapeutic factors in relation with the temporomandibular joint, a meridian system, and the whole body.

Correlation between Insulation Diagnostic Test and AC Breakdown Test for 3.3 kV Class Induction Motor

  • The insulation diagnostic test and the AC breakdown test were performed under off-line using the 3.3 kV class induction motor which have been served for 10 years. These tests were conducted in means of nondestructive and destructive test. In this paper, we compared the correlation between the nondestructive and destructive test. Furthermore we setup an experimental condition with moisture and compared the insulation characteristics between moist and dry sample. From the results of the nondestructive and destructive test, it was found that the second AC current, which is the previous step of insulation breakdown, suddenly increased at a point of around 8.5 kV. The insulation breakdown of moist sample occurred at 12-14 kV, which is 4-5 kV lower than dry sample.

  • Background and Objectives : There are many diagnostic modalities to evaluate laryngopharyngeal reflux. However, ideal diagnostic methods have not been established yet. The purpose of this study is to evaluate the effectiveness of Water-Siphon test as a diagnostic test of laryngopharyngeal reflux. Subjects and Methods : From November 2006 to September 2007, we performed esophagography with Water-Siphon test and questionnaire, physical examination for 227 patients with laryngopharyngeal reflux symptoms. The results of Water-Siphon test were classified according to the degree of reflux, the number of reflux, the retention time and analyzed about the relationship with the reflux symptom index (RSI), reflux finding score (RFS). Results : The degree of reflux was related with pseudosulcus, thick endolaryngeal mucus. The number of reflux was related with thick endolaryngeal mucus. A positive predictive value of Water-Siphon test was 82.7% and sensitivity was 91.5%. Conclusion : Water-Siphon test is a reliable diagnostic test for laryngopharyngeal reflux.
