Development of Portable System for Measuring pH in Blood (휴대용 혈중pH 측정시스템의 구현)

  • We developed the portable blood analysis system, which can be measured pH of the blood. This system is composed to electronic circuit, mechanism, and system software. Electronic circuit is composed to the sensor, pre-amp part, temperature regulation part, fluid sensing part, A/D(analog to digital) conversion part, main and peripheral device processing part. And the mechanism is composed to the flow cell and the liquid flow part. The liquid flow part is consisted of blood and washing control system under the control of the 6-channel solenoid valve and syringe rump. The system software is composed to measurement program, calibration program, washing and diagnostic program. The program of each routine is designed as sequential process for an efficiency. And the portable pH analysis system used two-point calibration method using the two types of corrective liquid. As a result, we obtained the calibration curve and calculated the value of pH. For verifying the system, we confirmed the output voltage of the sensor, and estimated reappearance of system using the standard liquid.

Reference values for pulp oxygen saturation as a diagnostic tool in endodontics: a systematic review and meta-analysis

  • Objectives: This systematic review aimed to identify mean oxygen saturation values (SpO2) using pulse oximetry in permanent maxillary anterior teeth. Materials and Methods: The MEDLINE, Scientific Electronic Library Online, Cochrane Central Register of Controlled Trials, EMBASE, and Literatura Latino Americana em Ciências da Saúde electronic databases were searched. Combinations and variations of "oximetry" AND "dental pulp test" were used as search terms. Studies reporting means and standard deviations of SpO2 values were included. Two reviewers independently extracted data following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist. Heterogeneity was assessed using the I2 statistic, and all analyses were performed using R software. Study quality was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2 tool and the Newcastle-Ottawa scale. Results: Of the 251 studies identified, 19 met the eligibility criteria and were included (total sample, 4,541 teeth). In the meta-analysis, the mean SpO2 values were 84.94% (95% confidence interval [CI], 84.85%-85.04%) for the central incisors, 89.29% (95% CI, 89.22%-89.35%) for the lateral incisors, and 89.20% (95% CI, 89.05%-89.34%) for the canines. The studies were predominantly low-quality due to the high risk of bias associated with the index test, unclear risk regarding patient selection, and concerns about outcome assessment. Conclusions: Although most studies were low-quality, the oxygen saturation levels in normal pulp could be established (minimum saturation, 77.52%). Despite the risk of bias of the included studies, the reference values reported herein are clinically relevant for assessments of changes in pulp status.

Diagnostic performance of stitched and non-stitched cross-sectional cone-beam computed tomography images of a non-displaced fracture of ovine mandibular bone

  • Purpose: This study assessed the diagnostic performance of stitched and non-stitched cross-sectional cone-beam computed tomography (CBCT) images of non-displaced ovine mandibular fractures. Materials and Methods: In this ex vivo study, non-displaced fractures were artificially created in 10 ovine mandibles (20 hemi-mandibles) using a hammer. The control group comprised 8 hemi-mandibles. The non-displaced fracture lines were oblique or vertical, <0.5 mm wide, 10-20 mm long, and only in the buccal or lingual cortex. Fracture lines in the ramus and posterior mandible were created to be at the interface or borders of the 2 stitched images. CBCT images were obtained from the specimens with an 80 mm×80 mm field of view before and after fracture induction. OnDemand software (Cybermed, Seoul, Korea) was used for stitching the CBCT images. Four observers evaluated 56 (28 stitched and 28 non-stitched) images to detect fracture lines. The diagnostic performance of stitched and non-stitched images was assessed by calculating the area under the receiver operating characteristic curve (AUC). Sensitivity and specificity values were also calculated (alpha=0.05). Results: The AUC was calculated to be 0.862 and 0.825 for the stitched and non-stitched images, respectively (P=0.747). The sensitivity and specificity were 90% and 75% for the non-stitched images and 85% and 87% for the stitched images, respectively. The inter-observer reliability was shown by a Fleiss kappa coefficient of 0.79, indicating good agreement. Conclusion: No significant difference was found in the diagnostic performance of stitched and non-stitched cross-sectional CBCT images of non-displaced fractures of the ovine mandible.

Analysis of Image Factors of X-ray Films: Study for the Intelligent Replenishment System of Automatic Film Processor (자동현상기 지능화에 필요한 연산처리 기법의 개발을 위한 방사선 필름의 영상 지수의 분석)

  • We analyzed image factors to determine the characteristic factors that need for intelligent replenishment system of the auto film processor. We processed the serial 300 sheets of radiographic films of chest phantom without replenishment of developing and fixation replenisher. We took the digital data by using film digitizer which scaned the films and automatically summed up the pixel values of the films. We analyzed characteristic curves, average gradients and relative speeds of individual film using densitometer and step densitometry. We also evaluated the pH of developer, fixer, and washer fluid with digital pH meter. Fixer residual rate and washing effect were measured by densitometer using the reagent methods. There was no significant reduction of the digital density numbers of the serial films without replenishment of developer and fixer. The average gradients were gradually decreased by 0.02 and relative speeds were also gradually decreased by 6.96% relative to initial standard step-densitometric measurement. The pHs of developer and fixer were reflected the inactivation of each fluid. The fixer residual rates and washing effects after processing each 25 sheets of films were in the normal range. We suggest that the digital data are not reliable due to limitation of the hardware and software of the film digitizer. We conclude that average gradient and relative speed which mean the film's contrast and sensitivity respectively are reliable factors for determining the need for the replenishment of the auto film processor. We need more study of simpler equations and programming for more intelligent replenishment system of the auto film processor.

  • The goal of this study is to develop knowledge representation method for the construction and evaluation of ontology for diagnosis in oriental medicine. To develop the expert system for decision making on diagnosis and treatment, the systematic and structural knowledge which can be processible in EMR(Electronic Medical Record) must be precedent, and the Computational Process which control the system as well. This study set up an ontology as a trial model to represent the oriental medical knowledge into the machine processible one. Protege 2.1 has been used to build the ontology, and the serialization format of our ontology is the XML document based on OWL. The components of oriental medical diagnosis was arranged with the combination of symptoms which belong to the certain symptom patterns. Then natural language which expresses the oriental medical diagnosis components were converted into the logical sentence, and individual characteristic symptoms into each values of specific properties. In addition to the study, the diagnosis software for oriental medicine was developed and it used the ontology which we developed. Sequently, we tested the software to confirm the appropriateness of ontology. The result of the test shows that diagnostic questions are automatically formulated according to the diagnosis components of this ontology and that as such diagnostic results are induced. Therefore, the ontology system in this study will be efficient to develop the diagnosis program and useful as a tool for doctors to make decision. But, it is not recommendable to apply the system to the clinical environment until the clear diagnosis standards are introduced, and the more reliable diagnosis program can be developed based on the more appropriate ontology mentioned above.

A New Software for Quantitative Measurement of Strabismus based on Digital Image (디지털 영상 기반 정량적인 사시각 측정을 위한 새로운 소프트웨어)

  • Various methods for measuring strabismus have been developed and used in clinical diagnosis. However, most of them are based on the visual inspection by clinicians. For this reason, there is a high possibility of subjective evaluation in clinical decisions and they are only useful for cooperative patients. Therefore, the development of a more objective and reproducible method for measuring strabismus is needed. In this paper, we introduce a new software to complement the limitations of previous diagnostic methods. Firstly, we simply obtained facial images of patients and performed several preprocessing steps based on the spherical RGB color model with them. Then, the measurement of strabismus was performed automatically by using our 3D eye model and mathematical algorithm. To evaluate the validity of our software, we performed statistical correlation analysis of the results of the proposed method and the Krimsky test by two clinicians for ten patients. The coefficients of correlation for two clinicians were very high, 0.955 and 0.969, respectively. The coefficient of correlation between two clinicians also showed 0.968. We found a statistically significant correlation between two methods from our results. The newly developed software showed a possibility that it can be used as an alternative or effective assistant tool of previous diagnostic methods for strabismus.

Diagnostic Value of Human Epididymis Protein 4 Compared with Mesothelin for Ovarian Cancer: a Systematic Review and Meta-analysis

  • Background and Purpose: Ovarian cancer is the leading cause of death among gynecologic cancers because of the lack of effective early detection methods. Accuracies of the human epididymis protein 4 (HE4) and mesothelin in detecting ovarian cancer have never been systematically assessed. The current systematic review aimed to tackle this issue. Methods: MEDLINE, EMBASE, and Cochrane databases were searched (September 1995-November 2011) for studies on the diagnostic performances of HE4 and mesothelin in differentiating ovarian cancer from other benign gynecologic diseases. QUADAS items were used to evaluate the qualities of the studies. Meta-DiSc software was used to handle data from the included studies and to examine heterogeneity. All included studies for diagnostic performance were combined with sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), diagnostic odds ratios (DORs) with 95% confidence intervals (CIs), summary receiver operating characteristic (SROC) curves, and areas under the SROC curves (AUC). Results: A total of 18 studies and 3,865 patients were eligible for the final analysis. The pooled sensitivity estimates for HE4 (74.4%) were significantly higher than those for mesothelin (49.3%). The pooled specificity estimates for mesothelin (94.5%) were higher than those for HE4 (85.8%). The pooled DOR estimates for HE4 (26.22) were higher than those for mesothelin (24.01). The SROC curve for HE4 showed better diagnostic accuracy than that for mesothelin. The PLR and NLR of HE4 were 6.33 (95% CI: 3.58 to 11.18) and 0.27 (95% CI: 0.21 to 0.34), respectively. The PLR and NLR for mesothelin were 11.0 (95% CI: 6.21 to 19.59) and 0.51 (95% CI: 0.42 to 0.62), respectively. The combination of the two tumor markers or their combination with CA-125 increased sensitivity and specificity to different extents. Conclusion: The diagnostic accuracy of HE4 in differentiating ovarian cancer from other benign gynecologic diseases is better than that of soluble mesothelin-related protein. Combinations of two or more tumor markers show more sensitivity and specificity.

A review on Clinical Trials by Using a Computerized Tongue Diagnosis System (국내외 설진기를 활용한 인간 대상 연구현황)

  • Objectives The purpose of this study was to survey the status of clinical use of a computerized tongue diagnosis system (CTDS) Methods We searched domestic/international articles using the CTDS from online medical databases including OASIS, NDSL and pubmed. We selected articles on clinical application or reliability of CTDS but excluded articles on mechanical design or software programming for developing a new CTDS. Finally we found 15 articles and classified the articles according to the study purpose. Results Out of the 15 articles, 8 were focused on the clinical application including halitosis, cold/heat syndrome, lung cancer, xerostomia etc. Other 5 articles were aimed at evaluating and improving reliability of CTDS. The other 2 articles were studied for development of differential diagnostic criteria on tongue coating thickness. Conclusion We found out that until now the researches on clinical application of CTDS mainly had been performed for producing a variety of CTDSs. Considering the importance of the tongue color in the traditional Korean medicine, we suggest that at first standard operating procedure for CTDS be developed and researches to develop differential diagnostic criteria on tongue body/coating color be performed and then explore its applications.

Use of a gesture user interface as a touchless image navigation system in dental surgery: Case series report

  • Purpose: The purposes of this study were to develop a workstation computer that allowed intraoperative touchless control of diagnostic and surgical images by a surgeon, and to report the preliminary experience with the use of the system in a series of cases in which dental surgery was performed. Materials and Methods: A custom workstation with a new motion sensing input device (Leap Motion) was set up in order to use a natural user interface (NUI) to manipulate the imaging software by hand gestures. The system allowed intraoperative touchless control of the surgical images. Results: For the first time in the literature, an NUI system was used for a pilot study during 11 dental surgery procedures including tooth extractions, dental implant placements, and guided bone regeneration. No complications were reported. The system performed very well and was very useful. Conclusion: The proposed system fulfilled the objective of providing touchless access and control of the system of images and a three-dimensional surgical plan, thus allowing the maintenance of sterile conditions. The interaction between surgical staff, under sterile conditions, and computer equipment has been a key issue. The solution with an NUI with touchless control of the images seems to be closer to an ideal. The cost of the sensor system is quite low; this could facilitate its incorporation into the practice of routine dental surgery. This technology has enormous potential in dental surgery and other healthcare specialties.
