A Study on the Radiation Exposure Dose of Brain Perfusion CT Examination a Phantom (Phantom을 이용한 뇌 관류 CT검사에서 방사선 피폭선량에 관한 연구)

  • The purpose of this study, was Let's examine the exposure dose at the time of cerebral blood flow CT scan of acute ischemic stroke patients. In particular, long-term high doses of radiation sensitive organs and we Measured using phantom and a glass dosimeter. Apply the existing protocol suggested by the manufacturer (fixed time delay technique) and the proposed new convergence protocol (bolus tracking technique), reporting to measure the dose, dose reduction was to prepare the way. Results up to 39.8% as compared to the existing protocols in a new suggested convergence protocol, a minimum of 5.8% was long-term dose is reduced. Test dose of $CDTI_{vol}$ and DLP values decreased 25%, respectively, were measured at less than recommended dose. Try checking the protocol set out in the existing based on the analysis result of the above, by applying the proposed new convergence protocol by reducing the dose would have to contribute to improved public health. It is believed to be research continues to find the optimum protocol in the other tests.

  • The effects of Asaddition in amorphous selenium(a-Se) films for digital X-ray conversion material have been studied using the moving photocarrier grating(MPG) technique. This method utilizes the moving interference pattern generated by the superposition of the two frequency shifted laser beams for the illumination of the sample. This moving intensity grating induces a short circuit current, j$_{sc}$ in a-Se:As film. The transport parameters of the sample are extracted from the grating-velocity dependent short circuit current induced in the sample along the modulation direction. The electron and hole mobility, and recombination lifetime of a-Se films with arsenic(As) additions have been obtained. We have found an Increase in hole drift mobility and recombination lifetime, especially when 0.3% As is added into a-Se film, whereas electron mobility decreases with As addition due to the defect density. The transport properties for As doped a-Se films obtained by using MPG technique have been compared with X-ray sensitivity for a-Se:As device. The fabricated a-Se(0.3% As) device film exhibited the highest X-ray sensitivity out of 5 samples.

The Evaluation of Image Quality According to the Change of Reconstruction Algorithm of CT Images (재구성 알고리즘 변화에 따른 CT 영상의 화질 평가)

  • In this study, the correlation among the changes of Modulation Transfer Function(MTF) in the noise and high-contrast resolution and the change of Contrast to noise ratio(CNR) in the low-contrast resolution will be examined to investigate the estimation of image quality according to the type of algorithms. The image data obtained by scanning American Association of Physicists in Medicine(AAPM) phantom was applied to each algorithm and the exposure condition of 120 kVp, 250 mAs, and then the CT number and noise were measured. The MTF curved line of the high-contrast resolution was calculated with Point Spread Function(PSF) by using the analysis program by Philips, resulting in 0.5 MTF, 0.1 MTF and 0.02 MTF respectively. The low-contrast resolution was calculated with CNR and the uniformity was measured to each algorithm. Since the measurement value for the uniformity of the equipment was below ${\pm}$ 5 HU, which is the criterion figure, it was found to belong to the normal range. As the algorithm got closer from soft to edge, the standard deviation of CT number increased, which indicates that the noise increased as well. As for MTF, 0.5 MTF, 0.1 MTF and 0.02 MTF were all sharp algorithms, and as the algorithm got closer from soft to edge, it was possible to distinguish more clearly with the naked eye. On the other hand, CNR gradually decreased, because the difference between the contrast hole CT number and the acrylic CT number was the same while the noise of hole increased.

Study of Image Properties for Computed Radiography (Computed Radiography의 영상특성에 관한 연구)

  • Computed radiography(CR) has been widely used in the field of diagnostic radiography since digital X-ray image was introduced. The imaging performance of CR system was studied by analyzing the digital image data of the CR images which are the outcomes of the whole imaging system composed of image plate(IP), laser digitizer, analoge-digital convertor, and a given image processing unit. In this study, we used a conventional CR system made by Agfa. From the flat field image of 150$\times$150 image pixels, signal-to-noise ratio(SNR) was calculated. SNR of the CR image increases in proportion to logarithm value of the X-ray exposure irradiated on the IP. SNR is less than about 6 at the exposure below 0.2mR and is more than 10 at the exposure above 0.54mR. In our study, most of images obtained by the smaller exposures less than 2.0mR can not be readable. In general, the minimum value of the SNR ranges from 3 to 5. We obtained modulation transfer function(MTF) by analyzing the bar pattern image which was made under conditions as follows: X-ray tube potential was 55kVp, the IP exposure was 0.54 mR, and the distance between X-ray source to IP was 2m, where bar pattern was located on the IP. MTF is 23% at 2.5lp/mm spatial frequency. Provided that the MTF of noise equivalent modulation is 10%, the CR system has the limiting spatial resolution of 3.2lp/mm. If the image sharpness is evaluated by the spatial frequency where MTF is 50%. the corresponding spatial frequency is 0.5$\sim$0.75lp/mm. MTFA(Modulation Transfer Function Area) is 1.0lp/mm. Compared with the Fuji CR whose MTFA is 1.1lp/mm, Agfa CR in this study shows almost same MTFA performance.

  • Hemophagocytic lymphohistiocytosis (HLH) is characterized by fever, splenomegaly, jaundice, and pathologic findings of hemophagocytosis in bone marrow or other tissues such as the lymph nodes and liver. Pleocytosis, or the presence of elevated protein levels in cerebrospinal fluid, could be helpful in diagnosing HLH. However, the pathologic diagnosis of the brain is not included in the diagnostic criteria for this condition. In the present report, we describe the case of a patient diagnosed with HLH, in whom the brain pathology, but not the bone marrow pathology, showed hemophagocytosis. As the diagnosis of HLH is difficult in many cases, a high level of suspicion is required. Moreover, the pathologic diagnosis of organs other than the bone marrow, liver, and lymph nodes may be a useful alternative.

  • Purpose: Sacral dimples are a common cutaneous anomaly in infants. Spine ultrasonography (USG) is an effective and safe screening tool for patients with a sacral dimple. The aim of this study was to determine the clinical manifestations in patients with an isolated sacral dimple and to review the management of spinal cord abnormalities identified with USG. Methods: We reviewed clinical records and collected data on admissions for a sacral dimple from March 2014 through February 2017 that were evaluated with spine USG by a pediatric radiologist. During the same period, patients who were admitted for other complaints, but were found to have a sacral dimple were also included. Results: This study included 230 infants under 6-months-old (130 males and 100 females; mean age $52.8{\pm}42.6days$). Thirty-one infants with a sacral dimple had an echogenic filum terminale, and 57 children had a filar cyst. Twenty-seven patients had a low-lying spinal cord, and only one patient was suspected of having a tethered cord. Follow-up spine USG was performed in 28 patients, which showed normalization or insignificant change. Conclusion: In this study, all but one infant with a sacral dimple had benign imaging findings. USG can be recommended in infants with a sacral dimple for its convenience and safety.

Intrusion of Supraspinatus Outlet by the Humeral Head in Rotator Cuff Disease (회전근 개 질환에서 상완골 두의 극상근 출구의 침범)

  • Purpose of the study was to analyze the supraspinatus outlet image of sagittal MRI in rotator cuff disease. We analyzed the sagittal views of the shoulder MRI of 78 cases without cuff tear. The cases were divided into 51 cases of rotator cuff disease group and 27 cases of control group. Six parameters of acromial tilt, coracoacromial ligament angle, length and height of coracoacromial triangle, length of acromial side of the baseline and distance of intrusion of the humeral head were compared for each group. The distance of intrusion of the humeral head was the most significantly different one, 0.52cm for rotator cuff disease group and 0.15cm for control group. Intrusion of the humeral head to the supraspinatus outlet space from the bottom may be a contributing factor developing rotator cuff disease. The intrusion may precede to tearing of the rotator cuff.

  • Dual-energy X-ray absorptiometry (DEXA) is the most widely used technical instrument for evaluating bone mineral content (BMC) and density (BMD) in patients of all ages. In 2016, DEXA devices operating is 5617 in Korea. In this study we investigated the quality of management practices survey for DEXA equipment and we analyzed it. We got a survey response rate of 12.6%. Accurate bone densitometry test is used data for estimation a patient's risk of fracture. However, improper bone densitometry will increase the possibility of causing a false positive. Therefore. it is essential to use the proper aids accurate bone densitomenty to be performed, and the quality control of the device to reduce the error factor of the tester through the training to reduce error for the device and the attitude.

  • The rapid development in digital acquisition technology in radiography has not been accompanied by information regarding optimum radiolographic technique for use with an amorphus silicon flat panel detector. The purpose of our study was to compared image quality and radiation dose of an amorphus silicon flat panel detectors for digital chest radiography. All examinations were performed by using an amorphus silicon flat panel detector. Chest radiographs of an chest phantom were obtained with peak kilovoltage values of $60{\sim}150kVp$. Published data on the effect of x-ray beam energy on image quality and patient dose when using an amorphus silicon flat panel detector. It is important that radiographers are aware of optimum kVp selection for an amorphus silicon flat panel detector system, particularly for the commonly performed chest examination.

  • Pneumothorax associated with thoracic endometriosis is a rare clinical entity and it is called catamenial pneumothorax if the recurrence of pneumothorax is related to the period of menstruation. Several hypotheses about its pathogenesis are suggested including spontaneous rupture of the bulla, endometrial implants of the visceral pleura, and passage of air from the genital tract through endometrial fenestration of the diaphragm. Pneumothorax is associated with chest pain and dyspnea within 72 hours of the onset of menses in young women and developed usually at right side. We report a case of 32-year-old woman who had bilateral pneumothorax and thoracic endometriosis confirmed histopathologically in the visceral pleura by thoracotomy.
