Purpose : The purpose of this study was to find out useful radiological projection of shoulder subluxation in patients with post-stroke hemiplegia. Methods : A total of 33 patients with post-stroke hemiplegia were included(20 men and 13 women, mean age 62.3 years) and having the subluxed shoulder over one finger breath. The shoulder subluxation was determined as the ratio of the radiographic vertical and horizontal distance. The vertical distance was determined by measuring the distance between the most inferolateral point of the acromion and the central point of the humeral head. The horizontal distance was determined by measuring the distance between the central point of the glenoid fossa and the central point of the humeral head. To measure of the shoulder subluxation, the shoulder AP, axial and transthoracic lateral projections were taken on both affected and unaffected shoulders. We analyzed the difference of subluxation distance by t-test. Results : When patients was in sitting position, the average time of being shoulder subluxation was 123 second. The requirements for medical laboratories ISO 15189 is examined in organization and a quality management system, stressing the importance of evidence, document control, and control of records and clinical material. Medical services are provided from the areas of resource management, and pre-examination, examination and post-examination processes. The main goal of ISO 15189 accreditation is to improve the quality of laboratory services provided for patients and clinical users not only through compliance with consensually developed and harmonized requirements but also by adopting the philosophy of continual improvement using the Plan-Do-Check-Act cycle. Laboratory quality should be evaluated and improved in all steps of the testing process as the state-of-the art indicates that the pre- and post-analytical phases are more vulnerable to errors than the intra-analytical phase. The Korea Laboratory Accreditation Scheme (KOLAS), a national accreditation body, provides medical laboratory accreditations for appropriate approaches to evaluating the competence of a medical laboratory in providing effective services to its customers and clinical users. Adoption of ISO 15189 in 2010s as a government policy has been delayed, and only 5 laboratories have been accredited to date in Korea. The medical laboratories should seek the adoption of ISO 15189 with a positive attitude for quality improvement and strengthening of international competitiveness.
In this study, among various factors having influence on SUV, we intended to compare and analyze the change of SUV using CT(4 type) and MRI(3 type) contrast agents which are commercialized now. We used Discovery 690 PET/CT(GE) and NEMA NU2 - 1994 PET phantom as experimental equipment. We have conducted a study as follows; first, we filled distilled water to phantom about two-thirds and injected radioisotope(18F-FDG 37 MBq), contrast agent. Second, we mixed CT contrast agent with distilled water and MRI contrast agent with that water separately. And then, we stirred the fluid and filled distilled water fully not to make air bubble. In emission scan, we had 15minutes scanning time after 40 minutes mixing contrast agent with distilled water. In transmission scan, we used CT scanning and its measurement conditions were tube voltage 120 kVp, tube current 40 mA, rotation time 0.5 sec, slice thickness 3.27 mm, DFOV 30 cm. Analyzing results, we set up some ROIs in 10th, 15th, 20th, 25th, 30th slice and measured SUVmean, SUVmax. Consequently, all images mixed 3 types of MRI contrast agent with distilled water have high SUVmean as compared with pure FDG image but there was no statistical significance. In SUVmax, they have high score and there was statistical significance. And other 4 images mixed 4 types of CT contrast agent with distilled water have significance in both SUVmean and SUVmax. Attenuation correction in PET/CT has been executed through various methods to make high quality image. But we figured out that using CT and MRI contrast agents before PET/CT scanning could make distortion of image and decrease diagnostic value. In that reason, we have to sort out the priority of examination in hospital not to disturb other examination's results. Through this process, we will be able to give superior medical service to our customers.
In Nai-Gyung(內經), it is said that In-Yeong(人迎) means In- Yeong Maek(人迎脈), Gi-Gu m- eans Tson-Gu(寸口) or Maek- Gu(脈口), In-yeong, artery in the side of neck, is located before Yeong-Geun(瓔筋), called as Jok-Yang-Myung-Maek(足陽明脈),so the part of In-Yeong-Maek means In-Yeong-Hyul(人迎穴) of Jok-Yang-Myung- Wi-Gyung(足陽明胃經) showing the artery in the side of neck. The part of Tson-Gu-Maek is that of artery in Yo-Gol(橈骨), beating source of Soo-Tae-Eum-Maek (手太陰脈) Of In-Yeong-Gi-Gu-Maek, In-Yeong-Maek mainly consists of Yang(陽), Gi-Gu-Maek of Eum(陰), so In-Yeung means physical disease, short of extra Eum component. They said that if In-Yeong-Maek was more than Gi-Gu-Maek, it meant physical injury, while Gi-Gu-Maek was more than In- Yeo-ng-Maek, it meant internal injury. In-Yeong-Gi-Gu-Maek is the one to distinguish the external and internal, the inside and outside, Eum and Yang, but there is no definite classification method to distinguish it, also it is difficult to grasp the comparison of 1sung(1盛), 2sung(2盛), 3sung(3盛), interrelation with 12-Gyung-Rak(l2經絡), change of maek phases, so necessary to set up the clear definition for In-Yeong-Gi-Gu-Maek. For the two theories as to In-Yeong-Gi-Gu-Maek according to Nai-Gyung, the one is to diagnose the maek by comparing the In-Yeong-Hyul with Tson-Gu of Soo-Tae-Eum-Maek as Gi-Gu in the both sides of neck part, he other is to divide the left and right of Gi-Gu-Maek in to In-Yeong and Gi-Gu afterwards, but today it is difficult to compare and explain the medical theory afterwards owing to insufficient consideration of In-Yeong-Gi-Gu-Maek at Nai-Gyung. The diagnosis of In-Yeong-Gi-Gu-Maek at Nai-Gyung to distinguish the surplus and shortage of Eum and Yang up to now since Nai-Gyung has so important and diagnostic value as to grasp the exact meaning. Herewith, this researcher com-pared Nai-Gyung and medical theory afterwards, reported it to consider the fixed position and changing process of viscera and entrails arrangement of literatures introducing In-Yeong-Gi-Gu-Maek, examining the change of maek phases for normal maek and a-bnormal maek of In-Yeong-Gi-Gu-Maek, considering the 1 sung, 2 sung, 3 sung maek phases in In-Yeong-Gi- Gu-Maek. According to the above results, the conclusion was reached as follows. 1. In-Yeong as the outside indicates external disease(外感), showing the surplus and shortage of Yang symptom by having Boo-Maek(浮脈) as Py-ung-Maek(平脈), Gi-Gu as the inside indicates internal disease(內傷), showing the surplus and shortage of Eum symptom by having Chim-Maek(沈脈) as P-yung-Maek(平脈). 2. In Pyung-Maek of In- Yeong-Maek as Boo-Maek, g-radual sinking of more floated changing maek because of disease means the improvement of di-sease, in Pyung-Maek of Gi- Gu-Maek as Chim- Maek, gradual floating of more sunken changing maek because of disease means the improvement of disease. 3. They said that disease of Jok-Gyung-Rak(足經絡) is cha-nged to that of Soo-Kyung-Rak(手經絡) when Jo-Maek(躁脈) appears whether In-Yeong-Maek or Gi-Gu-Maek 4. With the exemples of 1 sung 2 sung 3 sung it was porned with the relation of Pyo-Ri-Soo-Jok(表裏手足). Therfore I can guess that this fact is a moment explained the Bu-You-Sa-Kyung(部有四經) mentioned in Nan-Gyung-18-Nan(難經 第18難). 5. I think that In-Yeong and Gi-Gu, as a diagnosis method which distinguish between the inside indicates internal disease and the outside indicates external disease, is required to study further researches.
General status of pediatric dentistry in Korea is to conduct vigorous academic activities and specialized medical care centering the Korean Association of Pediatric Dentistry (KAPD) that has about 1,000 pediatric dentists as members, pediatric dentistry departments of 11 Colleges of Dentistry, numbers of pediatric dentistry training institutions and private clinics specialized in children. From 1996, the accredited pediatric dentists were produced by the KAPD and from 2008, the state began to produce the accredited pediatric dentists. Since then, doctors with expertise in pediatric care had opened private clinics in addition to the university hospitals, it became the basis of a momentum to deepen the specialty of pediatric dentistry. The Dentistry community of Korea is going through rapid and profound changes recently, and the underlying reasons for such changes can be classified largely into a few categories: (1) Decreasing population and structural changes in population (2) Increase in numbers of dentists, (3) Changes in the pattern of dental diseases and (4) Changes in medical environment. In Korea, the children population in the age range of 0 ~ 14 years old had been decreased by 2 million in 2010 compared to that of 2000 due to reduction of birth rate. The current population of children in the age range of 0 ~ 4 years old in 2010 takes up 16.2% of the total population, but it is estimated that such percentage would decrease to 8.0% by 2050. Such percentage is largely behind the estimated mean global population of 19.6% by 2050. On the other hand, the number of dentists had been largely increased from 18,000 in 2000 to 25,000 in 2010. And it is estimated that the number will be increased to 41,000 by 2030. In addition, the specialized personnel of Pediatric dentistry had been shown as increased by 2.5 times during past 10 years. For the changes in the pattern of dental diseases, including dental caries, each df rate of 5 years old children and 12 years old children had been decreased by 21.9% and 16.7% respectively in 2010 compared to 2000. Each df Index also had been decreased by 2.5 teeth and 1.2 teeth respectively. The medical expenditure of Korea is less than that of OECD and more specifically, the expenditure from the National Health Plan is less than OECD but the expenditure covered by households is larger than OECD. These facts indicate that it is considered as requiring the coverage of the national health plan to be reinforced more in the future and as such reinforcement needs continuous promotion. In medical examination pattern of Pediatric dentistry, the preventive and corrective treatment were increased whereas the restorative treatment was decreased. It is considered that such change is caused from decrease of dental caries from activation of the prevention project at national level. For the restorative treatment, the restorations in use of dental amalgam, pre-existing gold crowning and endodontic treatment had been decreased in their proportion while the restorative treatment in use of composite resin had been increased. It is considered that such changes is caused by the change of demands from patients and family or guardians as they desired more aesthetic improvement along with socio-economic growth of Korean society. Due to such changes in dentistry, the pediatric dentistry in Korea also attempts to have changes in the patterns of medical examination as follows; It tends to implement early stage treatment through early diagnosis utilizing various diagnostic tools such as FOTI or QLF. The early stage dental caries so called white spot had been included in the subjects for dental care or management and in order to do so, the medical care guidelines essentially accompanied with remineralization treatment as well as minimally invasive treatment is being generalized gradually. Also, centering the Pediatric dentists, the importance of caries risk assessment is being recognized, in addition that the management of dental caries is being changed from surgical approach to internal medicinal approach. Recently, efforts began to emerge in order to increase the target patients to be managed by dentists and to expand the application scope of Pediatric dentistry along with through such changes. The interest and activities of Pediatric dentists which had been limited to the medical examination room so far, is now being expanded externally, as they put efforts for participating in the preventive policy making process of the community or the state, and to support the political theories. And also opinions are being collected into the direction that the future- oriented strategic political tasks shall be selected and researches as well as presentations on the theoretical rationale of such tasks at the association level.

Purposes: The objective of this study was to develop a management program for Korean elementary school students suffering from asthma, which would be based on the Australian Asthma-Friendly Schools (AFS) program. Methods: On the basis of the AFS program, we designed a 6-month pilot project for asthmatic students in two elementary schools in a rural area and one elementary school in an urban area of Korea. The pilot project consisted of the following processes: identifying students with asthma in a school, educating school staffs and the parents of an asthmatic child, registering those with asthma, and installing emergency kits for asthma attacks in school health rooms. In order to evaluate these processes, group discussions were held between project team members and school staffs in each area. In addition, we conducted a postal survey of 144 households having an asthmatic child. Results: The screening process adopted in this program resulted in the early diagnosis in asthma; however, it needs to be evaluated economically due to expensive diagnostic test for asthma. For the school nurses, asthma lessons were evaluated as being very helpful for their tasks, while teachers tended to take less interest in the program with only 45% of all teachers attending these lessons. Almost all participating parents reported that such lessons would be beneficial for the care of their child, even though only 24.2% of the survey respondents (122 households) attended the lessons. Installing emergency kits in school health rooms was regarded as a key feature of this project. The introduction of a register card containing more specific health records of asthmatic students was considered necessary to replace the existing list of students with asthma. Conclusion: This study has merit in that a Korean asthma-friendly schools program was developed for the first time, despite the many obstacles to such programs becoming more common.
The study aims to investigate the clinical utility of Bayley-III using US norm in Korea. A total of 98 preterm infants and 93 term infants were assessed with the K-Bayley-III. The performance pattern of preterm infants was analyzed with mixed design ANOVA which examined the differences of scaled scores and composite scores of Bayley-III between full term- and preterm- infant group and within preterm infants group. Then, We have investigated agreement between classifications of delay made using the BSID-II and Bayley-III. In addition, ROC plots were constructed to identify a Bayley-III cut-off score with optimum diagnostic utility in this sample. The results were as follows. (1) Preterm infants have significantly lower function levels in areas of 5 scaled scores and 3 developmental indexes compared with infants born at term. Significant differences among scores within preterm infant group were also found. (2) Bayley-III had the higher scores of the Mental Development Index and Psychomotor Developmental Index comparing to the scores of K-BSID-II, and had the lower rates of developmental delay. (3) All scales of Bayley-III, Cognitive, Language and Motor scale had the appropriate level of discrimination, but the cut-off composite scores of Bayley-III were adjusted 13~28 points higher than 69 for prediction of delay, as defined by the K-BSID-II. It explains the lower rates of developmental delay using the standard of two standard deviation. This study has provided empirical data to inform that we must careful when interpreting the score for clinical applications, identified the discriminating power, and proposed more appropriate cut-off scores. In addition, discussion about the sampling for making the Korean norm of Bayley-III was provided. It is preferable that infants in Korea should use our own validated norms. The standardization process to get Korean normative data must be performed carefully.
The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure.
