PURPOSE. The purpose of this diagnostic study was to assess the accuracy and time efficiency of a digital method to draw the denture foundation extension outline on preliminary casts compared with the conventional technique. MATERIALS AND METHODS. A total of 28 preliminary edentulous casts with no anatomical landmarks were digitized using a laboratory scanner. The outlining of the entire basal seat of the denture was performed on preliminary casts and digitized. Casts with no extension outline were digitized and outlines were drawn using software (DWOS, Straumann). The accuracy of the extension outlined between both techniques was evaluated in the software (GOM Inspect; GOM GmbH) by file superimposition. Specificity and sensitivity tests were applied to measure accuracy. The paired t-test (95% CI) was used to compare the mean total area and the working time. RESULTS. The accuracy ranged from 0.57 to 0.92. The buccal and labial frenulum showed a lower value in the maxilla (0.57); while the area between the retromolar pad and buccal frenulum (0.64) showed a lower score in the mandible. The maxillary denture foundation and the working time for both arches were significantly longer for the digital method (P < .001). CONCLUSION. The denture foundation extension outline exhibited a sufficiently excellent accuracy for the digital method, except for the maxillary anterior region. However, the digital method required a longer working time.
Occlusal disease is comparable to periodontitis in that it is generally not reversible. Occlusal disease, however, like periodontitis, often maintainable. It does itself to treatment and when restorative dentistry is utilized it becomes, in that sense, reversible. Moreover, a systematized and integrated approach will lead to a prognosis that is favorable and predictable. This approach facilitates development of optimum oral function, comfort, and esthetics, resulting in a satisfied patient. Such a systematized approach consists of four logical phase : (1) patient evaluation, (2) comprehensive analysis and treatment planning, (3) integrated and systematic reconstruction, and (4) postoperative maintenance. An integrated treatment plan is first developed on one set of diagnostic casts, properly mounted on a semiadjustable articulator using jaw relationship records. This is accomplished by using wax to make reconstructive modifications to the casts. These modified casts become the blueprint for planned occlusal changes and the fabrication of provisional restorations. The treatment goals are : (1) comfortably functioning temporomandibular joints and stomatognathic musculature, (2) adherence to the basic principle of occlusion advocated by Schuyler, (3) anterior guidance that is in harmony with the envelope of function, (4) restorations that will not violate the patient's neutral zone. This report shows the treatment procedures for a patient whose mandibular position has been altered due to posterior bite collapse. Migration of the maxillary anterior teeth had occurred, and the posterior occlusal contacts showed pathologic interference. Precise diagnosis using mounted casts was executed and prosthodontic reconstruction by the aid of an unconventional orthodontic correction on maxillary flaring was planned. An unconventional orthodontic correction can be accomplished by using preexisting natural teeth, which can be modified for use in active tooth movement or splinted together for orthodontic anchorage. This technique has an advantage over conventional fixed appliance orthodontic therapy because it can accomplish tooth movement concurrently with restorative and periodontal therapy. On occasion, minor tooth movement can be necessary to achieve the optimum occlusal scheme, crown form, and tooth position for the forces of occlusion to be displaced down the long axis of the periodontally compromised teeth. Once the occlusion, periodontal health, and crown contours for the provisional splinted restoration are acceptable, the final splinted restoration can be similarly fabricated, and it becomes an excellent orthodontic retainer.
The use of alginate impression materials today is prevalent because of its efficiency and simplicity in clinical settings. Unfortunately, the simplicity of the procedure tends to lull the dentist into a sense of well-being, and lead him into using careless or sloppy technique. Alginate impression materials are used to fabricate diagnostic and preliminary casts, and the final cast. Incorrect use of this material is known to affect the accuracy of the final prosthesis. The purpose of this study was to compare the effect of different mixing methods of alginate impression material and tray adhesive on the accuracy of the stone cast produced by each method. A total of 30 stone casts were produced by using 3 different types of mixing methods (10 stone cast for each mixing method, respectively). The first method utilized an automatic-mixing machine to mix alginate while the second method was carried out manually, strictly following manufacturer's instructions. The third method also involved manual mixing, but did not follow the manufacturer's instructions and was done in a random fashion. Also, 20 additional stone casts were produced by using alginate with or without tray adhesives were included in the study to evaluate effects of tray adhesives on the accuracy of alginate impression. 10 stone casts were produced by adding tray adhesives to the interior surface of the impression tray prior to taking the impression. The other 10 excluded this step. A total of 50 stone casts were analyzed by the three-dimensional measuring machine to measure and compare the dimensional changes of the impression material of each group. The results are as follows. 1. No significant difference was found between the automatic mixing group and the manually-mixing group(p>0.05). 2. For the group that followed manufacturer's instructions, less dimensional changes were record ed than the group that didn't in measuring distanced 4(p<0.05). 3. The group that used tray adhesives showed less dimensional changes(p<0.05). The findings revealed that mechanical methods of mixing alginate impression materials had little influence on dimensional changes. However, it is proven that following manufacturers instructions in alginate impression taking is an important step in acquiring accurate impressions and tray adhesives may play an important role in enhancing the results.
상악 전치부는 심미적으로 민감한 부위이며, 치아 외형과 치은의 조화도 치료의 성공과 밀접한 관련이 있다. 이를 위해서는 정확한 진단과 치료계획이 필수적이며, 필요 시 치아 주위 연조직에 대한 처치 등이 동반될 수 있다. 연조직 처치로서 외과적 치관연장술 중 하나인 치은절제술을 고려할 수 있으며 이를 통해 심미적인 보철 수복을 할 수 있다. 상악 전치부는 심미뿐 아니라 기능 또한 고려하여야 한다. 지대치 주모형과 순측만을 수정한 진단 납형 모형을 스캔 및 중첩하여 디자인 및 가공하면, 최소한의 교합조정을 통해 기존의 편안했던 교합을 재현해 줄 수 있다. 본 증례에서는 치은절제술 및 모형 중첩을 통해 심미 수복을 진행하였고, 그 결과 심미와 기능모두에서 환자의 만족도를 높였기에 이를 보고하는 바이다.
The patient had bruxism and epilepsy tendency. Inadequate or unstable posterior support was identified due to severe anterior attrition and decreased occlusal vertical dimension. Prematurities of posterior occlusal and wear facets increased the function of anterior teeth, resulting in severe wear. Wear facets displayed sharply defined peripheries, which are matched on articulated diagnostic casts. Also the patient showed C III malocclusion tendency, and lost some facial contour with drooping corners on the mouth. In this case, the alteration of OVD (Occlusal Vertical Dimention) may provide a biologically compatible adjunct to the treatment such as dentofacial esthetics, improved visual proportion in facial heightand mechanical solutions to the force-management of the masticatory system. The patient requires extensive restorative treatment to regain appropriate function, esthetics and comfort. According to the report by Farhad Fays, the average vertical distance from the maxillary to the mandibular mucolabial reflection in the region of the central incisors is approximately 34mm. However, the vertical distance of this patient was found to be 32mm, which was necessary to add gauge 20-sheets to apply vertical dimension. A removable occlusal overlay splint, which restores OVD to the estimated optimalposition, is the general first trial. The patient was observed periodically for 6 weeks, while appropriate adjustments were made vertical dimension to function. When patient felt comfortable with the splint, the teeth were prepared, and provisional restorations are placed for 3 months. The provisional restoration was fabricated by a diagnostic wax-up. When the patient felt comfortable with the provisional restoration, the final restoration mimics OVD, function, and esthetics that have been developed in the treatment restorations. Restoration of the extremely worn dentition presents a substantial challenge to thedentists. Therefore, careful evaluation of the etiology, history, and factors associated with occlusal vertical dimension should be preceded prior to the appropriate treatment planning.

Objective: Facial-type-associated variations in diagnostic features have several implications in orthodontics. For example, in hyperdivergent craniofacial types, growth imbalances are compensated by displacement of the condyle. When diagnosis and treatment planning involves centric relation (CR), detailed knowledge of the condylar position is desirable. The present study aimed to measure condylar displacement (CD) between CR and maximum intercuspation in three facial types of an asymptomatic orthodontic population. Methods: The study was conducted in 108 patients classified into three groups of 36 individuals each (27 women and 9 men; mean age, 20.5 years), based on the following facial patterns: hyperdivergent, hypodivergent, and intermediate. To quantify CD along the horizontal and vertical axes, the condylar position was analyzed using mounted casts on a semi-adjustable articulator and a mandibular position indicator. The Student t-test was used to compare CD between the groups. Results: Vertical displacement was found to be significantly different between the hyperdivergent and hypodivergent groups (p < 0.0002) and between the hyperdivergent and intermediate groups (p < 0.0006). The differences in horizontal displacement were not significant between the groups. In each group, vertical CD was more evident than horizontal displacement was. Conclusions: All facial types, especially the hyperdivergent type, carried a significantly high risk of CD. Therefore, the possibility of CD should be carefully evaluated and considered in the assessment of all orthodontic cases in order to accurately assess jaw relationships and avoid possible misdiagnosis.
총의치 제작 과정 중 정확한 최종인상을 얻기 위해서는 정확한 예비인상 채득이 선행되어야 한다. 본 증례에서는 비정형적인 악궁 형태를 가진 환자에서 기성 트레이 및 구강 스캐너로 채득하여 제작한 예비모형과 개인트레이를 비교하였다. 58세 남자 환자로 상, 하악 총의치 제작을 위해 본원 구강악안면외과에서 의뢰되었다. 상하악궁 모두 좁고 긴 형태로 비정형적인 형태를 보였다. 기성 트레이를 이용하여 채득한 예비모형은 구강 스캐너를 이용한 예비 모형에 비해 상악의 경우, 협측의 변연이 과연장되었고, 후방 구개 폐쇄 부위에서는 압박되어 있었다. 또한 하악에서도 과연장되었으나, 상악에 비하여 그 양이 많지 않았다. 그리고 구강 스캐너에 의해 채득된 예비모형은 기성 트레이로 채득된 예비 모형에 비해 저연장되었으나, 해부학적 지표는 충분히 인기되었다. 기성 트레이에 의해 제작된 개인 트레이는 IOS에 의해 제작된 개인 트레이에 비하여 구강 내 시적 시, 개인 트레이의 조정량이 많았고 진료 시간이 증가하였다. 본 증례를 통해 비정형적인 악궁을 가진 환자에게 3D 구강 스캐너로 인상을 채득하는 것이 대안이 될 수 있음을 보고하는 바이다.
본 연구에서는 한국인 정상교합자의 치아 크기와 치열궁 척도를 재평가하고자 하였다. 본 연구를 위하여 296명(남자 179명, 여자 117명)의 정상교합자 모형을 분석하였는데, 이 정상교합자 표본은 1999년부터 2002년까지 시행된 대규모의 구강 검진 사업에 대상이 되었던 15,836명의 성인 중에서 선발되었다. 0.01mm정밀성을 보이는 슬라이딩 디지털 캘리퍼스를 이용하여 치아의 근원심 폭경과 치열궁 폭경, 치열궁 기저부 폭경 및 치열궁 장경을 측정하였으며, 구치부 폭경 비율과 Bolton의 지수를 도출하였다. 계측의 신뢰성 증진을 위한 방안들을 면밀히 고려하였으며, 측정시의 실험자내 및 실험자간의 신뢰성 지수를 산출하였다. 그 결과 자료의 신뢰성은 매우 우수하였으며, 측정된 자료에서는 명백한 성차가 다시금 확인되었다. 20년전의 자료와 비교한 결과 의미있는 차이는 발견되지 않았다. 각종의 선형 계측치의 경우 명백한 성차 및 인종간의 차이가 인정되지만 각종 비율 척도의 경우 성별 및 인종에 상관없이 같은 값을 보였다. 본 연구의 결과물은 임상적으로 부정교합 환자에게 적용 가능할 것으로 생각되었다.
이 연구는 각 유형의 부정교합과 중심교합위-중심위 변위량 간의 상호관련성을 구명하기 위하여 시행되었다. 각 부정교합자 군에서 중심교합위-중심위 변위를 3차원적으로 비교 평가하고, 각 안면골격 형태의 특징에 따른 변위량의 차이 유무를 분석하고, 각 유형의 부정교합 골격형태에서 중심교합위-중심위 변위와 두개안면골격 계측요소 간의 상관성을 검정하였다. 242명의 부정교합자를 대상으로 중심교합위 상태에서 얻어진 측모두부X선규격사진을 계측하고 이들을 수평적, 수직적 관계에 의하여 9군으로 분류하였다. 또한 이들로부터 중심위와 중심교합위에서의 교합기록을 채득하고 face-bow로 안궁이전을 한 뒤 석고모형을 교합기에 중심위 상태로 탑재하였다. CPI를 이용하여 중심위와 중심교합위의 하악과두위치를 측정하고 이들과 각 군의 측모두부X선규격사진 계측치와의 상관성을 검정하기 위하여 통계처리한 결과 다음과 같은 결론을 얻었다. 1. 중심교합위-중심위 변위의 양과 방향에서 좌측과 우측은 증등도의 상관관계를 보였으며 전후방보다는 상하변위의 좌 우측 상관성이 다소 높았다. 2. II급 부정 교합군이 I 급과 III급 부정교합군에 비해 좌우측간의 중심 교합위-중심위 변위가 높은 상관관계를 보였다. 3. 각 부정교합군에서의 중심교합위-중심위 변위 유형은 유의한 차이가 없었다. 4. 중심교합위 상태의 측모두부X선규격사진상에서의 안면골격의 형태요소와 중심교합위-중심위 변위간에는 상관성이 적었다. 5. II급 brachyfacial skeleton과 III급 mesofacial skeleton의 일부 계측치에서 CPI수치를 예측할 수 있는 항목들이 나타났다. 중심교합위-중심위 변위의 양과 방향은 특정한 안면형태요소와는 무관하며 하악과두의 위치변이는 부정교합양태나 안면골격 형태와 관계없이 발현되었다.
본 연구는 III급 부정교합자에서 중심교합상태의 두부 방사선 사진과 MPI 수치를 이용하여 중심위로 전환한 두부 방사선 사진(Centrically related cephalometrics)을 계측하여 중심위 변위시 유의한 계측치 변화가 있는지 확인하고자 시행되었다. 교정치료의 경험이 없는 25명의 III급 부정교합군과 25명의 정상교합군을 대상으로 하여, 중심교합위 상태의 측모 두부 방사선 사진과 중심위 및 중심교합위 기록을 채득하고, 모형을 SAM II 교합기에 부착하였다. 중심위 변위 량은 MPI(Mandibular Position Indicator)를 이용하여 계측하였으며, MPI 수치와 중심교합위 상태의 측모 두부 방사선 사진의 투사도를 이용하여 Conversion work sheet상에서 중심위 전환을 시행하였다. 두부 방사선 계측 항목은 하악골의 위치와 관계되는 것을 선택하였다. 정상교합군과 III급 부정교합군에서 중심위와 중심교합위간의 계측치 변화를 연구하여 다음과 같은 결과를 얻었다. 1. 하악 과두에서의 중심위 변위량은 정상교합군에서 전후방 변위량$({\Delta}X)0.56{\pm}0.50mm$, 수직적 변위량$({\Delta}Z)-0.61{\pm}0.53mm$였으며, III급 부정교합군에서 전후방 변위량$({\Delta}X)0.93{\pm}0.77mm$, 수직적 변위량$({\Delta}Z)-0.64{\pm}0.89mm$로, 두군 모두 후하방 변위양상을 나타냈다. III급 부정교합군에서 정상교합군보다 전후방 변위량이 더 크게 나타났으나 통계학적인 유의성은 없었다. 2. 중심교합위와 중심위 두부 방사선 사진의 계측치를 비교해 본 결과 정상교합군에서 ANB, Facial angle, Facial convexity, ODI 둥의 항목이 유의한 차이를 나타냈다. III급 부정교합군에서는 ANB, Facial angle, Facial convexity, ODI, SNB, APDI, L1-FP 등 유의한 차이를 보이는 항목이 더 많았고 유의성도 더 높게 나타났다. 3. 두부 방사선 사진의 계측치 변화는 중심위 변위와는 연관성이 있었으나, 정상교합군과 III급 부정교합군에 대한 연관성이 없었다. 이상의 결과는 중심위 변위량이 임상적으로 재현할 수 있는 중심위 위치에서 정상적으로 변위되는 양보다 큰 경우 중심위는 교정 환자에서 적절한 진단과 치료 계획 수립을 위한 기준 점으로 고려되어야 함을 시사하였다.

① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는

- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
