  • Non-secretory multiple myeloma (NSMM) is a rare cancer of plasma cells characterized by the absence of detectable monoclonal M protein in the blood or urine. A 57-year-old woman presented with mandibular pain but without intraoral swelling. Imaging studies revealed multiple osteolytic lesions in her mandible and pronounced root resorption of the left mandibular second molar. Biopsy results showed atypical plasmacytoid cells positive for anti-kappa, CD138, MUM1, and CD79a antibodies, but negative for anti-lambda and CD20. These results were indicative of a malignant plasma cell neoplasm. No abnormalities were revealed by free light chain assay or by serum or urine protein electrophoresis, leading to a diagnosis of NSMM. The patient began chemotherapy in conjunction with bisphosphonate therapy and achieved remission following treatment. This case underscores the critical role of dentists in the early detection and prevention of NSMM complications, as the disease can initially present in the oral cavity.

So the many national researches about foundation, source and core technology are actively being made. As a result of these researches, the security has become an important part of success factor in R&D. And so various security diagnosis and evaluation is being conducted about national R&D program. Existing the research security evaluation models are classified domains in terms of security management and created evaluation indicators according to the domains. However the models are inappropriate in case of researchers doing self-diagnosis of research security. This paper set up the domains in aspect of research management and then proposed the evaluation indicator of research security according to the domains. The evaluation indicator model that is suggested can be utilized in self-diagnosis of research security effectively.

  • Prompt and accurate diagnosis of malaria is the key to prevent disease morbidity and mortality. This study was carried out to evaluate diagnostic performance of 3 commercial rapid detection tests (RDTs), i.e., Malaria Antigen Pf/Pan$^{TM}$, Malaria Ag-Pf$^{TM}$, and Malaria Ag-Pv$^{TM}$ tests, in comparison with the microscopic and PCR methods. A total of 460 blood samples microscopically positive for Plasmodium falciparum (211 samples), P. vivax (218), mixed with P. falciparum and P. vivax (30), or P. ovale (1), and 124 samples of healthy subjects or patients with other fever-related infections, were collected. The sensitivities of Malaria Ag-Pf$^{TM}$ and Malaria Antigen Pf/Pan$^{TM}$ compared with the microscopic method for P. falciparum or P. vivax detection were 97.6% and 99.0%, or 98.6% and 99.0%, respectively. The specificities of Malaria Ag-Pf$^{TM}$, Malaria Ag-Pv$^{TM}$, and Malaria Antigen Pf/Pan$^{TM}$ were 93.3%,98.8%, and 94.4%, respectively. The sensitivities of Malaria Ag-Pf$^{TM}$, Malaria Antigen Pf/Pan$^{TM}$, and microscopic method, when PCR was used as a reference method for P. falciparum or P. vivax detection were 91.8%, 100%, and 96.7%, or 91.9%,92.6%, and 97.3%, respectively. The specificities of Malaria Ag-Pf$^{TM}$, Malaria Ag-Pv$^{TM}$, Malaria Antigen Pf/Pan$^{TM}$, and microscopic method were 66.2%, 92.7%, 73.9%, and 78.2%, respectively. Results indicated that the diagnostic performances of all the commercial RDTs are satisfactory for application to malaria diagnosis.

  • The purpose of this paper is to develop the software for the diagnosis and evaluation of the nutritional ingestion status of inviduals or a group of family members who have inhabited in rural and urban, based on the suggested amount of nutrition and foods. This software can be used for the computation for the needed amount of nutrition, which is suitable to the economic conditions of individuals and group of family members, and for the reduction of effort to plan the balanced table menu and effort to manage the diets. However, those commercial softwares were hard to be purchased and expensive for the users, and even if they were purchased, it needs much time and efforts to operate and run the program. Especially those commerical software were not possible to revise when the user has faced with the need to revise and complement the input data according to his situations such as the individuals and the group of family members. The software which was developed to treat the table menu and the nutrition was very limited to the particular persons. That is, the software was not for the evaluation of the nutrition synthetically and reasonably, because it was planed to use for the patients and the group of hospital and research centers. However this software has a merit of providing the table menu and the nutritional diagnosis in that it can show the suggested amount of nutrition, the evaluation of nutrition, the methods of diet, and management of nutrition individually according to the age, sex, amount of exercise, and experience of sickness. The expected effect can be predicted to calculate the lacking amount of nutrition which the individual has taken, and be provided to the suitable table menu. In addition to this, the various table menu is based on the foods which can be purchased easily and inexpensively at any times.

  • Background: This study aimed to identify the characteristics of the referral and return of patients to clinics in the endocrinology and cardiology departments at the National Health Insurance Service Ilsan Hospital to evaluate the "referral and return of patients to clinics" program and reduce the rate of returning patients. Methods: From May 2018 to December 2020, we identified the number of visits to referral hospitals and hospital usage status at Ilsan Hospital after returning to clinics. We also identified the patients who returned to Ilsan Hospital within 6 months, defined as "failure to transport," among those recommended to be transported to clinics of the Medical Cooperation Center. Additionally, we evaluated the characteristics of the "failure to transport" patients. Results: Among the returning patients, the rate of visiting Ilsan Hospital within 6 months was higher in cardiology than in endocrinology (25.1% vs. 16.7%). Older age, more severe disease, and more number of visits to the department were associated with a high rate of failure to transport. The rate of failure to return was low in cases diagnosed with hyperlipidemia/lipoprotein metabolism disorder. With respect to diabetes, the rate of failure to transport differed according to each type of diagnosis of diabetes. Conclusion: The success rate of the "referral and return of patient to clinics" program differed based on each patient's characteristics, department of visit, and diagnosis. Individualizing according to the visit department and diagnosis is required to ensure successful transfers, and infrastructure expansion and institutional arrangements must be facilitated.

  • Backgrounds/Aims: Historically, the incidence and prognosis of patients diagnosed with intrahepatic cholangiocarcinoma (ICC) have been inadequately understood. Survival analysis in ICC has yet to be investigated in a population-based study. Methods: We conducted a retrospective cohort study using the Surveillance, Epidemiology, and End Results Program (SEER) 18 Registry. Risk ratios were estimated via Poisson regression. Hazard ratios for 5-year survival were estimated using hierarchical Cox regression models. Results: Males show a higher rate of age-adjusted ICC incidence. Blacks carried a decreased risk of ICC diagnosis than Whites, while Asians revealed a higher risk of ICC diagnosis when compared with Whites. The observed survival rates at 12, 36, and 60 months were 36.3%, 12.8%, and 8.1%, respectively. Compared with Whites, Blacks showed an increased risk of death (p < 0.01). Lymph node resection during surgery was associated with a 64.1% reduced risk of mortality (p < 0.01). A higher T stage at diagnosis was associated with poor survival (p < 0.01). Surgery combined with chemoradiotherapy, radiotherapy, or chemotherapy was associated with a reduced risk of mortality compared with nonsurgical interventions (p < 0.01). Conclusions: ICC incidence has been increasing since 2000, especially in White males. The risk of ICC rises with age. Lymph node removal is associated with better survival. In recent years, survival had worsened, and surgical intervention improved survival compared with nonsurgical management.
