A Study of the Knowledge and Attitude of Diabetics (당뇨병환자의 자가조절 (Self -Control)의 지식 및 태도 조사연구)

  • Diabetes Mellitus is a worldwide disease affecting millions of persons and appears to be on the increase in Korea. At the present time it can not be cured but can be controlled. To achieve control the patient must know about the disease. Education of the patient is an essential part of the management of the diabetic and is the responsibility of the nurse. The educational program must be adapted according to patient's knowledge and attitude about their diseases. Therefore, the opportunity to assess the knowledge and attitude of diabetics must provided. The objectives of this study were : 1. To describe the general characteristics of the study population. 2. To find out the characteries of experience of treatment. 3. To exams the knowledge and attitude of diabetics about the self-control. 4. To learn the influencing factors affecting the knowledge and attitude of diabetics about self - control. The study population defined and selected was 65 diabetics registered at the Diabetic Clinic of 5. hospital who visited the Diabetic Clinic from April 16, 1976 to May 14, 1976. The direct interview method was used. Statistical analysis of the data was X²- test. The following results were obtained : A. General characteristics of the study population : The respondent's average age was 50. 8 years old, sex ratio(male to female) revealed as 1 : 1.1, economic background was upper middle class, the most respondents had better than- average education, 75.4 Percents of the respondents lived in Seoul, 47. 2 percents were unemployed, the average size of family was 5.3 and 47.7 percents of respondents had religions. B. The characteristics of clinical experience : The average duration was 7.0 years, 46.1 percents of respondents had experience of hospitalization, 56.9 percents was taking oral drugs, 67.7 percents examined urine- sugar at home, 60.0 Percents had treat compilations and 20.0 percents had experience of coma. C. There was significant difference in the relationship : Between complications and durations of diabetes, hospitalization and therapeutic methods.

A retrospective study of the dental implants placed in the controlled diabetes mellitus patients (조절되는 당뇨환자에게 식립된 치과 임플란트의 생존율에 대한 후향적 연구)

  • Purpose: The aim of the present study was to analyze 7-year cumulative survival rate (CSR, %) of dental implants in the controlled diabetic patients and to evaluate the influence of the position, diameter and length of fixture, bone quality, age, gender and the method of maxillary sinus elevation on the survival rate. Methods: The data of 342 placed implants in the 104 diabetic patients collected between 1995 and 2007 at the Department of Periodontology in Yonsei University Hospital were analyzed. Results: Seven-year CSR of the 342 dental implants in the 104 controlled diabetic patients was 96.5%. The survival rates of the placed implants according the position have no statistically significant difference. The survival rates according to the length or diameter of the fixtures have no statistically significant difference. The survival rates according to the bone quality were 100% (Type I), 97.1% (Type II), 97.7% (Type III) and 85.7%(Type IV). The difference between the survival rate of Type I, II and III and that of Type IV was statistically significant. The survival rates according to patient gender were 96.8% (male), 95.5% (female). The survival rates according to patient age were 100% (${\leq}59$), 93.8% (${\geq}60$). The survival rates according to the method of sinus elevation in the maxillary posterior area were 96.8% (without sinus elevation), 92.9% (lateral approach) and 89.8% (crestal approach). Conclusions: Dental implants can be used successfully in the controlled diabetic patients. In case of upper posterior region which has poor bone density and older patients, the implant treatment should be more properly planed, executed, and followed-up.

The effects of low dose doxycycline regimen on gingival crevicular fluid enzyme actmty of diabetic patients with periodontitis and adult periodontitis patients (저용량 독시싸이클린 투여가 당뇨환자 마치 성인성 치주염 환자의 치은열구액내 효소 활성도에 미치는 영향에 관한 비교연구)

  • It was reported that low dose doxycycline(LDD) regimen could inhibit pathologically elevated collagenase activity in the gingival crevicular fluid of petients with adult periodontitis without producing typical antibiotic side effects. The purpose of this study was to evaluate the effects of LDD regimen(20mg) administered during non-surgical therapy on clinical index and gingival crevicular fluid enzyme activity in diabetics who are at high risk for periodontal disease. Forty-nine subjects having at least two sites with probing pocket depths greater than 4mm were selected. In this double-blind, placebo-controlled study, the patients were administered 20 mg doxycycline capsule or placebo capsule b.i.d. for 2 weeks. Clinical parameters of dental plaque, gingival inflammation, probing pocket depth and probing attachment level were assessed at week 0, 2, 4, and 8. Gingival crevicular fluid samples were collected at the same time to evaluate the activities of collagenase and elastase. Clinical parameters and elastase activity were significantly reduced in all groups compared to the baseline value after treatment. Significantly greater reduction in pocket depth and gain in attachment level was shown in the LDD-administered group compared to placebo group in both adult periodontitis and diabetic patients. Total collagenase activity was also reduced significantly in all groups after treatment, but the greater reduction was seen in the LDD-administered diabetics group compared to relevant placebo group(at 4, 8week). Significantly greater reduction in active collagenase activity was also seen in the LDD-administered group compared to placebo group in diabetic patients(at 2, 4, 8week). These results indicated that use of low dose doxycycline could be aueseful adjunct to instrumentation therapy in the management of diabetic patient with periodontitis as well as adult periodontitis patient.

A Prospective Study on Nutritional Status and Nutrient Intake of Hemodialysis Patients Based on Coexistence of Diabetes (혈액투석 환자에서 당뇨병 유무에 따른 영양상태와 영양섭취량 비교)

  • It is well known that malnutrition is a predictor of mortality in hemodialysis patients. The number of diabetic nephropathy patients is increasing rapidly. This study aimed to investigate nutritional status and nutrient intake according to the presence of diabetes among hemodialysis patients. The nutritional intake and general characteristics of outpatients at Ajou University Hospital (24 with diabetes and 30 without diabetes) were investigated between July and September 2015. Patients' general data were collected, and nutritional status by Patient-Generated Subjective Global Assessment (PG-SGA) was evaluated. Nutrient intakes were assessed according to 3-day food records. There was no significant difference regarding body weight between the two groups. However, the non-diabetic group showed a better nutritional status by Patient-Generated Subjective Global Assessment (PG-SGA) ($5.2{\pm}4.4$ vs. $8.0{\pm}4.3$ score, non-diabetics vs. diabetics, P<0.05). There was no difference in daily calorie intake ($1,473.9{\pm}370.5$ vs. $1,503.8{\pm}397.5kcal$) and protein intake ($60.3{\pm}19.7$ vs. $65.6{\pm}20.5g$) in those with diabetes. Intakes of vegetables protein, sodium, potassium, vitamin C, folic acid and fiber were significantly higher in the diabetic group compared to those of the non-diabetic group. There was no difference in daily nutrient intake between the hemodialysis weekday and weekend groups. The non-diabetic group had higher calorie and sodium intakes per meal in the hemodialysis weekday group, but the difference was not significant. Based on these results, intervention should be performed to improve nutritional status in consideration of diabetes and dietary patterns.

Diagnostic Ex-Vivo Assay of glucose Using Diabetic-Control Circuits

  • For ex-vivo diabetic control, the voltammetric diagnosis of glucose (GU) was conducted with a modified carbon nanotube paste electrode, using handheld analytical circuits. The optimum analytical conditions were attained within the 0.5-4.0 ug/L working range and at the 0.06 ug/L detection limit, which system was interfaced to the feedback circuits and was applied to human urine for diabetic-patient diagnosis. It can be used for ex-vivo flow control analysis, vascular flow detection and other medicinal assays. The equations of the patients' urine are y=36.65x+12.13 and $R^2=0.987$, those of the healthy person of y= 2.5x+10.9 and $R^2=0.928$ (patients: 118 ug/L; healthy person: 12.34 ug/L).

  • Diabetic foot ulcers (DFUs) are among the most serious complications of diabetes and are a source of reduced quality of life and financial burden for the people involved. For effective DFU management, an evidence-based treatment strategy that considers the patient's clinical context and wound condition is required. This treatment strategy should include conventional practices (surgical debridement, antibiotics, vascular assessment, offloading, and amputation) coordinated by interdisciplinary DFU experts. In addition, several adjuvant therapies can be considered for nonhealing wounds. In this narrative review, we aim to highlight the current trends in DFU management and review the up-to-date guidelines.

  • Chronic Kidney Disease (CKD) is a major global health burden. Sodium-glucose cotransporter-2 (SGLT2) inhibitors have demonstrated potential in slowing CKD progression. We evaluated the expanding role of SGLT2 inhibitors, emphasizing their renoprotective benefits in diabetic and non-diabetic CKD patients. We also investigated the underlying mechanisms, including the reduction of glomerular hypertension via modulation of tubuloglomerular feedback. Our study critically analyzed current indications for SGLT2 inhibitor therapy based on recent clinical trial data. To optimize patient outcomes, we present a comprehensive analysis of practical considerations for the prescription of SGLT2 inhibitors, including the potential initial decline in the estimated glomerular filtration rate and a review of adverse events.

Short-term Glycemic Control and the Related Factors in Association with Compliance in Diabetic Patients (당뇨병 환자의 치료순응도에 따른 단기간 혈당조절정도와 관련 요인)

  • Objectives : Generally, it seemed that the therapeutic result in diabetic patients was changed by compliance. This study was conducted on the basis of assumption that the therapeutic result id diabetic patients could control according to compliance. This study was conducted to analyze the related factors in association with compliance to drug, diet and exercise therapy. Methods : 224 diabetic patients in Kyungpook National University Hospital were selected through the interviews and HbA1c values from 1 Jan. to 28 Feb.1997. The drug compliance was tested by regularity of drug administration, the diet compliance was tested by restriction of food, exactly allocation, balance of nutrient, measuring food and the exercise compliance was tested by regularity of exercise per day. We assessed compliance by percentage, $x^2-test$ and generalized logit regression model(method:enter). Results : The significant variable was the satisfaction to medical personnels in drug, the knowledge to disease in diet, the participation of the diabetic education in exercise therapy and the satisfaction to medical personnels in HbA1c. Using the generalized logit model(method : enter) in compliance change, the significant variables were the satisfaction to medical personnels and the complication in drug; the significant variables were the age at the first diagnosis, the family history, the concern of health, the knowledge of disease, the self-exertion for therapy and the complication in diet: the only significant variable was the gender in exercise therapy. Conclusions : The degree of glycemic control in diabetic patients was influenced by compliance. In order to improve patient's compliance, we must foster the knowledge on the diseases, lead participation for diabetic education. Because the satisfaction to medical personnels was the important variables, we must build up good relationship between doctors and patients.

A Case Report of Diabetic Retinopathy Vitreous Hemorrhage (당뇨망막병증성 유리체출혈 환자 치험 1례 보고)

  • Objectives : The aim of this study is to report the effect of Korean medicine treatment on diabetic retinopathy vitreous hemorrhage. Methods : A 66-year-old female who had diabetes for over 20 years showed vitreous hemorrhage of Grade 4 at initial visit. For about 2 months, acupuncture, moxibustion and herbal medicine treatment were performed. Prognosis was confirmed by funduscopic examination, automatic refractive examination and visual acuity test. Results : In the fundus photographs, we can observe the recovery of turbidity in Grade 4, which decreases to Grade 1, and the result of refraction in the right eye and visual acuity that was not measured when the turbidity is severe. Conclusions : This study was a case of a patient with diabetic retinopathy vitreous hemorrhage complaining of decreased visual acuity. Patients were given herbal medication, acupuncture and moxibustion. Through the funduscopic examination, the treatment effect on the absorption of vitreous hemorrhage was confirmed.
