육미지황환(六味地黃丸)으로 호전된 당뇨병성 신경병증 1례 (A Case of Yukmijihwang-hwan's effect on Diabetic neuropathy)

  • Abnormal sensation is the main manifestation of diabetic neuropathy. We treated 53 year-old male patient suffering from diabetic neuropathy with Yukmijihwang-Hwan, herb complex, which has been used to treat Yin deficiency syndrome in Oriental Medicine, The patient complained abnormal sensation(numbness, pain) in the back portion and the both of plantar arch. We could observe the symptom improved after 1 months since the beginning of administration and disappeared after 9 months. So we suggest that long-term administration of Yukmijihwang-Hwan has therapeutic effect on abnormal sensation caused by diabetic neuropathy with the diagnosis of Yin deficiency in Oriental medicine.

  • Objectives: The purpose of this study is to report the clinical application of acupuncture, herbal medication, and antibiotic treatment to a patient with a diabetic foot infection. Methods: We treated the patient-who had ulcerations of the 1st, 2nd, and 4th toes and osteomyelitis on the right 1st and 2nd toes-with acupuncture, herbal medication, and antibiotics. We measured the state of this case by changes in his MRI signal and ulceration of his toes. Results: After treatment, the MRI signal indicated that the osteomyelitis had disappeared and the ulceration was diminished. Conclusions: A combination of acupuncture, herbal medication, and antibiotic treatment is effective for treatment of ulceration and osteomyelitis caused by diabetic foot infection.

  • Objective: To carry out the oriental medical treatment on a patient with vitreous hemorrhage in the left eye caused by diabetic retinopathy and record the results of the treatment. Methods: 1. Diagnosis: Fundus photography, Colored paper, Dr. Hahn's standard test chart for 5M, Blood sugar measurement. 2. Treatment: Acupuncture, Electro-Acupuncture, Indirect moxibustion, Western medicines, Oryoungsan(Crude drug preparations) Results: Oriental treatment using Ohaeng-acupuncture, Electro-Acupuncture, Indirect moxibustion resulted in the Unaided visual acuity of 0.1 while it used to be the left eye visual acuity with only light sense I month ago. Looking from Fundus photography result, progress was achieved and diabetic retinopathy was found to be in progress in fluorescein fundus angiography to right eye also by revisiting the patient after treatment. Conclusions: 1. Vitrectomy has many advantages but there are instances where patients do not recover their visual acuity due to complications. Therefore it is necessary to prove the effect oriental medical treatment through more cases in future. 2 For diabetic retinopathy patients, diabetes must be treated together with visual acuity.

한국인 제2형 당뇨 환자의 당뇨발 궤양 예방교육 프로그램의 효과: 체계적 문헌고찰 (Effectiveness of Korean Patient Education in Preventing Diabetic Foot Ulcer: A Systematic Review)

  • Purpose: This study aimed to systematically review the contents and effectiveness of education programs for preventing diabetic foot ulcer among patients with type 2 diabetes mellitus in Korea. Methods: Six electronic databases were searched using search terms, and 748 articles were identified. Ten articles were eligible based on inclusion and exclusion criteria. Article quality was evaluated using a critical evaluation checklist for manuscripts before performing the systematic review. Results: Although education programs for preventing diabetic foot ulcers have been continuously studied for 20 years, the number of studies is small, and to our knowledge, there have been no studies reporting on the subsequent prevalence of foot ulcers or amputations. While the effect of these education programs on knowledge (ES=1.23) and self-management behavior (ES=.96) was shown to be statistically significant, it was not shown to be significant in preventing diabetic foot ulcers (ES=.03). Conclusion: The actual preventive effect on the prevalence of foot ulcers, through education programs for preventing diabetic foot ulcer could not be determined. Our findings highlight a potential need for studies aimed at developing effective programs to improve education on preventing diabetic foot ulcers.

The Effect of Korean Medical Treatment on Suspected Diabetic Muscle Infarction (DMI)

  • Objectives : This study reports the clinical effects of Korean medical treatment on a patient with suspected diabetic muscle infarction (DMI). Methods : A patient diagnosed with spinal stenosis was suspected of DMI. The patient was treated with acupuncture, bee-venom pharmacopuncture and gastrocnemius stretching. Symptoms were evaluated by verbal numeric rating scale (vNRS), Oswestry disability index (ODI) and Manchester foot pain and disability index (MFPDI). Results : After approximately 5 weeks of Korean medical treatment, including 8 bee-venom treatments, vNRS, ODI, and MFPDI all decreased. Conclusion : The results suggest that Korean medical treatment is effective for treating pain caused by DMI.

고암모니아혈증을 동반한 전신성 염증으로 인한 당뇨병성 케톤산증 환자 치험 1례 (A Case Report of a Patient Diagnosed with Diabetic Ketoacidosis Accompanied by Hyperammonemia from Systemic Inflammation)

  • A 42-year-old male patient with prolonged throat pain and discomfort, dry mouth, and general weakness and recently diagnosed with tonsillitis, pulmonary embolism, and venous thrombosis was admitted to the internal Korean medicine department. A sudden onset of diabetic ketoacidosis with hyperammonemia was diagnosed on the second day of treatment. During admission, the patient received insulin therapy, hydration, and traditional Korean medicine treatment, including herbal medicine. Subjective symptom change was evaluated daily and blood glucose level checked five times per day. At discharge, the patient's fasting and postprandial blood sugar levels were adequate. After an additional two weeks of herbal treatment, the symptoms were significantly ameliorated. Thus, having been admitted with dysregulated glucose metabolism leading to a hyperglycemic crisis after a series of inflammatory events, the patient showed symptomatic improvements and decreased blood glucose after 18 days of hospitalization and treatment.

비만형 당뇨환자의 웹기반 당뇨 교육 효과 (Effects of Web-based Diabetic Education in Obese Diabetic Patients)

  • Purpose: The purpose of this study was to evaluate the effect of web-based diabetic education on plasma glucose and serum lipids in obese people with diabetes. Method: A random allocation design with control and experimental groups being assessed pre- and post-intervention was used. Eighteen patients were randomly allocated to an intervention group and 16 to a control group. Participants were requested to input their blood glucose levels weekly for 3 months at http://www.biodang.com by cellular phone or wire Internet. The researcher sentoptimal recommendations to each patient weekly for 3 months using a short message service (SMS) of the cellular phone and wire Internet. Results: Patients in the intervention group had a mean decrease of $1.2\%$ in glycosylated haemoglobin ($HbA_{1}c$) levels and those in the control group had no difference in HbA1c levels. There was a significant mean change in 2-hour post prandial blood glucose (2HPPG) for the intervention group, with a mean change of -120.1 mg/dl. The mean change in the control group, however, was not significant. Conclusion: These findings indicate that this web-based intervention using SMS of the cellular phone for 3 months improved $HbA_{1}c$ and 2HPPG, but did not affect total cholesterol, triglyceride, and high density lipoprotein cholesterol in obese type 2 diabetic patient.

족궐음간경 자침을 이용한 당뇨병성 안근마비 1례 (One Case of Diabetic Ophthalmoplegia Which Was Treated Acupuncture at Jok-Gwoleum-Gan-Gyeong)

  • Objectives : This study is to report one case of the diabetic ophthalmoplegia by acupuncture at Jok-Gwoleum-Gan-Gyeong. Methods : The patient was treated with acupuncture and herbal medicine for about 5 weeks. We evaluated the results of the treatment by observing the patient's symptoms. Results : After acupuncture the patient's symptoms such as strabismus, headache, ptosis, Rt eye pain and discomfort and both shoulder pain were considerably reduced. Conclusions : These results support that acupuncture at Jok-Gwoleum-Gan-Gyeong can have a meaningful effect in improving symptoms of diabetic ophthalmoplegia.

당뇨병성 하지궤양을 가진 농촌 노인의 상처간호 사례보고 (Wound Care in an Older Rural Patient With Diabetic Lower Extremity Ulcers: A Case Report)

  • Purpose: This study describes a case report of wound care performed by a community health practitioner for a rural older adult with diabetic ulcers. Methods: Data were collected from a rural older adult who had a diabetic lower extremity ulcer, and wound dressing was applied over 20 weeks after the skin graft operation. Results: The patient visited a primary healthcare post for regular ulcer care, and a community health practitioner applied appropriate wound dressings and managed the patient's diabetic condition. Conclusion: In rural areas, where healthcare resources are scarce compared with urban areas, rural nurses, such as community health practitioners, are primarily required to manage rural residents' health problems. Advanced nursing practices such as complicated wound care should also be appropriately provided to patients by enhancing rural nurses' competencies in the changing health care environment.

Using 3D Bioprinted Autologous Minimally Manipulated Homologous Adipose Tissue for Limb Salvage in Treating Diabetic Foot Ulcer

  • Reconstructive surgeons face challenges when considering limb salvage methods for the treatment of diabetic foot ulcers (DFUs). In this article, we present our experience with autologous fat grafting as a viable alternative in cases where flap reconstruction is difficult. We encountered a 78-year-old female patient with a nonhealing DFU who had multiple comorbidities, including renal failure and severe peripheral arterial disease. During the initial multidisciplinary meeting, due to extensive necrosis and osteomyelitis, amputation was recommended. However, the patient expressed a strong preference for a salvage procedure and refused amputation. After careful consideration, we opted to reconstruct the patient's foot using three-dimensional bioprinted autologous minimally manipulated homologous adipose tissue. The AMHAT was engrafted well without complications such as autolysis, graft failure, or infection. After the operation, the large defect with partial bone exposure was covered with healthy granulation tissue. The size of the wound decreased to less than half its original size after 6 weeks of surgery, and it decreased to less than 25% after 12 weeks of surgery. The AMHAT may be an appealing treatment option for diabetic foot patients who are unsuitable for flap reconstruction due to comorbidities.
