Effects of PALMIWON on Cell Viability of Immune Cell and ${\beta}-cell$ (취장소도세포와 면역세포에 미치는 팔미원의 영향)

  • In order to investigate the usability of PALMIWON as antidiabetic immuno-modulating prescription for Insulin-dependent diabetes, we studies the effects of PALMIWON on immune cell and ${\beta}-cell$. U937 was used as the model of immune cell and RINm5F as the model of ${\beta}-cell$. The effects of PALMIWON was measured by cell viability in terms of MIT assay. As a result, PALMIWON and the compositional drugs showed the different effects m immune cell and ${\beta}-cell$. Cell viability of U937 was significantly decreased wheras that of RINm5F was no significantly difference between drug treated group and control, or significantly less reduction compared with U937. It implies that PALMIWON is useful as immunotherapeutic agents in the prevention and therapy of type 1 diabetes.

Hot water extract of Loliolus beka attenuates methylglyoxal-induced advanced glycation end products formation in human umbilical vein endothelial cells

  • Over production of methylglyoxal (MGO) a highly reactive dicarbonyl compound, has been associated in progressive diabetes with vascular complication. Therefore, we investigated whether hot water extract of Loliolus beka meat (LBM-HWE) presents a preserve effect against MGO-induced cellular damage in human umbilical vein endothelial cells (HUVECs). The LBM-HWE extract showed to inhibit MGO-induced cytotoxicity. Additionally, the LBM-HWE reduced mRNA expression of pro-inflammatory cytokines, and reduced MGO-induced advanced glycation end product (AGEs) formation. Furthermore, LBM-HWE induced glyoxalase-1 mRNA expression and reduced MGO-induced carbonyl protein formation in HUVECs. The results implicate that LBM-HWE has protective ability against MGO-induced HUVECs toxicity by preventing AGEs formation. In conclusion, LBM-HWE could be used as a potential treatment material for the prevention of vascular complications of diabetes.

Health and Economic Costs of Physical Inactivity

  • Physical inactivity has reached epidemic levels in developed countries and is being recognized as a serious public health problem. Recent evidence shows a high percentages of individuals worldwide who are physically inactive, i.e. do not achieve the WHO's present recommendation of 150 minutes of moderate to vigorous intensity per week in addition to usual activities. Living in sedentary lifestyle is one of the leading causes of deaths and a high risk factor for several chronic diseases, like cancer, cardiovascular disease, diabetes type 2, and osteoporosis. This article summarizes evidence for relative risk of the civilization diseases attributable to physical inactivity and the most important conclusions available from the recent investigations computing the economic costs specific to physical inactivity. The findings provide health and economic arguments needed for people to understand the meaning of a sedentary lifestyle. This may be also useful for public health policy in the creation of programmes for prevention of physical inactivity.

Association of Demographic and Socioeconomic Factors With Risk Factors for Chronic Kidney Disease

  • Objectives: The goal of this study was to examine the association of various demographic and socioeconomic factors with risk factors for chronic kidney disease (CKD). Methods: We used nationally representative pooled data from the Korea National Health and Nutrition Examination Survey (KNHANES), 2007-2013. We estimated the glomerular filtration rate (GFR) using the Modification of Diet in Renal Disease equation. We defined CKD as a GFR <60 mL/min/$1.73m^2$, and 1304 of the 45 208 individuals included in the KNHANES were found to have CKD by this definition. The outcome variable was whether individual subjects adhered to the CKD prevention and management guidelines recommended by the Korea Centers for Disease Control and Prevention. The guidelines suggest that individuals maintain a normal weight, abstain from alcohol consumption and smoking, manage diabetes and hypertension, and engage in regular exercise in order to prevent and manage CKD. Results: This study found that individuals with CKD were more likely to be obese and have hypertension or diabetes than individuals without CKD. In particular, male and less-educated CKD patients were less likely to adhere to the guidelines. Conclusions: Although the prevalence of CKD, as indicated by the KNHANES data, decreased from 2007 to 2013, the prevalence of most risk factors associated with CKD fluctuated over the same time period. Since a variety of demographic and socioeconomic factors are related to the successful implementation of guidelines for preventing and managing CKD, individually tailored prevention activities should be developed.

Clinical Practice Guideline for Sasang Constitutional Medicine: Prevention and Risk Fators of Sasangin Disease (사상체질병증 임상진료지침: 예방 및 위험인자)

  • Objectives This research was proposed to present Clinical Practice Guideline (CPG) for Prevention of Sasangin disease pattern of Sasang Constitutional Medicine (SCM) and diseases closely related with Sasang constitution. Each CPG was developed by the national-wide experts committee consisting of SCM professors. Methods At first, we searched the literatures related to SCM such as "Dongeuisusebowon", Textbook of SCM and Clinical Guidebook of SCM. Also we searched the articles related to the studies about risk factors for Sasangin disease pattern of both at home and abroad. Finally, we selected leading risk factors of Sasangin disease pattern and developed CPG for prevention of Sasangin disease pattern of SCM. And then, we searched the literatures related SCM such as "Dongeuisusebowon" and the articles on the correlation between disease and Sasang constitution using case-control studies, observational studies or cross sectional studies of both at home and abroad. Next, we selected diseases closely related with Sasang constitution on the basis of articles including prevalence rate and odds ratio between disease and Sasang constitution and finally developed CPG for these diseases. Results and Conclusions We categorized risk factors of Sasang disease pattern into 2 types: non-modifiable and potentially modifiable. 3 items (age, sex and genetic factors) were classified as non-modifiable risk factors of Sasang disease pattern. 6 items (original symptom, stress, diet and nutrition, physical activity, alcohol and drug misuse) were classified as less well-documented or potentially modifiable risk factors of Sasangin disease pattern. We found out Sasang constitution is more likely to develop some diseases. It was proven that Sasang constitution increase the risk of hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. And there is high probability of Sasang constitution being potential risk factor for obesity, hyperlipidemia, allergy and cancer. Also, we found out Taeeumin is independent risk factor for hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. Therefore we recommend that Taeeumin need to prevent these disease by regular checkups and aggressive management.

Difference in Knowledge and Learning Needs of the Coronary Artery Disease according to the General Characteristics of the Patients with Type 2 Diabetes Mellitus (제 2형 당뇨병 환자 특성에 따른 관상동맥질환 지식과 교육요구도 차이)

  • Purpose: The purpose of this study was to assess the difference in knowledge and learning needs of the coronary artery disease according to the general characteristics of the patients with type 2 diabetes mellitus. Method: The participants were 188 patients who had diabetes mellitus. Data were obtained between January and April 2006 by asking the knowledge and learning needs of the coronary artery disease. Data were analyzed using SAS program. Results: Participants' knowledge level was high in the cause and prevention, but the level was low in the symptom and occurrence of pain. Learning need for "Influence of smoking on heart disease" and "The structure and functions of the heart" was great, but learning need for "Complete diagnosis" and "Management of pain and pressure on the sternum" was little. No significant differences were found in the knowledge level of the coronary artery disease according to the general characteristics. Learning needs were greater in participants under 60 years of age (p=0.011) and in those with low education level (p=0.049). There was a significant correlation between knowledge and learning needs of the coronary artery disease (p=0.003). Conclusion: In planing the education programs, the general characteristics of the patients should be considered.

Risk Prediction Using Genome-Wide Association Studies on Type 2 Diabetes

  • The success of genome-wide association studies (GWASs) has enabled us to improve risk assessment and provide novel genetic variants for diagnosis, prevention, and treatment. However, most variants discovered by GWASs have been reported to have very small effect sizes on complex human diseases, which has been a big hurdle in building risk prediction models. Recently, many statistical approaches based on penalized regression have been developed to solve the "large p and small n" problem. In this report, we evaluated the performance of several statistical methods for predicting a binary trait: stepwise logistic regression (SLR), least absolute shrinkage and selection operator (LASSO), and Elastic-Net (EN). We first built a prediction model by combining variable selection and prediction methods for type 2 diabetes using Affymetrix Genome-Wide Human SNP Array 5.0 from the Korean Association Resource project. We assessed the risk prediction performance using area under the receiver operating characteristic curve (AUC) for the internal and external validation datasets. In the internal validation, SLR-LASSO and SLR-EN tended to yield more accurate predictions than other combinations. During the external validation, the SLR-SLR and SLR-EN combinations achieved the highest AUC of 0.726. We propose these combinations as a potentially powerful risk prediction model for type 2 diabetes.

Physical and Dietary Characteristics in Women with Gestational Diabetes Mellitus (임심성 당뇨병환자의 신체적 특성과 식습관)

  • The present study was conducted to investigate physical characteristics, food habits of pregnant women with gestational diabetes mellites(GDM). The subjects were consecutive pregnant women, 21 GDM and 43 non-GDM controls who received antenatal care at the Obstetrics and Gynecology clinic. There were no significant difference in the mean age between the two groups(GDM:30.8years, non-GDM:30.5years). Significant differences were observed in several anthropometric characteristics such as height(GDM:159.7cm, non-GDM:162.3cm), prepregnancy weight(GDM:61.6kg, non-GDM:54.1kg), body mass index(GDM:23.9kg/$m^2$, non-GDM:20.5kg/$m^2$), and triceps skin fold thicknesses(GDM:30.9mm, non-GDM:24.7mm) between the two groups. The GDM group had an high incidence of(52.4%) family history of diabetes mellitus than those of control group(23.3%). In the GDM group, more irregularities of meals and more bed-time snacking were observed than those of control group. The intake amount of carbohydrates as well as the total energy were significantly higher in the GDM group when thery were compared with the control. These results suggest that the GDM women in Korea do exhibit clearly distinctive anthropometric and dietary characteristics. Nutritional management such as weight control before conception and throughout pregnancy, monitoring total energy and carbohydrate intakes, and implementing more rigorous meal scheduling might be beneficial for the prevention or reduction of the prevalence of the GDM or its complications. Management of Diabetes from HbA1c Test

  • HbA1c test measures the amount of glycated hemoglobin in blood. HbA1c shows the average of blood glucose levels for the past three months, this is a better indicator of how overall diabetes is doing. HbA1c gives a much better idea of how the body is breaking down the glucose. Therefore, this HbA1c is very important tool for maintaining normal glucose levels for pre-and diabetic patients. Total 408 participants were tested HbA1c voluntarily from Chosunilbo Health Expo (8th~11th, July 2010). Through this small-scaled direct HbA1c, about 54.7% (207 out of 408) was shown glucose tolerance and diabetes. However, 61 from 157 participants who were shown under 6.9% HbA1c (normal and pre-diabetic stage) are taking only antidiabetic drugs to maintain a normal blood glucose. Regular HbA1c test can bring an important management and awareness about controlling blood sugar level and prevention of diabetic complications.

Research Trend on Diabetes Mobile Applications: Text Network Analysis and Topic Modeling (당뇨병 모바일 앱 관련 연구동향: 텍스트 네트워크 분석 및 토픽 모델링)

  • Purpose: The aim of this study was to identify core keywords and topic groups in the 'Diabetes mellitus and mobile applications' field of research for better understanding research trends in the past 20 years. Methods: This study was a text-mining and topic modeling study including four steps such as 'collecting abstracts', 'extracting and cleaning semantic morphemes', 'building a co-occurrence matrix', and 'analyzing network features and clustering topic groups'. Results: A total of 789 papers published between 2002 and 2021 were found in databases (Springer). Among them, 435 words were extracted from 118 articles selected according to the conditions: 'analyzed by text network analysis and topic modeling'. The core keywords were 'self-management', 'intervention', 'health', 'support', 'technique' and 'system'. Through the topic modeling analysis, four themes were derived: 'intervention', 'blood glucose level control', 'self-management' and 'mobile health'. The main topic of this study was 'self-management'. Conclusion: While more recent work has investigated mobile applications, the highest feature was related to self-management in the diabetes care and prevention. Nursing interventions utilizing mobile application are expected to not only effective and powerful glycemic control and self-management tools, but can be also used for patient-driven lifestyle modification.
