Merkel cell carcinoma: A series of seven cases

  • Background Merkel cell carcinoma (MCC) is a rare neuroendocrine malignancy affecting the skin, for which timely diagnosis and aggressive treatment are essential. MCC has most often been reported in Caucasians, and case reports in Asians are rare. This study presents our experiences with the surgical treatment and radiotherapy of MCC in Asian patients. Methods We retrospectively reviewed the records of seven MCC patients between 2000 and 2018 from a single institution, and analyzed patient characteristics, tumor characteristics, surgical treatment, sentinel lymph node evaluation, reconstruction, adjuvant radiation therapy, and prognosis. Results Eight MCC lesions occurred in seven patients, most commonly in the head and neck region. All patients underwent surgical excision with reconstruction. The final surgical margin was 1.0 cm in most cases, and reconstruction was most commonly performed with a splitthickness skin graft. Five patients received adjuvant radiotherapy, and two patients received sentinel lymph node biopsy. During the follow-up period, three patients remained well, two died from other causes, one experienced recurrence, and one was lost to follow-up. Conclusions We treated seven Asian MCC patients and our series confirmed that MCC is a very dangerous cancer in Asians as well. Based on our experiences, thorough surgical excision of MCC with histopathological clearance should be considered, with sentinel lymph node evaluation if necessary, followed by appropriate reconstruction and careful postoperative observation. Adjuvant radiation therapy is also recommended for all Asian MCC patients. The results of this case series may provide guidance for the treatment of Asian MCC patients in the future.

Efficacy of Combination Treatment of Herbal Medicine and Western Medicine for the Treatment of Type 1 Diabetes in children : Systemic Review and Meta-analysis (소아 1형 당뇨의 한약과 양약 병용 치료에 대한 체계적 문헌 고찰 및 메타분석)

  • Objectives The purpose of this study is to investigate the clinical studies on the effect of herbal medicine in type 1 diabetes in children and to seek better approach of herbal medicine to treat type 1 diabetes in children. Methods This study researched randomized controlled trials through various databases in the world about herbal medicine treatments in type 1 diabetes in children. Results 10 out of 337 studies were selected and analyzed. All studies were conducted in China. All studies were using herbal medicines, as an adjunctive treatment to the main regimen. As a result, the integrated Chinese medicine and western medicine lowered FPG by -1.56 mmol/L and 2hPG by -1.94 mmol/L on average, respectively. The HbA1c also decreased by -1.11% in the treatment group compared to the control group. Total efficacy of the treatment was 1.21 times more effective in the treatment group than in the control group. Conclusions Based on the results of the studies, it seems that the herbal medicine for the treatment of type 1 diabetes in children will be effective as a combination with conventional medicines. Further research is needed to prove the findings of this observatoional studies.

A Case Report of Fibromyalgia Improved by Korean Medical Treatment (섬유근육통의 한방치료 증례보고 1례)

  • Introduction: The aim of this study is to report the effect of traditional Korean medical treatment on fibromyalgia. Case presentation: A patient with fibromyalgia was treated with acupuncture, pharmacopuncture (Bee venom, Hwangryunhaedok-tang), cupping therapy, moxibustion therapy and herbal medicine for 37 days. To reduce the patient's symptoms, we provided herbal medicine three times a day, as well as acupuncture, pharmacopuncture, cupping therapy and moxibustion therapy once a day for 37 days. To evaluate the results of this treatment, we used the Numeral Rating Scale (NRS), Score of Sleeping Quality (SSQ) and Fibromyalgia Impact Questionnaire (FIQ). After treatment, the patient showed that pain was reduced, the score of NRS was improved, FIQ improved from 91 to 69 and SSQ improved from 3 to 1. No adverse reaction was noted. Conclusion: According to the result, traditional Korean medical treatment can be effective and safe for the treatment of fibromyalgia.

Protective effects of saffron against zearalenone-induced alterations in reproductive hormones in female mice (Mus musculus)

  • Objective: Zearalenone (ZEA) is a mycotoxin with potent estrogenic effects. Saffron is an herbal product that has antioxidant activities. The objective of this study was to investigate the protective role of saffron against reproductive toxicity induced by ZEA in female mice. Methods: Ninety 8-week-old female mice were randomly allocated into three treatment groups. The first group received an intraperitoneal injection of ZEA (2.5 mg/kg) on alternate days. The second group received ZEA (2.5 mg/kg) on alternate days plus oral saffron daily (50 mg/kg). The third group was treated with a vehicle of 1% dimethyl sulfoxide (DMSO) on alternate days, as a control. Ten mice were euthanized from each group at 30, 60, and 90 days of treatment. Serum levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), estradiol ($E_2$), and progesterone (P) were assessed. The uterus and ovaries were examined for changes in size or morphology. Results: Serum levels of LH, FSH, $E_2$, and P in the female mice treated with ZEA plus saffron were significantly higher than in those treated with ZEA alone, and were not significantly different from those treated with 1% DMSO. The female mice treated with ZEA alone showed a reduction in size of the uterus and abnormal architecture of the ovaries. Conclusion: The administration of saffron to female mice resulted in a significant reduction in ZEA-induced alterations in reproductive hormone levels, the size of the uterus, and the morphology of the ovaries.

Venous malformations of the head and neck: A retrospective review of 82 cases

  • Background Venous malformations (VMs) are a common type of vascular malformation. However, their causes and management remain unclear, and few studies specific to VMs of the head and neck have been reported. This study describes our experiences with VMs of the head and neck. Methods This retrospective study included 82 patients who underwent treatment for head and neck VMs, among 222 who visited our vascular anomalies center. Medical records between 2003 and 2016 were reviewed to identify common features in the diagnosis and treatment. The diagnosis of suspected head and neck VMs was based on the results of imaging studies or biopsies, and the VMs were analyzed based on magnetic resonance imaging, computed tomography, and Doppler sonography findings. Results VMs were slightly more common in female patients (59.8%), and 45.1% of patients developed initial symptoms at the age of 10 or younger. Lesions were slightly more common on the right side (47.3%). The main sites involved were the cheek (27.7%) and lip area (25.5%). The muscle layer was commonly involved, in 98.7% of cases. Small lesions less than 5 cm in diameter accounted for 60.8% of cases, and well-defined types were slightly more prevalent at 55.4%. Improvement was observed in 77.1% of treated patients. Conclusions Early and accurate diagnosis and appropriate treatment according to individual symptoms are important for successful treatment of VMs. If treatment is delayed, the lesions can worsen, or recurrence becomes more likely. Therefore, VMs require a multidisciplinary approach for early and accurate diagnosis.

Consciousness Recovery by Rituximab after Seizure Control in Cryptogenic New-Onset Refractory Status Epilepticus (잠복신발현난치뇌전증지속상태에서 발작이 조절된 후에 투여한 리툭시맙에 의한 의식 회복)

  • Background: New-onset refractory status epilepticus (NORSE) occurs in people without a history of seizures. In these cases, the seizure causes are unclear, and the seizures are not controlled by standard treatment. Autoimmune encephalitis (AIE) can be a cause of NORSE. Cryptogenic NORSE may be associated with AIE, but antibodies associated with the condition have not yet been identified. Primary immunotherapy may not be effective for AIE. Rituximab has improved the prognosis in some cases. Case Report: We treated a cryptogenic NORSE patient with a combination of antiepileptic drugs and immunotherapy. On the 13th hospital day, the seizures were controlled, but the patient remained in a coma. The patient rapidly recovered after administration of rituximab started on the 26th hospital day. Conclusion: Rituximab may be helpful for cryptogenic NORSE patients in whom primary immunotherapy controls seizures, but fails to improve consciousness.

Evaluation of Anti-Inflammatory Effect of Pulsed Electromagnetic Field on DNCB-Induced Atopic Dermatitis Using Principal Component Analysis (주성분 분석을 이용한 펄스형 전자기장 자극을 통해 DNCB로 유발된 아토피성 피부염의 개선 효과 분석)

  • Atopic dermatitis (AD), a chronic inflammatory skin disease, is characterized by itchy and age-dependent lesions. Previous studies have shown that pulsed electromagnetic field (PEMF) significantly improved chronic ulcers and ununited fractures, providing an evidence for the application of PEMF in resolving inflammation caused by AD. This study investigated the anti-inflammatory effect of PEMF on DNCB-induced AD in animal models. Five male hairless mice (6 weeks old) per group were assigned to a normal group, a sham group, and two PEMF groups (15Hz, 75Hz). Mice were treated with 2,4-Dinitrochlorobenzene (DNCB) to induce uniform AD among all groups excluding a normal group. To examine the inflammatory progress and the improvement of AD after the PEMF stimulation, images are taken with various cameras for non-invasive evaluation and the results are expressed using principal component analysis (PCA) for visualization. The results of this study demonstrated that PEMF effectively improved skin lesions without the use of drugs.

  • Ascites is the accumulation of fluid in the peritoneal cavity, and it is generally allied with liver disease like cirrhosis of the liver. The ascites is commonly associated with liver cirrhosis, malignancy, or cardiovascular disease today. The liver cirrhosis is the most significant cause of ascites in developed countries as per the available data. In the modern medicine system, the treatment includes the restriction of sodium intake, use of diuretics, and paracentesis done in severe cases. There are many side effects of synthetic chemical diuretics, and their long-term use is not recommended. The alternative medicine like Unani system of medicine provides natural diuretics and drugs which also have protective effects on kidney, liver and other internal organs. It is well known in all drug systems that the prognosis is poor in ascites, but the use of Unani herbal medicines can minimize the side effects caused by synthetic chemical diuretics. The natural herbs used for the management of ascites have fewer side effects as compared to synthetic drugs. The natural drugs can be used for longer duration and provide some dietary supplement which improves the quality of life. This review addressed the thorough treatment of ascites by natural diet and medications.

Efficacy of Korean Medicine Combination Treatment on Recurrent Neck Pain After Medical Procedures: A Retrospective Study

  • Background: The aim of this study was to evaluate the efficacy of Korean medicine combination treatment on recurrent neck pain after medical procedures. Methods: This retrospective study included 158 inpatients of the Daejeon Jaseng Hospital of Korean Medicine who were diagnosed with "Cervical disc disorder with radiculopathy (M50.1)" between December 14th, 2017 and May 29th, 2019. The patients were assigned to 1 of 2 groups based on whether they received medical procedures on the cervical spine at least once. Korean medicine combination treatment was evaluated using EuroQol-5 dimensions index (EQ-5D), numeric rating scale (NRS), and neck disability index (NDI) scores. Results: Before and after treatment, the patients who received medical procedures on the cervical spine at least once before admission (Group A) showed a statistically significant difference in the NDI and NRS scores but not in the EQ-5D scores. This was similar to the patients who had not received medical procedures on the cervical spine before admission (Group B) they showed a statistically significant difference in the NDI and NRS scores but not in the EQ-5D scores. When comparing the results of Group A and Group B before and after treatment, no statistically significant differences were observed in the EQ-5D, NDI, and NRS scores. Conclusion: Korean medicine combination treatment improves the neck functional disability of patients who suffer from recurrent neck pain despite patients having undergone medical procedures.

Clinical and radiographic features of facial cosmetic materials: A systematic review

  • Purpose: The aim of this study was to systematically screen the literature for studies reporting cosmetic material in the oral and maxillofacial complex to shed light on the types of cosmetic materials, their radiographic appearance, and possible complications. Materials and Methods: Five electronic databases were reviewed for eligible studies. The general search terms were "cosmetic," "filler," "face," and "radiograph." Demographics, material types, clinical and radiographic presentation, and complications were recorded. Results: Thirty-one studies with 53 cases met the inclusion criteria. The mean age was 52.6±15.4 years with a 4 : 3 female-to-male ratio. The most common material was calcium hydroxyapatite (CaHa) (n=14, 26.4%), found incidentally. The materials were generally located within the upper cheek and zygoma (n=35, 66.0%), radiographically well-defined (n=44, 83%), and had no effects on the surrounding structures (n=27, 50.9%). The internal structure was radiopaque (calcification, hyperdensity) for gold wires, CaHa, bone implants, and secondary calcification or ossification. Outdated cosmetic materials or non-conservative techniques were infiltrative, had effects on the surrounding structures, and presented with clinical signs, symptoms, or complications. Conclusion: Conventional radiography, cone-beam computed tomography, and multi-detector computed tomography are useful to differentiate several cosmetic materials. Their magnetic resonance imaging appearance was highly variable. The infrequent inclusion of cosmetic materials in the differential diagnosis implies that medical and dental specialists may be unfamiliar with the radiographic appearance of these materials in the face.
