A Study on the Characteristics and Consultation Request Type of Inpatients Referred for Depressive Symptoms (우울 증상으로 의뢰된 입원환자의 임상적 특징 및 자문 의뢰 형태에 관한 연구)

  • Objectives : The purpose of this study is to investigate the characteristics of depressive patients who admitted to general hospital. We examined the clinical characteristics of patients who were referred to the Department of Psychiatry as depressive symptoms, according to the type of consultation request, and comparing 'with re-consultation' and 'without re-consultation' groups. Methods : We performed a retrospective chart review of 4,966 inpatients who were referred to the Department of Psychiatry from August 2005 to December 2011. Results : For about 6 years, among the inpatients referred for psychiatric consultation, a total of 647 patients were referred for depressive symptoms, accounting for 13.82% of the total consultations. The average age of depressive patients was 58.6 years, which was higher than the average of 56.4 years of overall patients. Among the depressive patients, 275 patients were included in 'with re-consultation' group and there was no statistically significant difference when comparing 'with re-consultation' group and 'without re-consultation' group. However, there was a difference in the tendency of the two groups in the type of consultation request. 'With re-consultation' group was in the order of frequency of consultation type 3-2-1, whereas the 'without re-consultation' group was in the order of frequency of consultation type 2-3-1. Conclusions : The group of inpatients who were referred for depressive symptoms in general hospital showed the largest proportion of the group of patients referred to the Department of Psychiatry. 'With re-consultation' group had a higher rate of re-consultation due to the occurrence of new symptoms after hospitalization compared to 'without re-consultation' group. Therefore, doctors in each department and psychiatrists should pay attention to the depressive symptoms of inpatients and actively discuss treatment plans to improve the quality of medical services, identify risk factors, and make efforts to intervene early if necessary.

A PRELIMINARY STUDY OF CHILDREN WITH LEARNING DISORDER IN KOREA (한국에서의 학습장애 아동에 대한 예비적 연구 - 종합병원 학습장애 특수 클리닉 내원 아동을 중심으로 -)

  • This is a preliminary report on the first segment of a continuing and prospective teaming disorder study project in Korea. Study subjects were 197 children, aged between 6 and 15 referred for psychiatric evaluation of scholastic problems. Demographic data, psychiatric diagnoses and intelligence and achievement test results were reviewed and analyzed. Analyses of data lead to the following conclusions : (1) About 20.8% of children referred for scholastic problems were diagnosed of teaming disorder(LD). The most prevalent diagnosis among these children with scholastic problem was emotional disorder, especially depressive disorder(33%), (2) The comorbid rate of attention deficit/hyperactivity disorder(ADHD) of 41 children with LD was 44%, (3) Male/female ratio was 5.8:1 among all of the LD children, 17:1 among children with LD and ADHD and 3.6:l among children with LD but without ADHD, (4) 83% of children with LD scored above middle level on socioeconomic status(SES), (5) Age, SES, IQ, family psychiatric history, past history of medical and psychiatric illness, onset of age, pattern of peer relationship, number of friends, presence of adaptation problem and academic achievements of children with LD and ADHD compared to those of children with LD but without ADHD. No significant differences between two groups were found on age, SES, IQ, family psychiatric history, past history of medical and psychiatric illness, pattern of peer relationship, number of friends and presence of adaptation problem. However, there were significant differences in academic achievements of Korean language total, speaking and listening score, arithmetic score, social science score and music score of children with LD and ADHD compared to those of children with LD but without ADHD. Also there was an ealier onset of age in LD and ADHD group when compared to LD but without ADHD group.

Lifetime Prevalence and Comorbidity in Obsessive-Compulsive Disorder and Subclinical Obsessive-Compulsive Disorder in Korea (강박장애 및 아임상형 강박장애의 평생 유병률과 병발성)

  • Background : In spite of the worldwide relevance of obsessive-compulsive disorder Ed-highlight : Unclear. Perhaps consider changing word choice. (OCD), there are considerable differences in prevalence, sex ratio, comorbidity patterns, and sociodemographic correlates. Data on subclinical OCD have been sparse to date. Methods : Data stemmed from the Korea Epidemiologic Catchment Area (KECA) study which had been carried out from April to December 2001. Korean versions of DSM-IV adapted Composite International Diagnostic Interview were administered to a representative sample of 6275 persons aged 18-64 living in the community. DSM-IV based criteria for subclinical OCD were applied. Results : The lifetime prevalence rates for OCD and subclinical OCD were 0.8% and 6.6%, respectively. In both OCD and subclinical OCD, the rates for males and females were not statistically different. OCD was demonstrated to be associated with depressive disorder, bipolar disorder, social phobia, generalized anxiety disorder, and alcohol and nicotine dependence. Additionally, subclinical OCD was associated with posttraumatic stress and somatoform disorders. Comorbidity rates in subclinical OCD were lower than those in OCD. Conclusions : The lifetime prevalence rate for OCD was less than 1% in the Korean general population. Age distribution and comorbidity patterns suggest that subclinical OCD represents a broad and heterogeneous syndrome and not simply a milder form of OCD.

The Effect of Multidimensional Five Senses Treatment on Patients with Depression or Anxiety (오감을 이용한 다각적 치료의 불안, 우울 증상 치료 효과)

  • Objectives: This study aimed to examine the effects of multidimensional five senses treatment on patients with depression or anxiety. Methods: This study examined the effects of multidimensional five senses treatment for patients with depression or anxiety who visited the neuropsychiatric clinic of Korean medicine. STAI-X1,X2, BDI, BAI, PAS were used to determine whether the mood (depression or anxiety) of patients was improved. Results: Our results showed that multidimensional five senses treatment improved the depressive symptoms or anxiety symptoms. In addition, after the multidimensional five senses treatment, PAS score in patients with panic disorder was improved. Conclusions: The multidimensional five senses treatment was effective for the improvement of patients with depression or anxiety.

Antidepressant-Induced Somnambulism:A Case of Mirtazapine (항우울제와 몽유병:Mirtazapine에 의한 증례 1례)

  • Somnambulism is classified as a parasomnia and has been reported with the use of antidepressants and other psychotropics. However, to our knowledge, there have been no reports of somnambulism associated with the use of mirtazapine (a noradrenergic and specific serotonergic antidepressant;NaSSA). We experienced a case of the mirtazapine (30 mg/day)- induced somnambulism in a 65 year-old woman with major depressive disorder who has never been diagnosed as panic disorder, somnambulism, other parasomnias, neurological disorders (including epilepsy), and other medical diseases. The sleepwalking symptoms disappeared after the antidepressant was replaced with paroxetine (20 mg/day).

  • Bibliotherapy is the field of clinical counceling to treat the emotional, psychiatric and social maladjustment. Neurofeedback is a means by which participants can team voluntary control of the EEG and has been applied to a range of clinical conditions such as epilepsy, attention deficit hyperactivity disorder, depressive disorder. 'Ul-zeong' comes from obstruction of qi by stress. The mind affect the body. The patient has depressed mood, irritable sign, chest discomfort, flank pain, angry state and some strange feeling on the throat. In this case, a female patient, 30 years old, who complained of amnesia, deficiency of the power of the attention and concentration depersonalization, depressed mood, insomnia ect. We treated the patient with bibliotherapy, neurofeedback and oriental medical treatment such as heral medicine, acupuncture treatment and aroma therapy. In result, the symptoms which she complained were improved.

Familial, Cognitive, and Behavioral Characteristics of Adolescents with Depression

  • Objectives: Adolescent depression is a complex disorder influenced by a variety of personal and familial factors. In this study, we compared the familial, cognitive, and behavioral characteristics of adolescents with and without diagnosed depression. Methods: Forty adolescents with depressive disorder were recruited from two psychiatric clinics, along with 46 healthy adolescents from a middle school and a high school. We then compared the participants' cognitive and behavioral characteristics and the child-rearing attitudes of their parents. Results: Compared to the healthy adolescents, the adolescents with depression exhibited lower self-esteem, higher emotional reappraisal, greater disruptive behavior, and lower attention. Furthermore, compared to the mothers of the healthy adolescents, the mothers of those with depression reported less affective, less autonomic, and more rejecting parenting attitudes towards their children. Conclusion: We found that attentional problems, negative parenting attitudes, negative self-cognition, and expressive suppression are all associated with adolescent depression. Parenting education and interventions appear to be needed to correct the negative cognitions of adolescents with depression.

  • Lithium is mainly prescribed for manic and depressive disorder, also frequently prescribed for the other diseases such as migraine, cluster headache, alcoholism, and obsessive-compulsive disorder. An acute lithium intoxication occurs in cases of patients ingesting large amount lithium at a time, a chronic lithium intoxication occurs in patients on chronic lithium therapy. Acute or chronic lithium poisoning occurs frequently in case of patients on chronic lithium therapy ingesting larger dose than prescribed. Manifestations of lithium poisoning are various. It is possible nervous, cardiovascular, renal, gastrointestinal and endocrine systems to be involved. Due to intracellular high concentration, mortality rate is high in acute lithium intoxication patients on chronic lithium therapy. We report a case of acutely intoxicated 40-year-old male on chronic lithium therapy. His chief complaints were deterioration and high fever. On his arrival to an emergency department, he was in cardiac arrest. He restored return of spontaneous circultion (ROSC) 5 minutes later after cardiopulmonary cerebral resuscitation (CPCR) and referred to department of internal medicine for hemodialysis. Vigorous treatment was given to the patient, but he was expired at 4th hospital day.

A Study of the Job Stress in Auto Part Manufacturing Company (자동차부품 제조업체에서의 직무스트레스에 관한 연구)

  • The job stress is rising as a social issue in all industries. So many manufacturing companies have been trying to prevent the job stress. Recently, the job stress is the important cause which lead to WMSDs, depressive disorder, anxiety disorder and sleep disturbance. However, the cause of the job stress is very variety. So management is very difficult. The purpose of this study is to investigate the group of the job stress by Karasek model from workers in auto part manufacturing company. The survey was using Karasek job stress model and was administered to 181 workers of auto part manufacturing company. Job demand median and job decision median was in 30.0 and 50.0. From this study, following results were obtained. 1) The gender of workers according to the groups of job stress was statistically significant(p<0.05). 2) The duration of working of workers according to the groups of job stress was statistically significant(p<0.05). 3) The intensity of work of workers according to the groups of job stress was statistically significant(p<0.01). The result of this study will help to identify the group of job stress and improve personal coping ability, systematic measure of company.

  • Neuralgic amyotrophy (NA) is an idiopathic disease characterized by muscular atrophy accompanied by neuralgia, where acute pain in the limb, mostly including the shoulders, leads to muscle weakeness and atrophy in the limb days to weeks after the onset of pain. Although its exact pathogenesis is unknown, genetic and nongenetic factors, such as infection, surgery, and trauma are suspected contributors. We report this case as we diagnosed NA of the right lower limb via enhanced MRI as well as EMG and have followed up the patient's prognosis and radiologic changes for one and a half years. In addition, the patient had been diagnosed with neuropsychiatric disorders, such as major depressive disorder and somatic symptom disorder.
