한국 노인의 구강보건지수와 치과보철물 필요도가 구강건강관련 삶의 질에 미치는 영향 (The Effect of Oral Health Index and Dental Prosthesis Needs on Oral Health Related Quality of Life in Korean Elderly)

  • 이 연구는 한국 노인의 구강건강상태와 치과보철물 필요도가 구강건강관련 삶의 질에 미치는 요인을 경험적으로 규명함으로써 향후 노인대상의 구강건강사업에 필요한 기초자료를 제공하고자 한다. 이 연구자료는 2010년 1월부터 12월까지 질병관리본부에서 시행한 '국민건강영양조사 제5기 1차 및 2차년도를 활용 구강검사까지 완료한 65세 이상 노인 2,389명 중 검사가 미흡한 노인 116명을 제외한 2,273명을 최종 연구대상으로 하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여 전산통계 처리하였으며, 분석방법은 빈도분석, 다중회귀분석, 구조방정식 모형을 이용하였다. 주관적 구강건강상태에 영향을 미치는 요인은 나이가 적을수록, ST지수가 높을수록, FS-T 지수가 높을수록, T-Health 지수가 높을수록, 상악 크라운 요구도가 낮을수록, 상 하악 의치 요구도가 낮을수록 주관적 구강건강상태가 높은 것으로 나타났다. 씹기문제에 영향을 미치는 요인은 소득수준이 높을수록, T-Health 지수가 높을수록, 상악 크라운 요구도가 낮을수록, 상 하악 의치 요구도가 낮을수록 씹기 문제가 불편하지 않는 것으로 나타났다. 말하기 문제에 영향을 미치는 요인은 ST지수가 높을수록, T-Health 지수가 높을수록, 상악 임플란트 요구도가 낮을수록, 상악 의치 요구도가 낮을수록 말하기 문제가 불편하지 않는 것으로 나타났다. 이상의 결과를 토대로 구강건강상태와 치과보철물 필요도는 구강건강관련 삶의 질에 상당한 영향을 미치고 있음을 알 수 있었고, 향후 노인의 구강건강 관련 삶을 증진시키기 위해서는 노인을 위한 구강건강증진사업과 치과보철 사업이 필요가 요구된다.

노인의 구강건강상태 및 구강건강행태가 의치필요와의 관련성 (A study on Relationship between the Appearance of Needing Denture Oral Health Condition and Oral Health Behavior of the Elderly)

  • 본 연구는 고령화 사회를 맞이하여 노인들의 구강건강상태와 의치필요도를 알아보고자 하였다. '국민건강 영양조사 제5기 1차년도 (2010)원시자료'의 건강설문조사와 구강검진을 활용하여 노인의 주관적 구강건강 수준과 구강관리 행태에 따른 의치필요도를 조사하기 위하여 분석하였다. 목표 모집단을 우리나라에 거주하고 있는 대한민국 국민이며, 65세 이상 노인표본 1,419명을 조사하였다. 연구대상자의 구강건강상태에 따른 의치필요도의 상관관계를 알아보기 위하여 카이제곱검정과 교차비 확률을 이용하여 분석하였다. 연구결과 주관적인 구강건강상태 중 씹기 불편(p<0.001)과 말하기 불편(p<0.001)이 유의미한 관련성이 있는 것으로 검토되었다. 객관적 구강건강상태에서는 치아우식증 유병률(p<0.001)이 있는 노인에서 의치필요도가 높게 나타났다. 구강건강관리 행태에 따른 의치 필요도는 구강위생보조용품을 사용하지 않는 노인과 치과치료를 받지 않는 노인에서 의치필요도가(p<0.001)매우 높은 것으로 검토되었다. 본 연구의 결과로부터 노인의 주관적 객관적 구강건강상태 및 구강건강관리 행태에 따른 의치필요도가 파악되었으며 이러한 결과의 바탕으로 노인의 구강건강상태가 지속적으로 유지 보존될 수 있도록 활발한 구강보건교육 및 전문가 관리가 활발하게 이루어질 수 있도록 프로그램이 계발되어야 할 것이다.

PHOTOCATALYTIC ANTIEUNGAL ACTIVITY AGAINST CANDIDA ALBICANS BY $TiO_2$ COATED ACRYLIC RESIN DENTURE BASE

  • Statement of problem. Proliferation of Candida albicans is primarily within the plaque on the fitting surface of the denture rather than on the inflamed mucosa. Consequently, the treatment of the denture is equally important as treatment of the tissue. Cleansing and disinfection should be efficiently carried-out as the organisms can penetrate into the voids of the acrylic resin and grow in them, from which they can continue to infect and reinfect bearing tissues. Purpose. The purpose of this study was to evaluate the applicability of photocatalytic reaction to eliminate Candida albicans from acrylic resin denture base, and to investigate the anti-fungal effect with various UVA illumination time. Materials and Methods. The specimens were cured by the conventional method following the manufacturer's instruction using thermal polymerized denture base resin (Vertex RS: Dentimex, Netherlands). $TiO_2$ photocatalyst sol(LT), which is able to be coated at normal temperature, was made from the Ti-alkoxide progenitor. The XRD patterns, TEM images and nitrogen absorption ability of the $TiO_2$ photocatalyst sol(LT) were compared with the commercial $TiO_2$ photocatalyst P-25. The experimental specimens were coated with the mixture of the $TiO_2$ photocatalyst sol(LT) and binder material (silane) using dip-coater, and uncoated resin plates were used as the control group. Crystallinity of $TiO_2$ of the specimen was tested by the XRD. Size, shape and chemical compositions were also analyzed using the FE-SEM/ EDS. The angle and methylene blue degradation efsciency were measured for evaluating the photocatalytic activity of the $TiO_2$ film. Finally, the antifungal activity of the specimen was tested. Candida albicans KCTC 7629(1 ml, initial concentration $10^5$ cells/ ml) were applied to the experiment and control group specimens and subsequently two UVA light source with 10W, 353 nm peak emission were illuminated to the specimens from 15cm above. The extracted $2{\mu}l$ of sample was plated on nutrient agar plate ($Bacto^{TM}$ Brain Heart Infusion; BD, USA) with 10 minute intervals for 120 minute, respectively. It was incubated for 24 hours at $37^{\circ}C$ and the colony forming units (CFUs) were then counted. Results. Compared the characteristics of LT photocatalyst with commercial P-25 photocatalyst, LT were shown higher activity than P-25. The LT coated experimental specimen surface had anatase crystal form, less than 20 nm of particle size and wide specific surface area. To evaluate the photocatalytic activity of specimens, methylene blue degradation reaction were used and about 5% of degradation rate were measured after 2 hours. The average contact angle was less than $20^{\circ}$ indicating that the LT photocatalyst had hydrophilicity. In the antifungal activity test for Candida albicans, 0% survival rate were measured within 30 minute after irradiation of UVA light. Conclusion. From the results reported above, it is concluded that the UVA-LT photocatalytic reaction have an antifungal effect on the denture surface Candida albicans, and so that could be applicable to the clinical use as a cleaning method.

65세 이상 노인의 가철성 의치만족도와 구강건강관련 삶의 질의 관련성 (The Relevance of Oral Health Related Quality of Life of the Oral-Cavity for Removable Denture Satisfaction of Senior Citizens Over 65 Years of Age)

  • Objectives : This study explored ways to improve the quality of life related to oral health and the oral health of senior citizens by identifying the relevance of oral health-related quality of life issues, and satisfaction with dentures in elderly 65 years of age or older. Methods : This study surveyed 220 senior citizens who are currently using dentures and 65 years of age or older living in Busan. As a result, 200 questionnaires were returned. All of these questionnaires were used in the final analysis. The method of analysis was descriptive statistics using SPSS VER. 19.0, t-test, ANOVA, Pearson correlation coefficients, and regression analysis. Results : First, when verifying the difference in the quality of the health-related life of the oral cavity in accordance with the general characteristics, only average monthly income showed a significant difference. Second, the results of the analysis for factors that affect the quality of the health -related life of the oral cavity, fixation satisfaction, and mastication satisfaction, in that order, affected the general satisfaction of the elderly. Conclusions : Dentures satisfaction and the quality of the health-related life of the oral cavity show a high correlation and causality.

구미 지역 노인의 주관적 건강상태 및 구강건강상태 수준비교 (Comparison of subjective health condition and subjective oral health condition of the elderly in Gumi)

  • Objectives : Rapid aging is a big social issue and aging influences on the quality of life in elderly people. Oral health in elderly people related to the general health condition and it is necessary to improve the quality of life in the elderly people. The aim of the study is to investigate the influencing factors of oral health in the elderly people. Methods : Self-reported questionnaire included general characteristics, subjective health and oral condition and denture use. Subjects were 177 elderly people in Gumi, Korea. SAS (Ver.9.2) Program was used for the collected data to perform frequency analysis, cross tabulation, t-test and ANOVA. Results : Higher subjective oral health score were found in younger age, highly educated, high income, and married elderly people. Elderly people who were older and had low education and low income tended to use denture Conclusions : It is necessary to develop oral health care management and prevention program for the elderly people.

Systematic review of the determinants of oral health-related quality of life (OHRQoL) among Korean elderly adults

  • Objectives: This study conducted a literature review to systematically confirm the quantitative results of individual studies on oral health-related quality of life (OHRQoL) among elderly adults. Methods: Studies on quality of life related to oral health among Korean elderly adults were selected as targets, and literature published in Korean journals until January 10, 2019, was examined. A total of 179 papers were reviewed using RISS, KISS, eArticle, and DBpia, and 17 papers meeting the final conditions were selected for analysis. Factors were categorized with publication year, sample size, gender, and statistical analysis as main variables. Relevant factors and effect sizes were then encoded, and the values of correlation coefficients and correlation effect sizes were coded. Results: After classifying the factors affecting OHRQoL among elderly adults according to common characteristics, four studies related to food and diet, five studies related to xerostomia, and four studies related to dentures and denture satisfaction were found to be related. Conclusions: Overall, various factors affected OHRQoL among elderly adults. Therefore, oral health among elderly adults must be developed and managed considering the various factors related to OHRQoL.

지역사회 노인의 구강건조증 정도 및 구강건강관련 삶의 질 영향요인 (Degree of Dry Mouth and Factors Influencing Oral Health-related Quality of Life for Community-Dwelling Elders)

  • Purpose: This study was conducted to investigate the degree of dry mouth and oral health-related quality of life and to identify factors contributing to oral health-related quality of life for community-dwelling elders. Methods: A descriptive correlational study design was used. Participants were 156 older adults from two senior welfare centers. Data were collected on February 21, 22 and 29, 30, 2009 using structured questionnaires. Enter type multiple regression analysis was used to identify factors influencing oral health-related quality of life according to general and oral health characteristics. Results: There were significant differences in oral health-related quality of life according to living arrangement, insurance, smoking, number of natural teeth, and denture type. The oral health-related quality of life had significant correlations with the number of chronic disease, number of medications, and dry mouth. Factors influencing oral health-related quality of life for community-dwelling older adults were dry mouth, number of chronic disease, and medical aid, which explained about 47.9% of total variance. Conclusion: These results indicate that in order to promote oral health-related quality of life for older adults, prevention or management of chronic diseases as well as oral health and dry mouth are needed for this population, and especially economically poor elders.

일부 농촌지역 노인의 일반 특성 및 구강상태와 치주염유발세균의 관련성 (Association of periodontitis-related bacteria complex with socio-demographic and oral health condition among the elderly in a rural area)

  • Objectives: The objectives of this study were to investigate the distribution and level of periodontopathic bacteria with the general characteristics and oral health condition of the elderly. Methods: A total of 335 elderly individuals aged 65 years or older who lived in Ganghwa-gun, Incheon, were included in the study. Oral examination, investigation through a questionnaire, and collection of saliva were carried out. The collected saliva was analyzed for the distribution and levels of bacteria (red and orange complex bacteria) by real-time polymerase chain reaction. Statistical analyses were performed using chi-square test, t-test, one-way analysis of variance, and Pearson's correlation coefficient with SAS statistical software version 9.4. Results: Among the general characteristics, there were significant differences in the distribution of Porphyromonas gingivalis, Treponema denticola, and Parvimonas micra depending on sex, age, and dental visits (p<0.05). The number of remaining teeth and denture use were related to the distribution of periodontopathic bacteria, except T. denticola (p<0.05). Additionally, periodontitis was related to the distribution of P. gingivalis (p<0.05). As the number of remaining teeth increased, the copy number of red and orange complex bacteria also increased (p<0.05). Those individuals who did not use dentures and had periodontal disease had more periodontopathic bacteria (p<0.05). Conclusions: The distribution and copy number of periodontopathic bacteria in the elderly were more related to oral health condition than to general characteristics. In particular, the distribution and copy number of periodontopathic bacteria were higher in subjects with multiple remaining teeth, no dentures, and periodontal disease.

  • Purpose: The recent proliferation of the aging population, oral health care is closely related to the mental health of the elderly is very important being recognized. Provide basic data for improving the quality of life for the elderly, to determine whether the oral health of the elderly whether oral self-perception of health and any related wish. Methods: Surveys to May 20 from April 15, 2011, subjects were elderly and living in Daejeon and Chungnam 277 people were examined. All the statistics using SPSS 18.0 for the determination of statistical significance, and the significance level was 0.05. Results: 1. Recognized, the most common oral health, self-aware bad the state (46.2%) there was no significant difference in the general characteristics according to self-Oral health status, remaining natural dimensions or the intra-oral prosthetic, there was no significant difference. 장기입원환자와 일반인의 구강건강관련 특성 비교 (Comparison of oral health related characteristics between a long-term patients and general population)

  • Objectives: The purpose of the study was to compare the oral health related characteristics between a long-term patients and general population. Methods: A direct interview questionnaire was completed by 160 patients and 165 general people from January 16 to April 31, 2014. The questionnaire consisted of general characteristics of the subjects, subjective health status, oral health behaviors, and needs of oral health. The data were analyzed using SPSS 18.0 program. Results: The long-term patients reported that most of them were denture wearers(38.1%), and had oral disease symptoms(62.5%) and xerostomia(65.6%). Most of the long-term patients were ex-smoker(31.3%) and did not receive regular dental check-up(92.5%). They did not know tooth brushing method(31.3%) and brushed their teeth less than twice a day(47.5%). Those who used tooth brush for more than 6 months accounted for 47.5%. Most of them did not use auxiliary oral hygiene devices(85.6%). The patients answered that oral health is not important(6.9%), oral health education is not necessary(7.5%), and oral cleaning(26.3%) should be included in oral health education. Conclusions: The self-reported oral health status of the long-term patients much more serious than the general population. It is necessary to educate the continuing oral health management program for the long-term patients.
