증례 보고-Talon Cusp (CASE REPORTS : TALON CUSP)

  • Talon cusp는 전치부의 치관부에 부가적으로 생성된 교두양 구조물로서 치과이상중 하나이다. Talon cusp는 비심미성, 교합 장애, 치아변위, 발육부의 우식으로 인한 치수 괴사 및 치근단 병변 등을 야기할 수 있다. 그밖에 교모나 치주적인 문제, 혀에의 자극, 그리고 턱관절 동통등의 문제점을 야기할 수 있다. 따라서, Talon cusp의 정확한 조기 진단이 필요하며 각각의 증례에 맞는 처치를 통해 Talon cusp로 인한 문제점을 방지할 수 있다. 본 증례는 각각 영구전치와 유전치에 형성된 Talon cusp로서 이차 상아질을 유도하면서 점차적으로 교두를 삭제하는 치료와 근관 치료를 동반해 교두를 완전 절단하는 치료를 시행하였다.

지각과민 치아에 대한 처치약재의 임상적 연구 (A clinical study of desensitizing agent on hypersensitive teeth)

  • The purpose of this study was to evaluate the periodic effect of desensitizing drug such as potassium oxalate(D.D.S. # I&II), strontium chloride (ZAROSEN)$^{(R)}$, and placebo group. The 193 teeth of 93 patients who had been complained dental hypersensitivity, and were divided into three groups by application agent and desensitizing treatment was completed. The interval of observation and treatment period were immediately, 1 week, 2 week, 3 week, 4 week, before and after treatment. The data was statistically analized and the results were as followed. 1. Group I showed best desensitizing effect to the stimuli, followed by Group II, Group III. 2. There was a significant difference (p < 0.005) in desensitizing effect among the Group I, Group III and Group II, Group III but there was no significant difference (p < 0.005) in Group I, Group II. 3. The cold stimuli was most effective in desensitization and there was a significant difference (p < 0.005) in cold, air-blast, but there was no significant difference (p < 0.005) in other stimuli. 4. There was no significant difference (p < 0.005) in effect of the desensitization of the cause of exposed dentine. 5. Anterior teeth was more effective than posterior teeth in desensitization and there was a significant difference (p < 0.005) between anterior teeth and posterior teeth. 6. In analysis of stimuli on the potassium oxalate, there was a significant difference (p < 0.005) in cold, air-blast but there was no significant difference (p < 0.005) in other stimuli.

근관충전용 글라스 아이오노머 시멘트의 근단 폐쇄성과 치질 적합성에 관한 연구 (EVALUATION OF APICAL MICROLEAKAGE AND ADAPTATION TO THE DENTINAL WALL OF THE GLASS-IONOMER ROOT CANAL CEMENT)

  • The purpose of this study was to compare the apical leakage and adaptation to the dentinal wall of a glass ionomer root canal cement(Ketac-Endo) with those of AH-26 and Tubli-Seal. 102 single-rooted teeth were insrumented with step-back technique and randomly divided into four groups according to kinds of root canal cements: Ketac-Endo, AH-26, Tubli-Seal, and no root canal cement. Four experimental groups were as follows; Group 1 : Filling with gutta-percha and Ketac-Endo Group 2 : Filling with gutta-percha and AH-26 Group 3 : Filling with gutta-percha and Tubli-Seal Group 4 : Filling with gutta-percha without root canal cement All the specimens were obturated with lateral condensation technique, and stored in 100% humidity for 4 days. 20 teeth in each group were placed in centrifuge tube filled with India ink, and then centrifuged for 20 minutes at 3,000 rpm. And they were evaluated for linear dye leakage using a stereoscope and analyzed by one-way ANOVA. 22 teeth were split longitudinally and evaluated the adaptation to the dentinal wall using scanning electron microscope(4 teeth in each group) and stereoscope(2 teeth in 1, 2, 3 group). The obtained results were as follows ; 1. In case of not used root canal cement(Group 4), there was showed much degree of dye penetration comparerd with used root canal cements (Group 1, 2, 3)(P<0.01). 2. In the case of used root canal cements(Group 1, 2, 3), there was no significant difference in linear leakage in groups(P>0.01). 3. In the case of used root canal cements(Group 1, 2, 3), there was showed intimate adaptation to the dentinal wall independent on kinds of root canal cements. 4. Fractured surfaces indicated failure of cohesion in Ketac-Endo, and failure of adhesion to dentine in AH-26 and Tubli-Seal. 5. According to similar apical leakage and adaptability to the dentinal wall to another root canal cements, it is possible to be used glass-ionomer cement as root canal cement.

Evaluation of the effects of two novel irrigants on intraradicular dentine erosion, debris and smear layer removal

  • Objectives: To evaluate the effects of copolymer of acrylic acid and maleic acid (Poly[AA-co-MA]) and calcium hypochlorite ($Ca(OCl)_2$) on root canal dentin using scanning electron microscope (SEM). Materials and Methods: Twenty-four single-rooted teeth were instrumented and the apical and coronal thirds of each root were removed, leaving the 5 mm middle thirds, which were then separated into two pieces longitudinally. The specimens were randomly divided into six groups and subjected to each irrigant for 5 min as follows: G1, $Ca(OCl)_2$; G2, Poly(AA-co-MA); G3, $Ca(OCl)_2$ + Poly(AA-co-MA); G4, sodium hypochlorite (NaOCl); G5, ethylenediaminetetraacetic acid (EDTA); G6, NaOCl+EDTA. The specimens were prepared for SEM evaluation. Smear layer, debris and erosion scores were recorded by two blinded examiners. One image from G3 was analyzed with energy dispersive spectroscopy (EDS) on suspicion of precipitate formation. Data were analyzed using the Kruskal-Wallis and Dunn tests. Results: G1 and G4 showed the presence of debris and smear layer and they were statistically different from G2, G3, G5 and G6 where debris and smear layer were totally removed (p < 0.05). In G1 and G4, erosion evaluation could not be done because of debris and smear layer. G2, G3 and G5 showed no erosion, and there was no significant difference between them. G6 showed severe erosion and was statistically different from G2, G3 and G5 (p < 0.05). EDS microanalysis showed the presence of Na, P, and Ca elements on the surface. Conclusions: Poly(AA-co-MA) is effective in removing the smear layer and debris without causing erosion either alone or with $Ca(OCl)_2$.

상아질 표면처리가 글라스 아이오노머 시멘트의 변연누출에 미치는 영향에 관한 연구 (THE EFFECT OF THE DENTINE PRETREATMENT ON THE MARGINAL LEAKAGE OF A GLASS IONOMER CEMENT)

  • The purpose of this study was to evaluate the effect of the dentin pretreatment on the marginal leakage of a glassionomer cement. 1n this study, 60 molars with sound and healthy crown portion were used. The dentin surface of these teeth were exposed and polished with 600 grit silicon carbide paper. Square - shaped cavities were prepared on the flattened dentin surfaces and these were divided into 4 groups according to the dentin pretreatment procedures. Group I : Dentin pretreatment with distilled water as a control group. Group II : Dentin pretreatment with 5% sodium hypochlorite solution. Group III : Dentin pretreatment with Ketac conditioner. Group IV : Dentin pretreatment with 40% polyacrylic acid. The degrees of dye penetration in the cavity walls were assessed using a stereoscope at ${\times}40$ magnification according to the maximum dye penetration. The results were analyzed by using Mann - Whitney U test. The results were as follows : 1. All groups showed varying depth of dye penetration. 2. Distilled water group showed the most severe marginal leakage when compared with the other groups(P<0.05). 3. 40% polyacrylic acid group showed the least amount of marginal leakage compared with the other groups (P<0.05). 4. There were significant differences between Goup I(distilled water) and Group IV (40% polyacrylic acid)(P<0.05), but there were no significant differences among Group I(distilled water), Group II(sodium hypochlorite), Group III(Ketac conditioner) (P>0.05).

Comparison of root canal preparation by three Ni-Ti instruments

  • The aim of this study was to compare the effects of three Ni-Ti instruments on leaning ability by evaluating the volumetric and morphological changes in the apical 6mm of the root canals before and after preparation, using three-dimensionally reconstructed root canals of extracted human teeth. Forty-five teeth were used in this study. They were opened the chambers and removed the all pulp remnants ultrasonically. Subsequently, the canal wall was coated with silver paste and prepared using ProTaper, ProFile and GT rotary files according to the manufacturers instructions. Before and after root canal preparation, all the specimens were scanned with micro computed tomography and examined the differences in dentine volume removed, canal straightening, the proportion of the unchanged area and canal transportation. Quantitative analysis revealed that instrumentation increased in canal volume ranging between 0.081 and $1.866{\;}\textrm{mm}^3$. On average, the large apical preparation produced by ProTaper demonstrated smaller proportions of unchanged surface areas compared to the two other instruments in small canals. But in large canals like maxillary central incisor, the preparation of ProTaper instruments was not enough. ProTaper instrument was tended to increase more in canal volume as compared with the other two instruments but unchanged area was no significant difference. These results showed that three instruments had similar preparation ability and micro computed tomography in combination with the coated wall of root canal using silver paste is a nondestructive and valuable tool to study root canal geometry and changes after preparations in detail.

Dentinal defects induced by 6 different endodontic files when used for oval root canals: an in vitro comparative study

  • Objectives: To compare the formation of dentinal defects using stainless-steel hand K-files (HFs), rotary files, reciprocating files, and Self-Adjusting File (SAF), when used for oval root canals. Materials and Methods: One hundred and forty extracted human mandibular premolar with single root and oval canal were selected for this study. Oval canals were confirmed by exposing to mesio-distal and bucco-lingual radiographs. Teeth with open apices or anatomic irregularities were excluded. All selected teeth were de-coronated perpendicular to the long axis of the tooth, leaving roots segments approximately of 16 mm in length. Twenty teeth were left unprepared (control), and the remaining 120 teeth were divided into 6 groups (n = 20) and instrumented using HF (size 40/0.02), Revo-S (RS; size 40/0.06), ProTaper NEXT (PTN; size 40/0.06), WaveOne (WO; size 40/0.09), RECIPROC (RC; size 40/0.06), and the SAF (2 mm). Roots were then sectioned 3, 6, and 9 mm from the apex, and observed under stereomicroscope, for presence of dentinal defects. "No defect" was defined as root dentin that presented with no visible microcracks or fractures. "Defect" was defined by microcracks or fractures in the root dentin. Results: The control, HF, and SAF did not exhibit any dentinal defects. In roots instrumented by RS, PTN, WO, and RC files exhibited microcracks (incomplete or complete) in 40%, 30%, 55%, and 50%, respectively. Conclusions: The motor-driven root canal instrumentation with rotary and reciprocating files may create microcracks in radicular dentine, whereas the stainless-steel hand file instrumentation, and the SAF produce minimal or less cracks.

Interface between calcium silicate cement and adhesive systems according to adhesive families and cement maturation

  • Objectives: This study aimed to evaluate the interface between a calcium silicate cement (CSC), Biodentine and dental adhesives in terms of sealing ability. Materials and Methods: Microleakage test: 160 standardized class II cavities were prepared on 80 extracted human molars. The cavities were filled with Biodentine and then divided into 2 experimental groups according to the time of restoration: composite resin obturation 15 minutes after Biodentine handling (D0); restoration after 7 days (D7). Each group was then divided into 8 subgroups (n = 5) according to the adhesive system used: etch-and-rinse adhesive (Prime & Bond); self-etch adhesive 2 steps (Optibond XTR and Clearfil SE Bond); self-etch adhesive 1 step (Xeno III, G-aenial Bond, and Clearfil Tri-S Bond); and universal used as etch-and-rinse or self-etch (ScotchBond Universal ER or SE). After thermocycling, the teeth were immersed in a silver nitrate solution, stained, longitudinally sectioned, and the Biodentine/adhesive percolation was quantified. Scanning electron microscopic observations: Biodentine/adhesive interfaces were observed. Results: A tendency towards less microleakage was observed when Biodentine was etched (2.47%) and when restorations were done without delay (D0: 4.31%, D7: 6.78%), but this was not significant. The adhesives containing 10-methacryloyloxydecyl dihydrogen phosphate monomer showed the most stable results at both times studied. All Biodentine/adhesive interfaces were homogeneous and regular. Conclusions: The good sealing of the CSC/adhesive interface is not a function of the system adhesive family used or the cement maturation before restoration. Biodentine can be used as a dentine substitute.

Trend Analysis of Pediatric Dentistry Patients at Seoul National University Dental Hospital: a Study on the Impact of the COVID-19 Outbreak

  • This study aims to investigate changes in dental service use among pediatric patients during the COVID-19 pandemic. The analysis was performed by collecting information about the number of visits, age, gender, diagnoses, and treatments for patients at the Pediatric Dentistry Department of Seoul National University Dental Hospital from January 2017 to December 2022. Compared with the period from 2017 to 2019, the total number of visits decreased during the COVID-19 pandemic, with a substantial decline in 2020. Patients in all age groups, except those aged 10 - 14, experienced decreased visits, and there was no significant gender difference. Z29.8 (need for other specified prophylactic measures) declined in all age groups, especially among those aged 0 - 4, except for those 20 years and older. K02.1 (caries of dentine) was sought by the age groups in the following order: 10 - 14, 15 - 19, 5 - 9, and 0 - 4 years, with the most significant decrease in the 0 - 4 years age group compared to others. K07.4 (malocclusion, unspecified) decreased in younger groups aged 9 and below but increased in older groups aged 10 and above. During the period from 2020 to 2022, preventive, restorative, pulp, and trauma treatments decreased, but surgical and orthodontic treatments increased compared with the previous three years. The use of sedation decreased and general anesthesia increased during COVID-19 compared to pre-pandemic. This study describes the impact of the COVID-19 pandemic on pediatric dentistry, making it a valuable reference for future pandemics.

재광화 완충용액의 pH 변화가 상아질의 재광화에 미치는 영향 (THE EFFECT OF THE pH OF REMINERALIZED BUFFER SOLUTIONS ON DENTIN REMINERALIZXATION)

  • 치아 우식증은 구강 영역에서 가장 흔한 질환이나 여러 가지 복잡한 요소가 작용하기 때문에 이에 대한 발생기전과 치료법에 대해서는 완전히 이해되지 않은 상태이다. 특히 초기우식의 재광화에 있어서 pH의 영향에 대하여 논란의 여지가 있다. 본 연구에서는 유산 완충용액을 이용하여 상아질에 인공 우식을 형성하고 이때의 무기질 소실을 측정하고, 탈회된 시편을 pH가 다른 세 가지 재광화 용액 (pH 4.3, 5.0, 5.5 군)으로 재광화시켰을 때 나타나는 무기질의 침착되는 양과 침착이 일어나는 부위를 Microradiograph를 이용하여 정량적으로 비교 분석하였다. 또한 주사전자현미경을 이용하여 수산화인회석 결정의 정상 상태 탈회된 상태, 그리고 재광화가 일어났을 때의 상태를 정성적으로 비교하여 다음과 같은 결과를 얻었다. 1. Microradiograph분석시 pH가 증가할수록 탈회된 상아질에서 재광화양이 유의차 있게 증가되는 경향을 보였고, pH가 낮아질수록 재광화가 일어나면서 더 깊게 탈회되는 양상을 보였으나, pH 5.5 군은 전반적으로 재광화가 일어나는 경 향을 보였다 (p<0.05). 2. 주사전자현미경 소견에서 상아질 우식의 재광화는, 유기기질 망을 둘러싸고 있는 수산화인회석 결정의 표면으로부터 진행되며 결국에는 탈회 시 파괴된 공간을 채워가는 양상으로 관찰되었다. 3. 재광화 5일 후 pH 4.3. 5.0군에서 무기질이 침착되어 수산화인회석결정이 정상보다 커졌으며, pH 5.5군에서 재광화된 부위의 결정은 정상으로 회복되는 양상을 보였다. 재광화 10일 후 pH 4.3, 5.0 군에서 재광화 5일 후에 정상보다 커졌던 결정들이 정상상아질의 결정 크기로 줄어들었으며 pH 5.5군에서는 결정의 크기가 2배정도 커진 경우도 관찰되었다. 본 실험의 결과에 의하면 상아질의 우식과 재광화 과정은 단순한 탈회와 재광화의 독립적인 과정이 아니고 동력학적으로 탈회와 재광화가 동시에 일어나는 과정이며, 이때 재광화는 기존의 수산화인회석 결정의 표면으로부터 진행되었다.
