치과용 유니트 수관에서 분리한 세균의 부착 및 바이오필름 형성 능력 (Adhesion and Biofilm Formation Abilities of Bacteria Isolated from Dental Unit Waterlines)

  • 우리 연구의 목적은 DUWL에서 배출되는 물에서 분리한 균주의 부착 능력과 바이오필름 형성 능력을 확인하고 두 능력 사이 관계를 확인하는 것이다. DUWL로부터 분리한 12균주를 실험에 사용하였다. 각 균주의 부착 능력을 확인하기 위해, 12-well plates의 각 well에 멸균된 glass coverslip, R2A 액체 배지, 그리고 $1{\times}10^8CFU/ml$의 농도로 조정된 세균 현탁액을 넣고 $26^{\circ}C$ 배양기에서 7일 동안 배양하였다. 배양 후, glass coverslip에 부착한 정도에 따라 1~3점으로 점수를 부여하였다. 분리 균주의 바이오필름 형성 능력을 확인하기 위해, 96-well polystyrene flat-bottom microtiter plate에 R2A 액체 배지와 세균 현탁액을 넣고 $26^{\circ}C$에서 7일 동안 배양하였다. 배양 후, plate에 형성된 바이오필름은 R2A 액체 배지에 현탁했고, 현탁액을 R2A 고체 배지에 도말하였다. $26^{\circ}C$에서 7일 배양한 후 세균의 집락을 계수하고 CFU/ml를 계산하였다. DUWL로부터 총 56균주가 분리되었으며, 12속과 31종을 포함하였다. 실험에는 속당 1균주씩 선택하여 총 12균주가 사용되었다. 12균주 중에 S. echinoides, M. aquaticum, C. pauculus의 부착 능력 점수는 +3으로 가장 높았다. 바이오필름 축적량은 C. pauculus가 가장 많았고, M. testaceum이 가장 적었다. 대부분의 부착 능력이 높은 균주는 바이오필름 형성 능력 또한 높았다. 본 연구의 결과는 DUWL 바이오필름의 형성 기전을 파악하는데 도움을 주며 나아가 바이오필름 형성을 억제하는 방법의 개발에 기본적인 정보를 제공할 수 있을 것이다.

Incidence of postoperative pain after using single continuous, single reciprocating, and full sequence continuous rotary file system: a prospective randomized clinical trial

  • Background: Extrusion of debris is a major factor that results in postoperative pain during root canal treatment with various instruments and instrumentation techniques. Therefore, instrumentation techniques that extrude minimal debris into the periapical area while reducing pain are desirable. This study aimed to compare the incidence of postoperative pain and intake of analgesic medication (frequency and quantity) after endodontic treatment of mandibular posterior teeth using two single files and full-sequence continuous rotary systems with different kinematic motions. Methods: Thirty-five of 105 patients were assigned equally to three groups according to the instrumentation system used: ProTaper Next (PN) X2, 25/06 (Dentsply, Maillefer, Ballaigues, Switzerland), One Shape (OS), #0.25/06 (Micro Mega, Besancon, France), and Wave One Gold (WG), Red - #0.25, 0.07 (Dentsply, Maillefer, Ballaigues, Switzerland). Five specialists were included in this study design; each professional prepared 21 teeth, and randomly selected 7 per instrument system. The VAS sheet ranging from 0 to 10 was used to record the initial and postoperative pains at 24, 48, and 72 h, and 7th day after single visit endodontic treatment in mandibular premolars and molars with a diagnosis of asymptomatic irreversible pulpitis with or without apical periodontitis. Postoperatively, an analgesic, ibuprofen 400 mg was administered for intolerable pain at a dose of 1 tablet for 6 h. The patients were asked over the telephone regarding postoperative pain at intervals of 24, 48, and 72 h, and 7th day using a visual analogue scale. Result: There were no statistically significant differences among the PN, OS, and WG systems (P > 0.05) with regard to the incidence of postoperative pain at any of the four time points assessed. Conclusion: The intensity of postoperative pain, frequency, and analgesic intake were similar across all three types of instrument systems; however, the reciprocating single file (WG) was associated with less postoperative pain than the full sequence continuous rotary file.

치과 진료실내의 세균오염도와 영향인자에 관한 연구 (A Study on Bacterial Concentrations in Dental Offices)

  • Objectives: The purpose of this study was to identify the stains causing infections in dental clinics by analyzing bacterial contamination, as well as to suggest improvements for infection control in dental clinics. Methods: In this study, a questionnaire survey of 47 dental hospitals and clinics located in Gyeonggi-do and Incheon, South Korea was administered from June 2013 to September 2013 and used to investigate the practice rates of infection control by dental hygienists and to analyze the bacterial contamination levels in dental offices. Results: In the studied institutions, the bacterial contamination levels of water lines were $20.9{\times}10^3$ colony forming units (CFU)/mL for three-way syringes, $12.7{\times}10^3CFU/mL$ for high-speed handpieces and $9.8{\times}10^3CFU/mL$ for gargling water. The bacterial contamination levels of surfaces were $44.9{\times}10^3CFU/mL$ in cuspidors, higher than in unit chairs ($2.9{\times}10^3CFU/mL$) and light handles ($6.7{\times}10^3CFU/mL$). The mean bacterial cell count of water lines and surfaces was relatively high in all establishments founded 11 years ago or more, and the mean bacterial cell count of waterline handpieces was $6.27{\times}10^3CFU/mL$ in establishments founded between one and five years ago, $11.16{\times}10^3CFU/mL$ six to ten years ago and $20.04{\times}10^3CFU/mL$ 11 years ago or more, which suggests that earlier foundation is associated with higher bacterial contamination levels with a statistical difference (p<0.01). Similarly, the mean bacterial cell count of cuspidors using water from water lines was also $70.16{\times}10^3CFU/mL$ in at least 11-year-old establishments, statistically significantly higher among in one- to five-year-old ($4.61{\times}10^3CFU/mL$) and six- to ten-year-old clinics ($47.89{\times}10^3CFU/mL$) (p<0.05). Conclusion: This study may be utilized to improve the bacterial contamination levels in dental offices by controlling the characteristics and environmental factors of dental offices that affect the microbial contamination of waterlines and surfaces in such institutions.

방사선조사가 백서타액선조직에 미치는 영향에 관한 실험적 연구

  • This study was undertaken to observe the salivary gland of the white rat when exposed to single and fractionated doses of Cobalt-60 irradiation. One hundred fifty white rats of the experiment were devided into control and 3 experimental groups. In experimental groups, group I receivcd 1200 rads everyweek untill 4800 rads reached, group II received 1500 rads and group III received 2000 rads with single dose. irradiation was carried out using a RAC-120 Cobalt-60 Teletherapy Unit with a dose rate 84.3 r/min, field size 4×5 cm measured at 80 from source. Rats were serially sacrificed at the following postirradiation time intervals: 1, 3, 5, 7, 10, 14, 21, 28, 35, and 42days. At sacrifice, the parotid and submandibular glands were dissected out in toto, and stained with: 1) hematoxylin and eosin; 2) periodic acid Shiff; 3) toludine blue.

One Piece Casting법에 의한 Fixed Bridge (A CASE REPORT FOR CONSTRUCTION OF FIXED BRIDGE BY ONE PIECE CASTING TECHNIQUES)

  • This present paper is concerned with four unit fixed bridge construction by one piece casting technique in the case of missing of upper left second premolar and first molar. The authors had good clinical results with following advantages. 1. The complete bridge can be delivered to the patient for two appointments. 2. The laboratory procedures for solder joint are not necessary. 3. A one piece casting can be properly shaped without weak joints. 4. The casting can be heat-treated to insure an ideal molecular structure. 5. Porcelain veneers can be applied without weakening or melting solder joints.

Maryland Bridge의 적용분포 및 결합실패에 관한 임상적 연구(I) (A Clinical Study on the Distribution and The Bond Failure of Etched(Maryland) Bridge: A Preliminary Report of 135 Cases)

  • The purpose of this was to examine the distribution and the bond failure of the acidetched ceramometal retainer (Maryland Bridge). 126 subjects who treated by faculty and residents of Department of Prosthodontics, Seoul National University Hospital from Dec. 1982 to Dec. 1986 were selected for this study. From the foregoing study author obtained the following conclusions. 1. A total of 135 restorations were placed in the mouths of patients ranging in age from 11 to 70 years (Man 62, woman 64) 2. Most restorations were applied to replace anterior teeth. 3. It was found that of the total number of bridges constructed 59.3 percent were the three-unit type. 4. Replacing one tooth missing was the most frequent cases(74.1 percent). 5. Of the total number of cases, 10.4percent showed bond failure. 6. The bond failure, author suggest, be due to one or more of mis-fit of framework, occlusion, material in itself, faulty case selection and lack of technique.

  • The object of this paper is develope the PC based controlled high voltage power supply and studies 1.2kW(120kV, 10mA) pulse power X-ray generator possible to adapt fluoroscopy of Dental CT X-ray generator and industrial X-ray pulse power equipment. The developed pulse power X-ray generator consisted of mono-block tank include X-ray tube and high voltage X-ray power supply circuit and high voltage control unit with RS232C/422 communication port. The PC control program of pulse power X-ray generator uses LabVIEW, and the size of high voltage transformer and high voltage generator is minimized by high voltage high frequency inverter has 100kHz switching frequency. Also this paper shows result of X-ray tube voltage and tube current correspond to variable load.

  • The "surgery-first" approach, defined as a team approach between surgeons and orthodontists for orthognathic surgery without preoperative orthodontic treatment, is aimed at dental decompensation. A brief historical background and indications for the surgery-first approach are reviewed. Considering the complicated mechanism of postoperative orthodontic treatment, the proper selection of patients is a vital component of successful surgery-first approach.

  • It has been established that disk displacement of the temporomandibular joint(TMJ) can cause mandibular asymmetry in growing subjects. One of the causes of internal derangement of TMJ seems to be the result of poor positioning of the joint structure in unilateral cross bite, and the subsequent occlusal trauma transferred to the functioning unit of the mandible, the joint and disc. Transverse discrepancy of the maxillary and the mandibular posterior dentoalveolar units was often found in mandibular asymmetric subjects. Most of the asymmetry in growing subjects becomes worse with further growth if left untreated. However once sufficient posterior overjet is gained through orthodontic treatment, many cases have shown improvement in facial asymmetry. Furthermore, the position of condyles in computed tomogram(con-beam CT) changed from anatomically unfavorable position to more concentric position.

간접 골성고정원(indirect skeletal anchorage)을 이용한 상악구치 압하

  • Molar extrusion is a quite common problem in prosthodontic patients. It is caused due to the loss or infraocclusion of opposing teeth. A more conservative approach than reduction of the coronal part of extruded tooth is to intrude the malaligned molar orthodontically. Several authors have presented the cases of molar intrusion, by cither removable or fixed appliances. However, the design of those appliances was complex so that many teeth were included as an anchorage unit. This increased the patient's discomfort inevitably. Moreover, the results could be unpredictable. Instead of these conventional methods, skeletal anchorage has been suggested for ideal force system to intrude a molar without any side effects. Many recent clinical reports presented the cases using microscrew or miniplate as a direct anchorage, which included some limitation. The purpose of this report is to introduce the indirect skeletal anchorage for intrusion of extruded maxillary molar.
