Competencies for Entry into the Profession of Dental Hygiene

  • This study was conducted to derive core and detailed competencies of dental hygienists to be utilized as educational targets to be reached by graduation as well as basic data that can be reflected in the development and improvement of dental hygiene curricula. This study analyzed publication reports from the Korean Dental Hygienists Association, the International Federation of Dental Hygienists, the Commission on Dental Accreditation, and the American Dental Education Association. Based on the academic classification system for dental hygiene studies, the components of core and detailed competencies of dental hygiene school at the time of graduation were extracted and developed through expert panel discussions. This study defined competencies at the graduation level of dental hygiene school and derived eight core competencies and their 52 detailed competencies to serve as educational objectives from four areas: professionalism, communication, clinical practice, and community and health promotion. In the future, it will be necessary to conduct self-assessments of competencies based on those developed in this study, at time of the graduation from dental hygiene school, as well as to continuously develop competency-based curricula according to entry level, knowledge level, and graduation level. Thus, it is urgently necessary to develop a system that can evaluate the competencies of dental hygienists after graduation and put this system into practical use.

체계적 문헌고찰을 통한 국내 치과공포 연구 동향분석 (2007-2017) (A systematic review of Korean research trends in dental fear between 2007-2017)

  • Objectives: The purpose of this study was to investigate the overall research trends and factors influencing dental fears in the last 10 years (2007-2017) and provide recommendations for future research. Methods: The literature review focused on dental fear research in Korea. Inclusion criteria of selected studies were as follows: studies that examined fear in middle school students or older, studies undertaken between 2007-2017, and studies that examined trends and factors relating to dental fears. Results: Findings from this literature review showed that the most commonly used tool for measuring dental fear was the Dental Fear Survey (DFS). A lack of trust towards the dentist increased dental fear among patients. Women managed fear better than men. Past pain experiences increased dental fear. The most frequent intervention for reducing fear was sound (41.7%). Most of the intervention studies demonstrated a reduction of fear, with the exception of interventions using ear plugs. Fear was increased in studies involving ear plugs. Conclusions: Based on the results of the study, specific measures should be taken to alleviate past pain experiences, such as the fear of anesthetic injections and sensations of the dental drill. Continuous research is needed to reduce dental fear.

Endotracheal Intubation Using Submandibular Approach for Maxillofacial Trauma Patients: Report of 2 Cases

  • The indication for submandibular intubation is the requirement for intraoperative maxillomandibular fixation (MMF) in the presence of injuries that preclude nasotracheal intubation. Thus, We reported 2 cased of endotracheal intubations via submandibular approach that is applicable in patients with skull base fractures for a reliable general anesthesia. Endotracheal intubation via submandibular approach was applied during general anesthetic procedures for open reduction in three patients with Le Fort II, III or nasoorbitoethmoid (NOE) fractures. No complications due to submandibular intubation, such as infection, postoperative scarring, nerve injury, hematoma, bleeding, or orocutaneous fistula, were observed following submandibular intubation. Endotracheal intubation via submandibular approach is effective in patients with skull base fractures. In our method, the tube connector is removed in orotracheal intubation in order to avoiding the tube removal or displacement. The advantages of this method are very simple, safe, and to provide the good operation field.

치과종사자의 치과건강보험 산정기준에 대한 인지도 조사 (Research on the Oral Health Professional's Awareness of the Dental Health Insurance Standard)

  • Purpose. This research is implemented to identify the status of educations about the pricing standard, status of the cognition of the pricing standard, and the solutions to the questions on the pricing standard of the oral health professionals. it will provide the easier accessibility to the annual changes in Dental Health Insurance Standards to the oral health professionals. Methods. The research subjects are the total 204 oral health professionals in limited area, and it was analyzed with structured questionnaires. The collected data is analyzed by IBM SPSS ver. 19.0, a statistical program (IBM Co., Armonk, NY, USA) for the frequency and percentage, and ANOVA. The result is as following Results. The awareness of dental health insurance standard was statistically significant differences by age (p<0.01), career (p<0.001), the prime task (p<0.01), and dental insurance claimants (p<0.05). The awareness of the standard was statistically significant differences by educational training within last 6 months (p<0.05), the cognition of the standard (p<0.001), and solutions for the questions (p<0.05). Conclusions. As the results of the research, it is necessary to develop the educational program under condition of age, career, main task, and better understanding in Dental Health Insurance Standard for oral health professional.

골유도재생술을 동반한 임플란트의 생존율에 대한 연구 (Long-term Retrospective Study on Cumulative Survival Rate of Implants with Guided Bone Regeneration)

  • Purpose: The purpose of this study was to analyze the cumulative survival rate of dental implants installed with guided bone regeneration (GBR), and also elucidate the factors related with the survival of dental implants. Material and Methods: This retrospective study was conducted on 148 dental implants installed in 76 patients by one specialist (Y.K.) at the Department of Periodontology and Implant Center, Seoul National University Dental Hospital from 2001 to 2010. The cumulative survival rates were obtained by the Kaplan-Meier method. The correlations between various factors and dental implant survival were analyzed by using the log-rank test and Cox proportional hazards model. Results: Among 148 dental implants installed in 76 patients, 8 implants in 7 patients were lost and the cumulative survival rates up to 5-years and 10-years were 97% and 89%, respectively. Gender, smoking status and location of implant were significantly associated with the cumulative survival rate of implants (p < 0.05). Age, history of hypertension and diabetes were not significantly associated with the cumulative survival rate of implants (p > 0.05). Conclusion: The dental implants installed with guided bone regeneration is predictable technique according to the results of cumulative survival rate over 10 years.

Comparison of marginal bone loss between internal- and external-connection dental implants in posterior areas without periodontal or peri-implant disease

  • Purpose: The purpose of this retrospective study with 4-12 years of follow-up was to compare the marginal bone loss (MBL) between external-connection (EC) and internal-connection (IC) dental implants in posterior areas without periodontal or peri-implant disease on the adjacent teeth or implants. Additional factors influencing MBL were also evaluated. Methods: This retrospective study was performed using dental records and radiographic data obtained from patients who had undergone dental implant treatment in the posterior area from March 2006 to March 2007. All the implants that were included had follow-up periods of more than 4 years after loading and satisfied the implant success criteria, without any peri-implant or periodontal disease on the adjacent implants or teeth. They were divided into 2 groups: EC and IC. Subgroup comparisons were conducted according to splinting and the use of cement in the restorations. A statistical analysis was performed using the Mann-Whitney U test for comparisons between 2 groups and the Kruskal-Wallis test for comparisons among more than 2 groups. Results: A total of 355 implants in 170 patients (206 EC and 149 IC) fulfilled the inclusion criteria and were analyzed in this study. The mean MBL was 0.47 mm and 0.15 mm in the EC and IC implants, respectively, which was a statistically significant difference (P<0.001). Comparisons according to splinting (MBL of single implants: 0.34 mm, MBL of splinted implants: 0.31 mm, P=0.676) and cement use (MBL of cemented implants: 0.27 mm, MBL of non-cemented implants: 0.35 mm, P=0.178) showed no statistically significant differences in MBL, regardless of the implant connection type. Conclusions: IC implants showed a more favorable bone response regarding MBL in posterior areas without peri-implantitis or periodontal disease.

  • Sodium hydroxide or caustic soda is a corrosive agent that can cause extensive damage to the oral mucosa, lips, and tongue when ingested either accidentally or intentionally. These injuries include microstomia, shallow vestibule, ankyloglossia, speech impairment, loss of teeth and impairment in facial expression. In the present article, we report a unique case of tongue adhesion to the mouth floor and its surgical management in a 66-year-old female patient, who had a history of caustic soda ingestion.
