Anatomical variations of trabecular bone structure in intraoral radiographs using fractal and particles count analyses

  • Purpose : This study was performed to evaluate possible variations in maxillary and mandibular bone texture of normal population using the fractal analysis, particles count, and area fraction in intraoral radiographs. Materials and Methods : Periapical radiographs of patients who had full mouth intraoral radiographs were collected. Regions of interest ($100{\times}100$ pixels) were located between the teeth of the maxillary anterior, premolar, and molar area, as well as the mandibular anterior, premolar, and molar areas. The fractal dimension (FD) was calculated by using the box counting method. The particle count (PC) and area fraction (AF) analyses were also performed. Results : There was no significant difference in the FD values among the different groups of age, gender, upper, and lower jaws. The mean FD value was $1.49{\pm}0.01$. The mean PC ranged from 44 to 54, and the mean AF ranged from 10.92 to 11.85. The values of FD, PC, and AF were significantly correlated with each other except for the upper molar area. Conclusion : According to the results, patients with normal trabecular pattern showed a FD of approximately 1.5. Based on these results, further investigation would be recommended if the FD value of patient significantly differenct from this number, since the alteration of this value indicates microstructural modification of trabecular pattern of the jaws. Additionally, with periapical radiographs, simple and cost-effective, PC and AF could be used to assess the deviation from the normal.

섬유성이형성증 유래세포의 특성연구 (CHARACTERISTICS OF FIBROUS DYSPLASIA DERIVED CELLS)

  • Purpose: Fibrous dysplasia (FD) is a fibro-osseous disease associated with activating missense mutations of the gene encoding the $\alpha$-subunit of stimulatory G protein. FD may affect a single bone (called monostotic form) or multiple bones (called polyostotic form). The extent of lesions reflects the onset time of mutation. In this study, cells from monostotic FD in maxilla of a patient were isolated and cultured in vitro for characterization. Materials and Methods: The single cells were released from FD lesion which was surgical specimen from 15 years-old boy. These isolated cells were cultured in vitro and tested their proliferation activity with MTT assay. In osteogenic media, these cells underwent differentiation process comparing with its normal counterpart i.e. bone marrow stromal cells. The proliferated FD cells were detached and transplanted into the dordsal pocket of nude mouse and harvested in 6 weeks and 12 weeks. Results and Summary: FD cells have an increased proliferation rate and poor differentiation. As a result, cells isolated from FD lesion decreased differentiation into osteoblast and increased proliferation capacity. MTT assay presented that proliferation rate of FD cells were higher than control. However, the mineral induction capacity of FD was lesser than that of control. Monostotic FD cells make fewer amounts of bone ossicles and most of them are woven bone rather than lamellar bone in vivo transplantation. In transplanted FD cells, hematopoietic marrow were not seen in the marrow space and filled with the organized fibrous tissue. Therefore, they were recapitulated to the original histological features of FD lesion. Collectively, these results indicated that the FD cells were shown that the increased proliferation and decreased differentiation potential. These in vitro and in vivo system can be useful to test FD cell's fate and possible.

  • Objectives: This study was focused on the influence of chronic aircraft noise exposure on children's continuous performance, intelligence and reading skill. Methods: We enrolled 586 children in 4-6th grade of 7 primary schools near air base in Korea. Continuous performance was measured using the computerized ADS program. We analyzed 477-512 children on the visual continuous performance test, auditory continuous performance test, intelligence test, and reading and the vocabulary test. Intelligence was measured using vocabulary, digit span, block design, and digit symbol tests of K-WISC-III. Results: The commission error and variability deviation of auditory continuous performance test and reading test were significantly higher among children in schools with the helicopter noise and the fighting plane noise compared to children in the low noised schools. Conclusion: There was a possibility that chronic aircraft noise exposure was associated with impairment of the school performance. The result of our study also shows chronic aircraft noise was associated with reading ability.

지역별 요양기관의 분포에 영향을 미치는 인구관련 요인 (Population-related factors affecting the regional distribution of medical institutions in Korea)

  • Few public health researchers have paid research attention to the location of medical institutions in Korea. Previous studies were published in geography journals, and relied on limited data in terms of geographic regions and the type of medical institutions. This study utilized nationwide data covering 8 types of medical institutions. We obtained data from Health Insurance Review and Assessment Service and National Population and Housing Census. The correlation coefficients of resident, daytime, university-graduate population, and the population of different age groups (fewer than 15, 15~64, 65 or more) were compared to understand their relative association with the location of medical institutions. Medical clinic, dental clinic, oriental medical clinic, and pharmacy, all of which are almost completely operated by private sector, showed strong positive correlation with population. Hospital-level medical institutions, which are operated by both public and private sector, had moderate positive correlation. Daytime population and university-graduate population, rather than resident population, were more correlated with the location of medical clinics. The correlation coefficients of the population of 15~64 age group and the location of medical institutions were greater than that of other age groups. The results showed that daytime and university-graduate population are more important than resident population to explain the location of medicalrelated facilities. The results also suggests that the population of age groups (especially, 15~64) might be one of important influence factors in the location of medical institutions.

강화형 strip crown의 사용 (GLASS FIBER REINFORCED STRIP CROWN IN PRIMARY TEETH)

  • 본 연구의 목적은 유전치 심미수복의 새로운 방법으로 glass fiber를 이용한 강화형 strip crown의 사용을 제시하고자 하는 것이다. Strip crown의 제작을 위하여 celluloid crown form(3M, USA)과 복합레진은 Z100(P shade, 3M, USA)을, 유동성 복합레진은 Aeliteflo(Bisco Inc., USA)를 사용하였다. 상아질 결합제는 Clearfil SE Bond(Kuraray Medical Inc., Japan)를 사용하였고 강화를 위하여 그물형 glass fiber(TESCERA Fiber Mesh, Bisco Inc, USA)를 사용하였다. 적절한 크기의 celluloid crown form을 성형하여 삭제된 치아에 시적한 후 레진 접착제와 유동성 복합레진으로 처리한 그물형 glass fiber를 설측 내면에 삽입하고 광중합하였다. 잔여 공간에 복합레진을 채우고 치아에 끼운 다음 광중합하였다. 복합레진의 심미성은 유지하면서 glass fiber로부터 기계적 장점을 얻을 수 있을 것으로 사료된다. 새로운 강화형 strip crown의 물성에 대한 후속연구가 필요하다.

항생제를 사용한 유치의 치수치료 (PULP TREATMENT OF PRIMARY TEETH USING A COMBINATION OF ANTIBIOTICS)

  • 본 연구는 항생제를 복합 투여하여 유치의 근관 및 치근단 주위 감염에 대한 효과를 임상적으로 관찰하고자 하였다. 근관 및 치근단 주위 병소의 멸균을 목적으로 metronidazole, ciprofloxacin 및 minocyclin을 증류수 및 근관시멘트와 혼합하여 적용하였다. 유치의 근관 입구로부터 하방에 $Fissurotomy^{(R)}$ bur(SS White, USA)를 이용하여 약제 공간을 형성하였고 약제는 치수실 바닥까지 채웠다. 본 증례들에서 단회 적용으로 임상 증상이 개선되었다. 치은종창, 동통, 누공 등 모든 증상이 술 후 1주일 내에 소실되었다. 임상적으로 활용하기 위해서는 보다 많은 수의 증례와 장기간의 임상적 평가가 필요하며 아직까지 근관내 항생제 사용에 따른 부작용에 대한 보고가 없지만 항생제에 민감한 환아에 대해서는 사용에 주의가 필요할 것으로 사료된다. 저자는 행동조절이 곤란한 소아에서 생리적 치근흡수가 진행 중인 감염된 유치를 가진 환아들을 대상으로 항생제를 복합 사용한 바 증상의 개선을 관찰하였기에 보고하는 바이다.

부가중합 실리콘 인상재 및 모형재의 젖음성에 관한 비교 연구 (WETTABILITY OF POLYVINYLSILOXANE IMPRESSION MATERIALS AND IMPROVED STONE MATERIALS)

  • Wettability of addition silicone impression material is very important property for making an accurate restoration. This study examined the impression quality in clinical condition and the wettability of impression and die material. Four commercially available addition silicone impression material (Express, Examix, Contrast, Perfect) and three die materials (Die-Keen, Vel-Mix, Fuji-Rock) were studied. A total of 50 putty/wash and heavy body/wash impressions of wet intact permanent molar teeth were examined for definition of the gingival sulcus reproduction and then classified in quality ranking. The percentage of the sulcus reproduction ability of each material was calculated from the sulcus depths of cross-sectioned epoxy resin casts from the impressions and clinically measured sulcus depths, The same impression materials were used to produce 3 groups of die stone casts form void entrapment die had been exposed to milk. Voids in the impression body and stone casts ere counted under a stereoscopic microscope. From the experiment, the following results were obtained : 1. Sulcus reproduction ability of additional silicone impression material were diminished in order of Express, Examix, Perfect, Contrast. The significant difference was found between Perfect and other material. Heavy body/wash combination was superior In putty/wash method n Perfect impression material. 2. In direct observation, Contrast showed least void in impression body but correlations ere not found between sulcus reproduction and void production. 3. In void entrapment laboratory test, wettability were diminished in order of Examix, Contrast, Express, Perfect. Clinical impression recording seems not to correlate with laboratory test. 4. The wettability of die material to impression material was not different in Express, Examix, Contrast. But, in Perfect, Die-Keen had superior wettability to others.

구강악안면재건을 위한 복직근 유리피판의 해부학적 고찰 (Anatomical Review of Rectus Abdominis Muscle Free Flap for the Oral and Maxillofacial Reconstruction)

  • Midfacial reconstruction following resection of extensive malignant oral cavity tumors constitutes a challenging problems for reconstructive surgeons. Rectus abdominis muscle free flap (RAMFF) can be considered as the optimal reconstructive option in this case, because this flap has some advantages including consistent deep inferior epigastric artery anatomy, easy to dissect with well defined skin boundaries, acceptable donor site morbidity and the ability to perform simultaneous flap harvest with oral cancer ablation surgery. The rectus abdominis muscle forms an important part of the anterior abdominal wall and flexes the vertebral column, which is a long strap-like muscle divided transversely by three tendinous intersections, fibrous bands which are adherent to the anterior rectus sheath, which is thickly enclosed by the rectus sheath, except for the posterior part below the arcuate line that is usually located midway between the umbilicus and symphysis pubis. Below the arcuate line, this muscle lies in direct contact with the transversalis fascia and parietal peritoneum. For the better understanding of RAMFF as a routine reconstructive procedure in oral and maxillofacial surgery, the constant anatomical findings muse be learned and memorized by the young doctors in the course of the special curriculum periods for the Korean national board of oral and maxillofacial surgery. This review article will discuss the anatomical basis of RAMFF with Korean language.

The effect of bonded resin surface area on the detachment force of lingual bonded fixed retainers: An in vitro study

  • Objective: The aims of this study were to evaluate the relationship between the detachment force and bonding resin surface are and to determine the resin bonding surface area that would provide adequate bonding strength with minimum resin volume. Methods: One hundred and sixty human premolars were randomly divided into 4 groups of 40 teeth each. The diameter of the resin surface area in each group was as follows: group 1, 1.5 mm; group 2, 2.5 mm; group 3, 3.5 mm; and group 4, 4.5 mm. Respond Dead Soft straight (length 0.0175 inch) was used to fabricate the retainers, and $Transbond^{TM}$ XT was used to fix the retainers to the tooth surfaces. A pair of teeth was embedded in acrylic blocks for each specimen. Thus, each group comprised 20 samples. Fixed retainers were bonded to the teeth, and vertical force was applied at the middle of wire. The force was measured using a universal testing machine. Results: The mean value of detachment force was the highest for group 4 ($102.38{\pm}2.92N$), followed by group 3 ($63.54{\pm}2.21N$), group 2 ($51.95{\pm}1.61N$), and group 1 ($24.14{\pm}1.38N$). Conclusions: The detachment force of lingual fixed retainers was significantly affected as the area of the resin bonding surface increased. Considering the minimum bonding strength of brackets, a resin bonding surface area with a diameter of 3.5 mm would provide adequate bonding strength.

Stability of bimaxillary surgery involving intraoral vertical ramus osteotomy with or without presurgical miniscrew-assisted rapid palatal expansion in adult patients with skeletal Class III malocclusion

  • Objective: The aim of this study was to evaluate the stability of bimaxillary surgery involving bilateral intraoral vertical ramus osteotomy performed with or without presurgical miniscrew-assisted rapid palatal expansion (MARPE) in adult patients with skeletal Class III malocclusion. Methods: A total of 40 adult patients with skeletal Class III malocclusion were retrospectively divided into two groups (n = 20 each) according to the use of MARPE for the correction of transverse maxillomandibular discrepancy during presurgical orthodontic treatment. Serial lateral cephalograms and dental casts were analyzed until 6 months after surgery. Results: Before presurgical orthodontic treatment, there was no significant differences in terms of sex and age between groups. However, the difference of approximately 3.1 mm in the maxillomandibular intermolar width was statistically significant (p < 0.001). Two days after surgery, the mandible had moved backward and upward without any significant intergroup difference. Six months after surgery, the maxillary intercanine (2.7 ± 2.1 mm), interpremolar (3.6 ± 2.4 mm), and intermolar (2.0 ± 1.3 mm) arch widths were significantly increased (p < 0.001) relative to the values before presurgical orthodontic treatment in the MARPE group; these widths were maintained or decreased in the control group. However, there was no significant difference in surgical changes and the postsurgical stability between the two groups. No significant correlations existed between the amount of maxillary expansion and postsurgical mandibular movement. Conclusions: MARPE is useful for stable and nonsurgical expansion of the maxilla in adult patients with skeletal Class III malocclusion who are scheduled for bimaxillary surgery.
