중년층의 구강건강지식, 실천 및 보철치료 만족실태 조사 (A study for middle-aged on oral health knowledge, oral health care and satisfaction with prosthetic treatment)

  • Objectives : The purpose of this study was to examine the oral health knowledge and actual oral health care of the selected subjects, their decision making about prosthetic treatment, the state and characteristics of their prosthetic treatment and their satisfaction with prosthetic treatment in an attempt to provide some information on the improvement of the quality of life related to oral health and the promotion of oral health. Methods : The subjects in this study were 250 people who received education in two different lifelong education institutions in the city of Busan. After a survey was conducted from May 23 to June 15, 2011, the answer sheets from 217 respondents were analyzed. Results : 1. As for the general characteristics of the respondents, the men(52.1%) outnumbered the women. Those who were in their 60 and up(47.5%) made up the largest age group, and the married people(65.4%) outnumbered the unmarried ones. By occupation, the company employees(20.3%) made up the biggest group. By education, the greatest group was high-school graduates(36.1%). By monthly mean income, the biggest group gained an income of 2.01 million won or more(36.9%). As to medical security, community-based insurance was most prevailing (36.9%). In terms of health promotion, the largest group worked out to stay fit(52.4%). 2. Regarding oral health knowledge, they had a good knowledge about the cause of dental caries(56.7%), but they weren't well aware of the right time for regular dental checkup(47.9%). In relation to oral health care, regular scaling wasn't prevailing(16.9%). 3. Regarding connections between the general characteristics and satisfaction with prosthesis, the less-educated respondents expressed significantly more dissatisfaction(p=0.015). By monthly mean income, those who had a smaller income were dissatisfied in general(p=0.028). Conclusions : The findings of the study illustrated that it's required to spread awareness of the importance of oral health among people in general, and that differentiated incremental oral health care should be provided for different age groups. In order to raise the satisfaction of patients with prosthesis, how to relieve their pain and anxiety and how to adjust prosthetic treatment cost properly should carefully be considered. In addition, the government should take measures to offer assistance for the low-income classes in preparation for an increase in the elderly population.

임플란트를 이용한 고정성 보철물 장착 전후 구강건강관련 삶의 질 변화 (Change of the Oral Health Related Quality of Life After Fixed Prosthetic Treatment Using Implant Therapy)

  • 본 연구는 2009년 8월부터 2009년 11월까지 편의추출된 서울, 인천, 대구지역 치과 병의원 3곳에서 1개 이상 3개 이하 치아에 대해 임플란트 시술을 받은 20세 이상의 남녀 성인 환자 중 102명을 대상으로 하여 3개 이하의 임플란트 고정성 보철물 장착 전후의 구강건강관련 삶의 질(OHIP-14) 변화에 관하여 조사하고 다음과 같은 결론을 얻었다. 1. 3개 이하의 임플란트 고정성 보철물 장착 후 OHIP-14 총점은 치료 전에 비해 유의하게 감소하여(p<0.001) 구강건강관련 삶의 질이 향상된 것으로 나타났다. 2. 인구통계적 특성에 따른 보철물 장착 전 시점의 OHIP-14 총점은 성별(p=0.01), 경제적 수준별(p=0.04), 교육수준별(p=0.01)에는 유의한 차이가 있었으나 보철물 장착 후에는 OHIP-14 총점의 유의한 차이가 나타나지 않았다. 3. 3개 이하의 임플란트 고정성 보철물 장착 후 North Texas Periodontal Associates에서 제시한 모형의 저작 기능, 사회적 기능, 심리적 기능 만족도에 있어서도 유의하게 변화하였으나(p<0.001) 세부지표 중 구강건강에 대한 염려의 전후 변화는 유의하지 않았다. 4. 3개 이하의 임플란트 고정성 보철물 장착 후 구강건강관련 관심도 변화에 있어서 잇솔질 횟수 증가(p<0.001), 주기적 구강검진(p<0.001), 구강검진필요 인지(p<0.001), 구강질병치료 인지(p<0.001) 등에 있어서도 유의하게 변화하였다. 따라서, 본 연구에서는 소수 치아 결손 시 임플란트를 이용한 고정성 보철물 장착이 구강건강관련 삶의 질, 저작기능, 사회적 기능, 심리적 기능 만족도, 구강건강관련 관심도를 향상시킬 수 있다고 판단되었다.

In-vitro performance and fracture strength of thin monolithic zirconia crowns

  • PURPOSE. All-ceramic restorations required extensive tooth preparation. The purpose of this in vitro study was to investigate a minimally invasive preparation and thickness of monolithic zirconia crowns, which would provide sufficient mechanical endurance and strength. MATERIALS AND METHODS. Crowns with thickness of 0.2 mm (group 0.2, n=32) or of 0.5 mm (group 0.5, n=32) were milled from zirconia and fixed with resin-based adhesives (groups 0.2A, 0.5A) or zinc phosphate cements (groups 0.2C, 0.5C). Half of the samples in each subgroup (n=8) underwent thermal cycling and mechanical loading (TCML)(TC: $5^{\circ}C$ and $55^{\circ}C$, $2{\times}3,000cycles$, 2 min/cycle; ML: 50 N, $1.2{\times}10^6cycles$), while the other samples were stored in water ($37^{\circ}C/24h$). Survival rates were compared (Kaplan-Maier). The specimens surviving TCML were loaded to fracture and the maximal fracture force was determined (ANOVA; Bonferroni; ${\alpha}=.05$). The fracture mode was analyzed. RESULTS. In both 0.5 groups, all crowns survived TCML, and the comparison of fracture strength among crowns with and without TCML showed no significant difference (P=.628). Four crowns in group 0.2A and all of the crowns in group 0.2C failed during TCML. The fracture strength after 24 hours of the cemented 0.2 mm-thick crowns was significantly lower than that of adhesive bonded crowns. All cemented crowns provided fracture in the crown, while about 80% of the adhesively bonded crowns fractured through crown and die. CONCLUSION. 0.5 mm thick monolithic crowns possessed sufficient strength to endure physiologic performance, regardless of the type of cementation. Fracture strength of the 0.2 mm cemented crowns was too low for clinical application.

Comparative study of prosthetic complications associated with the bar-clip, milled bar, and Locator attachments for implant overdentures: a retrospective study

  • Purpose. The purpose of this study was to compare the type and frequency of prosthetic complications associated with attachment types for implant overdenture. Material and methods. In this retrospective study, 38 patients (mean age, 63.5 years) have been treated with implant overdentures from 2007 to 2014. Ten patients received a bar-clip attachment. Eleven patients had received a milled bar with Locator attachment. Seventeen patients had received a Locator attachment. The mean follow-up period was 36.9 months (range, 15-83 months). The type and frequency of prosthetic complications was recorded. The frequency was analyzed to determine the statistical difference among the 3 different attachments by using one-way ANOVA (${\alpha}=.05$) and Bonferroni post hoc method at a 5% level of significance. Results. The total number of prosthetic complications was higher in the bar-clip attachment (55 events) than in the milled bar with Locator attachment (39 events) and the Locator attachment (34 events). There were no statistically significant differences, and the most common prosthetic complication was the loss of retention. In the bar-clip attachment group, the average frequency of prosthetic complications was 3.0 events per prosthesis during the first year. In the milled bar with Locator attachment and Locator attachment groups, the average frequencies were 1.45 events and 2.35 events, respectively. Statistically significant differences were observed in the frequency of the complication. (p = .043) Conclusions. Compared to the bar-clip attachment, implant overdentures that use milled bars with the Locator attachment have a significantly lower incidence of prosthetic complications in the first year of follow-up after placement.

Comparison of effect of desensitizing agents on the retention of crowns cemented with luting agents: an in vitro study

  • PURPOSE. Many dentists use desensitizing agents to prevent hypersensitivity. This study compared and evaluated the effect of two desensitizing agents on the retention of cast crowns when cemented with various luting agents. MATERIALS AND METHODS. Ninety freshly extracted human molars were prepared with flat occlusal surface, 6 degree taper and approximately 4 mm axial length. The prepared specimens were divided into 3 groups and each group is further divided into 3 subgroups. Desensitizing agents used were GC Tooth Mousse and $GLUMA^{(R)}$ desensitizer. Cementing agents used were zinc phosphate, glass ionomer and resin modified glass ionomer cement. Individual crowns with loop were made from base metal alloy. Desensitizing agents were applied before cementation of crowns except for control group. Under tensional force the crowns were removed using an automated universal testing machine. Statistical analysis included one-way ANOVA followed by Turkey-Kramer post hoc test at a preset alpha of 0.05. RESULTS. Resin modified glass ionomer cement exhibited the highest retentive strength and all dentin treatments resulted in significantly different retentive values (In Kg.): GLUMA ($49.02{\pm}3.32$) > Control ($48.61{\pm}3.54$) > Tooth mousse ($48.34{\pm}2.94$). Retentive strength for glass ionomer cement were GLUMA ($41.14{\pm}2.42$) > Tooth mousse ($40.32{\pm}3.89$) > Control ($39.09{\pm}2.80$). For zinc phosphate cement the retentive strength were lowest GLUMA ($27.92{\pm}3.20$) > Control ($27.69{\pm}3.39$) > Tooth mousse ($25.27{\pm}4.60$). CONCLUSION. The use of $GLUMA^{(R)}$ desensitizer has no effect on crown retention. GC Tooth Mousse does not affect the retentive ability of glass ionomer and resin modified glass ionomer cement, but it decreases the retentive ability of zinc phosphate cement.

요양병원 노인환자의 구강건강상태와 구강건강관리 요구도 (Oral health status and care needs of elderly patients in long-term care hospital)

  • Objectives: The purpose of this study is to identify oral health status and oral health care needs of elderly patients in long-term care hospital. Methods: Oral health examination was carried out by a dentist and questionnaire was completed by direct individual interview by a dental hygienist for 245 elderly patients in seven long-term care hospitals in Daegu from March 12 to April 16, 2011. Results: Those who need dental care were 188(76.7%) and was done by oral health examination by a dentist. 93 patients(38.0%) wanted dental care and 63 patients(25.3%) need dental care. The most important dental service in the elderly patients was denture and prosthetic service. As demand for denture and prosthetic service exceeds the supply, but only 35.9 percent of the elderly received the dental care service by the estimation of the caregivers. Conclusions: It will be necessary to establish the customized dental care service for the elderly patients in long term care hospitals. The oral health education for the caregivers is very important and the continuing education program must be implemented in the future.

Wax-up으로 조각한 치관과 CAD로 설계한 치관의 형태 비교 (Shape comparison of Wax-up carved crown and CAD-designed crown)

  • Purpose: With the development of digital dental technology, we are manufacturing prosthetic crown in various ways. However, the wax-up method that makes existing prosthetic crown is also used steadily. Thus, we will compare the shape of the crown designed with wax-up and the crown designed with CAD. Methods: Sculpt the crown with wax-up on the model to 10 dental technicians. The same model was used to design the crown CAD. Measure the size of the sculpted crown. The shapes were compared in three dimensions. Results: As a result, we could observe a difference in size and shape in crown designed with wax - up crown and crown. Conclusion: These results suggest that there are differences according to the method of carving crown and the method of processing.

양질의 보철 제작을 위한 치과의사와 치과기공사 간의 의사소통의 중요성

  • 일반적인 보철치료 과정에서 치과의사와 치과기공사 간의 의사소통 부족으로 인한 재치료 사례가 빈번하게 일어나고 있다. 대규모 치료의 성공은 작은 치료에서 부터 시작한다. 이 보고서에서는 그것과 관련한 간단한 사례를 통해 커뮤니케이션의 중요성을 되새긴다. 좀 더 나은 색과 형태를 재현하기 위해서 치과의사는 치과기공사에게 구체적이고 정확한 정보와 의견을 전달해야 하고 치과기공사는 이것을 올바로 분석하고 제대로 표현할 수 있는 기술을 보유해야 한다. 이 과정에서 반드시 서로가 의견교환을 통해 좀더 나은 보철을 제작할 수 있는 환경을 구축해야 한다.

마모가 심한 환자의 전악 고정성보철물 제작 증례 (Full-mouth rehabilitation of a patient with severe tooth wear using fixed prosthesis)

  • This case study is aimed at introducing a full-mouth rehabilitation of a patient with severe tooth wear using fixed prosthesis. This is a case report of a patient with severe wear dentition with changing vertical dimension. In line with using prosthetic treatment, patient adaptation was verified with provisional restorations followed by diagnostic wax-up. Function, esthetics, and occlusal stability were verified during a 4-week follow up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory results after delivery of definitive prosthesis. Prosthetic treatment of severe wear dentition was functionally and esthetically successful. Dentists and dental technicians would be able to develop better treatment approaches using fixed dental prosthesis.

시멘트리스 나사 유지형 임플란트 시스템을 이용한 보철물 수복 증례 (Prosthetic restorations with cementless screw-retained implant systems: a case report)

  • This study aims to describe the clinical experience of single and bridge crowns fabricated using a cementless screw-retained implant prosthesis system. In the case of single crown (#37), regular link (HDL) was used, and bridge crowns (#15~#24), (#26~#27), (#17~#14) (#24~#26) were fabricated by selecting regular link and short link considering the vertical height. One abutment was hex shaped to ensure that it could be mounted while preventing insertion and prosthesis rotation. The advantages of cementless implant prosthesis include shorter chair time and periodic care, strong retention with LINK abutment, safety from inflammation, bacterial infection, and complications due to peri-implant cement, and high patient satisfaction. Dentists should double-check the position of the implant fixture and dental technicians should continuously manage the fit of the link and prosthesis with digital equipment to reduce screw loosening and fractures.
