Finite element analysis in static and dynamic behaviors of dental prosthesis

  • In recent years, implants have gained growing importance in all areas of medicine. The success of the treatment depends on many factors affecting the bone-implant, implant-abutment and abutment-prosthesis interfaces. In this paper, static and dynamic behaviors of the dental prosthesis are investigated. Three-dimensional finite element models of dental prosthesis were constructed. Dynamic loads in 5 sec applied on occlusal surface. Therefore, FEA was selected for use in this study to examine the effect of the static and dynamic loads on the stress distribution for an implant-supported fixed partial denture and supporting bone tissue.

구치부 수복용 복합레진의 파괴특성에 관한 Acoustic Emission 연구 (ACOUSTIC EMISSION ANALYSIS FOR FRACTURE CHARACTERISTICS OF DENTAL POSTERIOR COMPOSITES)

  • Dental composite resin is a kind of the particle - reinforced composite material, and is widely used in recent dental restoration of anterior and posterior tooth region. The purpose of this study was to investigate the fracture behaviour according to volume fractions and external findings of the filler particles for better interpretation of the fracture characteristics of posterior dental composite resins by analytic method of fracture mechanics. The plane strain fracture toughness($K_{IC}$) and Acoustic Emission were determined with three - point bending test using the single edge notch specimen according to the ASTM - E399, and its analyzed data was compared with filler volume fractions derived from the standard ashing test and scanning electron fractographs of each specimen including the unfilled experimental resin as a control. The results were that the value of fracture toughness of the composite resin material was in the range from 0.85 MPa$\sqrt{m}$ to 1.60 MPa$\sqrt{m}$ and was higher than the value of the unfilled experimental resin, and the fracture behaviours dervied from Acoustic Emission analysis show prominent differences according to the volume fraction and the size of filler particles used in each composite resin. The degree of resistance against crack propagation seems to be increase and the fractographs demonstrate the high degree of surface roughness and irregularity according with the increase of fracture toughness value.

전라북도 치과기공사들의 호흡기계 건강에 관한 조사연구 (1993년도와 2001년도 비교연구) (A Study on the Respiratory System Health Condition of Dental Laboratory Technicians in Jullabuk-do)

  • The purpose of this study was to examine the respiratory system health condition of dental technicians and to be of use for more successful health management and health promotion for them. The subjects in this study were 155 dental technicians who were working in North Jeolla province. Out of them, 70 dental mechanics were investigated in 1993, and 85 were surveyed in 2001. And the reports of the two groups on respiratory distress, including cough, sputum, phlegm, the notable sound of breathing, nasal discharge, coryza, shortness of breath and gasping, were compared. The findings of this study were as below: 1. The cough report rate was 24.3% in 1993 and 16.5% in 2001. There appeared approximately 7.8% decrease between the two years. The most powerful variables included working hours and age. 2. The complaint rate of sputum and phlegm was 47.1% in 1993 and 43.5% in 2001. The rate of 2001 reduced by 3.6%. Whether or not they smoked was identified as the most influential variable. 3. The report rate of shortness of breath and gasping was 7.4% in 1993 and 12.9% in 2001, and this rate showed about 1.5% increase in the latter year. The biggest variables were working hours and career. 4. The complaint rate of nasal discharge and coryza was 41.4% in 1993 and 44.7% in 2001, which showed about 3.3% increase. The largest variables were smoking and career. 5. The report rate of the notable sound of breathing and breathing in light little gasps was 12.0% in 1993 and 17.6% in 2001. There was approximately 5.6% increase, and the most powerful variables were working hours and career.

Evaluation of Tooth Movement and Arch Dimension Change in the Mandible Using a New Three-dimensional Indirect Superimposition Method

  • Purpose: To analyze the amount and pattern of tooth movement and the changes in arch dimension of mandibular dentition after orthodontic treatment using a new three-dimensional (3D)-indirect superimposition method. Materials and Methods: The samples consisted of fifteen adult patients with class I bialveolar protrusion and minimal anterior crowding, treated by extraction of four first premolars with conventional sliding mechanics. After superimposition of 3D-virtual maxillary models before and after treatment using best-fit method, 3D-virtual mandibular model at each stage was placed into a common coordinate of superimposition using 3D-bite information, which resulted in 3D-indirect superimposition for mandibular dentition. The changes in mandibular dental and arch dimensional variables were measured with Rapidform 2006 (INUS Technology). Paired t-test was used for statistical analysis. Result: The anterior teeth moved backward, displaced laterally, and inclined lingually. The posterior teeth showed statistically significant contraction toward midsagittal plane. The amounts of backward movement of anterior teeth and forward movement of posterior teeth showed a ratio of 6 : 1. Although the inter-canine width increased slightly (0.8 mm, P<0.05), the inter-second premolar, inter-first molar, and inter-second molar widths decreased significantly with similar amounts (2.2 mm, P<0.05; 2.3 mm, P<0.01; 2.3 mm, P<0.001). The molar depth decreased (6.7 mm, P<0.001) but canine depth did not change. Conclusion: A new 3D-indirect superimposition of the mandibular dentitions using best-fit method and 3D-bite information can present a guideline for virtual treatment planning in terms of tooth position and arch dimension.

안모유형에 따른 교정치료 (ORTHODONTIC TREATMENT RELATED TO FACIAL PATTERNS)

  • Certain malocclusion are associated with specific "facial type," and it is important for the clinician to classify the common facial characteristic of each patient. Because the reaction to treatment mechanics and the stability of the denture is depended upon the analysis of the facial pattern. Basically, there are 3 district facial types or patterns under which almost all malocclusion can be classified. 1. mesofacial is the most average growth. 2. brachyfacial which is a horizontal growth pattern has a week muscle, with dental arch, deep bite. 3. dolichofacial which is a vertical growth pattern has a strong muscle, narrow dental arch, open bite. Brachyfacial pattern show a resistant to mandibular rotation during treatment can accept a more protrusive denture and are prominantly nonextraction, whereas dolichofacial patterns tend to open during treatment require a more retracted denture in order to assure post-treatment stability. Brachyfacial pattern would better treat to use extrusive force system, whereas dolichofacial pattern treat to use intrusive force system with head gear and intermaxillary elastics.

교정용 미니임플란트를 이용한 하악 제2, 3대구치의 전방이동 : 증례보고 (Mandibular second and third molar protraction with orthodontic mini-implants: case report)

  • This case report describes the management of a 30-year-old woman with hopeless mandibular first molars and right maxillary second premolar. The treatment plan included mandibular second and third molar protraction after extraction of mandibular first molars. Mini-implants were placed between roots of first and second premolar. Sliding mechanics with lever arm was used to prevent inclination of molars. A good functional occlusion was achieved in 38 months without clinically significant side effects. Most of the extraction space of mandibular first molar was closed by protraction of second and third molars. The skeletal Class II pattern was improved by counterclockwise rotation of mandible through reduction of wedge effect. Mandibular molar protraction with orthodontic mini-implants in adequate cases would be a great alternative to prosthetic implant and reduce the financial and surgical burden of patients.

구강 환경에서 사용된 자성 Dyna 어태치먼트의 부식현상 (CORROSION PHENOMENA OF MAGNETIC ATTACHMENT USED IN ORAL ENVIRONMENT)

  • The purpose of this study was to investigate further the mechanics of failure of magnets used for denture retention. Dyna magnets were retrived from denture that had failed after 34 months of clinical use. The magnets were observed and sectioned in order to analyse with high resolution scanning electron microscope. From this study, corrosion behaviors of used magnetic attachment were analysed. The results were as follows ; 1. In Nd-Fe-B based magnetic materials, the erosion-corrosion was started at ununiformed part of stainless steel cover. 2. Corrosion was initiated at weared stainless steel surface and then magnetic materials were spalled by corrosive solution. 3. Spatting was occurred in Nd-Fe-B magnet materials due to corrosion products and then corrosion rate was increased drastically. 4. Corrosion started from ununiformed stainless steel surface as well as welded zone. In conclusion, the failure of magnets may occur by either breakdown of the welding or breakdown of the encapsulating material. So, it is considered that the corrosion problem of dental magnetic materials could be solved to some extent with surface treatment of dental magnetic materials.

Three-piece basal archwire를 이용한 치열 정중선 불일치의 교정치료 (Treatment of patients with midline discrepancies using three-piece basal archwire)

  • 경미하거나 중등도의 골격성 비대칭을 동반한 치열 정중선 불일치를 치료할 때 진단시 간과한 경우 교정치료의 마무리시기에 발견하여 치료기간이 더 길어지거나 만족스럽지 못한 결과를 초래하게 된다. 이때 비대칭 악간 고무줄을 사용한 통상적인 방법으로 장기간 치료하면 교합평면이 변화되거나 치아가 경사 이동되는 등 여러 가지 부작용들이 발생할 수 있다. 이를 예방하기 위해 초진시부터 세심한 진단과 정확한 역학의 적용이 필요하다. Dr. Burstone이 제안한 three-piece basal archwire는 posterior anchorage unit, anterior segment와 intrusion arch 로 구성된다. Three-piece basal archwire는 전치부 함입과 후방견인을 동시에 도모할 수 있으며, 기울어진 전치부 교합평면의 개선도 가능하다. 편측으로만 견인력을 적용하여 치열 정중선을 개선할 때 절치가 경사이동되는 것을 방지하기 위해 Three-piece basal archwire를 응용하여 견인하는 방향의 반대측에 모멘트를 부가하면 절치의 편측 치체 이동을 도모할 수 있다. 이러한 방법으로 치열 정중선 불일치를 개선한 치료 증례를 살펴보고자 한다.

  • 비대칭은 두개안면증후군 같은 선천적 요인과 외상과 같은 환경적 요인에 의해 발생할 수 있으며 심한 골격성 비대칭에서부터 경미한 치열 비대칭에 이르기까지 그 정도는 매우 다양하며 이에 대한 환자의 인지 또한 주관적이다. 심한 비대칭의 경우 악교정 수술이 필요할 수도 있으나 비대칭이 심하지 않고 환자가 수술을 원하지 않을 경우 교정치료 만으로 비대칭의 개선을 도모할 수 있다. 하지만 비대칭 고무줄을 사용한 통상적인 방법으로 장기간 치료하면 교합평면이 기울어지거나 치아가 경사이동이 되는 등의 부작용이 발생하여 만족스럽지 못한 결과를 초래할 수 있다. 이를 예방하기 위해 초진 시부터 주의 깊은 진단과 충분한 역학적 고려를 포함한 철저한 치료 계획이 필요하다. 이에 하악의 편위가 발생한 안면 비대칭 환자에서 수술 없이 비대칭을 악화시키지 않는 조절된 교정력을 적용하여 만족스런 비대칭 절충 치료 결과를 얻었기에 이를 보고하고자 한다.

  • After scientific verification of the osteointegration of dental implants, the overall efficiency of dental implants has been generally accepted. Thus, implants now play a major role in the clinical treatment of an edentulous mandible, and in the prosthetic maintenance equipment for partial edentulous mandible patients. Yet, for the successful long-term maintenance of implants, careful consideration of the bio-mechanics is needed to ensure that the maximum stress in the mandible as a result of chewing is maintained under a critical value. Accordingly, this study focuses on reducing the maximum stresses in an implanted mandible, especially in the cortical bone. Thus, the stresses in the implant and mandible are analyzed using finite element packages, including I-DEAS and NISA II/DISPLAY III, using a local zooming technique for a concentrated stress analysis. In addition, the von-Mises stress and principal stress in the mandible are both checked to determine the best combination.
