3D-printed titanium implant with pre-mounted dental implants for mandible reconstruction: a case report

  • Background: This clinical case presented a novel method of segmental mandible reconstruction using 3D-printed titanium implant with pre-mounted dental implants that was planned to rehabilitate occlusion. Case presentation: A 53-year-old male who suffered osteoradionecrosis due to the radiation after squamous cell carcinoma resection. The 3D-printed titanium implant with pre-mounted dental implant fixtures was simulated and fabricated with selective laser melting method. The implant was successfully inserted, and the discontinuous mandible defect was rehabilitated without postoperative infection or foreign body reaction during follow-ups, until a year. Conclusions: The 3D-printed titanium implant would be the one of the suitable treatment modalities for mandible reconstruction considering all the aspect of mandibular functions.

Intraoral HIV-associated Burkitt's lymphoma: a rare case report with special emphasis on differential diagnosis

  • Individuals with human immunodeficiency virus (HIV) infection present with unique intraoral manifestations of various neoplasms. Intraoral HIV-associated Burkitt's lymphoma is a rare presentation, especially in patients of Indian origin and may present as an initial sign of HIV. The objective of this paper is to report a rare case of Burkitt's lymphoma in an HIV-positive Indian patient along with a special emphasis on differential diagnosis. A 30-year-old Indian female presented with a solitary, well-defined, exophytic mass extending anteroposteriorly and buccolingually from the 35th to 38th regions with no evidence of intraosseous extension. An incisional biopsy was performed, and histopathology showed sheets of neoplastic lymphoid cells with numerous tingible body macrophages with clear cytoplasm, presenting a starry sky appearance, suggesting a diagnosis of BL. The tumor cells were positive for CD10, CD20, c-myc, and Epstein-Barr virus, with a nearly 100% Ki-67 proliferative index. The patient tested positive for HIV. This report indicates the importance of immunohistochemical analysis to differentiate Burkitt's lymphoma from other similar lesions like diffuse large B-cell lymphoma. Thorough knowledge of the clinical presentation, etiopathogenesis, histopathology, and immunoprofile of intraoral HIV-associated Burkitt's lymphoma is essential among clinicians and pathologists.

Recent Advances of Therapeutic Targets for the Treatment of Periodontal Disease

  • Periodontal disease is primarily associated with bacterial infection such as dental plaque. Dental plaque, an oral biofilm harboring a complex microbial community, can cause various inflammatory reactions in periodontal tissue. In many cases, the local bacterial invasion and host-mediated immune responses lead to severe alveolar bone destruction. To date, plaque control, non-surgical, and surgical interventions have been the conventional periodontal treatment modalities. Although adjuvant therapies including antibiotics or supplements have accompanied these procedures, their usage has been limited by antibiotic resistance, as well as their partial effectiveness. Therefore, new strategies are needed to control local inflammation in the periodontium and host immune responses. In recent years, target molecules that modulate microbial signaling mechanisms, host inflammatory substances, and bone immune responses have received considerable attention by researchers. In this review, we introduce three approaches that suggest a way forward for the development of new treatments for periodontal disease; (1) quorum quenching using quorum sensing inhibitors, (2) inflammasome targeting, and (3) use of FDA-approved anabolic agents, including Teriparatide and sclerostin antibody.

Office-based 2-stage Posterior Maxillary Segmental Osteotomy for Mandibular Implant Placement: Clinical Study

  • Purpose: This clinical study presented the effectiveness of 2-stage posterior maxillary segmental osteotomy (PMSO) under local anesthesia in gaining interarch space to restore the posterior mandibular segment with dental implants. Materials and Methods: Nine patients who received two-stage PMSO for mandibular implant placement from 2003 to 2011 were included in the study. Of the 9 patients, 7 were female and 2 were male. Ages ranged form 28 to 72 (mean 46.6). Potential complications were investigated such as sinus infection, survival of bone segment, inflammatory root resorption of adjacent teeth, relapse of bone segment and timing of implant placement, delivery of implant prosthesis and stability of bone segment. Result: None of the patients showed relapse or complication. Bone segments were stabilized by opposed implant prosthesis. Conclusion: Office-based 2-stage PMSO under local anesthesia can be considered a stable and predictable procedure. Also pedicle damage can be avoided by allowing favor of blood supply to the bone segments. From these advantages, it can be concluded that this surgical procedure can decrease post-operative complications.

최근 5년간 구강악안면 감염 환자의 임상통계학적 연구 (A CLINICOSTASTICAL STUDY OF ORAL AND MAXILLOFACIAL INFECTED PATIENTS FOR THE LAST 5 YEARS)

  • Infections of the oral and maxillofacial region are one of the most common conditions for which a patient presents to a maxillofacial surgeons. Although these infections can arise from a variety of source, dental disease is the most common etiology. So, odontogenic infection are frequently encountered in the practice of oral and maxillofacial surgery. These infections often respond to antimicrobial chemotherapy or surgical intervention, such as extraction of teeth, incision and drainage through clinical features. But, odontogenic infections have the potential to spread via the fascial spaces in the head and neck region, and, they spread to cavernous sinus, deep musculofascial space and other vital structure. We have undertaken clinical studies on infections in the oral and maxillofacial regions by analyzing retrospectively hospitalized patients in the Dept. of Oral and Maxillofacial Surgery, Chonbuk National University Hospital past 5 years from 2000 to 2004. And, the patients' age, sex, medical history, causes of the infection, surgical intervention, and other clinical parameters were reviewed. The obtained results were as follows : 1. The most frequent cause of oral and maxillofacial infection was odontogenic. And in the odontogenic cause, dental caries was the most common cause (47.2%). 2. The most common fascial space involved was the submandibular space (15.7%), followed by the buccal space (14.8%). 3. 60.4% of all patients required surgical drainage of the abscess, endodontic treatment or tooth extraction or periodontal treatment with drainage. 4. The most causative organism isolated from the pus culture were streptococcus viridans (53.9%). 5. Underlying medical problems were found in 136 patients (41.9%), the most common being hypertension (27.9%) and diabetes (14.7%).

  • Wiskott-Aldrich 증후군은 WAS 단백질 유전자의 돌연변이에 의해서 야기되는 면역결핍질환으로 X 염색체 열성으로 유전되는 양상을 보인다. 혈소판감소증과 그로 인한 출혈경향, 면역결핍으로 인해 반복되는 감염, 아토피성 피부염등의 소견을 보인다. 첫 번째 증례는 서울대학교 치과병원 소아치과에 내원한 2세 11개월의 남아로 서울대학교 어린이병원에서 골수이식수술 전에 감염성 요소의 제거를 위해 의뢰되었다. 두 번째 증례는 2세 6개월의 남아로 구각부 및 치은의 연조직 질환과 치아 및 치은의 착색을 주소로 본과에 의뢰되었다. 이들 두 환자에서 보이는 Wiskott-Aldrich 증후군의 특징적인 치과적 소견과 치과치료시의 주의사항에 대해 보고하는 바이다.

  • Infective endocarditis remains an important, life-threatening infection despite improvements in diagnosis and management. Despite the decrease in rheumatic heart disease and the improvements in antibiotic prophylaxis, infective endocarditis has been reported with increasing frequency in the last few decades. Presumably, this is due to the rise in the incidence of intravenous drug users, carriers of prosthetic valves and other intracardiac devices, and the longer survival of patients with congenital heart disease. Despite the great advances in medical and surgical treatment, infective endocarditis is still a life-threatening disease with an estimated mortality of 27%. Infective endocarditis represents one of the few potentially fatal infections that may occur in a dental patient. Efforts to reduce the incidence of this disease usually take the form of appropriate antibiotic coverage before dental treatment, together with the establishment and maintenance of good oral health. This study is a case report of a patient who developed infective endocarditis after multiple tooth extractions due to chronic periodontitis of dental origin.

  • Bacterial infection and smoking are an important risk factors involved in the development and progression of periodontitis. However, the signaling mechanism underlying the host immune response is not fully understood in periodontal lesions. In this study, we determined the expression of janus kinase (JAK)/signal transducer and activator of transcription (STAT) on Porphyromonas gingivalis lipopolysaccharide (LPS)- and nicotine-induced cytotoxicity and the production of inflammatory mediators, using osteoblasts. The cells were cultured with 5 mM nicotine in the presence of $1{\mu}g/ml$ LPS. Cell viability was determined using MTT assay. The role of JAK on inflammatory mediator expression and production, and the regulatory mechanisms involved were assessed via enzyme-linked immunosorbent assay, reverse transcription-polymerase chain reaction, and Western blot analysis. LPS- and nicotine synergistically induced the production of cyclooxgenase-2 (COX-2) and prostaglandin $E_2$ ($PGE_2$) and increased the protein expression of JAK/STAT. Treatment with an JAK inhibitor blocked the production of COX-2 and $PGE_2$ as well as the expression of pro-inflammatory cytokines, such as tumor necrosis factor-${\alpha}$, interleukin-$1{\beta}$ ($IL-1{\beta}$), and IL-6 in LPS- and nicotine-stimulated osteoblasts. These results suggest that JAK/STAT is closely related to the LPS- and nicotine-induced inflammatory effects and is likely to regulate the immune response in periodontal disease associated with dental plaque and smoking.

  • Purpose. The purpose of this study was conducted to provide the hospital infection control and personal protection, protection of the technician based on data by examining the relationship of hepatitis B knowledge of AIDS knowledge and attitudes of dental hygiene students and the purpose of the present study. Methods. The research subjects were 390 dental hygiene students in limited and data were collected through structured questionnaires. The collected data is analyzed by spss version 14.0 a statistical program for the frequency and one-way ANOVA, multiple linear regression analysis. Results. The received training students of AIDS knowledge and attitudes were higher, hepatitis B knowledge scores and attitudes about AIDS was a statistically significant difference (p <0.001), The experience clinical practice students was AIDS knowledge and attitudes were hepatitis B knowledge high score and difference was statistically significant(p <0.05), The knowledge AIDS and hepatitis B, higher score showed the attitudes significant increase was found to be a static relationship between two variables by both statistically (p <0.001). Conclusion. In order to provided with a thorough prevention program for infectious disease, strengthen school health education and clinical training to help achieve a mutually constructive relationship with the patient through proper education about infectious diseases.

비스포스포네이트 연관 악골괴사증(BRONJ) (Bisphophonate-Related Osteonecrosis of the Jaw (BRONJ))

  • 근래에 비스포스포테이트(bisphosphonate) 투약의 부작용으로 악골괴사에 관한 보고들이 점차적으로 증가하고 있다. 최근에 보고된 유병율은 대략 0.8% - 12 % 정도이다. 하악은 상악보다 2배 정도 호발하는 것으로 보고되었고 이의 60 - 70 %는 치과진료와 연관성이 거론되었다. 임상적으로 골괴사 현상이 일어나기 전에 건강한 치주조직에 변화가 일어나는데 치유지연 점막궤양, 치아동요와 원인을 알 수 없는 연조직 감염등이 관찰되었다. 치아발거가 치과영역에서 가장 흔한 사전 처치행위로 알려져 있다. 장기적인 비스포스포네이트 투약시 세심한 예방적 치과진료가 이러한 심각한 괴사로 이어지지 않도록 하는데 절대적이다.
