The purpose of this study was to examine the knowledge of dental hygiene students about hepatitis B including its seriousness, dissemination, infection route and prevention in an effort to lay the foundation for dental hygiene health education geared toward dental hygiene students who were going to work in clinical field, The subjects in this study were 134 dental hygiene freshmen, 124 sophomores and 102 juniors in some region. The survey was conducted from April 1, to April 14, 2008. An analysis of frequency and X2-test were performed through SPSS 12.0 program. After a survey was conducted, the following findings were given : 1. As for the seriousness of hepatitis B, by academic year the rates of the juniors, sophomores and freshmen who had a correct knowledge stood at 92.6 percent, 84.8 percent and 84.4 percent respectively. Thus, the students were better knowledgeable with academic year. 2. As for dissemination of hepatitis B, by academic year the rates of the juniors, sophomores and freshmen who had a correct knowledge stood at 60.3 percent, 54.8 percent and 52,7 percent respectively. Thus, the students were better knowledgeable with academic year. 3. As for infection route, by academic year the rates of the juniors, sophomores and freshmen who had a correct knowledge stood at 42.9 percent, 44.8 percent and 56.7 percent respectively. Thus, the students were lower knowledgeable with academic year. 4. As for prevention of hepatitis B, by academic year the rates of the juniors, sophomores and fresh men who had a correct knowledge stood at 71.3 percent, 66.5 percent and 58.4 percent respectively. Thus, the students were better knowledgeable with academic year. 5. As for general knowledge of hepatitis B by academic year the rates of the juniors, sophomores and freshmen who had a correct knowledge stood at 54.7 percent, 52.6 percent and 50.7 percent respectively. Thus, the students were better knowledgeable with academic year The findings of the study indicated that many of the students gave the right answers to the questions about the seriousness and prevention of hepatitis B, and that the rate of the correct answers about the route of dissemination was lowest, Accordingly, students should be taught to have a correct understanding of hepatitis B to help provide appropriate preventive treatment and take proper measures.
본 연구는 경기도와 인천지역의 치과 병원급 19개 기관, 의원급 28개 기관 진료실의 유닛체어 등받이, 라이트 손잡이, 타구대 표면의 검체를 채취하여 실험하였다. 우선 치과 진료실 내 표면의 세균수는 타구대 $44.82{\times}10^3CFU/mL$, 라이트 손잡이 $5.47{\times}10^3CFU/mL$ 유닛체어 $16.28{\times}10^3CFU/mL$로 타구대가 높게 측정되었으며, 의료기관의 규모로는 병원급이 높게 나타났고, 환자수가 많을수록 타구대에서 세균수가 높게 측정되었다. 표면 세균 동정 결과는 Gram positive 균주는 47.3%, Gram negative 균주는 52.7%였으며, Gram positive 균주 중 Micrococcus luteus 10.9%, Bacillus pumilus, Staphylococcus aureus 균주가 각각 3.6%로 확인되었다. Gram negative 균주로는 Acinetobacter ursingii 5.5%로 가장 많이 검출되었으며, Brevundimonas diminuta, Chryseobacterium (Flavo.) indologenes (CDC IIb), Methylobacterium sp.가 각각 4.5%로 나타났다. 이에 본 연구는 치과 진료실 내 표면 세균 오염도를 측정하고, 세균의 종류를 확인함으로써 진료실 내 감염관리의 중요성을 인식시키고, 감염방지에 대한 구체적인 계획 수립의 기초 자료가 될 것으로 사료된다.
본 연구의 목적은 치과 치료 시 라텍스 감염방지구가 핸드피스의 오염을 방지하고 고압 멸 균 소독과 같은 전통적 소독을 대신하는 방법이 될 수 있는 지 조사하는 것이다. 구강 내 상주균 대신 사용한 균주는 E. fecaelis (ATCC 29212) 이며 구강을 대신하는 2cm 의 정방형 cavity 가 형성된 실험용 bowl 을 제작하였다. 라텍스 재질의 덮개는 $Orokeeper^{(R)}$ (Orobiotech Co., Korea)를 사용하였으며 모든 실험 핸드피스는 고압멸균소독기에서 멸균 후 사용하였다. 4가지 실험으로 이루어 졌다 : 핸드 피스의 다양한 부위에 대한 세균 오염 평가, 알코올 소독솜으로 핸드피스의 전면 및 헤드부 에 소독 후 소독시간에 따른 세균오염 평가, 핸드피스 사용 후 air-water spray 시간에 따른 핸드 피스의 전면 및 헤드부에 대한 세균오염 평가, 라텍스 덮개의 반복 사용에 따른 핸드피스의 오염도에 대한 평가 등이다. 실험결과 라텍스 감염방지구로 덮여진 부분과 비교할 때 덮여져 있지 않은 부위의 오염 도가 매우 심하였다. 그러나 핸드피스 사용 후 15초간 알코올 솜으로 닦아내거나, 15초간 기수분사를 한 후에는 소독 수준 까지 균이 감소하였다. 결론적으로 오로 키퍼 같은 라텍스 감염방지구는 15초간의 알코올 세척 또는 사용 후 15초 기수분사와 함께 사용하면 멸균 소독을 할 수 없는 어려운 상황에서 교차감염 방지를 목적으로 사용이 가능하리라 사료된다.
Objectives: The purpose of this study was to investigate the current status of dental hygiene curricula related to clinical practice in Korea. Methods: Clinical work included the categories 'history taking, infection control, oral prophylaxis, preventive treatment, education/counseling, radiography/reading, assisting/cooperation, impression/bite registration, anesthesia, etc.', and 66 works were finally selected based on the frequent tasks of dental hygienists. The subjects were made to answer nine questionnaires. Results: It was found that the theory and practice of the main works operated quite differently in lectures and practice in each school. All types of practice were applied to all schools in the case of 'scaling'. The evaluation of clinical practice was also found to be very different from school to school. Conclusions: For dental hygienists to establish expertise in clinical practice and promote quality improvement, it is necessary to develop a core curriculum focusing on clinical practice. The standardized curriculum should be improved to an efficient and competency-centered one defining clearly the role of dental hygienists considering the needs and importance of clinical practice.
Subsequent to an allogenic stem cell transplantation(ASCT) on patients with hematologic malignancy(AML, ALL, CML, multiple myeloma, lymphoma etc.), chronic GVHD(graft versus host disease), which is an immunological reaction, occurs. With treatment results from patients who were diagnosed with ALL(acute lymphocytic leukemia), undergone BMT(bone marrow transplantation) and showed oral and skin lesions due to GVHD, treatment of oral manifestations of leukemia and its general management were studied. 90% of patients with chronic GVHD show change in the oral mucosa causing oral manifestations such as leukoplakia, lichenoid change of the oral mucosa, mucosal atrophy, erythema, ulceration and xerostomia. In treating GVHD, extensive systemic immunosuppression cause bacterial, viral, fungal infection that are fatal, and even if the treatment is successful, the patient is already in a severe immunosuppressed state. Therefore, localized target therapy is preferred. In another words, topical application(rinse, cream, ointment etc.) of cyclosporin and steroid in treating oral chronic GVHD is highly recommended, and the use of PUVA(Psoralen Ultraviolet A) and thalidomide is reported to be effective. In treating such diseases, dental treatment to control pain and prevent secondary infection of oral manifestations is very important. To those patients with systemic diseases who show limited effect by general dental treatment, non-invasive treatment such as the dental laser, in addition to the use of drugs, may be necessary to actively treat pain and help the healing process. For greater results, new effective methods are to be developed for treatment.
Objectives : This study was performed to enhance infection control and to establish the fundamental and practical improvement by the survey on personal protection of dental personnels with health science college students. Methods : We performed the survey with 585 individual students affiliated with health science in Jeonbuk province and statistically analyzed by SPSS 12.0 program. Results : The frequencies of the clinic gown as a personal protection were 96.0% of dentists (p<0.01) and 88.0% of dental hygienists (p<0.05) respectively in college-associated dental hospital when we compared with institution scale. The frequencies of the medical gloves as a compulsory personal protection were 47.2% of dentists and 34.6% of dental hygienists respectively (p<0.001). The frequencies of the medical mask as a compulsory personal protection were 84.0% of dentists (p<0.001) and 52.2% of dental hygienists (p<0.05) respectively shown by freshman students. The frequencies of the safety glasses as a compulsory personal protection were 12.3% of dentists and 2.5% of dental hygienists shown by freshman students (p<0.001). Conclusions : The results reflect that current dental personnel's concern of personal protection need to be increased continuously in accordance with current demands and eventually suggest their effort to put their action on personal protection in their dental office.
Pyogenic orofacial infections are most commonly odontogenic in origin. Although such infections are usually self-limiting and spatially confined, purulent material may occasionally borrow deeply into contiguous fascial space or planes far from the initial site of involvement. The incidence of orofacial infection remains low in this modern era of preventive dental care and antibiotic therapy, but severe orofacial infections are most frequently observed in the medically compromised patients. We experienced 5 cases of severe orofacial odontogenic infection associated with medical diseases, and then concluded as follows : 1. The average hospitalized period was about 5 weeks, and the signs that indicated that the infections were controlled usually appeared in third week after incision and drainage. 2. The involved medical diseases were diabetes mellitus iatrogenic Cushing's syndrome, rheumatoid arthritis, malnutrition, etc. 3. The medical diseases should be treated coincidently with control of infection.
Mortality associated with maxillofacial infection is relatively low due to the development of antibiotics, and improved oral care. However, inappropriate treatment, delayed treatment, old age, underlying systemic disease, and drug-resistant micro-organisms can potentially result in life threatening situations such as cavernous sinus thrombosis, mediastinitis, and sepsis. Sepsis is the most dangerous state with high mortality, ranging from 20~60%. The treatment of sepsis involves properly monitoring vital functions, fluid resuscitation, surgical drainage, and empirical use of high doses of antibiotics until culture results are available. Ventilatory support maybe be required as well. We encountered a 64-year-old patient who died from sepsis that developed as the result of an odontogenic infection. The initial diagnosis was right temporal, infraorbital, buccal, pterygomandibular space abscess. Despite surgical and medical supportive care, the condition progressed to sepsis and after four days the patient died due to multiple organ failure.
Purpose: The aim of this work is to evaluate a surgical technique for mobilization of mal posed dental implant in anterior area. Methods: A 38-year-old patient consulted our unit for esthetic dissatisfaction with the implant treatment of a central incisor. An implant was observed in 11 and 21, where 11 was 3 mm above the ideal limit, with excessive vestibular angulation. The choice was made to perform a segmental osteotomy and mobilize the bone block and the implant down and forward; a bone block extracted from the mandibular ramus was installed between the implant block and the bed to stabilize the segment. Results: After 4 months, a conventional fixed prosthesis was created and the esthetic result achieved was close to what the patient wanted, with no need for further surgery. The surgical condition was stabilized and maintained for the long-time and no complications how necrosis, infection or bone defects was present. Conclusions: It was concluded that the procedure is efficient, and the biological arguments in favor of the procedure are discussed.

Human mouth environment is known to include a variety bacteria, including Streptococcus spp., Staphylococcus spp., Actinomyces spp., Lactobacillus spp., Candida spp., Enterobacteriaceae, et al. Human oral microorganisms can cause dental caries, gingivitis, periodontitis, respiratory tract infection, and cardiovascular disease. Thus, right denture cleaning is essential to oral and general human health. The aim of this study was to evaluate the bactericidal effect of a sodium dichloroisocyanurate-based effervescent tablet (Aos Denti Germ, Aos Company, Chungbuk, Korea) against oral microorganisms. A total of 5 species Streptococcus spp. (Streptococcus anginosus, Streptococcus mitis, Streptococcus mutans, Streptococcus oralis, and Streptococcus sobrinus), Actinomyces oris, Candida albicans, and Escherichia coli were used in this study. All strains were exposed to the distilled water prepared with effervescent tablet. After the exposure, the mixture of strains and effervescent tablet was inoculated onto blood agar or MacConkey agar plate and cultured at $36^{\circ}C$. All strains were killed immediately on exposure to effervescent tablet. The results suggested that effervescent tablet could be used as an effective denture cleanser for dental hygiene.
