한국인의 임플란트 주위 질환에서 real time PCR법을 이용한 치주세균 평가: pilot study (Evaluation of periopathogenic bacteria of peri-implantitis using real-time PCR in Koreans: pilot study)

  • 목적: 한국인에서 임플란트 주위 질환의 심도에 따른 미생물학적 차이를 알아보기 위해 real time Polymerase Chain Reaction(real-time PCR)법을 이용하여 5종의 치주세균의 정량적, 정성적 분석을 시행하였다. 연구 재료 및 방법: 임플란트가 식립된 총 60명의 환자를 치근단 방사선 사진 및 임상지수 검사를 통해 3군(건강군, 임플란트 주위 점막염군, 임플란트 주위염군)으로 나누었다. 멸균된 curette기구를 이용해 치은연하에서 미생물 샘플을 채취한 후 치주세균 5종에 관해 real time PCR을 시행하였고 comparative delta-CT method를 이용하여 분석한 후 미생물의 상대적 발현량을 비교하였다. 결과: Eikenella corrodens, Treponema denticola의 상대적 발현량은 임플란트 주위염 그룹에서 유의하게 높게 나타났다(P < 0.017). 반면 Fusobacterium nucleatum, Porphyromonas gingivalis의 상대적 발현량은 질환의 심도와는 관련 없이 건강한 임플란트 그룹에서 가장 높게 나타났다. Prevotella intermedia의 상대적 발현량은 건강한 임플란트 그룹에서 유의하게 낮게 나타났다(P < 0.017). 결론: 한국인의 임플란트 주위질환에서 대표적인 치주염 세균이 검출되었으나 치주염과 유사한 미생물학적 분포를 보이지는 않았다.

안전한 임플란트 식립을 위한 하악골 내측면의 CBCT를 이용한 해부학적 연구 (An anatomical study on the mandibular medial surface by CBCT analysis for safer implant placement)

  • Introduction: This study examined the anatomical morphology of the medial surface of the posterior mandible using 3-dimensional cone-beam computed tomography (CT) images to reduce the number of complications related to dental implant placement. Materials and Methods: Fifty patients were enrolled in this study with an average age (${\pm}$standard deviation) of 44.28 (${\pm}13.05$). On the coronal views cone-beam CT of the first molars, the distance between the top of the canal and alveolar crest vertical distance (VD), the distance between the upper-most point of the canal and the point perpendicular to the lingual cortical margin of the mandible lingual distance (LD), the location of the starting point of VD for reducing from the vertical reference line (VD point), and the inclination of the mandibular medial surface (lingual inclination) were measured, and a statistical evaluation was performed using SPSS for Windows version 15.0. Results: The mean VD0 was $16.91{\pm}2.47\;mm$ and VDx decreased with increasing x value. The mean LD was $5.27{\pm}1.36\;mm$. The VD began to decrease at the mean location of $6.12{\pm}0.96\;mm$ from the vertical reference line. The mean lingual inclination was $1.52{\pm}0.72^{\circ}$. Conclusion: These results will assist in the accurate placement of dental implants and the reduction of complications, particularly in the case of preoperative implant planning using only 2-dimensional imaging methods. (ex. panoramic radiography)

Enhancement of peri-implant bone formation via parathyroid hormone administration in a rat model at risk for medication-related osteonecrosis of the jaw

  • Purpose: Dental implant-associated medication-related osteonecrosis of the jaw has been frequently reported in patients administered bisphosphonates (BPs) to prevent osteoporosis. The aim of this study was to investigate the effect of intermittent administration of parathyroid hormone (PTH) on peri-implant bone in the maxillae of ovariectomized rats systemically administered BPs. Methods: Thirty 8-week-old female Sprague-Dawley rats were randomly divided into 3 groups. The OVX-ZP group included ovariectomized rats administered 60 ㎍/kg of zoledronate once a week for 6 weeks and 30 ㎍/kg PTH after implant installation. The OVX-Z group included ovariectomized rats administered 60 ㎍/kg of zoledronate once a week for 6 weeks and saline after implant installation, and the control group included rats that underwent a sham operation and were then administered saline. Rats were sacrificed 4 weeks after implant placement for histomorphometric and micro-computed tomography (CT) analyses. Results: The average bone area percentage was greater in the OVX-ZP group than in the OVX-Z group (53.4%±4.0% vs. 28.9%±9.5%, P=0.01). The bone-to-implant contact ratio was 50.8%±1.4% in the OVX-ZP group and 16.9%±2.4% in the OVX-Z group (P=0.012). The average bone volume ratio as shown on micro-CT was 31.3%±19.8% in the OVX-ZP group and 19.4%±9.3% in the OVX-Z group (P=0.045). The OVX-ZP and OVX-Z groups displayed similar trabecular thickness (0.06±0.004 mm vs. 0.06±0.002 mm) (P>0.05) and trabecular separation (0.21±0.02 mm vs. 0.29±0.13 mm) (P>0.05). However, the number of trabeculae in the OVX-ZP group was significantly higher than that in the OVX-Z group (4.3±1.33/㎣ vs. 2.2±0.19/㎣) (P=0.024). Conclusions: The present findings indicate that intermittently-administered PTH can promote peri-implant bone formation and suggest that PTH administration may aid in effective treatment for medication-related osteonecrosis of the jaw after dental implantation.

하악의 전산화 단층사진에서 횡단면이 임플랜트를 위한 가용골 높이의 결정에 미치는 영향 (The Effect of the Axial Plane on Measurement of Available Bone Height for Dental Implant in Computed Tomography of the Mandible)

  • 임플랜트의 성공을 위해서 정확한 방사선학적 검사는 가용골의 높이와 하악관의 위치를 평가하고 중요구조물의 손상을 방지하는데 있어 필수적이다. CT촬영법 중 영상재구성법을 이용한 방법(CT/MPR)은 하악관의 위치를 잘 보여준다. 치아에 평행한 협설 단면 (cross-sectional plane)을 얻기 위해서는 횡단면(axial plane)이 교합면과 평행하도록 해야 한다. 횡단면(axial plane)이 교합면과 평행하지 않으면 재구성된 협설 단면(cross-sectional plane)은 계획된 fixture의 방향과 각을 이루게 되어 그 fixture의 실제 dimension을 보여주지 못하게 된다. 협설단면(cross-sectional view)에서 측정한 가용골 높이가 실제 가용골 높이보다 너무 큰 경우, 수술시 하악관을 침범할 가능성이 있게 된다. 이 연구의 목적은 하악 임플랜트 CT 촬영시 횡단면이 임플란트를 위한 가용골 높이의 측정에 미치는 영향을 알아보는 것이다. 하악 대구치 부위의 임플랜트를 계획하고 radiographic stent를 만든 후 치과 방사사선과에서 하악 임플랜트 CT를 촬영한 40명의 환자, 45개 부위를 선택하였다. 임플랜트 CT의 central panoramic view에서 치조정과 하악관, stent를 tracing한 후, 치조정과 하악관간의 거리(available bone height, ABH)를 재구성된 협설단면상에서 측정하였다(uncorrected ABH). 다음으로, .stent 방향으로 그은 직선상에서 측정하였다(corrected ABH). 두 거리사이의 각을 측정하였다. 두 거리로부터 각각 fixture의 길이를 결정하였다. 연구 결과, 두 가용골의 높이간에는 제 1 대구치와 제 2 대구치에서 모두 유의성있는 차이를 보였다 (p<0.001). 차이가 1 mm 이상인 경우는 제 1 대구치에서 8.7%, 제 2 대구치에서 15.5% 였다. 차이가 2 mm 이상인 경우는 제 1 대구치에서 2.0%, 제 2 대구치에서 6.6%였다. 최대값은 제 1 대구치에서 2.5 mm, 제 2 대구치에서 2.2 mm 였다. 두 가용골의 높이차와 각과의 상관 관계는 제 1 대구치와 제 2 대구치에서 모두 양의 상관관계를 보였다. 상관 계수는 제 1 대구치에서 0.534, 제 2 대구치에서 0.728 였다. 제 2 대구치가 더 강한 양의 상관관계를 보였다. 두 가용골의 높이로부터 결정한 fixture의 길이가 일치하지 않는 경우는 제 1 대구치에서는 24.4%, 제 2 대구치에서는 28.9%였다.

Computer-Assisted Mandibular Reconstruction with Monocortical DCIA Flap; A Case Report

  • Recently, computer-assisted surgery is popular for performing well-planned operations. Computer-aided navigation system is helpful in maxillofacial surgery with real time instrument positioning and clear anatomic identification. Generally, segmental mandibulectomy and reconstruction flap surgery have done by extra-oral approach such as, submandibular approach. This case report describes performing intra-oral segmental mandibulectomy and reconstruction with monocortical deep circumflex iliac artery (DCIA) flap and CT guided implant surgery by using computer-aided surgical guide and navigation for managing ameloblastoma in a 31 years old female patient.

  • Objectives : The aim of this study was to evaluate the effects of a composite of bone substitute and collagen barrier membrane (bone patch) for local ridge augmentation at peri-implant dehiscence defects on the clinical efficacy and positional stability in dogs. Materials and methods : Implant placement and ridge augmentation procedure were performed at surgically created peri-implant dehiscence defects in canine mandible (n=6). Four treatment modalities were randomly applied: i) bone patch group, ii) Guided bone regeneration (GBR) without pin fixation group (bone graft and collagen membrane), iii) GBR with pin fixation group, and iv) negative control group. After 12 weeks, clinical, micro-CT and histological analyses were performed. Results : Histologic analysis showed that bone patch group had similar results to GBR group and GBR with fixation group in terms of new bone formation. Micro-CT analysis revealed similar results to histologic analysis in terms of total volume maintenance. Operating time was shorter in bone patch group compared to GBR group and GBR with fixation groups. Conclusions : GBR using bone patch could simplify the ridge augmentation procedure with reduced operating time and equivalent biological performance compared to the conventional procedure.

The evaluation of the correlation between histomorphometric analysis and micro-computed tomography analysis in AdBMP-2 induced bone regeneration in rat calvarial defects

  • Purpose: Micro-computed tomography (micro-CT) has been widely used in the evaluation of regenerated bone tissue but the reliability of micro-CT has not yet been established. This study evaluated the correlation between histomorphometric analysis and micro-CT analysis in performing new bone formation measurement. Methods: Critical-size calvarial defects were created using a 8 mm trephine bur in a total of 24 Sprague-Dawley rats, and collagen gel mixed with autogenous rat bone marrow stromal cells (BMSCs) or autogenous rat BMSCs transduced by adenovirus containing bone morphogenic protein-2 (BMP-2) genes was loaded into the defect site. In the control group, collagen gel alone was loaded into the defect. After 2 and 4 weeks, the animals were euthanized and calvaria containing defects were harvested. Micro-CT analysis and histomorphometric analysis of each sample were accomplished and the statistical evaluation about the correlation between both analyses was performed. Results: New bone formation of the BMP-2 group was greater than that of the other groups at 2 and 4 weeks in both histomorphometric analysis and micro-CT analysis (P=0.026, P=0.034). Histomorphometric analysis of representative sections showed similar results to histomorphometric analysis with a mean value of 3 sections. Measurement of new bone formation was highly correlated between histomorphometric analysis and micro-CT analysis, especially at the low lower threshold level at 2 weeks (adjusted $r^2=0.907$, P<0.001). New bone formation of the BMP-2 group analyzed by micro-CT tended to decline sharply with an increasing lower threshold level, and it was statistically significant (P<0.001). Conclusions: Both histomorphometric analysis and micro-CT analysis were valid methods for measurement of the new bone in rat calvarial defects and the ability to detect the new bone in micro-CT analysis was highly influenced by the threshold level in the BMP-2 group at early stage.

ConeBeam CT로 임플란트 검사 시 MAR 기능 유무와 선량조건에 따른 정량적 및 정성적 평가 (Quantitative and Qualitative Evaluation according to Radiation Dose Conditions when using MAR function in Implant examination from Cone Beam CT)

  • 본 연구는 각각의 방사선 선량 조건을 나눠 검사할 때 MAR 기능의 사용과 영상 품질을 평가하기 위해 수행되었다. 실험을 위해 임플란트가 박힌 간단한 Periapical Phantom을 만들었고, CBCT 장비를 이용하여 선량 조건별로 MAR 기능의 유무에 따라 영상을 획득하고 실험에 임하였다. 본 연구에서는 Image J 프로그램을 이용하여 MEAN, SNR, 그리고 CNR 값들을 획득하였고, 통계 분석을 통해 Abutment of Implant 부분은 100 kVp, 8 mA 조건일 때, Center of Implant와 Apex of Implant 부분에서는 100 kVp, 9 mA 조건일 때 영상이 가장 정량적이고 적합한 값이었다. 따라서 방사선량이 증가하면 영상의 Pixel value, SNR, CNR 값이 상승해 화질이 향상되고, MAR 기능을 사용하면 Artifact가 줄어드는 것을 확인할 수 있었다.

국내에서 개발된 3차원 임플란트 가상시술 시스템에 의한 영상재구성상의 정확도 (The accuracy of reformatted images using a new virtual 3-dimensional dental implant system)

  • Purpose: To compare the measurements of the mandible and the detectability of the mandibular canal on reformatted images using a newly developed 3-dimensional implant simulation program with traditionally used CT multiplanar reconstruction program and true measurements. Materials and Methods: Ten dry dog mandibles were used in this study. Occlusal templates for CT examination were fabricated and marked with gutta perch a at ten sites. Axial CT scans were taken and reconstructed using DentaScan (D group) and Vimplant program (V group), and each mandible was sectioned at the previously marked sites (R group). Maximum vertical height (H) and maximum width (W) of the mandible, the distances from buccal border of the mandibular canal to the most buccal aspect of the mandible (X), and the distance from the superior border of the mandibular canal to the alveolar crest (Y) were measured, and the mandibular measurements in each group were compared. Detectability of mandibular canal was evaluated using a 3-point scale in both V and D groups by three oral radiologists and compared. Results: H in the V group was slightly greater than that in the D group, and Wand X in the V group was slightly less than those in the D group. H in the V group was less than that in the R group, and Wand X in the V group was larger than those in the R group. The detectability of the mandibular canal did not show statistically significant differences between V and D groups. Conclusion: The results of the experiment show that the newly developed, inexpensive Vimplant/TM/ simulation program can be used as an alternative to the traditionally used, and more expensive CT multiplanar reconstruction program.
