일개 치과대학병원 응급실 운영시간 변경에 따른 환자의 방문시기와 주요상병의 차이 (Difference of visiting time and major cause of disease by operating time changes of a university dental emergency room)

  • Objectives: The purpose of the study is to investigate of visiting time and major cause of disease by operating time changes of a university dental emergency room. Methods: This study was a retrospect study carried out by reviewing 9,172 records visiting the university emergency room from January 1997 to December 2009. Data were analyzed by frequency test, chi-square test, and logistic regression using SAS version 9.3. Results: Twenty four hours emergency room had 19 percent more patients than those in daytime emergency room. Daytime emergency room had more male patients during spring(26.6%) and winter(20.2%) than female patients(p<0.05). Female patients exceeded male patients during summer(24.4%) and autumn(36.2%)(p<0.05). Twenty-four hours emergency room had the maximal patients in autumn and the minimal patients in winter(p<0.05). The most common injury in the daytime was tooth fracture from 0 to 19 years old(p<0.05). Tooth fracture, jaw fracture, and soft tissue injury were the most common injury in dental emergency room and majority of the patients were the male(p<0.05). Conclusions: Twenty-four hours dental emergency room had a variety of causes of dental emergency thant that in the daytime. Twenty four hours dental emergency room must be prepared for the dental emergency patients at any time provided with personnel and facilities.

  • Background: Emergencies in dentistry can be classified as medical and dental. Medical emergencies occur mainly during dental treatment in patients with a systemic disease. Dental emergency departments are largely divided into dental emergency rooms located in dental college hospitals and medical emergency rooms located in medical institutions. This study aimed to analyze the characteristics of and provide help to dental emergency patients in a dental hospital. Methods: Overall, 1806 patients admitted to a dental emergency room at Yonsei University Dental Hospital for 1 year were included. The data collection period was from October 1, 2014 to September 30, 2015. An investigator reviewed medical records from the electronic medical record (EMR) system and radiographs. Results: The patients were 1,070 men and 736 women. The sex ratio was 1.45:1. The commonest age group was of 0-9 years, including 451 (25.0%) patients, followed by 20-29 years, including 353 (19.5%) patients, and 30-39 years, including 277 (15.3%) patients. Of the 108 patients transferred to the Severance emergency department, 81 had trauma, 19 were in pain, 4 were bleeding, and 4 had other complaints. Among chief complaints, 1,079 patients (60.3%) had trauma, 564 (31.5%) had pain, and 75 (4.2%) had bleeding. Twenty-three cases (1.3%) were caused by temporomandibular disorder (TMD). Conclusion: Dentists should be able to adequately assess patients in a dental emergency room and treat trauma, pain, and bleeding.

  • Dental emergency varies from toothache to trauma of oromaxillofacial area. Recently, the number of emergency cases after dental treatment is increasing because surgical treatments such as implant installation rise. Dental emergency patients have been looking for emergency room of general hospital and they could be treated only when the dentist of night duty exists. Although there are many analyses on the dental emergency patients visiting general hospitals, research on the dental treatment only has not been provided enough. We anticipated different aspects between the dental emergency patients visiting independent dental hospital and general hospitals. We analyzed dental emergency patients' chief complaints and received procedures for almost 1-year period in Won-Kwang dental hospital in Dae-jeon where there had been no dental emergency settings. With this 1574 patients' result, we tried to find appropriate treatment plans regarding dental emergency patients occurring at night and holidays.

H 병원 치과응급실에 내원한 치아 외상 환자에 대한 임상적 고찰 (Dental trauma patients visiting the emergency room in H hospital)

  • Objectives : The purpose of this study was to analyze the types of dental emergencies. This study was carried out for dental trauma patients visiting the emergency room in H hospital from 2005 to 2006. Methods : Subjects were 252 patients. Demographic characteristics consisted of age, gender, dentition, and dental related injury. Results : Male patients had 1.65 times higher tooth injury than female. Teenagers had higher prevalence of tooth injury. Main cause of dental injury was falling down. Young children accounted for 41.7% of the injuries. Late evening was the highest outbreak time of injury. The most commonly affected teeth were central incisor and lateral incisor. The damage of oral soft tissue was more common than the that of alveolar bone. Main area of primary tooth loss was gingiva(10.7%), tongue or soft palate(7.5%), and frenulum(6.0%). Subluxation(28.6%) and luxation(28.6%) were main cause for the primary teeth. Tooth fracture(50.0%) were the most common injury. Conclusions : Thus, to understand the incidence, causes and patterns of dental trauma is to help preserving natural teeth. The results of this study could provide the clinical guidelines on the treatment of dental emergency patients.

Retrospective Study on the Flow and Characteristics of Dental Emergency Patients in Chosun University Hospital

  • Purpose: The aim of the present study is to assess the importance of proper treatment timing for dental emergency patients by characterizing current patient care in the emergency room. Materials and Methods: A retrospective chart review of 3,211 patients who visited the Chosun University Hospital's dental emergency department (Gwangju, Korea) was conducted from January 1, 2011 to May 31, 2014. Information regarding age, gender, onset date, main causes, and diagnoses were collected and analyzed. The main causes were divided into six categories: assault, household/play, sports, traffic, work, and others. Result: Emergency visits were more common for men (69%), and the ratio of males to females was 2.2:1 On average, the major cause was household/play (49.8%), followed by others (18.9%), traffic (16.6%), assault (9.1%), sports (2.9%), and work (2.6%). The most frequent diagnosis on average was dental trauma with 82.4%, followed by infection (10.7%), others (4.7%), and bleeding (2.2%). Conclusion: The main reasons for visits to the dental emergency department are dental trauma, dental infection, bleeding, and others. The most frequent reason for dental emergency patients to visit the emergency department was dental trauma (82.4%).

Using nasal cannula for sevoflurane deep sedation in emergency dental treatment

  • Background: Emergency room doctors run into difficulties in treating injured pediatric patients because usually they fell into panic after trauma. In these situation, deep sedation with sevoflurane is fully recommendable method. The conventional way can interrupt common dental treatment procedure. Methods: In the present study, nasal cannula was used for sevoflurane deep sedation in 11 dental emergency treatment. Age ranged from 0 to 3 years old (average of 1.8 years). Results: Treatment duration was from 10 to 35 minutes (average of 16.7 minutes). Average duration of sedation was 25.5 minutes ranging from 15 to 45 minutes. Conclusions: It has advantages to use nasal cannula for sevoflurane deep sedation rather than conventional intubation; saves time and secures good operation field.

대학병원 응급실로 내원한 치과 응급환자에 관한 임상적 연구 (A clinical study on the dental emergency patients visiting an University Hospital emergency room)

  • Introduction: In today's society, the rapid and appropriate care of the dental emergency patients is much more important. So, a retrospective study on the characteristics of emergency dental injuries and diseases will be very meaningful. Materials and Methods: This retrospective clinical study was carried by reviewing the radiographic films and emergency chart of 11,493 patients who had visited the emergency room of Hallym Sacred heart Hospital and were treated in the Department of Oral and Maxillofacial Surgery from January 2006 to December 2010. Results: The male to female ratio was 1.9:1. The highest monthly incidence was observed in May (10.4%) and June (8.9%) and the peak age distribution was the first decade (56.0%), followed by the second decade (16.0%). Trauma was the most common cause in dental emergency patients, followed in order by toothache, odontogenic infection, temporomandibular joint (TMJ) disorder and oral hemorrhage. Soft tissue injury was most prevalent in the trauma group, followed by tooth injury and facial bone fractures. In the tooth injury group, tooth fracture (56.7%) showed the highest incidence followed in order by tooth subluxation (18.2%), tooth concussion (16.9%), tooth avulsion (11.5%) and alveolar bone fractures (3.7%). In the facial bone fracture group, mandibular fractures (81.8%) showed the highest incidence followed in order by maxilla fractures (15.7%), nasal bone fractures (9.0%), zygomaticomaxillary complex fractures (5.4%), orbital bone fractures (2.5%). In mandibular bone fractures, the most common location was the symphysis (70.1%), followed in order by the mandibular angle (33.0%), mandibular condyle (22.8%) and mandibular body (13.6%). In the infection group, a submandibular space abscess (46.2%) was most common followed in order by a buccal space abscess (17.4%), canine space abscess (16.9%) and submental space abscess (12.3%). TMJ dislocation (89.3%) showed the highest incidence in the TMJ disorder group, followed by TMJ derangement (10.7%). In the other group, a range of specific symptoms due to post operation complications, trigeminal neuralgia, chemical burns and foreign body aspiration were reported. Conclusion: For the rapid and appropriate care of the dental emergency patients, well-organized system should be presented in oral and maxillofacial surgery. And it is possible under analysis of pattern and the variation of the dental emergency patients.

응급실에서 소아외상환자의 치과적 처치를 위한 케타민 진정법의 사용 현황 (Use of Ketamine Hydrochloride for Pediatric Dental Patient at General Hospital)

  • 어린이는 쉽게 넘어지거나 부딪혀 구강안면부의 손상을 흔히 경험하며, 응급치료를 필요로 한다. 이러한 환자들에서는 공포와 불안으로 인해 협조도가 불량해지게 된다. 케타민은 일반 응급실에서 어린 환자의 치료와 검사를 위한 진정제로 잘 알려져 있다. 본 연구에서는 원주 세브란스 기독병원 응급실에서 케타민을 이용한 진정법을 시행한 현황에 대해 알아보았다. 2010년 1월에서 2014년 5월까지 치과적 응급처치를 위해 의뢰된 만 18세 이하의 환자들의 기록이 수집되었으며, 연령, 성별, 시행된 치과적 치료, 케타민 진정법 시행 여부에 대한 자료를 분석하였다. 총 659명의 소아 환자가 치과적 응급처치를 필요로 하였으며, 이중 118명이 케타민 진정법이 시행되었다. 조사 결과 열상의 봉합을 시행한 환자에서 진정법이 더 많이 시행되었고, 연령이 어릴수록 진정법이 시행되었던 상관성이 확인되었다. 케타민 진정법은 치과의사 단독으로 사용하지 않아야 하지만, 응급실에서 1차 진료를 담당하는 치과의사는 케타민 진정법이 적용된 환자들을 쉽게 접할 수 있다. 따라서, 응급실에서 근무하는 치과의사는 케타민의 임상적 효과, 고려사항, 그리고 발생 가능한 합병증 및 대처 방안에 대해 숙지하고 있어야 할 것으로 사료된다.

  • This is a retrospective clinical study on 2,955 patients who had visited the Emergency Room of Sang-gye Paik Hospital and then been treated in the Department of Oral and Maxillofacial Surgery during recent 4 years from Aug. 2002 to Dec. 2006. The obtained results were as follows. The total number of patients was 2,955 and the ratio of male to female was 1.72:1. The age distribution peak was the 1st decade (30.8%), followed by the 3rd decade (14.3%) and the 4th decade (14.0%). Trauma (62.3%) was the most frequent cause in dental emergency patients, pulpitis (13.0%) and infection patients were next in order of frequency. In trauma patients group, facial bone injury, tooth injury, soft tissue injury were included and soft tissue injury group was most prevalent, followed by tooth injury group and facial bone group. In total patient, the ratio of admission was 3.5%. We obtained the results of the distribution of primary emergency care in the traumatic injury, causal distribution of the jaw fracture, distribution of related medical department in multiple associated injuries, distribution of emergency care in infection, causal distribution and control methods of oral bleeding, distribution of TMJ disorder. The trauma patient group was major in the dental patients who had visited the emergency room, but other various groups were included. So we should analyze the pattern and the variation of the dental emergent patient to provide the proper treatment.
