Influence of dental services on satisfaction of old prosthetic patients (노인구강보철환자에게 미치는 치과 의료서비스 요인)

  • The main objective of this study is to investigate the improvement of the quality of dental health based on the dental service satisfaction factors of the old dental prosthesis patients of more than 60 years of age who use the dental institutes including dental hospital, clinic, and public health center and to identify the interest in the dental prosthesis as changing paint. Across the country through a questionnaire survey of a question and answer type conducted by the trained dental hygienists or dentist's. Major results of the empirical analysis are as follows. 1. looking at variation of the variables of socio-demographic features of the respondents, the group of female, age distribution of 60-65, average monthly income of 0.5-1 million Won, and schooling background of college showed significant difference. And the prosthesis treatment in terms of the purpose of isiting a dental institute, and the dental clinic in terms of the type of visiting dental institute showed a remarkably significant difference. 2. looking at variation of variables of the general features of the respondents, a prosthesis satisfaction service in terms of age showed significant difference: a prosthesis satisfaction service, dental staffs' service and dental facilities' service in terms of average monthly income showed significant difference: the entire conditions except for the dental facilities' service in terms of schooling and general features showed significant difference: and in terms of the purpose of visiting dental institute, it showed no significant difference. In terms of a visiting dental institute, the prosthesis satisfaction service, dental facilities, overall level of satisfaction, result of treatment, word of mouth and revisit showed significant difference. 3. looking at variation of the variables of post-installation prosthesis satisfaction of the respondents, the prosthesis satisfaction service, dentist's dental service, staffs' service, dental facilities' service and revisit in terms of the prosthesis type: and the prosthesis satisfaction service, word of mouth, revisit and overall level of satisfaction from the perspective of serious concern showed significant difference. The prosthesis satisfaction service and dentist's service in terms of the solving the economic burden: and the entire variables in terms of dissatisfaction elements showed a similar level of significant difference 4. in regard to the satisfaction of dental service and the change of the recognition of prosthesis patients, it was revealed that the level of dental prosthesis satisfaction of the respondents was closely related to the dentist's service, staffs' service, and dental facilities' service. Finally, looking at the influence of the dental service on the treatment result, satisfaction, word of mouth, revisit and the overall satisfaction level, it was revealed that they had a great impact on the prosthesis satisfaction service.

Effect of reflective journaling in team learning on the learning motivation of learners (팀학습에서 성찰일지 작성이 학습자의 학습동기에 미치는 효과)

  • Objectives : The purpose of this study was to examine the educational effects of team learning by having learners keep a journal to record, check and reflect on their own learning. It's basically meant to investigate the effects of team learning on learning motivation and academic achievements to provide some information on teaching-learning strategy setting tailored to learner characteristics. Methods : The subjects in this study were 49 college sophomores. After they were divided into an experimental group and a control group, the experimental group of 24 students engaged in team learning from March to June, 2012, and that group was asked to keep a reflective journal every week to determine whether writing a reflective journal in every session served to boost their learning motivation and academic achievement or not. Results : 1. After the experimental group kept a reflective journal, that group underwent a significant change in learning motivation from a mean of 3.34 in the pretest to a mean of 4.01 in the posttest(p<0.05). The experimental group showed a more positive improvement than the control group. 2. After the experimental group kept a reflective journal, they underwent a significant change in academic achievement from 22.7 in the pretest to 23.5 in the posttest(p<0.05). The control group showed no significant improvement in academic achievement. Conclusions : The findings of the study illustrated that keeping a reflective journal had a positive impact on the learning motivation of the learners, and the findings of the study are expected to lay the foundation for future research in new teaching methods for the department of dental hygiene.

A radiographic evaluation of graft height changes after maxillary sinus augmentation

  • Purpose: The aims of the present study were to quantitatively assess graft height changes after sinus lift procedures and to analyze the factors that influenced graft height changes, including the residual bone height before surgery, surgical approach, and tooth type. Methods: A total of 39 maxillary posterior implants placed during a simultaneous sinus lift procedure were evaluated. Panoramic radiographs of all patients were taken immediately after implant installation and at 3 months, 6 months, 1 year, 2 years, and 3 years. To analyze graft height changes over time, we measured the distance between the implant platform and the base of the grafted sinus floor at 3 locations. The radiographs were analyzed by a single examiner. Results: Graft height tended to decrease over time, and a statistically significant difference was observed at 2 years compared to baseline (P<0.05). There was no statistically significant difference in graft height change according to the surgical approach or tooth type. For residual bone height, a statistically significant difference in graft height change was found between those with 4-7 mm of residual bone height and those with ${\geq}7mm$ (P<0.05). Conclusions: Graft height after sinus lift procedures significantly decreased at 2 years compared to baseline after sinus augmentation. Further studies should be done with controlled variables, and prospective studies with 3-dimensional images are needed to clarify the factors that influence graft height changes.

Ridge preservation using basic fibroblast growth factor-2 and collagenated biphasic calcium phosphate in beagle dogs

  • Purpose: The aim of this study was to evaluate volumetric and histologic changes in edentulous alveolar ridge areas after ridge preservation using basic fibroblast growth factor-2 (bFGF-2) in combination with collagenated biphasic calcium phosphate (BCP). Methods: The experiments were performed in 6 adult male beagle dogs. The following 3 groups were created: 1) ridge preservation with bFGF-2 and collagenated BCP (experimental group), 2) ridge preservation with collagenated BCP (positive control group), and 3) a negative control group in which no ridge preservation procedure was performed. Volumetric change analysis was performed using an optical scanner and casts. Histological observations were made using light microscopy. Results: After the initial swelling subsided, the magnitude of the volumetric change in the experimental group and positive control group was smaller than in the negative control group. In the experimental group, a distinct trend was observed for the resorption of residual bone and collagen fibers at 4 weeks and for more mature bone and faster healing at 12 weeks. Conclusions: Based on the findings of the present study, bFGF-2 may be considered for use as a therapeutic molecule in ridge preservation procedures.

Effects of the repetitive tasting of different blending types of coffee on teeth stain during home bleaching (자가 미백 처치 기간 중 커피 종류별 적용이 치아 착색에 미치는 영향)

  • Objectives : The purpose of this study was to examine effect of the application of different types of coffee on teeth stain during home bleaching. Methods : Twenty five premolars that were extracted within the past month and healthy without any signs of dental caries or restorations were collected and divided into five groups. The teeth of a control group were bleached everyday for two weeks, and the teeth of four experimental groups were bleached everyday for two weeks and then deposited respectively in four different types of coffee solution: brewed coffee, black coffee, coffee with sugar, and coffee with sugar and cream. Afterwards, the color of the teeth of the four experimental groups was checked by a shade pilot. Results : As a result, there were significant differences in the change of lightness and color according to the blending type with the lapse of time during home bleaching. Concerning changes in lightness and hue, there were the largest differences in the control group and the experimental group deposited in coffee with sugar and cream, followed by coffee with sugar, black coffee and brewed coffee. When the extent of change in lightness and hue was investigated after home bleaching, the experimental group deposited in coffee with sugar and cream was stained the least, and the experimental group deposited in brewed coffee was stained the most. Conclusions : In conclusion, the intake of coffee that is the cause of stain should be reduced during bleaching, and in case of having a cup of coffee, it's advisable to have coffee with cream.

Tooth bleaching effect by light activation on the tooth surface and intra-pulpal temperature: an in vitro study (광활성 유무가 치아미백과 치아표면 및 치수 내 온도에 미치는 영향)

  • Objectives : The purpose of this study was to investigate of the color change, tooth surface and intra-pulpal temperature of tooth bleaching by light activation Methods : Forty-eight extracted bovine teeth were immersed into a tea solution for 24 hours. The specimens were randomly divided into four groups(n=15):(G1) 15% HP + without light activation, (G2) 15% HP + light activation, (G3) 25% HP + without light activation, (G4) 25% HP + light activation. All specimens were bleached for 15 minutes three times. The spectrophotometer (CM-2600d, Konica Minolta, Osaka, Japan) was used including before bleaching, immediately after bleaching, 1 week, 1 and 3 months after the end of bleaching. The temperature rise were measured in the pulpal chamber and tooth surface with a digital thermocouple thermometer(Termopar Digital Multimeter, Tektronix DMM916, USA). Between the tested time points, the specimens were stored in distilled water. The data were analyzed by ANOVA, t-test and Tukey's post hoc test set at 0.05. Results : There was no significant color change by the use of light after the bleaching treatment(p>0.05). The dental bleaching treatments of teeth with 15% HP and 25% HP did not seem to be more effective when light source was used. There was no difference in color stability between groups within three month(p>0.05). There was an increase in tooth surface and pulp temperature, but it was not sufficient to cause damage to the pulp. Conclusions :The use of light activation has no obvious effective impact on the tooth bleaching effect.

The location of midfacial landmarks according to the method of establishing the midsagittal reference plane in three-dimensional computed tomography analysis of facial asymmetry

  • Purpose: The purpose of this study was to evaluate the influence of methods of establishing the midsagittal reference plane (MRP) on the locations of midfacial landmarks in the three-dimensional computed tomography (CT) analysis of facial asymmetry. Materials and Methods: A total of 24 patients (12 male and 12 female; mean age, 22.5 years; age range, 18.2-29.7 years) with facial asymmetry were included in this study. The MRP was established using two different methods on each patient's CT image. The x-coordinates of four midfacial landmarks (the menton, nasion, upper incisor, and lower incisor) were obtained by measuring the distance and direction of the landmarks from the MRP, and the two methods were compared statistically. The direction of deviation and the severity of asymmetry found using each method were also compared. Results: The x-coordinates of the four anatomic landmarks all showed a statistically significant difference between the two methods of establishing the MRP. For the nasion and lower incisor, six patients (25.0%) showed a change in the direction of deviation. The severity of asymmetry also changed in 16 patients (66.7%). Conclusion: The results of this study suggest that the locations of midfacial landmarks change significantly according to the method used to establish the MRP.

A Study of change of oral health state score from gingivitis patients using toothbrushing method (칫솔질 방법에 따른 치은염 환자의 구강건강상태지수 변화)

  • Objectives : This study purpose were the effect of toothbrushing for decreasing halitosis for gingivitis and periodontitis patients. Methods : The university staffs were examined oral condition and analysised a change of oral health state score after using 3 types of toothbrushing. Results : The results were as followed : OHI-S shows that the decreasing effect takes place in the M. bass method and Tooth pick method, but shows no differences by each method. GI for Self test method shows decreasing effect after 2nd week during education while the M. bass method and Tooth pick method shows after 1st week during education. The M. bass method shows much greater effect of halitosis amongst 3 kind of method. PI for Self test method and Tooth pick method show decreasing effect. Decreasing effect during education shows until 2nd week by 3 kind of method, but it shows no differences after 3rd week. VSC(ppb) for M. bass method and Tooth pick method show decreasing effect. Decreasing effect during education shows after 3rd week by 3 kind of method, but it shows no differences until 2nd week. PHP for Self test method, M. bass method and Tooth pick method show decreasing effect after 2nd week during education. But, there is no differences of decreasing effect by among 3 kind of method. Conclusions : This study reports that it is necessary to carry outt further studies on the improvement of oral health management of adults based on the development of oral health education.

Oral health promotion behavior according to awareness-perception factors of dental hygiene and nonhealth-related students (치위생과와 비보건학과 학생의 인지-지각 요인에 따른 구강건강증진행위)

  • Objectives : The purpose of this study was to examine the awareness-perception factors and oral health promotion behavior of dental hygiene and nonhealth-related students in an effort to find out factors affecting their oral health promotion behavior. Methods : The subjects in this study were students in the three-year-course dental hygiene and students in the two-year-course nonhealth-related department in G college located in the city of Gwangju. After a survey was conducted, the collected data were analyzed and compared. Results : 1. The dental hygiene students were statistically significantly ahead of the nonhealth-related students in perceived oral health knowledge(p<0.001), awareness of the importance of oral health(p<0.001), perceived oral health status(p<0.001) and oral health interest(p<0.001). 2. Concerning the characteristics of oral health promotion behavior, the dental hygiene students were statistically significantly better at toothbrushing(p<0.001) and the use of dental floss(p<0.001) than the nonhealth-related students. 3. Regarding factors affecting oral health promotion behavior, oral health knowledge and oral health interest were identified as the factors that impacted on oral health promotion behavior. Out of the two, oral health interest exercised a greater influence on that. Conclusions : It seems that oral health education could motivate students to be concerned about their oral health, to acquire knowledge on that and eventually to change their oral health behavior and attitude. As for college students, oral health education provides a final chance for them to check their oral health knowledge, attitude and oral health promotion behavior before they start to work as full-fledged adults, and institutional measures should be taken to offer more intensive official oral health education.

Trends in the utilization of dental outpatient services affected by the expansion of health care benefits in South Korea to include scaling: a 6-year interrupted time-series study

  • Purpose: This study utilized a strong quasi-experimental design to test the hypothesis that the implementation of a policy to expand dental care services resulted in an increase in the usage of dental outpatient services. Methods: A total of 45,650,000 subjects with diagnoses of gingivitis or advanced periodontitis who received dental scaling were selected and examined, utilizing National Health Insurance claims data from July 2010 through November 2015. We performed a segmented regression analysis of the interrupted time-series to analyze the time-series trend in dental costs before and after the policy implementation, and assessed immediate changes in dental costs. Results: After the policy change was implemented, a statistically significant 18% increase occurred in the observed total dental cost per patient, after adjustment for age, sex, and residence area. In addition, the dental costs of outpatient gingivitis treatment increased immediately by almost 47%, compared with a 15% increase in treatment costs for advanced periodontitis outpatients. This policy effect appears to be sustainable. Conclusions: The introduction of the new policy positively impacted the immediate and long-term outpatient utilization of dental scaling treatment in South Korea. While the policy was intended to entice patients to prevent periodontal disease, thus benefiting the insurance system, our results showed that the policy also increased treatment accessibility for potential periodontal disease patients and may improve long-term periodontal health in the South Korean population.
