임상가를 위한 특집 2 - 턱관절장애와 관련된 교합변화의 치료 (Treatment of occlusal changes associated with temporomandibular joint disorder)

  • Temporomandibular joint disorder(TMJD) was mainly characterized with joint pain, motion limitation, joint sound, resulted from pathologic conditions in temporomandibular joint and around tissue. As temporomandibular joint is one of decisive factors determining the occlusion, disorders in temporomandibular joint may cause the occlusal changes. The causes of occlusal changes related with TMJD can be classified into 2 categories; (1) those related to progression of disorder, 2) those related to treatment of the disorder. The clinical manifestation of occlusal changes depend on their causes and affected site. Therefore, whenever possible, treatment should be directed to the relief of the underlying causes, However, it is not always possible to relieve the underlying conditions. Moreover, some occlusal changes may remain irreversible even after the considerable improvement in clinical symptoms. Regarding the treatment of the permanent occlusal changes, it has been reported that the extensive occlusal treatment including occlusal adjustment, prosthodontic treatment, orthodontic treatment should be applied. Here, we present with a case report of occlusal change caused by the progressive temporomandibular joint disorder, together with introducing the intermaxillary traction appliance as the possible treatment option.

Analysis of midpalatal miniscrew-assisted maxillary molar distalization patterns with simultaneous use of fixed appliances: A preliminary study

  • Skeletal anchorage-assisted upper molar distalization has become one of the standard treatment modalities for the correction of Class II malocclusion. The purpose of this study was to analyze maxillary molar movement patterns according to appliance design, with the simultaneous use of buccal fixed orthodontic appliances. The authors devised two distinct types of midpalatal miniscrew-assisted maxillary molar distalizers, a lingual arch type and a pendulum type. Fourteen patients treated with one of the two types of distalizers were enrolled in the study, and the patterns of tooth movement associated with each type were compared. Pre- and post-treatment lateral cephalograms were analyzed. The lingual arch type was associated with relatively bodily upper molar distalization, while the pendulum type was associated with distal tipping with intrusion of the upper molar. Clinicians should be aware of the expected tooth movement associated with each appliance design. Further well designed studies with larger sample sizes are required.

조절성 전방이동형 코골이 방지장치의 효과 (Antisnoring Effects of Adjustable Anterior Positioner: Case Study)

  • 구강내 장치 요법은 가벼운 혹은 중간 정도의 폐쇄성 수면 무호흡증의 일차 치료법이며 심한 폐쇄성 수면 무호흡증의 경우에도 CPAP 또는 수술요법에서 효과가 없을 때 좋은 대안적 치료법이 될 수 있다. 하지만 다양한 구강내 장치들마다 치료 효과가 다르며 또한 모든 환자들에서 효과를 기대하기 어려운 것도 사실이다. 본 연구는 목젖입천장인두절제술(uvulopalatopharyngoplasty, UPPP) 을 받았으나 증상의 개선이 없는 심한 폐쇄성 수면 무호흡증 (obstructive sleep apnea, OSA) 환자에서 단국대 치대 구강내과에서 개발한 구강내 조절성 하악전방이동형 코골이 방지장치 (Adjustable Anterior Positioner, AAP)를 사용하여 치료한 증례를 보고하고 그에 대한 고찰을 한다.

생쥐의 하악 치아 절단으로 인한 부정교합이 뇌경색에 미치는 영향 (Effect of Tooth-Cut Induced Dental Malocclusion on Mouse Model of Ischemic Stroke)

  • Objectives: Although intraoral balancing appliance therapy has been used effective to several diseases, verification studies through cerebral diseases are poorly reported so far. Thus we investigated the effect of tooth-cut induced dental malocclusion against mouse model of ischemic stroke. Methods: Tooth-cut and 90 min middle cerebral artery occlusion (MCAO) were loaded to C57BL/6 male mice, and total infarct area, neurological deficit scores (NDS), histological change of hippocampal region were observed. Production levels of reactive oxygen species (ROS) and inducible nitric oxide synthase (iNOS) in cerebral tissue were also measured. Results: The longer the tooth-cut period, the greater the area of cerebral infarction caused by MCAO, and NDS began to increase as the tooth was cut, and the results were more negative when MCAO was loaded. Histological change of hippocampal cells was significant when tooth-cut was maintained for 7 days. Those damages were thought to depend on the generation of ROS and iNOS in brain tissue. Conclusions: Since tooth-cut increased total area of cerebral infarction due to MCAO in mice, it is able to be confirmed that anomaly of the temporomandibular occlusion can affect neurological diseases.

Long-term stability of maxillary and mandibular arch dimensions when using rapid palatal expansion and edgewise mechanotherapy in growing patients

  • Objective: The purpose of this study was to assess the long-term stability of rapid palatal expansion (RPE) followed by full fixed edgewise appliances. Methods: This study included 67 patients treated using Haas-type RPE and non-extraction edgewise appliance therapy at a single orthodontic practice. Serial dental casts were obtained at three different time points: pretreatment ($T_1$), after expansion and fixed appliance therapy ($T_2$), and at long-term recall ($T_3$). The mean duration of the $T_1-T_2$ and $T_2-T_3$ periods was $4.8{\pm}3.5years$ and $11.0{\pm}5.4years$, respectively. The dental casts were digitized, and the computed measurements were compared with untreated reference data. Results: The majority of treatment-related increases in the maxillary and mandibular arch measurements were statistically significant (p < 0.05) and greater than expected for the untreated controls. Although many measurements decreased postretention ($T_2-T_3$), the net gains persisted for all of the measurements evaluated. Conclusions: The use of RPE therapy followed by full fixed edgewise appliances is an effective method for increasing maxillary and mandibular arch width dimensions in growing patients.

Orthodontic treatment in a patient with Moebius syndrome: A case report

  • Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

수종의 코골이장치 장착에 따른 인두공간의 변화 (Changes of the Pharyngeal Space by Various Oral Appliances for Snoring)

  • 코골이와 수면무호흡증은 대부분 상기도의 인두부위의 폐쇄에 의해 발생되므로, 이 부위의 폐쇄를 방지하기 위해 혀나 하악을 전방으로 이동시키는 다양한 구강내 장치가 코골이 및 수면무호흡증의 치료에 사용되고 있다. 본 연구는 세 종류의 코골이장치 즉, 하악전방이동장치(mandibular advancement appliance, MAA), 혀견인장치(tongue retaining appliance, TRA), 하악-혀전방이동장치(mandibular advancement-tongue retaining appliance, MATRA )가 구인두와 하인두 공간을 어떻게 변화시키는지를 비교하고자 하였다. Class I 교합을 갖고 코골이 및 수면무호흡증의 증상이 없으며 체질량지수 (BMI)가 정상인 남성 9명을 대상으로 상기 세종류의 코골이장치를 제작하였으며, 이때 MAA, MATRA의 하악전방이동량은 5mm로 TRA, MATRA의 혀전방이동량은 10-20mm로 설정하였다. 그리고, 코골이장치를 장착하지 않은 CO상태와 MAA, TRA, MATRA를 장착한 상태에서 측모두부 규격방사선사진을 촬영한 후, 구인두와 하인두 공간, 두경부 자세, 연구개와 설골의 위치와 관련된 20가지 측정항목에 대해 통계분석을 시행하였다. 분석결과 MAA, TRA, MATRA 모두 상부 구인두 공간을 유의성 있게 증가시켰으나, MATRA 만이 하부 구인두 공간을 유의성 있게 증가시켰다. 이러한 결과는 코골이와 수면무호흡증을 개선시키는데 있어 MATRA가 MAA와 TRA에 비해 효과적일 수 있음을 시사한다.

Managing oral biofilms to avoid enamel demineralization during fixed orthodontic treatment

  • Enamel demineralization represents the most prevalent complication arising from fixed orthodontic treatment. Its main etiology is the development of cariogenic biofilms formed around orthodontic appliances. Ordinarily, oral biofilms exist in a dynamic equilibrium with the host's defense mechanisms. However, the equilibrium can be disrupted by environmental changes, such as the introduction of a fixed orthodontic appliance, resulting in a shift in the biofilm's microbial composition from non-pathogenic to pathogenic. This alteration leads to an increased prevalence of cariogenic bacteria, notably mutans streptococci, within the biofilm. This article examines the relationships between oral biofilms and orthodontic appliances, with a particular focus on strategies for effectively managing oral biofilms to mitigate enamel demineralization around orthodontic appliances.

  • 구강내장치를 통한 자세조절훈련과 관련해 턱관절의 다차원적인 균형치료를 적용함에 따라 중쇠뼈 치돌기 및 가시돌기의 위치균형과 경추정렬이 변화되는 것이 방사선촬영상 실증적으로 관찰되었다.
