ASC52 attchment 이용한 하악편측 유리단 회복의 일례 (A CASE REPORT ON THE UNILATERALLY DESIGNED MOLAR RESTORATION)

  • Lower right first and second molar missings were restored by unilaterally designed removable partial denture, not to disturb the phonetic function. Terminal abutment teeth were splinted and ASC52 attachment aws used as a direct retainer. After that, oral examination and Roentgenographic examination were done periodically for 2 years. There were no remarkable changes on the abutment teeth and its surrounding structures. Also, there no phonetic disturbances by the appliance.

  • In order to know the recognition of dentist and laboratory technician about work order transmission and communication and troubles of restorations, this study was conducted for 84 practicing dentists and 122 dental laboratory technicians in Pusan during Mar. to Apr. 1988 using a questionnaire method and was analyzed by the use of percentage, $X^2$-test. Conclusions It is necessary that both dentist and laboratory technician clearly reconize and fulfill their responsibilities to one another. Dentist and dental laboratory technician should offer constructive criticisn to one another as interdependent member of a team. Effective use of work authorizations certainly promotes communication between the dentist and dental laboratory thechnician.

하악골에 발생한 Ameloblastoma의 증례 (A CASE REPORT OF AMELOBLASTOMA OF THE MANDIBLE)

  • A girl aged 16 years and I month, had a severe Angle's class III malocclusion, characterized by a retarded and constricted maxilla, anterior and posterior crossbite. This patient underwent extraction of two lower Ist premolar. After extraction, author placed multibanded system in lower dental arch to change the long axis of anterior teeth and delivered removable appliance with Coffin spring in the upper dental arch to expand dental arch. After 13 months, anterior & posterior crossbite was corrected and this patient's profile was improved. Superimposition of pretreatment and posttreatment cephalograms upon the SN line registered at S showed backward downward rotation of the mandible and retrusion of lower lip.

  • Conventional mouthguard fabrication process which consists of elastomeric impression taking and followed gypsum model making is changing into intraoral scanning and dental model printing with 3D printer. In addition, new 3D printing materials for mouthgurad, 3D Computer-Aided Design(CAD) software for dental appliance, evaluation of a virtual dentoalveolar model for testing virtually 3D designed mouthguard, and lightweight sensor technology will lead dental professionals to the new era of Sports Dentistry, including information technology integrated custom mouthguard fabrication and creating value with analytic data acquired from sensors in mouthguard.

소아에서 다수치아 상실 시 탄성의치 사용의 치험례 (REMOVABLE FLEXIBLE DENTURE FOR CHILD WITH LOSS OF MULTIPLE TEETH : A CASE REPORT)

  • 소아에서 외상, 다발성 치아 우식증, 전신 질환 등에 의한 다수 치아 상실시 저작력 저하, 영양의 불균형, 부정교합, 심미성의 저하등의 문제점이 발생할 수 있다. 이러한 경우 가철성 의치의 장착으로 저작력 회복, 치열 공간 유지, 심미성 증진 및 정확한 발음의 향상을 도모할 수 있다. 그러나 기존의 Wire와 아크릴릭 레진을 이용한 가철성 장치는 낮은 유지력으로 인해 저작력의 회복에 한계가 있고, wire의 노출로 인해 비심미적이다. 또한 구강내 많은 면적을 피개하는 아크릴릭 레진으로 인해 정확한 발음이 어렵고 저작력이 주로 지대치에만 가해지는 문제점이 있다. 그러나 소아에게 주조 금속 가철성 의치를 적용할 경우 유치에서 undercut을 얻기가 어렵고, 무게로 인해 적응하기가 어렵다. 또한 악골의 횡적인 성장에 방해가 될 수 있고 초기 영구치의 삭제가 필요한 경우도 있다. 이러한 단점을 보완할 수 있는 탄성 의치상재가 1950년도에 개발되었다. 의치상과 유지장치가 일체형으로 연조직의 undercut까지 이용 가능하고 재료 자체의 색조와 투명성으로 심미성이 향상되었다. 또한 얇게 제작 가능하기 때문에 발음의 불편함도 줄일 수 있고, 지대치 이외의 조직으로 저작력의 분산이 가능하여 잔존골의 흡수를 지연시키는데 도움을 준다. 본 증례에서는 여러 증례에서 기존의 가철성 의치의 단점을 보완할 수 있는 탄성 가철성 의치를 이용하여 양호한 결과를 얻었기에 보고하는 바이다.

혼합치열기 반대교합자에서 상악골 전방견인장치를 이용한 치료증례 (TREATMENT OF ANTERIOR CROSSBITE IN MIXED DENTITION USING MAXILLARY PROTRACTION APPLIANCE : A CASE REPORT)

  • The prevalence of class III malocclusion is approximately 5$\sim$9%, and about one fourth of this malocclusion is due to underdeveloped maxilla. Maxillary protraction appliance is an orthopedic device which promote the growth of a deficient maxilla by applying extraoral force to actively growing patients. The object of using maxillary protraction appliance is to guide a normal growth of maxilla and mandible and improve the occlusal relationship and also improve the facial profile. The author treated three patients whom were diagnosed as a class III malocclusion due to deficient maxilla using maxillary protraction appliance and the followings are the conclusions : 1. In these cases, anterior crossbite was corrected by anterior movement of maxilla and downward backward rotation of mandible and simultaneously, anterior facial height was increased. 2. The amount of dental change compare to skeletal change was greater as the patients got older. 3. When 500gm of force to each side was applied, the treatment period has been decreased. 4. As a result of applying the force between maxillary first primary molar and canine, there was a small degree of changes in palatal plane. So, it can be concluded that the maxillary protraction appliance is effective in treating growing patients with a deficient maxilla.

소아치과 임상에서의 Thermoformed Appliance의 적용 (APPLICATION OF THERMOFORMED APPLIANCES IN PEDIATRIC DENTISTRY)

  • 저자는 열성형재료와 진공성형기를 이용하여 소아치과 임상에서 사용되는 수 종의 장치를 제작하였다. 전공성형기의 구입에 따른 경제적인 부담은 있으나, 기존의 제작법에 비해 제작이 간편하여 시간이 단축될 뿐 아니라 plate 의 두께가 균일하므로 finishing이 거의 필요하지 않으며 더욱 심미적인 장치를 제작할 수 있다. 또한 조직과 접촉하는 부위는 soft한 열성형재료를 사용하여 치아 이동에 유리할 뿐 아니라, 기존의 resin에 비해 불쾌한 맛이 적다는 장점이 있다. Monomer가 없어 조직에 자극이 적고 mutagenous potential이 없으므로 생체 적합성이 뛰어나 조직의 적응이 용이하다. 복잡하거나 특수한 기능 및 견고성이 필요한 악기능교정 장치 등에서는 그 사용이 제한된다는 한계점이 있으나, 소아치과 임상에서 사용되는 대부분의 장치를 쉽고 간편하게 제작할 수 있어 앞으로 임상에서 널리 이용될 수 있을 것으로 생각된다.

Evaluation of growth changes induced by functional appliances in children with Class II malocclusion: Superimposition of lateral cephalograms on stable structures

  • Objective: To compare short- and long-term dentoalveolar, skeletal, and rotational changes evaluated by Björk's structural method of superimposition between children with Class II malocclusion treated by functional appliances and untreated matched controls. Methods: Seventy-nine prepubertal or pubertal children (mean age, 11.57 ± 1.40 years) with Class II malocclusion were included. Thirty-four children were treated using an activator with a high-pull headgear (Z-activator), while 28 were treated using an activator without a headgear (E-activator). Seventeen untreated children were included as controls. Lateral cephalograms were obtained before treatment (T1), after functional appliance treatment (T2), and after retention in the postpubertal phase (T3). Changes from T1 to T2 and T1 to T3 were compared between the treated groups and control group using multiple linear regression analysis. Results: Relative to the findings in the control group at T2, the sagittal jaw relationship (subspinale-nasion-pogonion, p < 0.001), maxillary prognathism (sella-nasion-subspinale, p < 0.05), and condylar growth (p < 0.001) exhibited significant improvements in the Z- and E-activator groups, which also showed a significantly increased maxillary incisor retraction (p < 0.001) and decreased overjet (p < 0.001). Only the E-activator group exhibited significant backward rotation of the maxilla at T2 (p < 0.01). The improvements in the sagittal jaw relationship (p < 0.01) and dental relationship (p < 0.001) remained significant at T3. Condylar growth and jaw rotations were not significant at T3. Conclusions: Functional appliance treatment in children with Class II malocclusion can significantly improve the sagittal jaw relationship and dental relationships in the long term.
