교정용 밴드를 이용한 구치부 수복 (MOLAR RESTORATION WITH AN ORTHODONTIC BAND)

  • 소아 및 청소년에서 유구치 및 대구치는 인접면이나 평활면에 광범위한 치아우식증으로 치질이 많이 파괴되거나, 치수치료로 인하여 치질 부위가 약해져서 파절되기 쉬운데, 치료방법으로 기성금속관이 사용되고 있다. 기성금속관은 1950년대 Humphrey에 의해서 소개되었고, 장점으로 내구성이 강하고, 임상적 수명이 길며, 성공률이 높지만, 치질 삭제로 인한 치수노출 위험성과 인접 치아의 삭제의 가능성이 있고, 다수의 치아 수복 시에 교합면 높이를 맞추기 어려우며, 큰 크기의 기성금속관을 선택한 경우 금속관의 변연에 의해 인접 치아의 맹출을 방해할 수 있다는 단점이 있다. 기성금속관의 단점을 보완하기 위한 방법으로 구치부 수복 시 교정용 밴드를 사용하는 것을 고려해 볼 수 있는데, 치아우식증으로 파괴된 치질 부위를 교정용 밴드와 복합레진, 또는 글래스아이오노머, 아말감 등으로 수복하는 방법이다. 교정용 밴드를 이용한 구치부 수복은 치경부와 교합면에 치아우식증이 있어 치질이 많이 파괴되거나, 치수치료한 치아, 법랑질 저형성된 대구치 등에서 사용될 수 있고, 시술 시간이 짧아 행동조절이 어려운 어린이에서 유용하다. 그러나, 치료 후 구강위생상태를 청결히 유지시키는 것이 중요하다. 이 증례들에서는 유구치와 대구치에 광범위한 치아우식증의 치료를 위해 내원한 환아에서 교정용 밴드를 이용한 보존적 수복치료를 시행하여 양호한 결과를 보여 이를 보고하는 바이다.

최근 5년간 광주지역 소아치과에 내원한 신환의 분포와 진료 내용에 대한 조사 (A STUDY ON THE CHANGES OF PATIENT DISTRIBUTION AND TREATMENT PATTERN FOR THE LAST 5 YEARS IN PEDIATRIC DENTAL PRACTICE OF GWANGJU)

  • 최근 사회 경제적인 변화로 소아치과의 진료 양상도 변화하고 있다. 이런 변화 양상을 파악하여 미래에 소아치과가 나아갈 방향성을 제시하고자 본 조사를 실시하였다. 2005년부터 2010년까지 조선대학교 소아치과와 광주의 소아전문 개인치과의원 두 곳에 내원한 모든 신환을 대상으로 조사하였다. 신환의 수는 꾸준한 증가 추세를 보였고, 이 중 남아의 비율이 더 높았으며, 연령별 분포에서는 3~4세군의 내원율이 가장 높았다. 조선대학교 소아치과의 경우 방학 중 내원율이 높았으나, 소아전문 개인 치과의원은 특정 기간과 상관관계가 없었다. 주소 분포에서는 충치가 가장 높은 비율을 보였으며, 예방 및 검진을 위한 내원율의 증가와 함께 불소도포가 증가하였다. 치료 중에서는 수복치료의 비율이 가장 높았으며, 이 중 레진과 글라스아이오노머의 수복이 가장 많았고, 아말감 수복은 감소하였다. 진정치료의 비율은 약간 감소하였고, 소아전문 개인치과의원과 비교해 조선대학교 소아치과에서 더 높은 비율을 보였다.

Air abrasive technique을 이용한 복합레진 수복 증례 (TREATMENT OF COMPOSITE RESIN RESTORATION WITH THE AIR ABRASIVE TECHNIQUE)

  • The air abrasive technique is a non-mechanical method by which teeth are treated before restoration and stains and calculi are removed from tooth surfaces using the kinetic energy of small particles. The air abrasive technique in dentistry was first introduced in the 1950's with as instrument called 'Airdent'. But, as the main restorative materials of the period were amalgam and gold, and the instrument's inability to control the flow of particles caused the particles to be spread throughout the clinics, widespread use was not possible. In the 1990's, as these techincal problems were solved and more interest in new restorative materials rose in an effort to preserve sound tooth structure, new developements took place in instruments related to the air abrasive technique. The air abrasive technique produces less pressure, vibration and heat that might cause patient discomfort and facilitates the preservation of sound tooth structure. It also reduces the need for anesthesia and is less harmful to the pulp. Other advantages include increase in dentin bonding strength of composite resin, lower possibility of saliva contamination and maintenance of a dry field. But there is not direct contact between the nozzle and the tooth, the operator cannot use his or her tactile sense and must rely solely upon visual input. Other disadvantages are: the tooth preparation depends on the operator's ability; alpha-alumina particles, after bouncing off the tooth surface, cause damage to dental mirrors; the equipment is expensive and takes up a certain amount of space in the clinic. The author conducted case report using the air abrasive technique on patient visiting the Department of Pediatric Dentistry at Seoul National University Dental Hospital and arrived at the following conclusions. 1. The tooth preparation capability of different air abrasive devices varied widely among manufacturers. 2. It was more effective in treating early caries lesions and stains compared to lesions where caries had already progressed to produce soft dentin. 3. The cold stream and noise caused by the evacuation system was a major cause of discomfort to pediatric patients. 4. As there is no direct contact with tooth surface when using the air abrasive technique for tooth preparation, considerable experience and skill is required for proper tooth preparation.

치수보호용 제재가 성견 치수조직에 미치는 영향에 관한 병리조직학적 연구 (A HISTOPATHOLOGICAL STUDY OF PULP TISSUE REACTION TO INTERMEDIATE RESTORATIVE MATERNAL IN YOUNG ADULT DOG'S TEETH)

  • This study was undertaken to evaluate the pulpal responses to the intermediate restorative materials such as Zinc phosphate cement, Polycarboxylate cement, IRM (zinc oxide eugenol cement), Dycal, Life, Cresatin, and Fluoride in caivties which were cut with high speed instrument. 5 dogs were used as experimental animals and devided into 8 groups. The intervals of observaobservation ranged 3 days, 1, 3, 4, 8 weeks after experiment respectively. The specimens were fixed with 10% formalin and decalcified in 5% nitric acid. All slides were stained with hemtoxylin-eosin and examined histopathologically. The results were as follows: 1. In control group, severe vacuolar degeneration and atrophy of odontoblasts were seen in 3 days, hemorrhage and congestion continued until 8 weeks. Necrosis of odontoblastic layer was seen in zinc phosphate cement group and polycarboxylate cement group. 2. In dycal group, vacuolar degeneration and atrophy of odontoblast were not seen. but in Life group, these were seen in 3 days and partially continued until 3 weeks. In 4 weeks, regeneration of odontoblast was occured. 3. In Crcsatin group, there was no pathosis except odontoblastic displacement. In Fluoride group, vacuolar degeneration of odontoblast was seen and soon disappeared. As compared with control group, pathological change of the pulp tissue in experimental group were decreased after amalgam restoration.

승홍 투여가 백서 악하선 세포의 조직병리학적 변화에 미치는 영향에 관한 연구 (AN HISTOPATHOLOGICAL STUDY ABOUT THE CHANGE OF SUBMANDIBULAR GLANDULAR CELL IN WHITE RAT FOLLOWING $HgCl_{2}$ INJECTION)

  • Mercury is one of the most frequently used heavy metal in dental clinic. Mercury poisoning rises up when someone is exposed to mercury chronically. In 1818, Amalgam was used for dental restorative procedure, and after then study about mercury toxicity has begun. Clinical signs of mercury toxicity in oral & maxillofacial area were increases of salivation, metallic taste, swelling and pain of tongue, redness and ulceration of oral mucosa, and increased mobility and loss of teeth. After we injected mercury($HgCl_{2}$) into intraperitoneum of rat, studied about histopathological changes of submandibular gland cell. Experimental group was divided into two groups by amount of mercury. (Group 1 was 0.5mg/Kg of mercury injection, group 2 was 1.0mg/Kg of mercury injection.) 1. After 3days of intraperitoneal injection, black granules were observed at macrophage cell in both group. In group 2, author found hyperchromatism of nucleus, and vacuolization of cellular matrix and nucleus of acinar cell. 2. After 1week of intraperitoneal injection, author found severe vacuolization of nucleus and cellular matrix, and irregular granules around nuclear membrane at mucous cell and serous cell in both group. Vacuolization of nucleus and cellular matrix was seen at duct cell in group 2. 3. After 2weeks of intraperitoneal injection, author could found severe vacuolization of cellular matrix, and sometimes nucleus was positioned in central area of cellular matrix at mucous and serous cell in both group. Vacuolization of nucleus and cellular matrix was found at vascular endothelial cell in group 2. 4. After 4weeks of intraperitoneal injection, destruction and distortion of gland cells were distinct. Vacuolization and destruction of nucleus and cellular matrix was found at duct cell in group 2. After intraperitoneal injection of mercury, we found equanimity of mercury and destruction of cellular matrix at serous cell, mucous cell, and duct cell of submandibular gland. So, we thought that metallic taste of mercury poisoning patient would be due to excretion of saliva containing mercury.

Comparative Analysis of Water Absorption and Water Solubility of Alkasite-based Restorative Material

  • Purpose: Cention N (Ivoclar Vivadent) was a recently introduced alkasite-based restorative material that was expected to replace amalgam and glass ionomer cement. This material was an esthetic restoration with adequate mechanical strength and release of fluoride and calcium. The purpose of this study was to measure the water sorption and water solubility of Cention N and evaluate its long-term durability compared to other esthetic restorations (Resin-Modified Glass Ionomer cement [RMGIC], Giomer, Composite Resin). Materials and Methods: Twenty specimens each of Cention N (CN), Resin Modified-Glass Ionomer Cement (FJ), Giomer (BF), and Composite Resin (FZ) were made. After each specimen was completely dried in a desiccator for 24 hours using a vacuum pressure pump, the specimen was weighed (m1). After that, the specimen was immersed in distilled water at 37℃ for 7 days, stored in a drying oven, and weighed (m2). After drying completely for 24 hours in a desiccator, the specimen was weighed (m3) to calculate the water absorption and water solubility using Formulas 1 and 2. The measured values were statistically processed and analyzed using SPSS, and the significance level was set at 0.05. Result: When measuring water sorption, FJ (122.61 ㎍/mm3) showed significantly higher water sorption than CN (35.42 ㎍/mm3) (P<0.05). There was no significant difference between FZ (18.03 ㎍/mm3) and BF (14.76 ㎍/mm3) (P=0.930). When measuring water solubility, CN (6.65 ㎍/mm3) showed significantly higher water solubility than FJ (1.47 ㎍/mm3) (P<0.05). Conclusion: Cention N had lower water sorption than RMGIC, but higher water solubility, indicating that it is more vulnerable to moisture and has lessened long-term durability.

최근 5년간 연세대학교 치과대학병원 소아치과의 환자 분포 및 치료 경향 (The Distribution of Patients and Treatment Trends in the Department of Pediatric Dentistry, Yonsei University Dental Hospital for Last 5 Years)

  • 본 연구는 환자분포 및 치료경향을 조사하여 소아치과의 변화하는 진료 양상을 알기 위해 시행하였다. 2008년 1월부터 2012년 12월까지 연세대학교 치과대학병원 소아치과에 내원한 환자를 대상으로 전자의무기록을 수집하고 분석하였다. 조사 결과 초진환자와 비선택 진료의사 1인당 환자 수는 감소하였고 외국인 환자수가 급증하였다. 신환은 만 0~2세의 연령층이 가장 많았고, 내원 주소는 치아우식증, 외상, 부정교합 순으로 높게 나타났다. 수복치료의 분포 결과 복합레진 수복, 아말감 수복, 치면열구전색술은 감소하였고, 자가중합형 글라스아이오노머, 예방적 레진수복이 증가하는 추세였다. 다내원근관치료가 감소하고 당일발수 근관 충전이 증가했으며, 치수복조술과 치수절단술과 같은 보존적 치수치료의 비율이 증가하였다. 환자의 불안을 감소시키기 위해 진정법을 선택하는 경우가 증가하였으며 아산화질소의 이용이 주를 이루었다. 내원 환자의 분포 및 특성을 파악하고, 최신 치료 경향을 제공함으로써 환자의 요구에 부합하는 적절한 치료계획을 수립하는데 도움이 될 것으로 생각된다.

수은 고노출지역 주민의 수은노출요인과 노출량 변화에 관한 연구 (Mercury Exposure Factors among Residents in the Highly Mercury Exposed Area, Seoksan-ri, Korea)

  • Objectives: The 2007 Korea National Survey for Environmental Pollutants in the Human Body found the highest blood mercury levels nationwide among residents in Seoksan-ri, Goro-myeon, Gunwi-gun, Gyeonsangbuk-do. With the aim to reduce the blood mercury levels of residents in this region, we conducted this study to identify the association between mercury exposure levels and shark meat consumption. Methods: This survey was conducted with 118 participants in Seoksan-ri before the Chuseok festival and 113 residents were added afterwards. Information on participants was collected via questionnaires. Total mercury concentrations in biological samples were measured using a mercury analyzer with the gold-amalgam collection method. Results: To identify this, we conducted mercury exposure level analysis before and after the Chuseok festival and found that blood and urinary mercury levels after Chuseok (GM of $6.9{\mu}g/L$ in blood and $1.68{\mu}g/g$_cr in urine) were higher than those before (GM of $5.29{\mu}g/L$ in blood and $1.44{\mu}g/g$_cr in urine). This area maintains a custom of using shark meat as one of the ancestral rite foods, and the performance of such rites and shark meat consumption have been identified as main sources of mercury exposure. Other than this, smoking, dental amalgam treatment and residential period in the area also contributed to an increase in mercury exposure levels. On the other hand, recent consumption of oriental medicine and vaccination did not have a significant influence on mercury levels. Conclusion: The results were attributed to the local custom of consuming shark meat with high mercury concentrations during rituals taking place during the festival and ancestral rites. Given that the blood mercury levels in 23.2% of the residents exceeded the HBM II values recommended by the German Commission on Human Biological Monitoring, it is suggested that further appropriate actions and follow-up measures be taken to reduce the mercury exposure levels of the residents that exceeded the reference values.

불소화합물을 첨가한 생리식염수에서 아말감의 부식거동에 관한 전기화학적 연구 (AN ELECTROCHEMICAL STUDY ON THE CORROSION BEHAVIOUR OF AMALGAMS IN THE SALINE SOLUTIONS WITH FLUORIDE COMPOUNDS)

  • The purpose of this study is to observe the corrosion characteristics of five dental amalgams(CAULK FINE CUT, OPTALLOY II, DISPERSALLOY, TYTIN, and VALIANT) in the solutions with fluoride compound through the anodic polarization curve obtained by using a potentiostat. After each amalgam alloy and Hg being triturated, the triturated mass was inserted into the cylindrical metal mold, and condensed by hydraulic pressure(160 kg/$cm^2$). Each specimen was removed from the metal mold. Specimens were polished with the silicone carbide grinding paper 24 hours after condensation and stored at room temperature for 1 week. The anodic polarization curves were employed to compare the corrosion behaviours of the amalgams in 0.9 % saline solution and in the saline solutions with 2.2 ppm, 0.05 %, 2 % NaF, and 8 % $SnF_2$ at $37^{\circ}C$ with 3-electrode potentiostat. After the immersion of specimen in electrolyte for 30 minutes, the potential scan was begun. The potential scan range was -1500m V to + 800m V(vs. S.C.E.) in the working electrode and the scan rate was 50 mV/sec. The results were as follows, 1. The corrosion potential, the potential of anodic current peak, and transpassive potential in the saline solutions with NaF shifted to lower direction than those in normal saline solution, and the current density in the saline solutions with NaF was higher than that in normal saline solution. The differences were increased as the concentrations of NaF became higher. 2. The corrosion potential and transpassive potential in the saline solution with $SnF_2$ shifted to higher direction than those in normal saline solution, and the current density in the saline solution with $SnF_2$ was higher than that in normal saline solution after the corrosion potential. The anodic polarization curves in the saline solution with $SnF_2$ had no outstanding anodic current peak. 3. The corrosion potentials for high-copper amalgams were much higher than those for CAULK FINE CUT and OPTALLOY II in normal saline solution, but, as the concentrations of fluoride compound became higher, the differences in corrosion potentials between them were decreased. The corrosion potentials had the similarity in the saline solution with 2% NaF or 8% $SnF_2$. 4. The current density for TYTIN was the lowest among the others in normal saline solution and in the saline solution with 2.2 ppm or 0.05 % NaF. 5. There was no significant difference in current density between Pd-enriched VALIANT and other high-copper amalgams.

수은 고노출 지역 초등학생의 수은노출수준과 노출요인 연구 (A Study on Mercury Exposure Levels among Schoolchildren and Related Factors in High Mercury Exposure Areas in Korea)

  • Objectives: Blood mercury levels among adults living in certain areas of the Gyeongsang Provinces have been shown to be very high (Kunwee County $29.6{\mu}g/L$, Yeongcheon-city $26.7{\mu}g/L$). The purpose of this project was to determine mercury exposure levels in schoolchildren and factors related with their mercury levels in high mercury exposure areas identifyed by the 2007 Korea National Environmental Health Survey. Methods: From June to September 2010, 1,097 students from grades 3 to 6 at 19 elementary schools participated in this study, including 294 students from 10 elementary schools in Kunwee County, 529 students from Yeongcheon City, 122 students from two elementary schools in Pohang City, North Gyeongsang Province, and 152 students from two elementary schools in Ulsan Metropolitan City. Biological samples from schoolchildren, including whole blood, urine and hair, were collected to measure total mercury at the time of a health check up. Information about children was collected by questionnaire. Total mercury concentrations in blood were measured using the Direct Mercury Analyzer 80 with the gold-amalgam collection method. Results: The mean mercury levels were $2.70{\mu}g/L$ in 1,091 blood samples, $2.25{\mu}g/g-creat.$ in 820 urine samples and $1.03{\mu}g/g$ in 1,064 hair samples. Blood mercury levels in the schoolchildren was slightly higher than the result of $2.4{\mu}g/L$ from a 2006 survey of elementary school children on exposure and health effects of mercury by the National Institute of Environmental Research. However, 0.3% and 4.5% of participants exceeded the reference level of blood mercury by CHBMII ($15{\mu}g/L$) and the US EPA ($5.8{\mu}g/L$), respectively. The reference level of urine by CHBMII ($20{\mu}g/L$) was exceeded by 0.4% of participants. As factors, residence period in the study areas, residence type, father's education level and income all showed significant associations with mercury level in the biological samples. The number of dental amalgam sides showed an association with urine mercury. Fish intake preference and fish intake frequency were important factors in mercury levels. In particular, intake of shark meat and recent intake of shark meat were associated with higher mercury levels. In this regard, participation in the performance of an ancestral rite showed a relation with higher mercury levels. Conclusion: The intake of shark meat was very important factor to high mercury exposure level. It is recommended to monitor and manage students with high mercury exposures who exceeded CHBM II and EPA guidelines, and include blood mercury testing in the Children's Health check up for this province.
