본 연구에서는 이중 시간 PET/CT 검사에서 지연검사 시간이 다른 두 그룹의 40명의 췌장암 환자에 대한 SUV 데이터의 통계 분석을 통해 최적의 지연검사 시간을 제시하였다. 췌장암의 유무를 확인해야 될 이중 시간 PET/CT 검사에서 새로운 수학적 예측 모델을 만들어 정확도 높은 최적의 지연검사 시간을 제시하였다. 시간에 따른 포도당 대사를 적용한 방사능 붕괴식을 예측 모델로 제시하였다. 지연검사 시간이 다른 두 그룹의 데이터를 가지고 시뮬레이션한 결과 PET/CT에서 췌장암 판별을 위한 최적의 지연검사 시간이 131.5분으로 제시하였다. 제안된 지연검사 시간은 췌장암 결정에 적용될 수 있는 통계적 유의 수준을 보였다(p<0.05). 제안된 지연 검사 시간 131.5분을 결정하여 검사를 진행할 때, 췌장암 조기 판별에 도움을 줄 수 있을 것으로 사료된다.
FDG PET/CT 검사에서 FDG는 신장 및 요료계를 통하여 배설된다. 이 방사능의 배설을 촉진 시키는 방법을 비교 및 분석하여 각 의료기관의 실정에 맞는 방법을 선택하는데 도움을 주고자 한다. 8개월간 본원에서 PET/CT 검사를 시행한 환자 중 신장에 병적 병변이 없고 기능이 정상인 환자 총 30명을 대상으로 하였다. FDG-PET/CT 기본검사를 마친 환자들을 무작위로 10명씩 다음 세 집단으로 나눠 지연검사를 촬영하였다. (1) 1시간 대기한 집단(Delay group), (2) 수분을 약 700 mL 섭취한 후 1시간 대기한 집단(Hydration group), (3) 이뇨제를 정맥주사하고 30분 대기한 집단(Lasix group). 평가는 육안평가와 정량분석을 하였다. 육안평가는 기본검사와 비교하여 각 집단별 지연검사 영상에서 신장의 방사능 배설을 네 단계로 나누어 비교 및 분석하였다. 정량분석은 기본검사의 표준화섭취계수에서 지연검사의 표준화섭취계수의 감소율을 측정하여 신장 방사능의 배설률로 활용하였다. 육안평가상 Lasix group이 가장 배설이 잘 되었고 그 다음은 Hydration group이었다. Delay group은 효과적인 배설이 이루어지지 않았다. 정량분석 결과 신장방사능 배설률은 Delay group $9.2{\pm}20.7%$, Hydration group $28.1{\pm}22.8%$, Lasix group $29.5{\pm}23.1%$이었다. Delay group과 Hydration group간, Delay group과 Lasix group간의 배설률은 통계적으로 유의한 차이를 보였다. Hydration group과 Lasix group간은 유의한 차이가 없었다. 본 연구에서 이뇨제를 사용하는 것이 신장의 방사능 배설을 촉진시키는데 가장 효과적임을 확인할 수 있었다. 또한 수분을 섭취하는 방법도 비슷한 효과를 보였으며 수분 섭취 없이 지연영상을 얻는 것은 효과가 없음을 확인 하였다. 이상의 방법들을 각 의료기관 실정에 맞게 적용함으로써 영상의 질 향상에 도움을 줄 수 있을 것이다.
복부 초음파 영상검사에서 부신 종괴가 확인된 세 마리의 개에서 지연형 조영증강 전산화단층촬영을 적용한 영상검사가 진행되었다. 조영제 주입 전과 주입 직후, 5분의 지연 시간 후에 CT 검사를 하였으며 위치, 조영 전과 후의 양상, 주변 장기와의 관계, washout 정도를 평가하였다. 세 마리 개 모두에서 부신절제술이 실시되었으며 조직병리 검사 결과 골수지방종을 동반한 크롬친화세포종, 부신샘종, 부신암종으로 진단되었다. 부신암종을 가진 한 마리의 개를 제외하고는 수술 후 회복하여 퇴원하였다.

The results were as follows; 1) CT findings of delayed CO sequleae were bilateral low density lesion in globus pallidus (l pt.), diffuse low density in white matter with bilateral low density in white matter (l pt.), diffuse low density in white matter with bilateral low density in globus pallidus (l pt.), diffuse low density in white matter with cortical atrophy (l pt.), bilateral low density in globus pallidus and diffuse low density in white matter with cortical atrophy (l pt.) and normal in 3 pts. 2) $^{99m}Tc-HMPAO$ Brain SPECT findings of delayed CO sequelae were decreased regional cerebral blood flow (rCBF) in frontal (1 among 8 pts.), frontal and basal ganglia (3 among 8 pts.), and diffuse patch decreased rCBF pattern (4 among 8 pts.) 3) $^{99m}Tc-HMPAO$ Brain SPECT study was well correlated with neurologic symptoms and signs in delayed CO sequelae. Our results may suggest that reduced cerebral blood flow contributes to the development of delayed CO sequelae.
Intraorbital wooden foreign bodies may present difficulties in diagnosis due to their radiolucent nature. Delayed recognition and management can cause significant complications. We present a case report that demonstrates these problems and the sequela that can follow. A 56-year-old man presented with a 3-cm laceration in the right upper eyelid, sustained by a slipping accident. After computed tomography (CT) scanning and ophthalmology consultation, which revealed no fractures and suggested only pneumophthalmos, the wound was repaired by a plastic surgery resident. Ten days later, the patient's eyelid displayed signs of infection including pus discharge. Antibiotics and revisional repair failed to solve the infection. Nearly 2 months after the initial repair, a CT scan revealed a large wooden fragment in the superomedial orbit. Surgical exploration successfully removed the foreign body and inflamed pocket, and the patient healed uneventfully. However, the prolonged intraorbital infection had caused irreversible damage to the superior rectus muscle, with upgaze diplopia persisting 1 year after surgery and only minimal muscle function remaining. We report this case to warn clinicians of the difficulties in early diagnosis of intraorbital wooden foreign bodies and the grave prognosis of delayed management.
펜타닐 첩포가 의학적으로 이용됨에 따라 간간이 펜타닐 중독 증례가 보고되고 있다. 지연성저산소성 백색질뇌증(delayed post-hypoxic leukoencephalopathy)은 저산소증에 의한 합병증의 하나이다. 이로 인해 급성 중독으로 인한 혼수상태에서 완전히 회복하더라도, 뒤늦게 신경병적 증상이 발생하고, 이후 백색질뇌증으로 진행하기도 한다. 여기 10개의 펜타닐 첩포를 한 번에 사용하고 13시간 동안 깨어나지 못하여 본원으로 내원한 65세 환자의 증례를 소개하였다. 첫 내원 당시 촬영한 CT 소견은 정상이었으나, 20일 뒤 지연성 신경병적 증상이 발생하여 촬영한 MRI 상 양측 대뇌 백색질에 대칭적인 불균일하고 융합하는 고신호 강도 병변이 T2와 FLAIR 영상에서 관찰되었으며 이 병변은 확산 제한을 보였다. 이후 환자의 신경병적 증상은 빠르게 악화되었고 1년 후 심한 전반적 뇌 위축으로 진행하였다.

Posttraumatic pseudoaneurysm of the face is caused by blunt, penetrating, or surgical trauma. Although its incidence is low, pseudoaneurysm rupture can cause a life-threatening, massive hemorrhage. A 48-year-old man visited our emergency center due to a fall-down accident. Three-dimensional computed tomography (CT) showed a comminuted zygomaticomaxillary complex fracture of the left face. After open reduction and internal fixation, the surgical wound healed without any complications. However, the patient was readmitted 10 days after surgery due to pus-like discharge from the wound. Contrast-enhanced CT to find the abscess unexpectedly revealed a pseudoaneurysm in the left retromaxillary area. Massive oral bleeding occurred on the night of re-hospitalization and emergency surgery was done. The bleeding site was identified as a pseudo-aneurysmal rupture of the posterior superior alveolar artery in the retromaxillary area. Hemostasis was achieved by packing Vaseline gauze in the maxillary sinus using an endoscope. Delayed rupture and massive bleeding of posttraumatic retromaxillary pseudoaneurysm after a zygomaticomaxillary fracture is a low-probability, but high-impact event. Therefore, additional contrast-enhanced CT should be considered to evaluate the possibility of a posttraumatic pseudoaneurysm in cases of severe comminuted zygomaticomaxillary fracture.
We describe two cases of tension pneumocephalus, one caused by ventriculoperitoneal shunt for communicating hydrocephalus, and the other caused by craniocerebral trauma. In the first case report, we examined the relationship between cerebrospinal fluid leakage and delayed onset tension pneumocephalus. The second case report, we addressed issues such as the diagnosis, management, and pathogenesis, as well as computerized tomography(CT) findings.
Objective: To evaluate the image quality of novel dark-blood computed tomography angiography (CTA) imaging combined with deep learning reconstruction (DLR) compared to delayed-phase CTA images with hybrid iterative reconstruction (HIR), to visualize the cervical artery wall in patients with Takayasu arteritis (TAK). Materials and Methods: This prospective study continuously recruited 53 patients with TAK (mean age: 33.8 ± 10.2 years; 49 females) between January and July 2022 who underwent head-neck CTA scans. The arterial- and delayed-phase images were reconstructed using HIR and DLR. Subtracted images of the arterial-phase from the delayed-phase were then added to the original delayed-phase using a denoising filter to generate the final-dark-blood images. Qualitative image quality scores and quantitative parameters were obtained and compared among the three groups of images: Delayed-HIR, Dark-blood-HIR, and Dark-blood-DLR. Results: Compared to Delayed-HIR, Dark-blood-HIR images demonstrated higher qualitative scores in terms of vascular wall visualization and diagnostic confidence index (all P < 0.001). These qualitative scores further improved after applying DLR (Dark-blood-DLR compared to Dark-blood-HIR, all P < 0.001). Dark-blood DLR also showed higher scores for overall image noise than Dark-blood-HIR (P < 0.001). In the quantitative analysis, the contrast-to-noise ratio (CNR) values between the vessel wall and lumen for the bilateral common carotid arteries and brachiocephalic trunk were significantly higher on Dark-blood-HIR images than on Delayed-HIR images (all P < 0.05). The CNR values were significantly higher for Dark-blood-DLR than for Dark-blood-HIR in all cervical arteries (all P < 0.001). Conclusion: Compared with Delayed-HIR CTA, the dark-blood method combined with DLR improved CTA image quality and enhanced visualization of the cervical artery wall in patients with TAK.
The usefulness of $^{99m}Tc-labeled$ RBC single photon emission CT (SPECT) scanning in the diagnosis of hepatic heminagiomas was evaluated in 22 patients with various focal hepatic lesions including 15 cases of hemangiomas, 3 cases each of hepatomas and metastasis and 1 case of abscess. The diagnoses were based on ultrasonography and/or CT scanning, clinical stability of lesion for at least 6 months or surgical exploration. Seven cases of 15 hemangiomas were detected by delayed planar RBC scanning, whereas 4 cases were detected by delayed RBC-SPECT scanning. The smallest hemangioma shown by delayed RBC-SPECT scanning was 1.0 cm in diameter. compared with 2.2 cm by planar RBC scanning. One small hemangioma (2.0 cm) located adjacent to the heart was not found by either method. The sensitivities in detecting the hemangioma according to the site by planar imaging were 16.6% $(1.0\sim1.9cm)$, 66.7% $(2.0\sim2.9cm)$ and 83.3% (more than 3.0 cm) and by SPECT were 50.0%, 66.7% and 100%, respectively. Seven cases of non-hemangiomatous lesions did not show any significant increase in activity in the delayed blood pool images. It is concluded that $^{99m}Tc-RBC$ blood-pool SPECT scanning is clearly more sensitive in detecting small hemangioma than planar scanning and is, therefore, a choice of method for the detection of hepatic hemangioma.
