EFFECT OF DRAINAGE AS A STRESS REDUCTION METHOD BEFORE EXTRACTION OF ADVANCED INFECTED TEETH IN DISABLED PATIENTS : REVIEW OF LITERATURE & REPORT OF CASES (장애환자에서 과도한 감염치아 발치전 스트레스 감소법으로서 배농술의 효과 : 문헌적 고찰 및 증례보고)

  • Dental extraction is potentially stress-inducing in many disabled patient. The body's response to dental stress involves the cardiovascular system(an increase in cardiovascular workload), the respiratory organ and the endocrine system(change in metabolism). To minimize the stress, the stress reduction method was established. The obtained contents were as follows: (1) Recognize the patient's degree of medical risk, (2) Complete medical consultation before dental therapy, (3) Schedule the patient's appointment in the morning, (4) Monitor and record preoperative and postoperative vital signs, (5) Use psychosedation during therapy, (6) Use adequate pain control during therapy, (7) Short length of appointment : do not exceed the patient's limits of tolerance, (8) Follow up with postoperative pain/anxiety control, (9) Telephone the risk patient later on the same day that treatment was given. Though the stress reduction method above was applied to the dental extraction in disabled patients with the advanced infected teeth, the complications(syncope, shock, bleeding & infection, etc.) may be occurred. For prevention of complications associated with the extraction, the authors treated the advanced infected teeth with endodontic drainage and incision & drainage before extraction. The final extraction and wound closure were then done after about 3 weeks.

Analysis of Experimental Studies on Master s and Doctoral Theses in Nursing (우리나라 간호계 석 . 박사학위 논문분석 -실험연구를 중심으로-)

  • This study was carried out to describe and analyze experimental studies conducted in graduates nursing degree. Of 170 experimental studies conducted during the past three decades between 1962 and August 1991, 150 studies were available, including 124 master’s and 26 doctoral theses. This study examined their general characteristics and detailed research methods using percentiles. The results were as follows 1. Most of the studies adopted a quasi-experimental design. 2. The subjects of the studies were chosen by convenience sampling except for two studies whose subjects were drawn by randomization. Studies comparing experimental and control groups were in the highest proportion and the most frequent sample sizes of each group were 21 to 30 for both experimental and control groups. 3. As to measurement, physiological measures were most frequent followed by psychsociological measures and active report questionnaires. Each study, on average, adopted two kind of measurement tools. Studies in which the data collection period was of 1~2 months were in the highest pro-portion. 4. All doctoral theses and 67.0% of master’s theses examined specific research hypotheses. Of these studies, the results of 92.5% supported the hypotheses. 5. Parametric statistics were the major analytical methods. In particular, t-test was used most frequently followed by Chi square, F-test, and Pearson Correlation Coefficients. 6. Patients were the most frequent study subjects. Frequent nursing interventions were information and education followed by support, distraction, and nursing treatments. 7. With regards to the dependent variables, “feelings” such as anxiety, pain, and depression were most frequent. In addition “exchanging” such as restoring, metabolism, cardiopulmonary function, infection and vital signs were adopted as the dependent variables in 29.1% of the studies examined, while 12.3% of the studies selected “choosing” such as stress, health behavior, or role performance.

A Recombinant $Plasmodium$ $vivax$ Apical Membrane Antigen-1 to Detect Human Infection in Iran

  • In Iran, $Plasmodium$ $vivax$ is responsible for more than 80% of the infected cases of malaria per year. Control interventions for vivax malaria in humans rely mainly on developed diagnostic methods. Recombinant $P.$ $vivax$ apical membrane antigen-1 (rPvAMA-1) has been reported to achieve designing rapid, sensitive, and specific molecular diagnosis. This study aimed to perform isolation and expression of a rPvAMA-1, derived from Iranian patients residing in an endemic area. Then, the diagnostic efficiency of the characterized Iranian PvAMA-1 was assessed using an indirect ELISA method. For this purpose, a partial region of AMA-1 gene was amplified, cloned, and expressed in pET32a plasmid. The recombinant $His-tagged$ protein was purified and used to coat the ELISA plate. Antibody detection was assessed by indirect ELISA using rPvAMA-1. The validity of the ELISA method for detection of anti-$P.$ $vivax$ antibodies in the field was compared to light microscopy on 84 confirmed $P.$ $vivax$ patients and compared to 84 non-$P.$ $vivax$ infected individuals. The ELISA cut-off value was calculated as the mean+2SD of OD values of the people living in malaria endemic areas from a south part of Iran. We found a cut-off point of OD=0.311 that showed the best correlation between the sera confirmed with $P.$ $vivax$ infection and healthy control sera. A sensitivity of 81.0% and specificity of 84.5% were found at this cut off titer. A good degree of statistical agreement was found between ELISA using rPvAMA-1 and light microscopy (0.827) by Kappa analysis.

Ultrasound Probe Contamination Classification using ATP Meter (ATP meter를 이용한 초음파 탐촉자의 오염도 분류)

  • In this study, the contamination level was measured using an ATP meter using adenosine triphosphate bioluminescent material to find effective infection control to compensate for the disadvantages of the microbial culture method used for hygiene control of ultrasound probe. The convex probes were selected from six ultrasound probe in the hospital, and the samples were taken in real time before and after cleaning to check the contamination of the probe. In order to classify the pollution degree using the APT meter was classified by category. A total of 78 samples were collected from the ultrasound probe. When the pollution levels before and after cleaning were classified by category, 76.6% of the samples were classified into category 3·4 before cleaning, but they decreased to 23.3% after cleaning. 13.3% before cleaning was in category 1, but increased to 43.3% after cleaning. By classifying the pollution level, it was confirmed that the pollution level was significantly reduced by category. Until now, there was no suitable criterion for determining the contamination level by using ATP meter in medical machines where sample area is small and reused. In this study, criteria for each category were set to measure the contamination level of ATP meter suitable for small sample area such as ultrasound probe, so that contamination level could be determined in real time at the site. Therefore, it is considered that hygiene management for ultrasound probe can be more actively performed.

Forehead Island Flap For Nasal Reconstruction (이마 섬피판을 이용한 코재건술)

  • The nose is the most prominent area of the face, therefore susceptible to trauma and skin cancer. When small sized defect is in nasal tip, it results in disturbance of the facial harmony even if replantation, composite graft, skin graft or median forehead flap has been used for the reconstruction. So it is needed that the best method reconstruction is performed according to the degree of defect or deformity. And at the same time the physiology and anatomy of nose were clarified and its aesthetic subunits were employed. How can we cover the about 3 cm sized nasal defect in nasal tip with cartilage exposure? At first, we can think forehead island flap is most appropriate. We performed 7 cases of the forehead island flap for reconstruction of the defect in nasal tip(4 cases: cancer, 3 cases: trauma) from March, 2001 to August, 2004. This result was satisfactory in the point of texture, color, donor scar, and there were no complication such as wound disruption, infection, flap atrophy, and hematoma. The advantages of forehead island flap are: 1) No injury of deep vessel and nerve, 2) control of shape and volume, 3) Short operation time, 4) primary closure of donor site, 5) one stage operation. Also, forehead island flap can cover the defect in nose where skin graft and local flap can not cover. But, operator always must take care for flap congestion and donor site scar. We thought forehead island flap is one of the best option of reconstruction of nasal tip defect.

Genotypic Diversity of the Complete Open-Reading Frame 7 Sequences of Porcine Reproductive and Respiratory Syndrome Viruses in Korea and Coexistence of Two Genotypes

  • To investigate the genotypic diversity of the porcine reproductive and respiratory syndrome viruses (PRRSV) in Korea, we examined 92 clinical samples from three provinces by RT-PCR and a nested PCR, and the complete open-reading frame 7 (ORF 7) sequences of 15 samples selected from 72 PCR-positive specimens were analyzed. When we compared nucleotide (amino acid) sequences of 80 isolates from Korea and overseas countries, the sequences of 7 samples belonged to North American (NA)-genotype, and those of 8 samples, to European (EU)-genotype. The nucleotide (amino acid) identities between two genotypes were 63.7% (59.8%) to 65.1% (63.1%). When compared with NA prototype VR-2332, the 7 strains of NA-genotype shared 89.8% (93.6%) to 91.2% (96.0%) identity of nucleotide (amino acid) sequence. The 8 strains of EU-type shared 93.6% (92.3%) to 94.3% (93.8%) identity of nucleotide (amino acid) sequence as compared to EU prototype Lelystad. In phylogenetic tree analysis by neighbor-joining method, all of the 8 EU-type strains were clustered into group 4 distinct from ED-prototype Lelystad (group 1). In NA-genotype, 24 domestic isolates reported previously and the 7 strains of NA-type determined in this study were clustered into group 1, while US prototype VR 2332 was classified into different group (group 2). These results suggest that emergence of EU-genotype and the dual-infection of NA- and EU-genotypes may be prevalent in the pig farms in Korea. The high degree of genetic diversity of field PRRSVs should be taken into consideration for control and preventive measures.

Clinical Experiences with Foreign Body Granuloma of the Nose: 7-Year Experience with 18 Patients (비부 이물 육아종의 임상적 경험)

  • Purpose: The injection of various materials, including medical fillers and unregulated products, is widespread, potentially causing the development of foreign body granulomas. Should this occur on the nose, the contour deformity and inflammatory signs that result from these granulomas are aesthetically undesirable to patients. The purpose of our study is to share our experiences using different surgical approaches, depending on the affected portion of the nose, to optimize management of this challenging problem and to evaluate patient's satisfaction using our in-house questionnaire along with degree of improvement by two independent plastic surgeons. Methods: We treated 18 patients who underwent surgical excision of nasal foreign body granulomas via a perilesional approach to the lesion (n=12) or by transcolumellar incision (n=6) at our hospital over a period of seven years from March 2003 to October 2010. Nonparametric statistics were used and are presented as medians (25th-75th). Patient satisfaction was evaluated on a scale of 1 to 5 using an in-house questionnaire. All pre-and post-operative photographs were analyzed by two independent plastic surgeons. Post-operative outcomes were evaluated based on the surgeons' consensus ratings. Results: All patients receiving the transcolumellar approach reported a high level of satisfaction with the results. All but two patients who received the perilesional approach were satisfied with the outcome. No outcomes were rated as no change or worse by the consensus ratings. Conclusion: For the upper two-thirds of the nose, perilesional surgical excision can lead to substantial patient satisfaction with modified contour deformity and infection control. The transcolumellar approach resulted in better outcomes and patient satisfaction for the lower one-third of the nose.

The Effect of e-PTFE Membrane Exposure on the Initial Healing of Periodontal Tissue in GTR Procedure (e-PTFE 차단막을 이용한 조직유도재생술시 e-PTFE 차단막의 노출이 치주조직의 초기치유에 미치는 영향)

  • The aim of the present study was to evaluate the effect of the expanded polytetrafluoroethylene (e-PTFE) membrane exposure on the initial healing of the periodontal tissue in guided tissue regeneration (GTR) procedure. 90 sites selected from 90 patients were treated with gingival flap surgery supported by an e-PTFE membrane. The material included angular bony defects with probing attachment loss of > 5mm or degree II furcation involvement. Treated sites were classified with membrane exposure group and non-exposure group at membrane removal and evaluated healing type. The results were obtained as follows. 1. e-PTFE membrane was exposed at 61 sites (67.8%) among 90 sites. 2. Thirteen sites (14.4%) depicted rapid healing type, 65 sites (72.2%) depicted typical healing type, 9 sites (10%) showed delayed healing type and 3 sites (3.3%) were categorized as adversed healing type. 3. In e-PTFE membrane exposure group, 1 site (1.6%), 51 sites (83.6%), 6 sites (9.8%) and 3 sites (4.9%) showed rapid healing type, typical healing type, delayed healing type and adverse healing type respectively. 4. In e-PTFE membrane non-exposure group, 12 sites (41.3%), 14 sites (48.3%) and 3 sites (10.3%) showed rapid healing type, typical healing type and delayed healing type respectively. Adverse healing type was not observed. 5. The rate of favourable healing between e-PTFE membrane exposure group and non-exposure group was not statistically significant(p=0.56). These results suggest that the prevention of membrane exposure may be important to obtain rapid healing type. However favourable healing could be obtained with stringent infection control program even if membrane was exposed.

  • The purpose is to determine the degree of contamination of the equipment for infection control in chest radiography of the radiology department. We confirmed by chemical and bacterial identification of bacteria of the equipment and established a preventive maintenance plan. Chest X-ray radiography contact area on the instrument patients shoulder, hand, chin, chest lateral radiography patient contact areas with a 70% isopropyl alcohol cotton swab were compared to identify the bacteria before and after sterilization on the patient contact area in the chest radiography equipment of the department. The gram positive Staphylococcus was isolated from side shoots handle before disinfection in the chest radiography equipment. For the final identification of antibiotic tested that it was determined by performing the nobobiocin to the sensitive Staphylococcus epidermidis. Chest radiography equipment before disinfecting the handle side of Staphylococcus epidermidis bacteria were detected using a disinfectant should be to prevent hospital infections.

Job Analysis of the Nursing Unit Managers of Women's Hospital Using DACUM Analysis (DACUM 직무분석 기법을 이용한 여성병원 간호단위관리자의 직무분석)

  • Purpose: To analyze the job of nursing unit managers working at women's hospital, using DACUM (developing a curriculum), DACUM is a method for analyzing job-focused competency. Methods: This study involved a descriptive survey. A DACUM workshop was held to define women's hospital nursing unit managers' role and identify their duties and tasks. For the workshop, a committee was formed consisting of 5 women's hospital nursing unit managers. Finally, after validation, the developed contents were made into a survey asking about nursing unit manager's duties and tasks. Results: Sixteen duties and 83 tasks were identified on the DACUM chart. The importance, difficulty, and frequency of the tasks were ranked in terms of A, B, and C, with A being the highest degree. Eight tasks received A's all in importance, difficulty, and frequency of performance. The 8 tasks were: 'taking over', 'taking care of seriously ill patients on handover', 'ward rounding', 'analyzing and resolving demands identified during handover and patient tour', 'reporting patient status during rounding', 'promoting breast-feeding', 'uterine contraction, and training for breast-feeding'. The duty with the biggest determinant coefficient (DC) was 'patients complaint management' (DC=7.09). Based on tasks, the one with the biggest DC was 'solving patient and patient guardian's complaints' (DC=7.53), followed by 'making infection control guidelines' (DC=7.5). Conclusion: When expanding the nursing staff of the hospital, women's hospitals nursing unit managers also need to use administrative functions as intermediaries to focus on the operation management of the entire hospital rather than direct nursing to suit their role.
