기능적 평가 측정을 통하여 전신냉기치료와 척추 감압기 적용이 요추 추간판 탈출증 환자에게 미치는 효과 (Effect of whole body cryotherapy with spinal decompression on lumbar disc herniation by functional assessment measures)

  • 본 연구는 요추 추간판 탈출증 환자 30명을 대상으로 전신냉기치료와 감압치료를 병행한 그룹과 감압치료를 적용한 그룹으로 나누어 2주간 실시한 후 통증과 요추 굴곡 관절가동범위, 그리고 전반적 인지효과에 미치는 효과에 대해 알아보기 위해 실시하였다. 본 연구의 측정은 시각적 상사 척도 검사와 수정판 쇼버 검사, 그리고 전반적 인지효과척도를 검사하였다. 대응표본 T검사에서 두 그룹 모두 효과적이었다. 그러나 독립표본 T검정과 공분산분석 결과 전신냉기치료와 감압치료를 병행한 그룹에서 더욱 효과적이었다. 요추 추간판 탈출증의 통증과 관절가동범위, 그리고 전반적 인지효과를 개선시키기 위해 전신냉기치료와 감압치료 병용 시 효과적이란 것을 검증하였다.

The effectiveness of decompression for patients with dentigerous cysts, keratocystic odontogenic tumors, and unicystic ameloblastoma

  • Objectives: The aim of this study was to verify the clinical effectiveness of decompression in decreasing the size of a cyst. In addition to the different types of cysts, we tried to reveal what effect host factors, such as the initial size of the lesion and the age of the patient, have on the velocity of cyst shrinkage. Materials and Methods: With the aid of a panoramic view, we measured the size of the cysts before and after decompression in 13 dentigerous cysts (DCs), 14 keratocystic odontogenic tumors (KTOCs), and 5 unicystic ameloblastoma (UA) cases. The velocity of shrinkage in the three cystic groups was calculated. Relationships between the age of the patient, the initial size of the cyst, and the shrinkage velocity were investigated. Results: The three types of cysts showed no inter-type differences in their velocity of shrinkage. However, there was a statistically meaningful relationship between the initial size of the lesion and the absolute velocity of shrinkage in the DC group (P=0.02, R=0.65) and the KTOC group (P=0.02, R=0.56). There was also a significant relationship between the age of the patient and the absolute velocity of shrinkage in the KTOC group (P=0.04, R=0.45) and the UA group (P=0.04, R=0.46). Conclusion: There was no difference in the decrease in size due to decompression among the different types of cysts. However, the age of the patient and the initial size of the lesion showed a significant relationship with the velocity of shrinkage.

Minimally Invasive Surgery without Decompression for Hepatocellular Carcinoma Spinal Metastasis with Epidural Spinal Cord Compression Grade 2

  • Objective : There is a lack of knowledge regarding whether decompression is necessary in treating patients with epidural spinal cord compression (ESCC) grade 2. The purpose of this study was to compare the outcomes of minimally invasive surgery (MIS) without decompression and conventional open surgery (palliative laminectomy) for patients with hepatocellular carcinoma (HCC) spinal metastasis of ESCC grade 2. Methods : Patients with HCC spinal metastasis requiring surgery were retrospectively reviewed. Patients with ESCC grade 2, medically intractable mechanical back pain, a Nurick grade better than 3, 3-6 months of life expectancy, Tomita score ${\geq}5$, and Spinal Instability Neoplastic Score ${\geq}7$ were included. Patients with neurological deficits, other systemic illnesses and less than 1 month of life expectancy were excluded. Thirty patients were included in the study, including 17 in the open surgery group (until 2008) and 13 in the MIS group (since 2009). Results : The MIS group had a significantly shorter operative time ($94.2{\pm}48.2minutes$ vs. $162.9{\pm}52.3minutes$, p=0.001), less blood loss ($140.0{\pm}182.9mL$ vs. $1534.4{\pm}1484.2mL$, p=0.002), and less post-operative intensive care unit transfer (one patient vs. eight patients, p=0.042) than the open surgery group. The visual analogue scale for back pain at 3 months post-operation was significantly improved in the MIS group than in the open surgery group ($3.0{\pm}1.2$ vs. $4.3{\pm}1.2$, p=0.042). The MIS group had longer ambulation time ($183{\pm}33days$ vs. $166{\pm}36days$) and survival time ($216{\pm}38days$ vs. $204{\pm}43days$) than the open surgery group without significant difference (p=0.814 and 0.959, respectively). Conclusion : MIS without decompression would be a good choice for patients with HCC spinal metastasis of ESCC grade 2, especially those with limited prognosis, mechanical instability and no neurologic deficit.

  • In this paper, we describe implementation of efficient wavelet image compression and decompression system for DVR(Digital Video Recoder). We used various methods to remove time redundancy, spatial redundancy and statistical redundancy of video camera inputs. Motion detection, wavelet transform, RLC(Run Length Coding) and huffman coding techniques are combined for efficient compression / decompression.

한글 한자 비트 맵 폰트의 압축과 복원에 관한연구 (A study on compression and decompression of hanguel and chinese character bit map font)

  • In this paper, a variable length block code for real time compression and decompression of hanguel and chinese character bit map font is proposed. The proposed code shows a good compression ratio in complete form of hangeul myoungjo and godik style and chinese batang and doddum style bit map font. Besides, a compression and decompression ASIC is designed and simulated on CAD. The 0.8 micron CMOS sea of gate is used to implement the ASIC in amount of 5,200 gates, and it runs at simple hardware and compress and decompress at 33M bit/sec at maximum, which is ideal for real time applications.

  • Lymphedema is the most common of complications after surgery and radiotherapy in patients with breast cancer or cervical cancer. The cancer itself is a worry, but the lymphedema ia an additional handicap for the patients, both physical and psychosoical. Recently the best treatment of lymphedema is complete lymphedema therapy, and Dr. Foldi developed in 1980s. But this treatment has not always given satisfactory and permanent results. We report a case of the lymphedema patient who was treated by partial decompression therapy.

Approach for Epiduroscopic Laser Neural Decompression in Case of the Sacral Canal Stenosis

  • Epiduroscopy is very useful in the treatment of not only low back pain caused by failed back surgery syndrome, epidural scar or herniated disc but also by chronic refractory low back pain which does not respond to interventional conservative treatment including fluoroscopically-directed epidural steroid injections and percutaneous adhesiolysis. Because cauterization using a laser fiber has become recently available, a wider opening is required to enter into the sacral canal in the case of epiduroscopic laser neural decompression (ELND). However, in a few patients, it is difficult to insert a device into the epidural space due to stenosis around the opening, and there is no alternative method. Herein, we report a case where a hiatus rasp specially designed for such patients was used to perform the operation.

흉요추 추간원반탈출증 권에서의 편측 추궁절제술 (Lateral Laminectomy in Thoracolumbar Disc Herniated Dogs)

  • Lateral laminectomy was performed far spinal decompression in 7 thoracolumbar disc herniated dogs. These dogs showed upper motor neurologic signs including kyphosisi urinary dysfunction, and paraplegia or paralysis in hindlimbs. The lesions were evaluated with myelographic findings and predominated in $T_{12}-T_{13}, T_{13}-L_{1} and L_{1}-L_{2}$ discs. Five dogs which were operated within 48 hours after the onset of paraplegia were recovered from kyphosis and sustained the normal gait and walkings furthermore normal urination and defecation were presented within 3 days of postoperation. One dog was expired with steroid induced intestinal bleeding. The other dog operated on 96 hours after the onset of paraplegia was not recovered from neurologic deficits. Spinal decompression technique was considered to be useful method fur improving neurological problems resulted firom thoracolumbar disc herniation, if dogs are operated on early stage of the onset of paraplegia.

  • Transmission of 3D shape model through Internet has become one of the hottest issues in these days. Presented in this paper is a new approach for the rapid transmission of the geometry data of the shape model. By analyzing the important three factors, the shape fidelity, the file size, and the decompression time, for the compression, we point out the potential problems of previous approaches of using the deltas between consecutive vertices and propose an alternative of directly using the position values of vertices of the model. It turns out that the proposed approach has smaller file size, has lesser distortion in the model, and the decompression is faster.

  • Epiduroscopic laser discectomy and neural decompression (ELND) is known as an effective treatment for intractable lumbar pain and radiating pain which develop after lumbar surgery, as well as for herniation of the intervertebral disk and spinal stenosis. However, various complications occur due to the invasiveness of this procedure and epidural adhesion, and rarely, cranial nerve damage can occur due to increased intracranial pressure. Here, the authors report case in which double vision occurred after epiduroscopic laser discectomy and neural decompression in a patient with failed back surgery syndrome (FBSS).
