초등학생들의 체중신장지수(WLI)와 치아우식증에 관한 연구 (A Study on the Weight Length Index and Dental Caries of Elementary School Students)

  • In Korea, changes in children's diet patterns accelerate their physical growth and development: frequent snacking has been presumed to be a major cause of increasing dental caries. The present study attempts to clarify the relationship between the physical development of growing children and their tooth decay. For this purpose, 632 six-grade children in 4 elementary school located in Urban(Seoul) and Rural(Po-gok, Yang-In) were classified into three groups based on the Weight Length Index(WLI), known to reflect the nutritional conditions of school-age children, and the relationship was analyzed between each group and the variables considered to be related with dental caries. The result is as follows: The average weight and height of the male is $44.88{\pm}10.89$ kg, $148.49{\pm}7.33$ cm and female is $43.35{\pm}9.60$ kg, $149.23{\pm}6.73$ cm, respectively, which are in the similar level with the Korean Physical Standard. The classification of the children by the WLI reveals a relatively high distribution of over-weighted child ren - 212 persons, 335% of the entire population. The DMFT Index was a little high in the rural area(3.15 teeth in urban and 3.31, in rural). Among the groups of children classified by the WLI, the over-weight group have the highest DMFT index(3.69 teeth). The relationship between the frequency of taking in basic nutrients and the DMFT index is also found: the relationship is not evident in case of the foods containing rich calcium, protein, as well as fruits and vegetables. But, in the protein-rich food, higher frequency of its intake means significantly lower DMFT index in the normal-weight group of the urban children. In case of carbohydrate, higher frequency of its intake means significantly higher DMFT index in all the groups of the rural children. The DMFT index has some correlations with the relevant variables: the index has a positive correlation with the frequency of snacking, and a negative correlation with the economic status. That is, the higher the frequency of snacking is, and the lower the economic status is, the higher the DMFT index may be. In the logistic multiple regression analysis conducted with the presence of DMFT as a dependent variable, only the frequency of tooth brushing is turned to be a variable affecting the presence of either decayed, missing, or filled teeth. Based on the above result, the variables affecting the DMFT index are a time spent on eating, frequency of intake of protein and carbohydrate for a week, frequency of snacking, regular dental check-ups, preventive behaviors for oral health(fluoride gargling, tooth brusing after each meal, proper tooth brushing method). These variables have a relationship with the DMFT index, but the degree is somewhat different between the groups classified either by the region or by the WLI. Therefore, appropriate nutrition management should be conducted according to the individual's nutritional conditions when the services like nutritional education are provided based on the closely-examined characteristics of each target group. And, at the same time, oral health education should be strengthened, and its importance should also be emphasized so that people can pay attention to their own oral health.

영역별 특성에 따른 구취 자각정도와 인지에 관한 조사 연구 (A study on characteristics to territorial among awareness of halitosis)

  • The purpose of this study was to examine the awareness of people in general about halitosis. The subjects in this study were 184 people who visited the clinical practice lab at J health college to get their teeth scaled. After a survey was conducted from May 1 to June 3, 2008, the analyzable answer sheets from 178 respondents were analyzed after four different areas were selected, which included smoking/nonsmoking, scaling experience, toothbrushing frequency and the use of oral hygiene supplies. SPSSWIN 12.0 program was utilized to make a frequency analysis and crosstabs analysis. The findings of the study were as follows: 1. Concerning subjective feelings of halitosis, 55.3 percent(99 people) of the respondents found themselves to have a moderate level of bad breath. 28.5 percent(51) deemed themselves to have a little foul breath, and 14 percent(25) didn't feel they had any bad breath. 2. As to the subjective level of halitosis, 89.8 percent(168) thought that their bad breath was a little perceived only by themselves, regardless of smoking, scaling experience, toothbrushing frequency and the use of oral hygiene supplies. 3. In regard to the cause of halitosis, 31 percent(56) cited plaque in the mouth as the cause, and 28.5 percent(51) pointed out the other causes that weren't mentioned in the questionnaire. 18.4 percent(33) cited decayed tooth, and 11.2 percent(20) pointed out gastroenteric disorder. 10.6 percent(19) viewed diabetes as the cause. 4. As to the time when they had the subjective symptom of halitosis, 114 respondents(63.7%) felt their own bad breath the most immediately after they got up 21.8 percent(39 respondents) did it when they were hungry 5.5 percent(9) did that before breakfast, and 4.5 percent(8) did that after having breakfast. 5. Regarding view of how to prevent halitosis, 52.5 percent(94) brushed their teeth frequently 21.2 percent(38) got their teeth scaled on a regular basis at a dentist's office, and 17.9 percent(32) drank water often. The above-mentioned finding seemed to suggest that the respondents weren't well aware of the fact halitosis was a sort of oral and systemic disease. Therefore the development of halitosis prevention and care programs geared toward practice lab visions were required.

치아 우식증에 따른 치수내 유전자 발현 변화에 관한 분석 (GENE EXPRESSION ANALYSIS OF THE DENTAL PULP IN HEALTHY AND CARIES TEETH)

  • 치아 우식증은 범발성 질환이고 이에 대한 생체반응은 단순하지 않으며 질병 과정과 숙주의 활성 모두를 반영하는 복합적인 반응이다. 이러한 반응을 이해하기 위해서는 질병의 세포학적, 분자학적인 면을 이해하는 것이 필수적이다. 이에 본 연구는 임상적으로 건강한 치아와 우식이 진행된 치아로부터 얻어진 치수 안의 유전자 발현을 규명하고 우식 병소에서 일어나는 치유 및 재생에 관계되는 분자와 면역 세포들 사이의 분자 생화학적 상호작용을 규명하기 위해서 우식치아와 건전치아의 치수를 이용하여 cDNA 미세배열(microarray) 분석과 역전사효소 중합효소 연쇄반응 (RT-PCR) 분석, 그리고 면역화학염색법 (immunohistochemistry)을 시행하여 다음과 같은 결론을 얻었다. 1. cDNA 미세배열 분석 결과, 건전치아군인 대조군에서는 143개의 유전자가, 우식치아군인 실험군에서는 377개의 유전자가 1.6배이상 발현되었다. 2. 역전사효소 중합효소 연쇄반응 분석에서 14개의 유전자를 선택하였고 cDNA 미세배열 분석결과와 동일한 결과를 확인하였다. 3. TGF-${\beta}1$의 면역조직화학적 관찰 결과, 건전치에 비해 우식치의 상아모세포와 치수에서 특히 강하게 발현되었다.

장애학생과 일반학생의 구강건강상태 조사연구 (A Study of Oral Health Status in Handicapped Students Comparing with Normal Students)

  • 본 연구는 신체적, 정신적으로 장애가 있는 장애학생의 구강 건강상태를 조사하였고 대조군으로는 일반학교에 다니는 일반학생을 대상으로 하였다. 6개의 장애학교의 학생들과 4개의 일반학교 학생들을 조사하였다. 조사기간은 1999년 8월부터 1999년 11월까지이며, 장애학생 597명과 일반학생 731명의 구강건강상태를 조사 평가해 보았다. 1. 장애학생과 일반학생의 구강건강상태는 우식영구치는 비슷한 수준이었고, 상실영구치는 일반학생($0.11{\pm}0.54$개)보다 장애학생($0.41{\pm}1.16$개)에서 높게 나타났으며(p<0.01), 치주질환도 일반학생(10.9%)보다 장애학생(29.1%)이, 부정교합에서도 일반학생(7.4%)보다 장애학생(25.6%)에서 더 높게 나타났다(p<0.01). 2. 장애유형에 따른 구강건강상태는 우식영구치에서 $2.24{\pm}2.14$개로 정신지체장애학생이 가장 높게 나타났으며, 감강기능장애의 학생은 $1.75{\pm}2.10$개로 가장 낮게 나타났다(p<0.05). 상실영구치는 장애유형에 따른 차이는 없었으나, 치주질환에서는 정신지체장애학생이 63.8%로 가장 높게 나타났다. 또한 부정교합에서도 정신지체장애학생이 59.5%로 지체장애학생(9.2%)이나 감각기증장애학생(31.3%)보다 높게 나타났다(p<0.01). 3. 정기적인 치아관리를 하고 있는 학교의 학생에 비해 그렇지 못한 학교의 학생이 치주질환과 부정교합이 많았으나, 우식영구치와 상실영구치는 비슷한 수준이었다. 집단숙식을 하는 학교의 학생과 그렇지 않은 학생의 학생에 대한 우식영구치, 상실영구치, 치주질환, 부정교합에서는 모두 비슷한 수준이었다.

김제시 일부지역 초등학교 학교계속구강건강관리사업의 효과 (Effectiveness of Incremental School Oral Health Program at Primary School in Some Regions of Gimje)

  • 본 연구는 김제시 B면 K면에 소재하고 있는 초등학교 중에서 학교구강보건실이 설치된 1개교(실험군), 학교구강보건실이 미설치된 3개교(대조군)를 표본학교로 선정하여 김제시 K보건지소의 학교계속구강건강관리사업이 중지되기 전 사업의 혜택을 받은 3~6학년, 사업이 중지된 후 혜택을 받지 못한 1~2학년을 비교대상으로 구강상태를 평가하여 다음과 같은 결과를 얻었다. 영구치우식경험자율은 전체 실험군 50.6%, 대조군 66.8%로 유의한 차이를 나타냈으며 (p<0.01), 사업혜택을 받은 3~6학년에서 실험군 58.9%, 대조군 76.1%로 나타나 유의한 차이를 나타냈다(p<0.05). 우식경험영구치율은 전체 실험군 34.8%, 대조군 56.5%로 나타나 유의한 차이를 보였고(p<0.001), 사업혜택을 받은 3~6학년 실험군 41.1%, 대조군 64.2%로 나타나 유의한 차이를 보였다(p<0.01). 우식경험영구치지수는 전체 실험군 1.35개, 대조군 2.97개로 유의한 차이를 보였고(p<0.001), 사업혜택을 받은 3~6학년 실험군 1.73개 대조군 3.66개로 나타나 역시 유의한 차이를 나타냈다(p<0.001). 우식영구치지수는 전체 실험군 0.72개, 대조군 1.96개로 나타나 유의한 차이를 보였고(p<0.001), 충전영구치 지수는 실험군 0.63개, 대조군 0.99개로 나타나 유의한 차이를 보였다(p<0.05). 사업중지 1~2학년 실험군에서 우식영구치지수 0.42개, 대조군 1.12개로 나타나 유의한 차이를 보였고(p<0.05), 사업혜택을 받은 3~6학년 실험군에서 우식영구치지수 0.89개 대조군 2.34개로 나타나 역시 유의한 차이를 보였다(p<0.01) 우식영구치율은 전체 실험군 57.23%, 대조군 64.16%, 충전영구치율은 실험군 42.77%, 대조군 34.81%로 나타났고 사업중지 1~2학년 우식영구치율은 실험군 63.89%, 대조군 76.17%로 나타났으며, 사업혜택을 받은 3~6학년의 우식영구치율은 실험군 54.81%, 대조군 60.98%로 나타났다. 이상의 결과를 토대로 초등학교 구강보건실에서 실시하는 학생계속구강건강관리사업의 효과를 확인하였으며, 학생들의 구강건강증진을 위해 계속적인 유지와 활성화를 위한 노력이 필요할 것으로 생각된다.

일부 농촌지역 60세 이상 노인의 생활습관병이 구강건강에 미치는 영향 (Influence of Lifestyle-Related Diseases on the Oral Health of the Rural Elderly in Korea)

  • 당뇨병, 고혈압, 비만, 고콜레스테롤혈증이 노인의 구강건강상태에 미치는 영향을 파악하고 구강질환의 예방 및 치료의 기초자료를 마련하고자 2006년과 2007년에 성주군에 소재한 보건소에서 시행하는 건강검진을 받은 만 60세 이상의 노인을 대상으로 설문조사와 구강검사를 실시하고 이 대상자 중 2000년과 2001년에 조사되어 6년간 추적 관찰된 399명을 대상으로 분석하였다. 당뇨병, 고혈압, 비만과 고콜레스테롤혈증 유병기간에 따라서 당뇨병은 우식치아 수와 우식경험 영구치아 수, 고혈압은 결손치아 수와 우식경험 영구치아 수, 비만과 고콜레스테롤혈증은 결손치아수와 우식경험영구치아 수가 증가하는 것으로 나타났고 (p<0.05), 두 가지 이상 질병 보유군에서 결손치아 수와 우식경험영구치아 수가 유의하게 많은 것으로 나타났다(p<0.01). 단순회귀분석 결과 당뇨병의 유병기간이 길수록 우식치아 수가 증가하고, 고혈압과 비만의 유병기간이 길수록 결손치아 수가 증가하는 것으로 나타났으며, 4가지 질병의 유병기간이 길수록 우식경험영구치아 수도 증가하는 것으로 나타났다. 다변량 회귀분석 결과 각 질병의 유병기간이 길수록 결손치아 수가 증가하는 것으로 나타났고, 당뇨병, 고혈압, 비만 등 각 질병의 유병기간이 길수록 그리고 보유질병 수가 많을수록 우식경험 영구치아 수가 증가하는 것으로 나타났다. 이와 같이 당뇨병, 고혈압, 비만, 고콜레스테롤 혈증 환자가 구강질환 발생위험이 상당히 높게 나타났다. 따라서 이들 생활습관병의 예방 및 관리는 구강건강상태에 크게 기여할 것이다.

전신 마취 하 장애인 치과치료에 대한 임상적 고찰 (DENTAL TREATMENT UNDER GENERAL ANESTHESIA: AN OVERVIEW OF CLINICAL CHARACTERISTICS OF SPECIAL NEEDS PATIENTS)

  • The aim of this study was to analyze the clinical characteristics of patients with cognitive and behavioral impairments receiving dental treatment under general anesthesia (GA-dental treatment). From August 2007 to April 2014, information was collected from 475 patients who received GA-dental treatment at the Clinic for Persons with Disabilities, Seoul National University Dental Hospital. The demographic factors (gender, age, disability, medication, GA history, residency type, caregiver, meal type, oral hygiene maintenance, and cooperation level) and dental status (operating duration, DMFT, malocclusion, periodontal disease, tooth defect, and treatment protocol) of the patients were evaluated. DMFT and malocclusion levels were compared among the patients with ANOVA and Sheffe's post-hoc test, and chi-square test, respectively. The correlation between the demographic characteristics and dental status of the patients were analyzed with the Pearson's correction test. The mean age of the patients was 27.1 (7 - 83) years and they had intellectual disabilities (55.4%), developmental disorders (17.9%), brain disorders (16.6%), neurocognitive disorders (4.6%), or others (5.5%). The mean DMFT (DT) was 8.6 (5.2) with a significant difference among the disability types (p<0.05). The incidence of malocclusion was higher in patients with intellectual disabilities and brain disorders than in the other types (p<0.05). The operation time ($191.4{\pm}91.2min$) was correlated with decayed or endodontically-treated teeth (p<0.05). Special needs patients requiring GA-dental treatment showed unfavorable oral conditions. Dental practitioners experience time restrictions and additional costs under a GA setting. Treatment planning and decision-making can be efficiently facilitated by evaluating the clinical characteristics of the patients.

Effect of Allergic Rhinitis and Asthma on Oral Health

  • Background: This study aimed to investigate allergic rhinitis and the relationship between allergic diseases, such as asthma and allergic rhinitis, and dental caries and periodontal disease using the raw data from third year of the 6th Korean National Health and Nutrition Examination Survey. Methods: A total of 3,729 subjects aged over 30 years who underwent examination for allergic diseases and an oral health checkup were selected. The data were analyzed using SPSS IBM SPSS ver. 25.0 (IBM Corp., USA). Composite sample cross correlation and composite sample logistic regression analyses were performed using the composite sample general linear model. Results: On examining the socioeconomic characteristics of the subjects suffering from allergic disease and the relationship between allergic disease and oral health, allergic rhinitis was more frequently found in younger subjects with a higher level of education. Periodontal disease and dental caries were more frequent among female, older age groups, lower income earners, and subjects with a lower level of education (p<0.05). On examining the relationship between the oral health characteristics of the subject and allergic diseases and oral health, allergic rhinitis was more common in subjects with a good oral health status perception than those who answered "bad" to the oral health status question. The community periodontal index of treatment needs (CPITN) score was higher in subjects who answered "poor" to the oral health status question, lower frequency of brushing, and higher in subjects using secondary oral hygiene products; the decayed, missing and filled teeth (DMFT) index was higher in subjects with a perception of poor oral health status (p<0.05). The DMFT index was high in the asthma group, and the CPITN score was high in the group who answered "no" to allergic rhinitis. Conclusion: There is a relationship between asthma and allergic rhinitis and the DFMT index and CPITN score. Corresponding oral programs for allergic patients need to be developed.

급성괴사성궤양성 치은염을 닮은 재발성 허피스 구내염 (Recurrent Herpes-Stomatitis Mimicking Acute Necrotizing Ulcerative Gingivitis)

  • Herpes simplex is caused by viruses of the herpesvirus hominus family. HSV have four categories: type 1, 2, 6, and 8. Generally HSV-1 affects the mouth. Once infected by HSV, the person's infection is permanent. Retrograde transport through adjacent neural tissue to sensory ganglia leads to a lifelong latent infection. Recently, we treated a patient with recurrent herpes-stomatitis mimicking acute necrotizing ulcerative gingivitis (ANUG). The results were satisfactorty so we report this case. 31 years old male patient showed sore throat, gingival ulceration, palpable both submandibular lymph node, and sulcular pus formation around posterior decayed teeth. This is the third time he has suffered from this symptom. Tentative diagnosis was acute necrotizing ulcerative gingivitis. Antibiotic therapy was started. But, intraoral symptom got worse in process of time. Especially ulcer of marginal gingiva got worse. Viral disease was suspected. We carried out viral cultivation. At the same time topical application of antiviral ointment (herpecid$^{(R)}$) was performed on oral ulcer unilaterally for the purpose of diagnosis and reducing pain experimentally. The next day pain was decreased dramatically on application area. Basing on the viral cultivation and clinical effect of antiviral ointment (herpecid$^{(R)}$), we have diagnosed it as a recurrent herpes-stomatitis and concluded that viral infection was major cause of disease and bacterial infection was secondary.

Socioeconomic Inequalities in the Oral Health of People Aged 15-40 Years in Kurdistan, Iran in 2015: A Cross-sectional Study

  • Objectives: The aim of this study was to evaluate socioeconomic inequalities in the prevalence of dental caries among an urban population. Methods: This study was conducted among 2000 people 15-40 years of age living in Kurdistan, Iran in 2015. Using a questionnaire, data were collected by 4 trained dental students. The dependent variable was the decayed, missing, and filled teeth (DMF) index. Using principal component analysis, the socioeconomic status (SES) of families was determined based on their household assets. Inequality was measured using the concentration index; in addition, the Oaxaca analytical method was used to determine the contribution of various determinants to the observed inequality. Results: The concentration index for poor scores on the DMF index was -0.32 (95% confidence interval [CI], -0.40 to -0.36); thus, poor DMF indices had a greater concentration in groups with a low SES (p<0.001). Decomposition analysis showed that the mean prevalence of a poor DMF index was 43.7% (95% CI, 40.4 to 46.9%) in the least privileged group and 14.4% (95% CI, 9.5 to 9.2%) in the most privileged group. It was found that 85.8% of the gap observed between these groups was due to differences in sex, parents' education, and the district of residence. A poor DMF index was less prevalent among people with higher SES than among those with lower SES (odds ratio, 0.31; 95% CI, 0.19 to 0.52). Conclusions: An alarming degree of SES inequality in oral health status was found in the studied community. Hence, it is suggested that inequalities in oral health status be reduced via adopting appropriate policies such as the delivery of oral health services to poorer groups and covering such services in insurance programs.
