Postoperative Follow up Study in ECD (심내막상 결손증 술후 추적 관찰에 관한 연구)

  • Endocardial cushion defect is a rare congenital heart disease. From September 1985, we experienced 20 cases of endocardial cushion defects and postoperative follow up was taken.We reviewed preoperative clinical data, echocardiography, cardiac cath data, operative method & time. After operation, we performed echocardiography and examined mitral valve function & integrity of patch closure sites. Postoperative follow up was taken regularly & follow up period was from 2 month to 91 months. Several postoperative complications were overcome with adequate treatment. There were 3 cases of early death and mortality rate was 15% [3/20].

Surgical management of spontaneous pneumothorax (자연기흉의 외과적 치료 -310례의 임상고찰-)

  • Total 310 cases of spontaneous pneumothorax in 281 patients were analyzed to review the results of surgical treatment for this condition. Clinical data on the age & sex distribution, recurrence, etiologic conditions and on the other aspects of spontaneous neumothorax were summarized. The results of surgical management of spontaneous pneumothorax are followings; 205 out of 310 cases[66.1%] were cured by closed thoracostomies. 82 cases[26.5%] were cured by thoracotomy. The indications of thoracotomy were 1] persistent air leakage, 2] history of recurrences, 3] blebs or bullae on thoracoscopy, 4] associated parenchymal lesion, 5] pneumothorax caused by paragonimiasis. Other reasons of thoracotomy were bilateral pneumothorax and inadequate expansion due to chronicity. Excision of blebs or wedge resection was performed in most cases with good result. Lobectomy [9 case] or pneumonectomy [3 cases] was carried out depending on the pathological involvement of the lung. There was no operative death and only one case showed recurrent pneumothorax during follow-up after thoracotomy.

Pulmonary Aspergillosis - Pulmonary Aspergillosis - (폐 Aspergillosis -6예 보고-)

  • Pulmonary aspergillosis is being recognized with increasing frequency in recent years and the-rising incidence of this infection parallels certain medical advances in antibiotics, chemotherapeutic and immunosuppressive therapy. The cavities of lungs resulting from tuberculosis, histoplasmosis or neoplasm are apt, to be infected by one of the species of the genus Aspergillus and eventually mycetomas are formed within the cavities. Authors have experienced 6 cases of pulmonary aspergillosis forming mycetoma in Dept. of Thoracic Surgery, Catholic Medical Center from Aug. 1976 to Feb. 1979. Hemoptysis or blood tinged sputum, the predominant symptom, occurred in all cases. All patients underwent pulmonary resection, 1 pneumonectomy, 3 lobectomies, 1 lobectomy with segmental resection and 1 segmental resection and survived well without death or complication. Primary aspergillosis was in 2 cases and underlying diseases were present in 4 cases: 3 pulmonary tuberculosis, 1 bronchiectasis. The common diagnostic study of intracavitary mycetoma was the posterioanterior chest roentgenogram; in cavities suspected of being diseased or in doubtful cases, tomography was most available to find fungus ball with air-meniscus shadow.

A Clinical Evaluation of 717 Chest Injuries (흉부외상 717례의 임상적 고찰)

  • A Clinical analysis was performed on 717 cases of the chest trauma experienced at department of thoracic and cardiovascular surgery, Soonchunhyang Chunan Hospital from Mar. 1985 to Jun. 1989. 1. The sex ratio was 2.89:1 with male predominance. The patients in 75 % of total cases was between 3rd to 5th decade. 2. The most common causes were traffic accident in non-penetrating thoracic injury and stab wound by knife in penetrating injury. 3. The hemothorax, pneumothorax and hemopneumothorax were observed in 306 cases[42.7 %]. 4. The left thorax was the preferred site of penetrating and non-penetrating thoracic injury. The rib fracture was prevalent between 4th to 8th rib. 5. The open thoracotomy was performed in 55 cases[7.7 %] 6. The overall mortality was 3.07 %[22 cases] and causes of death were hypovolemic shock, adult respiratory distress syndrome, sepsis, disseminated intravascular coagulation, renal failure and pneumonia.

Traumatic Diaphragmatic Injuries (Report of 22 Cases) (외상성 횡경막 손상 -22례 보고-)

  • 22 Cases of traumatic diaphragmatic injuries treated at the Department of Thoracic and Cardiovascular Surgery in Chon-Buk National University Hospital from Jan. 1979 to Oct. 1988 were reviewed in this study. Of the 22 cases, 18 were male and 3 were female, a ratio of 4.5:1. This ratio revealed high incidence in male patient. The age distribution ranged from 2 to 60 years and mean age was 31 years. The modes of injury were as follows: 11 stab wound, 5 traffic accident, 2 fall down, 2 fighting injury, 1 compression wound by sand bag, and 1 slip down injury. Useful diagnostic tools were chest X-ray with or without radiopaque dye swallowing, which was the most commonly diagnostic, UGI series, and thoracoscope. Operations were performed in 22 cases, and 18 cases were through thoracotomy. The herniated organs through the ruptured diaphragm were stomach, omentum, liver, spleen, colon, and small bowel. There were associated injuries, and the most commonly associated was rib fracture. There was no postoperative death.

  • We reviewed 10 cases of traumatic diaphragmatic injuries at Soonchunhyang University Gumi Hospital from January 1990 through April 1993. seven patients were male and three patients were female. The age distribution was ranged from 25 to 79 years, predominant 4th decades occurred in male. The traumatic diaphragmatic injuries were due to blunt trauma in 9 cases (traffic accident 7 and crash injury 2) and penetrating wound in 1 case (stab wound). The common symptom were dyspnea (60%), chest pain and abdominal pain in order frequency. In the blunt trauma and crash injury, te rupture site was all located in the left(9 cases). In the penetrating wound, the rupture site was located in the right(1 case). The surgical repair of 10 cases were performed with transthoracic approach in 9 cases and thoracoabodominal approach in 1 case. The postoperative mortality was 10% (1/10). The cause of death was multiple organ failure with pulmonary edema.

The Clinical Analysis of Flail Chest (동요흉 환자의 임상적 고찰)

  • From 1980 through 1993, sixty one patients having traumatic flail chest were analysised retrospectively at the Department of Thoracic and Cardivascular Surgery, Chonbuk National University Hospital. There were 47 men and 14 women, mean age, 49.3 years, age range 4 to 82 years. The most common mode of trauma was automobile accident, common combined other organ injuries were skeletal injury [ 36 patients and neurologic injury [ 20 patients . In the mode of treatment, ventilator therapy was done in 34 cases and operative stabilization was done in 18 cases [ Kirschner or steel wire: 9 cases, Judet`s strut: 9 cases . Sixteen patients died [26 % . The main factors associated with fatal outcome were shock [ p < 0.002 , head injury [ p < 0.005 , and more than 50 years of age [ p < 0.05 . In fatal cases, 14 patients died during in ventilator therapy [ 14/34, 41 % and 2 patients died following operative stabilization of chest wall [ 2/18, 11 % .The overall cause of death was septicemia, ARDS, ARF, hypovolemic shock and hypoxic brain damage.

  • Twenty-nine cases of ventricular septal defect operated in this department during the period from 1969 to May 1975 were presented. Out of 29 cases, there were 20 cases of type II defects, 8 cases of type I, and one case of type III, and the last case was multiple defect. The anomalies associated with ventricular septal defect were eight in all; three patent ductus arteriosus, one atrial septal defect, one tricuspid insufficiency and an aortic insufficiency. Over all mortality was 17%. The causes of death were complete heart block in a case, respiratory distress due to excessive administration of morphine in one, low cardiac output in two and perforation of the aortic annulus after repair of the ventricular septal defect associated with aortic insufficiency.

Analysis of medical disputes in Jeju (제주의 의료분쟁판례분석)

  • Medical disputes can always arise in the medical environment. We aimed to decrease the medical disputes by analysis for causes and results of cases of medical disputes. The cases of medical dispute were found on the homepage of the Supreme Court based on the judgment data which was searched using the keyword 'Jeju, Medical accident' and it was described as the area related to each medical accident. There was total of 13 cases related medical disputes in Jeju. The final states of the patients were different in each causes, but death accounted for 10 cases (76.92%), comatose state for one and disability for two respectively. The cases were 2 related with an injury from a fall. The major cause was violation of medical care obligation. Physician have to learn recent medical knowledge, have competence, and explain the detailed procedures and complications before the procedures dependent on patient autonomy.
