A Case of Strongyloidiasis with Hyperinfection Syndrome (분선충의 Hyperinfection 1례)

  • A 73-year-old Korean male was admitted to Jeil hospital with clinical complaints of backache, cough, sputum, vomiting and diarrhea. He had a history of long term administration of prednisolone. At admission he was comatose and showed Efneralized pitting edema and anasarca, Laboratory data revealed leukocytosis, hlvpoproteinemia with hypoalbuminemia, sepsis, anemia and brown-colored urine. Stool examination revealed rhabditiform nematode larvae. By fecal cultivation, filariform larvae of Strongyloides were obtained and the patient was diagnosed as hyperinfection syndrome due to 5. stercoralis infection. On the 3rd day of hospitalization, albendazole treatment was started and continued for 4 days. On the 7th day of hospitalization, sputum revealed filariform larvae. Total 619 parasitic adult females, ellpelled by chemotherapy, were collected from the diarrheal stool. However, the. patient was discllarged hopelessly and died at home.

Acute Kidney Injury and Postobstructive Diuresis Caused by a 4 mm Urinary Calculus (작은 요로 결석에 의한 급성 신후성 신부전 및 폐쇄 후 이뇨 1례)

  • Urinary obstructions from ureteral calculi are one of the causes of postrenal acute kidney injury (AKI). Here we present a case of AKI caused by a 4 mm ureteral calculus with postobstructive diuresis following the spontaneous passage of the calculus. A 13-year-old girl who underwent nephrectomy for the removal of a neuroblastoma eight years previously, visited our institution because anuria had developed over the preceding five days. The serum creatinine level was elevated at 13.4 mg/dL. Radiological examinations showed the right solitary kidney with moderate hydronephrosis and a 4 mm calculus in the upper right ureter. The patient immediately underwent hemodialysis. After the ureteral calculus was passed spontaneously on day 2 of hospitalization, urinary output increased to more than 5,200 mL per day. Intravenous fluid replacement with careful monitoring of weight, intake, output, and serum and urine electrolytes was performed. On day 5 of hospitalization, the patient's condition stabilized.

  • On July 2009, 5 fly larvae were discovered inside the nose of a 76-year-old female. She was living in Cheonansi, and in a state of coma due to rupture of an aortic aneurysm. Surgery was performed on the day of admission, and the larvae were found 4 days later. By observing their posterior spiracle, the larvae were identified as Lucilia sericata. Considering the rapid development of this species, the infection was likely acquired during hospitalization. Further investigation on the hospital environment should be needed to know the origin of the infection.

  • Ureteroscopic removal of the stone is now popular for the management of the lower ureteral stone. A clinical study was performed on our 75 patients with lower ureteral stone treated with ureteroscopy. Of the 75 stone manipulations 62(83%) were immediately successful and the final success rate including spontaneous delivery of stone or fragment after the procedure was 87 percents(65 cases). Of 57 smaller calculi than 1Cm(radiographic largest diameter) 50(88%) were removed successfully. Mean duration of postoperative hospitalization was 5.6 days. There were no interrelations between the success rate and anesthetic methods. Significant complications durinf or after procedure were not identified. We conclude that ureteroscopic removal of stones under direct vision can be done safely and be the first choice of procedure for the lower ureteral stones.

A Case of Korean Medicine Treatment for a Patient with Somatic Symptom Disorder Hospitalized after a Traffic Accident

  • Objectives: The purpose of this study was to report the effect of Korean medicine treatment on a patient with somatic symptom disorder hospitalized after a traffic accident. Methods: The subject for this study was a patient diagnosed with somatic symptom disorder that worsened after a traffic accident. During hospitalization, herbal medicine, acupuncture, psychotherapy, and other treatments were performed. Clinical effect was evaluated using Heart Rate Variability (HRV), BDI (Beck Depression Inventory)-II, BAI (Beck Anxiety Inventory), Global assessment (G/A), and Numerical rating scale (NRS). Results: After 36 days of treatment, BDI-II score was reduced from 17 to 5. BAI score was reduced from 23 to 8. Overall somatic symptoms and pain showed decreases. Conclusions: Korean medicine treatment might be effective for somatic symptom disorder that is worsened after a traffic accident.

Uroperitoneum Secondary to the Rupture of the Urinary Bladder in a Thoroughbred Foal

  • A 13-day-old Thoroughbred female foal weighing 59 kg was referred to the Jeju National University Equine Hospital with clinical signs including depression, labored breathing, tachycardia, anorexia, and marked distended abdomen. Uroperitoneum secondary to a rupture of the urinary bladder was diagnosed based on the history, clinical signs, and ultrasound imaging. An emergency laparotomy for cystorrhaphy under inhalation anesthesia was performed, and the patient recovered uneventfully. A course of extensive supportive therapy with systemic antibiotics after surgery was carried out. The foal fully recovered and was discharged after 14 days of hospitalization. The follow-up after seven months revealed the patient to be clinically healthy. This report describes a case of uroperitoneum secondary to the rupture of the urinary bladder in a Thoroughbred foal, along with the clinical outcomes of surgical repair and intensive treatment in detail.

  • A chlorine gas is a common irritant and when exposed, it result in mild occular, oropharyngeal, or respiratory symptoms. In severe case, however, it may result in pulmonary edema, interstitial pneumonia, or respiratory failure. We report the case of 29-year-old and 46-year-old men is accidentally exposed to chlorine gas during cleaning water. The patients complained dyspnea, chest tightness, cough and both eye pain. During hospitalization, they were treated with inhalation of humidified oxygen, beta-adrenergic agonist and $2\%$ sodium bicarbonate. After several days, patients were discharged without respiratory symptoms and complication. In treatment of chlorine gas toxicity the inhalation of sodium bicarbonate is a possible initial therapy can improve respiratory symptoms in spite of lack of evidence.

  • Objective: The present study reports on one patient diagnosed with Guillain-Barre syndrome.Method: One patient was treated by Sukjiyanggeun-tang, electroacupuncture, and physical therapy. Any improvement in symptoms was assessed by measuring changes in motor grade; scores in the modified Barthel index, the disability scale, and the numerical rating scale (NRS) of lower back pain; and ambulatory condition during hospitalization.Results: After 48 days of treatment, the patient's symptoms of Guillain-Barre syndrome showed improvement.Conclusions: Traditional Korean medical treatment appeared to be effective in reducing Guillain-Barre syndrome symptoms, but more research is required to confirm these results.

  • A 64-year-old man was admitted due to jaundice for 2 weeks. Radiologic findings revealed biliary stricture at the hepatic hilum with intrahepatic duct dilation suggesting Bismuth type IV Klatskin tumor. Jaundice improved spontaneously several days after hospitalization. Surgical treatment was considered but he only wanted to observe without specific treatment. Ten months later, he was re-admitted due to the recurrence of jaundice. Computed tomography (CT) showed no significant difference compared to previous results. Serum cancer antigen 19-9 and Immunoglobulin G4 were normal. Endoscopic forcep biopsy during endoscopic retrograde cholagiopancreatography (ERCP) revealed chronic inflammation. After steroid use under possible diagnosis of IgG4 related cholangiopathy, biliary stricture improved slightly. Four years later, he was hospitalized with the occurrence of acute cholangitis. Endoscopic retrograde biliary drainage was performed following endobiliary forcep biopsy. Pathology revealed well-differentiated adenocarcinoma at this time. Combined chemotherapy based on gemcitabine and cisplatin was performed. Six months later, CT revealed partial response.

Transfromation of Percutaneous Extracorporeal Life Support to Paracorporeal Ventricular Assist Device: A Case Report

  • Percutaneous extracorporeal life support (P-ECLS) is a useful modality for the management of refractory cardiac or pulmonary failure. However, venoarterial P-ECLS may result in a complication of left ventricular distension. In this case report, we discuss a patient with drug-induced dilated cardiomyopathy managed with venoarterial P-ECLS and a left atrial vent catheter. The venoarterial P-ECLS was modified to a paracorporeal left ventricular assist device (LVAD) by removing the femoral venous cannula. After 28 days of hospitalization, the patient was successfully weaned from the paracorporeal LVAD and discharged home from the hospital.
