Workers' Health Belief in Health Promotion Programs and Related Factors (사업장 건강증진 프로그램에 대한 근로자의 건강신념 및 관련요인)

  • Purpose: The purpose of the study was to investigate the relationships between worker's health belief in health promotion programs and their demographic, health, and health behavior characteristics. Methods: This study used survey data from 262 small- and large-scale workplaces. The study included 1149 male and female workers who completed the questionnaires. Results: The health belief score increased significantly with age and length of service. Male, married, shift-work, and white-collar workers showed a significantly higher health belief score than female, single, non-shift-work, and blue-collar ones. Workers with disease history, hypertension, or obesity had a significantly higher health belief score. Regular exercise was significantly correlated with the health belief score. When all the variables were included in the model, the health belief score was significantly higher in workers who were married, had a longer length of service, worked in the sales department, were diagnosed with hypertension, exercised regularly, and felt fatigue. Conclusion: The results showed that workplace intervention programs meet more the needs of workers who have a higher health belief score. In addition, given that healthier life style was related to a higher health belief score, further research is required to find how to change health behavior in workers.

A Comparative Study on the Relationships of Depression, Fatigue, BMI, and Health Locus of Control between Urban and Rural Female Middle School Students (도시 농촌 간 여중생의 우울, 피로, 체질량지수, 건강통제위 비교)

  • Purpose: The principal objective of this study was to assess the relationships of depression, fatigue, BMI, and health locus of control between urban and rural female middle school students. Methods: The study subjects included 377 girls in randomly-sampled middle schools in urban and rural provinces. The data were collected via self-recorded questionnaires from April 21.- to May 2. 2008, and analyzed using the SPSS Win 12.0 program. Results; 1) The mean score of depression was $10.22{\pm}5.72$ (urban), $10.81{\pm}4.92$ (rural). 2) The mean fatigue score was $50.43{\pm}1.57$ (urban), $51.90{\pm}9.70$ (rural). 3) BMI was classified into 3 groups: leptosomic, normal weight, and obesity groups. The mean BMI score was $18.73{\pm}2.05$ (Seoul), and $20.31{\pm}3.26$ (provincial). 4) The internal locus of control was higher than the external locus of control (urban). 5) Fatigue was correlated with depression (r=.512, p=.000) in both areas. The health locus of control was correlated with Fatigue (r=.153, p=.042) in province. Conclusion: An effective program to reduce the levels of fatigue and depression for middle school girls and to maintain normal weight is required. In particular, the high-risk depression group can be managed through community mental health care center deep interviews, and close observation.

Association between serum 25-hydroxyvitamin D levels and adiposity measurements in the general Korean population

  • BACKGROUND/OBJECTIVES: Obesity, which is a known risk factor for many chronic diseases, has also been associated with vitamin D deficiency. This study explored the relationship between serum 25-hydroxy-vitamin D [25(OH)D] concentrations and adiposity measures in a general Korean population using the most recent, nationally representative survey data. SUBJECTS/METHODS: The study sample consisted of 4,771 Korean adults (${\geq}19years$) who participated in the fifth Korean National Health and Nutrition Examination Surveys. Serum 25(OH)D was determined by radioimmunoassay. Body mass index (BMI), waist circumference (WC) and total body fat content were measured as adiposity measurements. Total body fat content was measured by dual-energy X-ray absorptiometry. RESULTS: The serum 25(OH)D level was significantly higher in men than in women. Serum 25(OH)D concentration was positively correlated with energy intake, and it was negatively correlated with total body fat content (P < 0.0001) and percentage body fat (P < 0.0001) after adjustment for age in both sexes, while was inversely correlated with BMI only in women. In multivariable regression analysis, serum 25(OH)D was inversely associated with the total body fat content after adjustment for age, BMI, education, region, smoking, alcohol consumption, physical activity, and energy intake only in men (P = 0.0047). However, the serum 25(OH)D concentration was not associated with WC or BMI, indicators of adiposity after adjustment for potential risk factors. CONCLUSIONS: Serum 25(OH)D concentration was independently associated with the total body fat content in a general Korean population, but it may be not associated with the indicators for estimating adiposity, such as WC or BMI.

Physical activity level, total daily energy expenditure, and estimated energy expenditure in normal weight and overweight or obese children and adolescents (소아청소년의 비만여부에 따른 신체활동수준, 1일 총에너지소비량 및 에너지필요추정량의 평가)

  • The purposes of this study were to assess the physical activity level (PAL) and the total daily energy expenditure (TEE) as well as to evaluate the validity of prediction equation for the estimated energy requirement (EER) in normal weight and overweight or obese children and adolescents. The subjects comprised of 100 healthy Korean students aged between 7-18. The anthropometric data was collected. PAL was calculated from the physical activity diary by the 24-hour recall method, and the resting metabolic rate (RMR) was measured by an open-circuit indirect calorimetry using a ventilated hood system. Daily energy expenditure was PAL multiplied by RMR. EER was calculated by using the prediction equation published in KDRIs. There was no significant difference in the means of age and height between the 46 obese subjects and 54 nonobese subjects. The weight and BMI of the obese group (60.2 kg, $25.3kg/m^2$) were significantly higher than those of the nonobese group (42.4 kg, $18.4kg/m^2$). However, PAL was not significantly different between the two groups (nonobese 1.45, obese 1.46). TEE of the obese group (2,212 kcal/day) was significantly higher than that of the nonobese group (1,774 kcal/day). EER (individual PA) and EER (light PA) were significantly higher than TEE (p < 0,001); however, EER (sedentary PA) was not significantly different with TEE in the two groups. These results showed that the levels of physical activity were the same as the sedentary activity both in the nonobese and obese Korean students; moreover, the predictive equation for EER published in KDRI overestimated the TEE of Korean children and adolescents. Therefore, in further research, a new predictive equation for EER should be developed for Korean children and adolescents through the doubly labeled water method.

The Dietary Characteristics of Middle-aged People by BMI and food Habits (체질량 지수와 식습관에 따른 중 .장년층의 식생활 특성)

  • This study was conducted to compare the dietary characteristics of middle-aged people with different obesity index or food habits. Questionnaires were collected from 480 subjects in Daejon. The data were analyzed by $X^2$-test and ANOVA. The subjects were distributed into the normal weight group(66.3%), overweight group(18.3%) and underweight(15.4%) based on BMI. Twenty-five percent of males and 23.0% of females in the 50's belonged to the overweight group. The least consumed food was milk or milk products among the five basic food groups. The frequency of exercise was low in their daily life. The had better food habits than the males. The overweight group had worse food habits than the normal weight or underweight group. The underweight group ate light meal and ate slowly. The better the subjects had, the more regularly they took breakfast(p<0.001). The good food habits group were more interested in their dietary life and were more careful in selecting foods for their health than the bad food habits group(p<0.001). Based on the results of this study, the practical education programs should be focused on more intake of milk or products and on daily exercise for the self-health maintenance of middle-aged people. Also, proper education programs should be developed for males and females in their 50's to practice weight control and regularity of meals in their life, and to utilize nutrition knowledge in dietary life. Further research on the comparision of dietary characteristics are needed with consideration of the subjects ratio according to BMI.

Nutrient Intake, Lifestyle Factors and Prevalent Hypertension in Korean Adults: Results from 2007-2008 Korean National Health and Nutrition Examination Survey (한국 성인의 고혈압 유병 관련 영양소 섭취 및 생활습관 위험 요인 분석: 2007-2008년 국민건강영양조사 결과 활용)

  • Hypertension is a well-known risk factor for cardiovascular disease. Previous studies have shown that changes in diet and lifestyle factors can prevent the development of hypertension, but the combined effects of these modifiable factors on hypertension are not well established. The objective of this study is to investigate associations of diet and lifestyle factors, evaluated both individually and in combination, with prevalent hypertension among Korean adults. We analyzed data obtained from the 2007-2008 Korean National Health and Nutritional Examination Survey, a nationwide cross-sectional study using a stratified, multistage probability sampling design. The associations of 12 nutrient intakes and lifestyle factors with risk of hypertension were explored using restricted cubic spline regression and logistic regression models among 6,351 adults. Total energy and several nutrients and minerals, including, calcium, vitamin A, vitamin C, and sodium, showed non-linear relationships with the risk of prevalent hypertension. In multivariate logistic regression models, dietary score, obesity and alcohol intake were independently associated with the risk of prevalent hypertension, but smoking and physical activity were not. Overall, participants whose dietary habits and lifestyle factors were all in the low-risk group had 68% lower prevalence of hypertension (OR: 0.32, 95 CI: 0.14-0.74) compared to those who were at least one in the high-risk group of any dietary or lifestyle factors. The result suggests that combined optimal lifestyle habits are strongly associated with lower prevalence of hypertension among Korean adults.

Endogenous catalase delays high-fat diet-induced liver injury in mice

  • Non-alcoholic fatty liver disease (NAFLD) has become the most prevalent liver disease in parallel with worldwide epidemic of obesity. Reactive oxygen species (ROS) contributes to the development and progression of NAFLD. Peroxisomes play an important role in fatty acid oxidation and ROS homeostasis, and catalase is an antioxidant exclusively expressed in peroxisome. The present study examined the role of endogenous catalase in early stage of NAFLD. 8-week-old male catalase knock-out (CKO) and age-matched C57BL/6J wild type (WT) mice were fed either a normal diet (ND: 18% of total calories from fat) or a high fat diet (HFD: 60% of total calories from fat) for 2 weeks. CKO mice gained body weight faster than WT mice at early period of HFD feeding. Plasma triglyceride and ALT, fasting plasma insulin, as well as liver lipid accumulation, inflammation (F4/80 staining), and oxidative stress (8-oxo-dG staining and nitrotyrosine level) were significantly increased in CKO but not in WT mice at 2 weeks of HFD feeding. While phosphorylation of Akt (Ser473) and $PGC1{\alpha}$ mRNA expression were decreased in both CKO and WT mice at HFD feeding, $GSK3{\beta}$ phosphorylation and Cox4-il mRNA expression in the liver were decreased only in CKO-HF mice. Taken together, the present data demonstrated that endogenous catalase exerted beneficial effects in protecting liver injury including lipid accumulation and inflammation through maintaining liver redox balance from the early stage of HFD-induced metabolic stress.

Comparison of Nutrient Intake, Dietary Behavior, Perception of Body Image and Iron Nutritional Status among Female High School Students of Urban and Rural Areas in Kyunggi-do (경기도 일부 도시 및 농촌지역 여고생의 영양섭취, 식행동, 체형인식 및 철분영양상태 비교연구)

  • This study was undertaken to investigate the nutrient intake, dietary behavior, perception of body image and iron deficiency of high school adolescent girls. The subjects were 463 high school girls in Inchon. This cross-sectional survey was conducted by questionnaires and data were analysed by SAS program. Nutrient intakes collected from 3 day-retails were analyzed by the Computer Aided Nutritional Analysis Program. The following anthropometric measurements were made on all participants height, weight, body fat (%), skinfold thirkness, subscapular skinfold thickness, mid-upper arm circumference and waist-hip circumference. Blood samples were obtained and analyzed for iron nutritional status. The mean obesity index of rural students were highest among students. However, over 18% of the urban and rural students belonged to the low-weight group. Proportions of skipping meals were most high in urban vocational students and the main reason was lack of time for meal. Most nutrient intakes of the students were below the RDA, in particular calcium, iron, vitamin A and vitamin B$_2$. Most of the students were concerned with body image and weight control. Also they were not satisfied with their body image. The mean fell hemoglobin and transferrin saturation of rural students were lowest among students. The total binding capacity of urban vocational student\ulcorner were Invest among students. Prevalence of iron deficiency was most high in urban general students when judged by Hb, MCV and serum ferritin. In conclusion, this study indicates that nutrient intake in the female high school students was considerably lower than RDA. They should also be notified that severe weight control is very harmful for health. And they had lower self -satisfaction for their body image and undesirable flood habits. Therefore, prefer nutrition education is required to maintain desirable flood habits and improve their iron status in female high school students.

Relationships of family support, diet therapy practice and blood glucose control in type II diabetic patients

  • The purpose of this study was to investigate the degree of family support for diabetic patients and the diet therapy practice of patients themselves, and to analyze the relationship between family support and diet therapy practice and blood glucose control, and thus to prepare basic data for the development of effective education programs to improve blood glucose control in diabetic patients. The study subjects were 82 patients with type II diabetes, aged over 20 in the Chungbuk area. The gender distribution of subjects was 52.4% males and 47.6% females, and BMI showed 29.3% overweight and 35.3% obesity. Among the 82 study subjects, the relationship between diet therapy related family support and blood glucose control was examined in 67 subjects who answered practicing diet therapy, and the results showed that the family support score of a group with excellent blood glucose control was significantly higher than those of groups with fair or poor control (p<0.001) and the correlation between the two factors was very high (r=0.341, p<0.001). For the relationship between diet therapy practice by patients themselves and blood glucose control, diet therapy practice of a group with excellent blood glucose control was significantly higher than those of other groups (fair or poor control groups) (p<0.001) and the correlation between two factors was very high (r=0.304, p<0.001). For other factors influencing blood glucose control, a group with diabetes education showed significantly better blood glucose control compared to other groups without education (p<0.05). From the above results, diet therapy practice by patients, family support, and the necessity of diabetes education were confirmed to control blood glucose of diabetic patients. In conclusion, development and operation of education program should include not only patients but also their family members.

Transcriptome analysis and promoter sequence studies on early adipogenesis in 3T3-L1 cells

  • To identify regulatory molecules which play key roles in the development of obesity, we investigated the transcriptional profiles in 3T3-L1 cells at early stage of differentiation and analyzed the promoter sequences of differentially regulated genes. One hundred and sixty-one (161) genes were found to have significant changes in expression at the 2nd day following treatment with differentiation cocktail. Among them, 86 transcripts were up-regulated and 75 transcripts were down-regulated. The 161 transcripts were classified into 10 categories according to their functional roles; cytoskeleton, cell adhesion, immune, defense response, metabolism, protein modification, protein metabolism, regulation of transcription, signal transduction and transporter. To identify transcription factors likely involved in regulating these differentially expressed genes, we analyzed the promoter sequences of up- or - down regulated genes for the presence of transcription factor binding sites (TFBSs). Based on coincidence of regulatory sites, we have identified candidate transcription factors (TFs), which include those previously known to be involved in adipogenesis (CREB, OCT-1 and c-Myc). Among them, c-Myc was also identified by our microarray data. Our approach to take advantage of the resource of the human genome sequences and the results from our microarray experiments should be validated by further studies of promoter occupancy and TF perturbation.
